Airway changes after Le Fort III osteotomy in craniosynostosis syndromes.

The purpose of this study was to investigate changes in the nasopharyngeal airway and surrounding structures after Le Fort III advancement in Apert's and Crouzon's syndromes. Sixteen status post Le Fort III osteotomy Apert's and Crouzon's patients with pre- and postoperative lateral cephalograms were evaluated. The cephalograms were traced, and linear and angular measurements were obtained. Pre- and postoperative data were compared using a Student's paired t-test. The correlation between changes in the nasopharyngeal airway and its surrounding skeletal structures was evaluated. Finally, the st

Le Fort III distraction osteogenesis versus conventional Le Fort III osteotomy in correction of syndromic midfacial hypoplasia: a systematic review.

PURPOSE: It has been debated whether the Le Fort III procedure using distraction osteogenesis (LFIII-DO) reduces the risk of postintervention relapse compared with conventional Le Fort III (LFIII) osteotomy in the correction of syndromic midfacial hypoplasia. Our objective was to evaluate the short- and long-term stability of the bony structures after midfacial advancement using conventional LFIII osteotomy versus LFIII-DO in patients with syndromic midfacial hypoplasia. MATERIALS AND METHODS: We performed a systematic review of the published data. An electronic search of 10 databases was per

Maxillary advancement and soft tissue response in Le Fort I osteotomy.

BACKGROUND: To evaluate the correlation between the amount of maxillary advancement and soft tissue changes, and to assess the long-term skeletal stability in patients who underwent Le Fort I osteotomy as part of bimaxillary orthognathic surgery. METHODS: This retrospective cohort study included 25 patients with skeletal Class III malocclusion who underwent bimaxillary orthognathic surgery. Cephalometric measurements were obtained at three timepoints: before orthodontic treatment (T0), after postsurgical orthodontic treatment (T1), and 1–3 years post-treatment (T2), with an average follow-up p

Trigeminal Cardiac Reflux During Le Fort I Osteotomy: A Case Report.

The trigeminocardiac reflex (TCR) is marked by significant cardiovascular reactions, such as bradycardia and asystole, triggered by trigeminal nerve stimulation. It is described as a brief episode of bradycardia, hypotension, or even cardiac arrest resulting from trigeminal nerve stimulation. The exact cause of TCR is not yet fully understood, but it is believed to involve the release of neurotransmitters, including acetylcholine, and the involvement of central neuronal integration. In this case report, we present an occurrence of trigeminal cardiac reflux during a Le Fort I osteotomy procedur

Le Fort I Osteotomy Complicated by Presence of Maxillary Sinus Septa.

Presence of maxillary sinus septa, an anatomical variant, has never been documented as a possible complication of Le Fort I orthognathic surgery procedure. This case report describes the difficulty faced by an experienced surgeon during Le Fort I down-fracture procedure. The osteotomized maxilla remained firm despite confirmation that all osteotomy cuts have been performed adequately. The presence of a bilateral sagittally oriented maxillary sinus septa extending from the floor to the roof of the sinus in this case had prevented separation of the lower maxilla during down-fracture maneuver. Pattern of pterygomaxillary disarticulation associated with Le Fort I maxillary osteotomy.

Pterygomaxillary disarticulation (PMD) contributes to surgical complications of Le Fort 1 osteotomy and is associated with undesirable fractures of the pterygoid plates. The aim of this paper was to investigate the patterns of PMD in Le Fort I osteotomies using Rowe's disimpaction forceps, and to evaluate correlations with age and anatomical measurements. Cone-beam computed tomography (CBCT) scans of 70 consecutive orthognathic patients were retrospectively evaluated to study four patterns of PMD: Type 1 - PMD at, or anterior to, the pterygomaxillary junction (PMJ); Type 2 - PMD posterior to t

A simple technique to facilitate the Le Fort I osteotomy downfracture.

In order to mobilize and reposition the maxilla when treating dentofacial deformities, the Le Fort I maxillary osteotomy is commonly used. Like other surgical procedures, this procedure is not without inherent difficulties. At times, it may be difficult to initially mobilize the maxilla after performing the osteotomies. Techniques used to help in the initial mobilization and downfracture of the maxilla from the pterygoid plates include digital pressure to the dentoalveolus, disimpaction forceps, spreaders, and other instruments. These techniques are similar in that they all apply a unidirectio

A Le Fort I osteotomy approach to lateral sphenoid sinus encephalocoeles.

Meningo-encephalocoeles of the skull base may present as spontaneous cerebrospinal fluid rhinorrhoea or acute meningitis. Previous approaches to midline skull base lesions have been either intracranial, via a craniotomy, or by transfacial or endoscopic extracranial approaches. This paper presents an alternative approach to lateral sphenoid sinus encephalocoeles through a Le Fort I osteotomy approach.

Blindness as a complication of Le Fort I osteotomy for maxillary distraction.

High Le Fort I osteotomy and maxillary distraction has become an accepted method for the treatment of maxillary retrusion in children and teenagers with cleft lip and palate or craniofacial anomalies. This procedure effectively corrects the dentofacial deformity in these patients. No major surgical morbidity has been reported. During the past 4 years, 94 cleft patients with maxillary hypoplasia received Le Fort I osteotomy and distraction osteogenesis at the authors' center. Two of them developed blindness after this operation. The first case was a girl with bilateral cleft lip and palate with

Vascular complications related to Le Fort I osteotomy: a scoping review.

BACKGROUND: Le Fort I (LFI) osteotomy represents a secure procedure employed for correcting facial deformities and may be coupled with other facial osteotomies. However, notwithstanding professional proficiency, this technique is not exempt from issues or morbidities. Therefore, the objective of this study was to relate the types of vascular complications associated with LFI. MATERIAL AND METHODS: A scoping review was conducted with published articles up to April 2024 sourced from PubMed, LILACS, EMBASE, Scopus, Web of Science, Cochrane Library and Google Scholar. The analyzed data comprised:

The influence of tissue redraping after Le Fort I type osteotomy.

Elongation of the upper lip, in particular the philtrum, and nose widening are common postoperative changes after a Le Fort I osteotomy. These changes can be induced by the transection of soft tissue and loosening of the underlying musculature. A methodology for soft tissue redraping was developed to limit these undesirable nasolabial changes. This study evaluates the effectiveness of the technique and influence of maxillary translocation on the nasolabial form. Anthropometric measurements, lip, philtrum length, and nose width, were taken two weeks prior to, and one year after, surgery. The me

Relevant Surgical Anatomy of Pterygomaxillary Dysjunction in Le Fort III Osteotomy.

BACKGROUND: Le Fort III osteotomy represents the foundation of surgical correction for midface hypoplasia. One serious complication of Le Fort III osteotomy is severing the internal maxillary artery or its branches during osteotome advancement for pterygomaxillary dysjunction. This study sought to characterize the relevant surgical anatomy of the infratemporal fossa and of the internal maxillary artery as it enters the pterygomaxillary fissure. METHODS: Bilateral midface dissections were performed on 15 fresh, normocephalic adult cadavers (30 hemifaces). Four superficial anatomical measuremen

Anatomic basis of Le Fort 1 impaction osteotomy: a radiological study.

PURPOSE: In Le Fort 1 osteotomy when a maxillary impaction is necessary, surgeons have to face different anatomical problems. (1) To determine the best bone resection route, they have to consider the situation of dental roots, infraorbital foramen and maxillary artery. (2) In case of Le Fort 1 osteotomy combined with a mandibular sagittal split osteotomy, the palate has to be replaced in horizontal position although there is no anatomical landmark. (3) In case of Gummy smiles, it can be due to either long face or short upper lip. The main objective was to identify safe bony landmarks to perfor
