Characteristics of a dengue outbreak in a remote pacific island chain--Republic of The Marshall Islands, 2011-2012.

Dengue is a potentially fatal acute febrile illness caused by four mosquito-transmitted dengue viruses (DENV-1-4). Although dengue outbreaks regularly occur in many regions of the Pacific, little is known about dengue in the Republic of the Marshall Islands (RMI). To better understand dengue in RMI, we investigated an explosive outbreak that began in October 2011. Suspected cases were reported to the Ministry of Health, serum specimens were tested with a dengue rapid diagnostic test (RDT), and confirmatory testing was performed using RT-PCR and IgM ELISA. Laboratory-positive cases were defined

An Integrated Academic Oncology Ecosystem for Hawai'i and the U.S.-Affiliated Pacific Islands.

Background: Delivering comprehensive cancer prevention, diagnosis, and treatment across Hawai'i and the U.S.-Affiliated Pacific Islands (USAPI) is constrained by geographic isolation, oncology workforce shortages, and persistent cancer inequities. Objectives: The University of Hawai'i Cancer Center, the state's only National Cancer Institute-designated cancer center, partners with community healthcare systems to address cancer health disparities. Here, we describe an implementation-focused strategy initiated in December 2024 that is designed to improve equitable access to evidence-based oncolo

Pacific Islands Families Study: adverse impact of food insecurity on child body composition.

AIM: COVID-19 has exacerbated food insecurity, unemployment, inequities and poverty in Aotearoa. Here, we tested the hypothesis that exposure to malnutrition due to household food insecurity during foetal life and early infancy is associated with body composition in adolescence. METHODS: As part of the Pacific Islands Families Study, 1,376 Pacific Island mothers were asked questions about food security at six weeks postpartum in the year 2000. At age 14 years, 931 youth completed in-school assessments of height and weight. Of these youth, 10 girls and 10 boys from each weight decile were rand

Breast, cervical, and colorectal cancer screening prevalence in the US-Affiliated Pacific Islands.

BACKGROUND: Breast, cervical, and colorectal cancer screening are recommended and can reduce mortality from these cancers, yet information on screening prevalence in the US-Affiliated Pacific Islands (USAPI) is limited. METHODS: We analyzed data from population-based cross-sectional surveys undertaken at different time points from 2016 to 2019 in American Samoa, the Commonwealth of the Northern Mariana Islands, the Republic of Palau, the Republic of the Marshall Islands, and the Federated States of Micronesia states of Pohnpei and Kosrae. We estimated the age-standardized percentage of never

Congenital abnormalities in Pacific Island Countries and Territories: a scoping review.

BACKGROUND: Pacific Island Countries and Territories experience disproportionately high rates of stillbirth, neonatal, and childhood deaths, yet the drivers of these outcomes remain unclear. The impact of congenital abnormalities within the Pacific has not been clearly established, despite these abnormalities being a leading global cause of infant and child mortality. This scoping review maps available literature to determine the current knowledge of congenital abnormalities in Pacific Island Countries and Territories. METHODS: Two independent reviewers searched six scholarly databases (PubMe

High resilience of Pacific Island forests to a category- 5 cyclone.

Assessing how forests respond to, and recuperate from, cyclones is critical to understanding forest dynamics and planning for the impacts of climate change. Projected increases in the intensity and frequency of severe cyclones can threaten both forests and forest-dependent communities. The Pacific Islands are subject to frequent low-intensity cyclones, but there is little information on the effects of high intensity cyclones, or on how forest stewardship practices may affect outcomes. We assess the resistance and resilience of forests in three community-stewarded sites on the island of Tanna,

Higher Heart Weight in New Zealand Māori and Pacific Islanders.

Heart weight is dependent on sex, age, height, and weight. Although previous autopsy studies showed no differences in heart weight between different ethnic groups, none have examined the New Zealand population of Māori and Pacific Islanders (Polynesians). The presented study compared heart weights between 101 European and 85 Polynesian suicide hanging deaths from New Zealand. Univariate linear regression coefficients for age, male sex, height, body weight, body mass index, and Polynesian ethnicity were positive and significant (P < 0.05). Apart from body mass index, subsequent multivariate ana

The Characterization of Violent Deaths Among Asian and Pacific Islander Americans.

INTRODUCTION: Health disparities in the Asian and Pacific Islander Americans (APIAs) community have not been well described, unlike non-Hispanic Black and Hispanic communities. However, there has been a rise in violence against the APIA community. This study explores and characterizes violent death by incident (e.g., homicide, suicide), weapon (e.g., firearm, strangulation), and location types among APIAs as they compare with other racial or ethnic groups. METHODS: We used the National Violent Death Reporting System from 2003 to 2018 to characterize violent deaths among APIA and compared them

Asian American and Pacific Islander substance use treatment completion.

Mental health professionals and administrators work to improve substance use treatment for racially and ethnically diverse populations. However, a national comparison study of Asian Americans and Pacific Islanders (AAPIs) and non-AAPIs substance use outpatient treatment completion has received little attention. The present study examined treatment completion by comparing AAPI clients to White and Latinx clients, and identified demographic and treatment-related moderating factors associated with treatment completion using discharge data from the Treatment Episode Data Sets-Discharge (TEDS-D). Late presentation of diaphragmatic hernia in a Pacific Island pediatric population.

BACKGROUND: Congenital diaphragmatic hernia (CDH) usually presents at birth with respiratory distress syndrome (RDS) and has a high mortality rate if not promptly recognized and treated. The incidence of CDH is reported to be 0.8 - 1.0/10,000 registered births. Less than 3% present after the neonatal period. In the latter, ie. late-presenting CDH (L-pCDH), the prognosis is improved because pulmonary hypoplasia does not develop. With the creation and application of a store-and-forward telemedicine system to the Pacific Island Health Care Project (PIHCP), we have come to realize that this rare c

Pacific Islands families study: the prevalence of chronic middle ear disease in 2-year-old Pacific children living in New Zealand.

OBJECTIVE: The aim of this study was to determine the prevalence of chronic middle ear disease in 2-year-old Pacific children living in New Zealand. METHODS: The data were gathered as part of the Pacific Islands Families: First Two Years of Life (PIF) Study in which 1001 two year olds were screened using tympanometry to identify children at risk for otitis media with effusion (OME) and other otological disorders. Those failing the screening tests were referred to Manukau Superclinic where they had a further tympanometry test, and if they failed this test, were followed up with a comprehensive

Vaccination Coverage Among Children Aged 2 Years - U.S. Affiliated Pacific Islands, April-October, 2016.

Vaccine-preventable diseases (VPDs) cause substantial morbidity and mortality in the United States Affiliated Pacific Islands (USAPI).* CDC collaborates with USAPI immunization programs to monitor vaccination coverage. In 2016, † USAPI immunization programs and CDC piloted a method for estimating up-to-date status among children aged 2 years using medical record abstraction to ascertain regional vaccination coverage. This was the first concurrent assessment of childhood vaccination coverage across five USAPI jurisdictions (American Samoa; Chuuk State, Federated States of Micronesia [FSM]; Comm

Diseases of livestock in the Pacific Islands region: setting priorities for food animal biosecurity.

Most Pacific Island countries and territories (PICTs) have developing economies and face a critical shortage of veterinarians with limited financial resources allocated to their animal disease surveillance programmes. Thus, animal health authorities have to set priorities for better focusing their scarce resources. The main objective of this study was to identify animal diseases perceived to be of importance by decision makers within selected PICTs, at the regional and national levels, to ensure better targeting of animal health resources. A second objective was to investigate whether the targ

Improvement of leptospirosis surveillance in remote Pacific islands using serum spotted on filter paper.

OBJECTIVES: Leptospirosis is a serious neglected disease in the Pacific. Because sensitive and specific laboratory tests are largely unavailable, the burden of disease and epidemiological data are often unreliable and do not allow informed disease prioritization and efficient control. We report the use of serum spotted on filter paper to improve the surveillance of leptospirosis in remote and resource-limited settings. METHODS: A total of 172 acute-phase serum samples collected from patients with suspected dengue at Yap State Hospital, Federated States of Micronesia, were spotted on filter pa

Larval development and osteology of Callanthias platei (Teleostei: Callanthiidae) from Desventuradas Islands, South Pacific.

Larvae of the Juan Fernández Splendid Perch, Callanthias platei Steindachner, 1898 (Teleostei: Callanthiidae), are described for the first time. The five available specimens, ranging from 3 mm notochord length (NL) to 6.1 mm standard length (SL), were collected with plankton nets near Desventuradas Islands, Southeast Pacific, during the austral spring of 2015 and 2016. The larvae are deep bodied with a narrow caudal peduncle, a large head and a triangular, coiled gut. The pigmentation is scarce in preflexion larvae but increases after flexion. All individuals have several groups of head spines

Gill monogenean communities (Platyhelminthes, Monogenea, Dactylogyridae) of butterflyfishes from tropical Indo-West Pacific Islands.

We studied the monogenean communities of 34 species of butterflyfish from the tropical Indo-West Pacific, identifying 13 dactylogyrid species (including two species that are presently undescribed). Monogenean assemblages differed significantly between host species in terms of taxonomic structure, intensity and prevalence. Parasite richness ranged from 0 (Chaetodon lunulatus) to 11 (C. auriga, C. citrinellus and C. lunula). Host specificity varied between the dactylogyrids species, being found on 2-29 of the 34 chaetodontid species examined. Sympatric butterflyfish species were typically parasi

The launch of the Pacific vector network: connecting Pacific Island Countries and areas to prevent and control vector-borne diseases.

The Pacific Island Health Officers' Association, the World Health Organization, and the Pacific Community co-organized the launch of the Pacific Vector Network (PVN) to address challenges posed by mosquito-borne diseases, including dengue fever, Zika virus disease, chikungunya, malaria, and lymphatic filariasis. The PVN was created as a new initiative under the Pacific Public Health Surveillance Network (PPHSN). This launch was a critical step in the build-up to PVN as a full-service network of PPHSN in the coming years. The Pacific Island Countries and areas (PIC)-led network comprises vector
