The pathophysiology of osteoarthritis.

Osteoarthritis (OA) is a complex disease whose pathogenesis includes the contribution of biomechanical and metabolic factors which, altering the tissue homeostasis of articular cartilage and subchondral bone, determine the predominance of destructive over productive processes. A key role in the pathophysiology of articular cartilage is played by cell/extra-cellular matrix (ECM) interactions, which are mediated by cell surface integrins. In a physiologic setting, integrins modulate cell/ECM signaling, essential for regulating growth and differentiation and maintaining cartilage homeostasis. Dur

Incidence and progression of ankle osteoarthritis: The johnston county osteoarthritis project.

OBJECTIVE: To determine the incidence and progression of ankle osteoarthritis (OA) and associated risk factors in a community-based cohort of African Americans and whites. METHODS: Data were from 541 participants who had standardized lateral and mortise radiography of the ankles in weight bearing at baseline (2013-2015) and follow-up (2017-2018). Incident radiographic ankle OA (rAOA) was defined as a Kellgren-Lawrence grade (KLG) ≥ 1 at follow-up among ankles with baseline KLG < 1; progressive rAOA was a ≥ 1 KLG increase at follow-up among ankles with KLG ≥ 1 at baseline. Symptoms were assess

Differentiation, diagnosis, and treatment of osteoarthritis, osteonecrosis, and rapidly progressive osteoarthritis.

Osteoarthritis, osteonecrosis, and rapidly progressive osteoarthritis are hip arthropathies that result in marked pain and disability. Although these diseases share features of arthritis-like symptoms and are all treated with total hip arthroplasty, they are separate diseases with distinct epidemiologic, radiographic, and histopathologic findings. In this article, the authors present clinical tips and techniques that will aid the surgeon in diagnosing and treating these different entities.

The epidemiology of knee osteoarthritis: results from the Framingham Osteoarthritis Study.

The Framingham Knee Osteoarthritis study is a population-based study of independently living elderly examining the prevalence of radiographic and symptomatic knee osteoarthritis. This group was assessed in the early 1980s at which time they had been observed for over 35 years and many risk factors for osteoarthritis had been ascertained. Results from this study suggest that knee osteoarthritis increases in prevalence throughout the elderly years, more so in women than in men. Also, studies of risk factors have shown that obesity precedes and increases the risk of knee osteoarthritis, especiall

Ankle Osteoarthritis Aetiology.

Ankle osteoarthritis affects 1% of the population and, unlike gonarthrosis or coxarthrosis, is secondary to previous trauma in more than 75% of cases. Another peculiarity of this disease is that it affects a younger and active population, with socio-occupational implications. Mechanical factors, such as incongruity, instability, malalignment, and impacts, which increase stress on isolated areas of the ankle cartilage, have been clearly associated with the development of osteoarthritis. However, we cannot ignore the importance of pro-inflammatory mediators present from the moment of fracture as

[Cervical spine osteoarthritis].

Cervical spine is particularly susceptible to anatomical osteoarthritis, but in a good half cases it remains asymptomatic. Painful cervical spine osteoarthritis results in neck pain, sometimes acute, more commonly chronic. But cervical osteoarthritis may induce compression of the neurovascular structures contained into the cervical spine: upper arm nerve root pain by nerve root compression; myelopathy by spinal cord compression; sometimes vertebro-basilar insufficiency by compression of vertebral arteries.

[Coxiella burnetii osteoarthritis].

A case of a 25-year-old female characterized by febrile osteoarthritis and arthromyalgias as the only clinical manifestation of a Q fever is discussed. Analytical data were compatible with those of an acute inflammation and serologic findings reached diagnostic levels. Response to Doxycycline treatment was totally satisfactory. Even though associated arthromyalgias are described in some cases of Q fever as well as sporadic cases of osteoarthal infection due to Coxiella burnetii, there are no oligosymptomatic cases published as the one discussed. Some physiopathologic considerations referred to

Metabolic osteoarthritis - relation of diabetes and cardiovascular disease with knee osteoarthritis.

OBJECTIVE: There is an interest in identifying a metabolic OA phenotype. We therefore assessed the relation of diabetes and cardiovascular disease to prevalent and incident radiographic (ROA) and symptomatic knee osteoarthritis (SxOA). DESIGN: In two large cohort studies of individuals with or at risk for knee OA, the Multicenter Osteoarthritis Study (MOST) and Osteoarthritis Initiative (OAI), participants self-reported diabetes and cardiovascular disease (CVD) at baseline. We assessed the relation of baseline diabetes and CVD (exposures) to ROA and SxOA cross-sectionally and after 60 (MOST)

The Phenotypic Approach to Osteoarthritis: A Look at Metabolic Syndrome-Associated Osteoarthritis.

Metabolic syndrome-associated osteoarthritis (Met-OA) is a clinical phenotype defined by the role of obesity and metabolic syndrome as risk factors and by chronic low-grade inflammation. Obesity is an established risk factor for osteoarthritis not only at the knee, but also at the hands. Metabolic syndrome is also a risk factor for osteoarthritis, and a cumulative effect of the various syndrome components combines with an independent effect of each individual component (diabetes, dyslipidemia, and/or hypertension). The higher incidence of osteoarthritis in patients with obesity is related to s

Studies in osteoarthritis of the hip. 3. Congenital subluxation and osteoarthritis of the hip.

Although congenital subluxation of the hip is an important primary abnormality, upon which OA hip may later supervene, other factors may also be important in individual patients. When osteoarthritis is seen with subluxation, it probably represents congenital subluxation and not displacement of the femoral head secondary to remodelling. Osteoarthritis in these cases is usually superolateral or supermedial in type, with the major impact of the disease in the superior articular cartilage. A similar form of osteoarthritis is seen in cases where leg-length disparity exists, with or without the pres

Early knee osteoarthritis.

Concepts regarding osteoarthritis, the most common joint disease, have dramatically changed in the past decade thanks to the development of new imaging techniques and the widespread use of arthroscopy that permits direct visualisation of intra-articular tissues and structure. MRI and ultrasound allow the early detection of pre-radiographic structural changes not only in the peri-articular bone but also in the cartilage, menisci, synovial membrane, ligaments and fat pad. The significance of MRI findings such as cartilage defects, bone marrow lesions, synovial inflammation/effusions and meniscal

[Erosive hand osteoarthritis].

Erosive hand osteoarthritis is a form of hand osteoarthritis. Phase of the normal process or separate entity? This remains debated. It is defined by radiographic erosions with a central subchondral bone collapse. It is more frequent than initially described as recently evidenced by ultrasonographic and MRI images which have shown better sensitivity to detect erosions and osteophytes than conventional radiographs. Synovitis, detectable by Doppler ultrasonography, is a common feature. The classical clinical picture is made of inflammatory flares with severe pain, joint swelling and important fun

Back pain: osteoarthritis.

Back pain is one of the chief complaints of the elderly. It may be either a chronic deep skeletal muscular pain or an acute circumscribed pain arising from nerve-root irritation. The main causes of back pain in older people are: 1) degenerative changes (spondylosis, osteoarthritis, ankylosing hyperostosis); 2) malignancy (multiple myeloma, metastases from carcinoma or lymphoma); and 3) metabolic disorders (osteoporosis, osteomalacia, chondrocalcinosis, Paget's disease). Mechanisms and variations are discussed in detail.

New developments in osteoarthritis. Posttraumatic osteoarthritis: pathogenesis and pharmacological treatment options.

Joint trauma can lead to a spectrum of acute lesions, including osteochondral fractures, ligament or meniscus tears and damage to the articular cartilage. This is often associated with intraarticular bleeding and causes posttraumatic joint inflammation. Although the acute symptoms resolve and some of the lesions can be surgically repaired, joint injury triggers a chronic remodeling process in cartilage and other joint tissues that ultimately manifests as osteoarthritis in a majority of cases. The objective of the present review is to summarize information on pathogenetic mechanisms involved in

Association of radiographic hand osteoarthritis with radiographic knee osteoarthritis after meniscectomy.

OBJECTIVE: To evaluate the association between radiographic hand osteoarthritis (OA), a disease with marked heredity, and radiographic knee OA in patients treated with meniscectomy. METHODS: We retrospectively identified 170 patients (mean age 54 years [range 33-87 years], 23% women) who had undergone isolated meniscectomy an average of 20 years earlier (range 17-22 years). Patients with cruciate ligament injury were excluded. All subjects were examined by standardized knee and hand radiography. Individual joints were considered to have OA when displaying radiographic features corresponding t

Per- and Polyfluoroalkyl Substances and Knee Osteoarthritis: Data From the Osteoarthritis Initiative.

OBJECTIVE: To examine whether concentrations of specific per- and polyfluoroalkyl substances (PFAS) and a mixture of PFAS relate to incident knee osteoarthritis (KOA) and knee pain progression. METHODS: Among a case-cohort sample from the OA Initiative (n = 1,878), we examined associations of serum concentrations of eight PFAS with odds of developing over the subsequent 48 months (1) symptomatic KOA and (2) a clinically meaningful change in the Western Ontario and McMaster Universities OA Index knee pain score. We used weighted logistic regression to assess single PFAS (continuous and quartil

Novel insights into the role of ferroptosis in temporomandibular joint osteoarthritis and knee osteoarthritis.

Osteoarthritis (OA) is a prevalent degenerative joint disease characterized by pain, limited movement, and joint stiffness, significantly impacting the quality of life and imposing substantial economic burdens. This review paper delves into the novel insights of ferroptosis, an iron-dependent form of cell death associated with lipid peroxidation, in the context of temporomandibular joint osteoarthritis (TMJ OA) and knee osteoarthritis (KOA). We explore the pathogenic characteristics of OA, including synovitis, chondrocyte death, and extracellular matrix (ECM) degradation, and discuss the limit

[Is osteoarthritis an inflammatory disease after all?; prednisolone effective in osteoarthritis of the hand].

There are very few treatment options that modify the course of osteoarthritis (OA) of the hand; the only current treatment options are physiotherapy and analgesics. At the moment, OA is regarded as a disease of the entire joint, with involvement of cartilage, bone, synovium, ligaments and muscles and new treatment options can focus on these structures. In a double-blind, placebo-controlled trial, treatment of patients with inflammatory OA of the hand (with pain, soft-tissue swelling and an ultrasound picture characteristic of inflammation) with 10 mg prednisolone per day for 6 weeks resulted i

Frequency of temporomandibular joint osteoarthritis and related symptoms in a hand osteoarthritis cohort.

OBJECTIVE: The prevalence of osteoarthritis (OA) in the temporomandibular joints (TMJs) in hand OA patients is largely unknown. Our aims were to explore (1) The frequency of TMJ-related symptoms and clinical findings; (2) The TMJ OA frequency defined by cone beam computed tomography (CBCT); and (3) The relationship between TMJ-related symptoms/clinical findings and CBCT-defined TMJ OA, in a hand OA cohort. METHODS: We calculated the frequencies of TMJ-related symptoms, clinical findings and diagnosis of TMJ OA by CBCT and clinical examination in 54 patients from the Oslo hand OA cohort (88% w
