Bacteriology and resistance patterns of otitis media with effusion.

OBJECTIVES: Following the increased use of antibiotics, the emergence of antibiotic-resistant species in pediatric patients with otitis media has become a problem in recent years. The aim of this study was to investigate change in bacterial species, antibiotic resistance, and detection rate of highly pathogenic species, such as Methicillin-resistant Staphylococcus aureus (MRSA) and Pseudomonas aeruginosa according to the number of repeated ventilation tube insertion (VTI) procedures in pediatric patients diagnosed with otitis media with effusion (OME). MATERIALS & METHODS: From May 2010 to Ju

Relation between adenoid size and otitis media with effusion.

AIMS: This study compares the efficacy of adenoidectomy on otitis media with effusion (OME) in patients with different size of adenoids and the connection between differently sized adenoids and middle ear effusion. MATERIAL AND METHODS: Children with a history of at least 3 months' OME underwent adenoidectomy and myringotomy without the insertion of a tympanostomy tube. Treatment assignment was stratified by adenoids' size causing choanal obstruction (grade I-III) and according to Eustachian tube ostium obstruction (grade A-C). The subjects were followed for 12 months. RESULTS: Adenoidectomy

Beta-trace protein in pediatric otitis media with effusion.

OBJECTIVES: The middle ear cleft connects by membranes to the inner ear and the subarachnoid space particularly in infants. In order to gain more insight about the permeability between the two compartments we quantified the concentration of beta-trace protein - a highly specific marker for CSF and perilymph but not for serum and mucosal effusion - in middle ear secretions from children with otitis media with effusion. METHODS: One-hundred and three patients were included and 93 samples from secretory otitis media were collected during myringotomy or explorative tympanotomy. Thirty-eight patie

Otitis media with effusion: is medical management an option?

Persistent middle ear effusion is a common cause of hearing impairment and remains underdiagnosed, particularly among younger children. Detection can be improved by adequate follow-up of otitis media. Decisions on management need to take into account the child's age, duration and severity of illness, degree of hearing impairment, and any evidence of learning difficulties. There is no definitive cure but both medical and surgical treatments may improve outcome. With increasing evidence that antihistamine-decongestant mixtures are ineffective, there may in the future be a role for antibiotics an

Allergy: a leading role in otitis media with effusion.

We have studied 35 children with seasonal allergic rhinitis confirmed by positive skin tests and RAST to grass pollen. Intranasal antigen provocation was performed to evaluate the function of the eustachian tube (ET) and middle ear pressure (MEP). Tympanometry was used to evaluate the changes in MEP, while Valsava and Toynbee tympanometric tests were employed to study the ET function, and the changes in nasal airway resistance were measured by anterior rhinomanometry. ET dysfunction (ETD) and a decrease in MEP and nasal airway resistance were demonstrated in the children under study as compare

Adenoidectomy for otitis media with effusion (OME) in children.

BACKGROUND: Otitis media with effusion (OME) is an accumulation of fluid in the middle ear cavity, common amongst young children. The fluid may cause hearing loss. When persistent, it may lead to developmental delay, social difficulty and poor quality of life. Management of OME includes watchful waiting, autoinflation, medical and surgical treatment. Adenoidectomy has often been used as a potential treatment for this condition. OBJECTIVES: To assess the benefits and harms of adenoidectomy, either alone or in combination with ventilation tubes (grommets), for OME in children. SEARCH METHODS:

Otitis media with effusion in two Boston terrier dogs.

Otitis media (OM) in dogs can occur as a primary condition instead of as an extension of an otitis externa (OE), characterized by the presence of fluid in the middle ear (ME) referred to as OM with effusion (OME). OME has been reported in a brachycephalic breed (boxer), and the same condition is described as primary secretory OM in Cavalier King Charles Spaniels. These dogs can be asymptomatic or present with pain, facial nerve paralysis and reduced hearing. This report describes two cases of OME with associated neurologic signs in Boston terriers with no previous history of OE, normal externa

Craniofacial morphology and otitis media with effusion in children.

Otitis media with effusion (OME) affects 28-38% of pre-school children, and it occurs due to the dysfunction of the auditory tube. Anatomical development of the auditory tube depends on the craniofacial growth and development. Deviations of normal craniofacial morphology and growth using cephalometric studies, may predict the evolution of otitis. Our goal in this paper is to determine if there are differences in craniofacial morphology between children with adenoid enlargement, with and without otitis media with effusion. This is a prospective study in which the sample consisted of 67 children

Carhart's notch: a finding in otitis media with effusion.

INTRODUCTION: Carhart's notch (CN) is a false depression of bone conduction (BC) thresholds at 2-4 kHz initially described in cases of stapes fixation. This study was designed to estimate the incidence and assess the clinical significance of CN in cases of otitis media with effusion (OME) in children. PATIENTS AND METHODS: Clinical records of 50 patients of OME that showed CN were analysed, retrospectively. First 24 were identified as seen in outpatients and 26 were found out of 100 consecutive cases of OME. The criteria of CN were a minimum 10 dB depression in BC at any frequency 500-4000 Hz

Risk factors of otitis media with effusion during infancy.

UNLABELLED: Associations of possible risk factors with prevalence of otitis media with effusion (OME) were prospectively studied in a cohort of 250 infants, aged 0-2 years. In order to determine OME, otoscopy and tympanometry were performed at 3-monthly intervals beginning at term date. Eighteen epidemiologically relevant features were inventoried by means of standardized questionnaires. Multivariate analysis controlled for possible confounding factors. Prevalence of OME was most strongly associated with age (P-value < 0.001). Other factors significantly associated with the prevalence of OME (

Adenoids and otitis media with effusion: a morphological study.

PURPOSE: Adenoidectomy, especially for the treatment of suppurative otitis media, has been used for a very long time. In this study, the role of adenoids in the origin of otitis media with effusion was investigated by using light microscopy, immunocytochemistry, enzyme chemistry, and electron microscopy. MATERIALS AND METHODS: A group of 28 children with otitis media with effusion (OME) was identified. Ages ranged from 3 to 12 years. A control group of 10 age-matched children without any middle ear and upper respiratory tract infection served as the basis for comparison. Specimens obtained at

Tympanic membrane changes in experimental otitis media with effusion.

A tympanometric in vitro model was used to measure the mechanoacoustic properties of the tympanic membrane. Impedance measurements were made during the early course (1-21 days) of experimentally induced otitis media with serous effusion in the Mongolian gerbil. A more severe form of otitis media with mucoid effusion was produced by electrocauterization of the nasopharyngeal orifice of the eustachian tube. The dynamic peak values of the acoustic susceptance were significantly reduced early in the course of serious otitis media, indicating increased stiffness of the tympanic membrane. In mucoid

Immunologic characteristics of cytokines in otitis media with effusion.

Levels of cytokines, interleukin (IL)-1 alpha, IL-1 beta, tumor necrosis factor (TNF), and granulocyte-macrophage colony-stimulating factor (GM-CSF) were investigated in samples of the middle ear effusions (MEEs) from 144 ears with otitis media with effusion (OME) by enzyme-linked immunosorbent assay, followed by cytologic analysis. Middle ear effusions of the acute purulent type contained a significantly higher concentration of cytokines compared with normal control sera (p < .001). Cytokines were observed at lower levels in MEE in adults than in children. Tests of children at the chronic sta

Implication of surfactant apoprotein in otitis media with effusion.

A two-site simultaneous immunoassay using monoclonal antibodies against human surfactant apoprotein (SAP) was used to measure SAP in middle ear effusions (MEEs). In 130 MEE samples from children with otitis media with effusion, SAP was detected in 54 samples (SAP-positive cases, 41.5%). In the remainder, the SAP concentration was below the sensitivity of the immunoassay (SAP-negative cases, 58.5%). A significant difference in periods of observation was found between the SAP-positive cases (17.3 +/- 16.8 months) and the SAP-negative cases (26.2 +/- 22.5 months) (p less than .01). The percentage

Histamine-induced mucociliary dysfunction and otitis media with effusion.

Histamine, which has been found in middle ear effusions, is a potent pharmacological mediator released at an early stage of allergic reactions or general inflammatory process, increasing permeability of small blood vessels. Histamine might be involved in the origin and chronicity of middle ear effusions. In this study we studied the effect of histamine on the mucociliary function. First we examined the effect of histamine and its H1 and H2 blockers on the ciliary activity in the middle ear. 10(-2) M of histamine deteriorated ciliary activity; however, at lower concentrations ciliostimulatory e
