Ototoxicity (cochleotoxicity) classifications: A review.

OBJECTIVE: Drug-mediated ototoxicity, specifically cochleotoxicity, is a concern for patients receiving medications for the treatment of serious illness. A number of classification schemes exist, most of which are based on pure-tone audiometry, in order to assist non-audiological/non-otological specialists in the identification and monitoring of iatrogenic hearing loss. This review identifies the primary classification systems used in cochleototoxicity monitoring. By bringing together classifications published in discipline-specific literature, the paper aims to increase awareness of their rel

[Reduced nutritional status enhances ototoxicity].

BACKGROUND: This study investigates whether the nutritional state of guinea pigs is a risk factor for the ototoxic side effects of cisplatin, gentamicin, and gentamicin in combination with ethacrynic acid. METHODS: A normal nutritional state was maintained with a standard 18.5% protein diet while nutritional deficiency was produced by feeding a 7% protein diet. Hearing loss was measured by auditory evoked brainstem responses. RESULTS: Guinea pigs on the low protein diet had a significantly higher drug-induced hearing loss. Cisplatin-induced hearing loss was 32 dB in undernourished animals bu

Quantitative assessment of torasemide ototoxicity.

Torasemide (1-isopropyl-3- ([4-(3-methyl-phenylamino)pyridine]-3-sulfonyl)urea), a new loop diuretic, was quantitatively tested for ototoxicity in cats. The toxic dose that causes a defined hearing loss in 50% of the animals (TD50) was determined. TD50 was calculated as 20.8 mg/kg. This is slightly but not significantly above the value for furosemide (18.37 mg/kg). Hearing function tended to recover after the acute effect. The main metabolite in man (1-isopropyl-3-([(3-carboxy-anilino)-3-pyridyl]sulfonyl)urea, M5) showed no ototoxic action even in excessive doses. To ensure complete recovery,

Chloramphenicol ototoxicity. A case report.

A case of bilateral profound sensori-neural hearing loss following systemic administration of chloramphenicol is reported because of its rarity. The hearing loss was gradually progressive, involving one ear; and with a repeat course of the drug, the other ear too was involved. An idiosyncratic or allergic response to previous exposure to the drug may be the cause of the toxicity. One should be careful in prescribing the drug repeatedly; when this is done, the patient must be carefully observed for possible toxic effects.

Ototoxicity and noise-drug interaction.

Diverse therapeutic agents, such as certain antibiotics, diuretics, and analgesics can cause functional and morphological damage to the inner ear. Noise present in recreational or occupational environment may also be potentially damaging to the auditory system when combined with some ototoxic therapeutic agents. This article documents recent research and controversies about the site and mechanism of action of major ototoxic drugs, noise-drug interaction, and possible prophylaxis against such ototoxicity.

What's New in Ototoxicity Management?

PURPOSE: Ototoxic medications and chemical agents in the workplace can put individuals' hearing and vestibular health at risk for permanent injury. Proactive ototoxicity management (OtoM) strategies aim to minimize exposure, avoid onset of symptoms, provide ongoing monitoring, and manage auditory and vestibular changes as the clinical needs of the patient evolve. During a 2021 American Speech-Language-Hearing Association Special Interest Groups Open House, members of the International Ototoxicity Management Group discussed how best to integrate OtoM into routine clinical practice, what tools t

Genetic susceptibility to aminoglycoside ototoxicity.

INTRODUCTION: Aminoglycosides are a well-known clinically relevant antibiotic family used to treat bacterial infections in humans and animals and can produce toxic side effects. Aminoglycoside-induced hearing loss (HL) has been shown to have a genetic susceptibility. Mitochondrial DNA mutations have been implicated in inherited and acquired hearing impairment. OBJECTIVE: Literature review of genetic mutations associated with aminoglycoside-induced ototoxicity. METHODS: PubMed was accessed from 1993 to 2017 using the search terms "aminoglycoside, genetic, ototoxicity, hearing loss". Exclusion

Cellular mechanisms of aminoglycoside ototoxicity.

PURPOSE OF REVIEW: To summarize advances in the study of the interaction between sensory hair cells and aminoglycoside antibiotics. RECENT FINDINGS: Aminoglycosides enter hair cells through mechanotransduction channels and initiate an active signaling pathway that leads to cell death. Early expression of heat shock proteins can protect hair cells from aminoglycosides, although signaling from surrounding supporting cells appears to promote hair cell death. Studies of certain human deafness mutations have revealed new insights into the role of mitochondria in aminoglycoside ototoxicity. SUMMAR

Some historical aspects of ototoxicity.

From a historical standpoint, the problems of ototoxicity came in two major stages. The first, in the nineteenth century, indirectly resulted from the chemical extraction, purification and, sometimes later, synthesis of the active components of traditional drugs, which led to their use in larger doses. The second stage, in the twentieth century, derived from the drive to find more and more effective antibacterial agents, which often had damaging side effects on the inner ear. These effects were highlighted by the development of more sophisticated techniques to detect and measure the ototoxic a

[Ototoxicity of spectinomycin (author's transl)].

Spectinomycin is an aminocyclitol antibiotic, but different in chemical structure from other aminocyclitol antibiotics such as streptomycin, kanamycin and gentamicin. Spectinomycin clinically has been proven to be highly effective in the intramuscular single-dose treatment of gonorrhea. According to NOVAK et al. (1974), all evidence from pharmacological and clinical efficacy studies to date indicates that spectinomycin is free from ototoxic effects at the dose levels recommended for single-dose treatment of gonorrhea (2 or 4 g) or repeated i.m. or i.v. doses, 8g/day. The present experimental s

Identifying targets to prevent aminoglycoside ototoxicity.

Aminoglycosides are potent antibiotics that are commonly prescribed worldwide. Their use carries significant risks of ototoxicity by directly causing inner ear hair cell degeneration. Despite their ototoxic side effects, there are currently no approved antidotes. Here we review recent advances in our understanding of aminoglycoside ototoxicity, mechanisms of drug transport, and promising sites for intervention to prevent ototoxicity.

Ototoxicity in Nigeria: why it persists.

No therapy is currently available to reverse the serious damage that can be caused by ototoxic drugs, such as permanent hearing loss and balance disorders. Otolaryngologists in various regions of the world have developed strategies aimed at curtailing drug-induced ototoxicity, but similar efforts in most developing nations have yet to be well established. We conducted a study to document our experience in Nigeria. Our study population was made up of 156 patients-66 males and 90 females, aged 5 to 85 years (mean: 32.1 ± 30.7)-who were diagnosed with drug-induced ototoxicity over a 3-year period

Memantine's action against aminoglycoside-induced ototoxicity.

The objectives of this study were (1) to assess the protective role of NMDA antagonists against the ototoxic effects of aminoglycosides, (2) to provide any possible evidence between ototoxicity due to aminoglycosides and excitotoxicity. An animal experiment was conducted. Twenty-eight, 3-month-old female New Zealand rabbits, weighing 1,000-1,500 g, were studied prospectively for 28 days after intramuscular administration of amikacin (15 mg/kg/day divided into two equal doses) for 14 days. Twenty-one rabbits were categorized into three equal treatment groups and seven animals received no medica
