Nummular and Side-locked Headaches for the Otolaryngologist.

Side-locked headaches are a common symptom having a wide-ranging differential. Unchanging in laterality, these headaches can represent neuralgias, trigeminal autonomic cephalgias, ophthalmologic disorders, otolaryngologic and craniofacial disorders, vascular disorders, and malignancy. In rarer situations, they have presented secondary to neurosurgical or dermatologic considerations. Loss of cranial nerves and visual changes warrant additional evaluation.

Caustic Ingestion in Children: The Otolaryngologist Perspective.

OBJECTIVES: Caustic ingestion in children is a significant cause of morbidity despite preventive measures. Upon arrival to the emergency department, these children are often initially seen by the otolaryngologist and later on by the gastroenterologist. This study aimed to determine which otolaryngological and gastrointestinal signs and symptoms can better predict abnormal findings on imaging, esophagogastroduodenoscopy (EGD), and complications development. METHODS: We performed a retrospective chart review of children 18 years or younger admitted because of caustic ingestion between January 2

Congenital cytomegalovirus (CMV) for the pediatric otolaryngologist.

Cytomegalovirus (CMV) is a double-stranded DNA virus and a member of the herpesvirus family. It is the most common congenital viral infection. For symptomatic infections, symptoms can vary widely but tends to have a predilection for the central nervous system and for the reticuloendothelial system. Sensorineural hearing loss (SNHL) is by far the most common sequelae of congenital CMV infection. For this reason, it is imperative to understand the screening, diagnosis, and possible treatment options for congenital CMV induced SNHL. This literature review explores the association of CMV with hear

E-cigarettes: An update on considerations for the otolaryngologist.

PURPOSE: We provide an update in the literature and national regulations regarding electronic cigarettes with special attention to the pediatric population. BACKGROUND: Electronic nicotine delivery systems (ENDS) are handheld battery operated devices that vaporize nicotine-containing liquids for inhalation. Use of these products has dramatically increased over the last several years, particularly among the youth. ENDS are being marketed with advertising techniques and flavors which appeal to the adolescent and young adult population. More reports of accidental pediatric exposures are being do

[Otolaryngologist's perspective in childhood adenotonsillar hypertrophy].

The hypertrophy of adenoid and tonsillar is the most common cause of obstructive sleep apnea hypopnea syndrome(OSAHS) in children, which require early diagnosis and standardized treatment to improve behavior, attention and cognitive abilities. Adenotonsillectomy is the primary treatment for pediatric OSAHS with adenotonsillar hypertrophy. Meanwhile, the interest in antiinflammatory agents, such as intranasal corticosteroids and leukotriene antagonists, for childhood OSAHS has also increased.

Imaging for headache: what the otolaryngologist looks for.

Diagnosing a rhinogenic cause of headache or facial pain outside of the classic definitions of chronic, acute, and subacute sinusitis can be challenging for the practicing otolaryngologist. Contact-point headaches have been clinically characterized as causing facial pain secondary to abutting mucosal contact from the lateral nasal wall to the septum. Imaging landmarks may help identify these potential contact points radiographically through revealing anatomic variants such as septal spurs and abnormally large lateral nasal structures. However, other potential rhinologic sources, such as barosi

The otolaryngologist's approach to the patient with Down syndrome.

As more patients with Down syndrome are living into adulthood, attention has focused on health factors that affect the quality of the patient's life and their ability to reach full potential. Patients with Down syndrome have several morphologic abnormalities that predispose them to problems with the ear, nose, and throat, and appropriate treatment can have a significant impact on the quality of life of these patients. Otolaryngologists are likely to see many patients with Down syndrome throughout their careers. This article reviews the literature to provide information and recommendations rega

Role of the otolaryngologist in the treatment of struma ovarii.

Struma ovarii, a rare germ cell tumor of the ovary composed of >50% thyroid tissue, is traditionally managed by gynecologic surgeons. Although struma ovarii is typically cured by simple excision, otolaryngology-head and neck surgeons may play a critical role in caring for these patients. In this article, we present two cases of struma ovarii and discuss the role of the otolaryngologist.

Role of otolaryngologists in health care for the homeless.

OBJECTIVES: This study describes the prevalence of head and neck disease in patients seen at an urban, federally qualified clinic for those experiencing homelessness and determines the need for otolaryngologists in providing health care for the homeless. METHODS: All adult patient visits from 2000 to 2004 were reviewed for ICD-9 diagnostic codes representing otolaryngological diseases. Chart review was performed for patients referred to a volunteer otolaryngologist to confirm diagnoses and referrals. RESULTS: During the study period, the clinic served 11,690 unique patients in 59,060 patient

West Nile virus: a primer for the otolaryngologist.

BACKGROUND: Since recognition in the United States with a 1999 New York City epidemic, West Nile virus has enduringly migrated westward, leaving few states unaffected. Infection rates are rising at an alarming rate, doubling every year since introduction, with more than 9800 cases in 2003 alone and more than 260 deaths. Patients may present with myriad symptoms including a maculopapular rash that affects the face and trunk and diffuse lymphadenopathy, both of which may result in the initial consultation of the otolaryngologist. We review the clinical history of West Nile virus and its epidemio

Congenital tracheal stenosis. The otolaryngologist's perspective.

Congenital tracheal stenosis is a rare congenital anomaly, with less than 70 reported cases in the literature. The presenting signs and symptoms of stridor, recurrent pneumonia, and respiratory distress are commonly seen in other conditions. The rarity of congenital tracheal stenosis and the diverse presentations make accurate early diagnosis difficult and frequently lead to inappropriate treatment. We treated three patients with congenital tracheal stenosis who presented with different sites of stenosis. Each patient displayed different symptoms and required individualized management. The tre

Are Otolaryngologists Seeing More Cough? Longitudinal Trends and Patterns.

OBJECTIVE: Otolaryngologists play an increasing role in managing cough, but little data exists examining the demographics of this patient population and the referral patterns that influence their access to care. This study sought to elucidate these factors using a longitudinal, nationwide database to minimize sampling bias and identify trends representative of the national population. STUDY DESIGN: Nationally representative survey. SETTING: National Ambulatory Medical Care Survey (NAMCS). METHODS: Visits with a diagnosis and chief complaint of cough between 2005 and 2019 in NAMCS were exami

What should otolaryngologists know about dural venous sinus stenting?

Dural venous sinus stenting is an emerging and exciting area in otolaryngology in collaboration with neurosurgeons and neuroradiologists. The first cases were reported 20 years ago. It is now considered part of the routine treatment of increased intracranial pressure due to transverse sinus stenosis. ENT doctors are the first to see these patients in their clinics, as sinus headaches, pulsating tinnitus, and dizziness are the most common symptoms. Previously, with limited success, high-dose diuretics and intracranial shunts had been the only options for treating these patients. Other methods,

Inter-Rater Variability of Reflux Finding Score Amongst Otolaryngologists.

BACKGROUND: Laryngopharyngeal reflux (LPR) is an extra-oesophageal variant of gastro-oesophgeal reflux disease. Patients often do not present with the classic reflux symptoms of heartburn or regurgitation. Accurate diagnosis of LPR can be challenging. The reflux finding score (RFS) is system based on the assessment of eight parameters seen on fibre optic laryngoscopy, used to determine the presence and the severity of laryngopharyngeal reflux (LPR). Scoring the RFS is subjective and highly dependent on the examiner's eye and experience. In this study, we investigated the inter-rater reliabilit

Interventions for Unhealthy Eating Patterns of Resident Otolaryngologists on Call.

OBJECTIVES: Resident physicians commonly experience elevated stress levels and disrupted sleep while on call. Many also report intense cravings for comfort food, typically high in fat, sugar, and salt. The objective of this pilot study was to explore the physiological and psychological factors contributing to the desire for unhealthy food among US otolaryngology residents on call, and to present strategies for reducing these behaviors. The hypothesis was that the introduction of a simple behavioral intervention would be associated with reduced unhealthy food consumption. METHODS: Twelve otola
