Preterm infants often have respiratory distress requiring non-invasive respiratory support. Those born with a cleft lip and palate pose significant challenges in maintaining positive pressure due to anatomic differences that can cause substantial air leak when using typical mask/cannula interfaces. This leads to difficulties with ventilation and oxygenation for these infants whose lung disease would otherwise not necessitate invasive ventilation. The purpose of this case report is to provide a multidisciplinary approach to a preterm infant with a unilateral cleft lip and palate for innovative

BACKGROUND: The authors' aim is to show, through a morphological analysis, how the use of a passive functional plate in hard acrylic resin can guide the growth of the maxillary segments in babies born with cleft lip and palate.
METHODS: The authors have obtained, with a scanner gray scales, bidimensional monochromatic images of nine babies' dental casts with cleft lip and palate at birth and at the time of the first cheilo-rhinoplastic surgery. A graphic analysis on these images was made comparing the charts drawn by Bolter's points identified on the maxillary alveolar arch.
RESULTS: The ove

Cone beam computed tomography (CBCT) technology is increasingly utilized in the head and neck region and is valuable in treatment planning for cleft palate patients, potentially enabling the creation of 3D-printed obturators to assist with feeding and speech. This technical report investigates the feasibility of using data from a 360-degree CBCT scan to accurately produce a cleft palate obturator and assesses whether a lower-dose 180-degree CBCT scan can achieve a comparable result. A simulated cleft palate was crafted on a dehydrated human skull, which was then scanned using both 360-degree a

OBJECTIVE: Severe nasopharyngeal stenosis after uvulopalatopharyngoplasty (UPPP) is a dreaded complication. Very little has been written about successful treatment because attempts at correction frequently fail. Severe stenosis of the nasopharynx drastically worsens speech, swallowing, and obstructive sleep apnea (OSA). We present our successful experience with severe stenosis focusing on a simple functional repair using topical mitomycin-c as well as differing daytime and nighttime palatal obturators.
METHODS: Three adults with total or near-total nasopharyngeal stenosis secondary to UPPP we

BACKGROUND: Soft palate defects created during oral cancer surgery may prevent complete palatal closure and trigger palatopharyngeal insufficiency. One current treatment employs a rigid obturator prosthesis; an extension of acrylic resin at the level of the hard palate ensures surface contact with the remaining musculature. Unfortunately, airflow escape often causes hypernasality, compromises speech intelligibility, and creates swallowing problems (including leakage of food and fluid into the nasal airway). We plan to test a new removable denture featuring a thick dental dam that serves as a m

Defects of the soft palate resulting from head and neck oncologic surgery traditionally have been rehabilitated using a prosthetic obturator. In general, the results of soft palate obturation have been satisfactory, with most patients achieving velopharyngeal competence during speech and swallowing. However, soft palate obturators have several disadvantages. Device stability and retention are impaired in patients who are edentulous. In addition, prosthetic obturators cover large areas of sensate mucosa in the hard palate, maxillary alveolus, and oropharynx, contributing significantly to the lo

Large defects of the hard palate can result from a congenital abnormality, trauma, or surgical treatment of oral masses. Closure of these defects can be challenging as dehiscence is common. Large, inoperable hard palate defects may be managed with prosthetic obturators, which recreate the separation between the oral and nasal cavities. This report describes the use of a palatal obturator to manage a large palatal defect that persisted after resection of a palatal fibrosarcoma. Three prior attempts at surgical closure of the defect had failed.

PURPOSE: Speech outcome measurements are valuable in guiding treatment and determining the effectiveness of rehabilitation with a maxillary obturator prosthesis in individuals with palatal resection. Although speech outcome data exist in the literature for such patients, relatively few reports have used clinical tools designed to measure the acoustic, physiologic, and perceptual bases of speech. This investigation reports these measures for individuals rehabilitated with a maxillary obturator.
MATERIALS AND METHODS: Speech measurements were collected prospectively at three clinical visit time

The aim of the study was to investigate the microbial colonization (by Candida species, anaerobic and facultative anaerobic bacteria) of maxillary obturators used for the restoration of maxillary defects, including during radiotherapy. Retrospective cohort study. Fifteen patients requiring a maxillary obturator prosthesis had swabs of their obturators and adjacent tissues taken at different stages of their treatment over a period of 8 years. Identification of microbial species from the swabs was carried out using randomly amplified polymorphic DNA polymerase chain reaction (RAPD PCR) analysis,

BACKGROUND: Although prosthetic obturation is the "gold standard" for restoration of hard-palate defects, obturators can be problematic. We present 10 cases of palatal reconstruction with the radial forearm free flap and compare patient satisfaction with defect-matched patients rehabilitated with prosthetic obturation.
METHODS: Twelve patients who underwent radial forearm free flap (RFFF) reconstruction of a hard-palate defect and eight patients, with similar-sized defects who were rehabilitated with a prosthetic obturator, were evaluated for donor site and recipient site complications, diet,

Coronavirus disease has taken a toll on everyone's lives either physically or emotionally throughout the world. One of the most serious complications occurring currently in covid-positive and recovered patients is the deadly invasive mucormycosis. There are five clinical types of mucormycosis, out of which rhino-orbito-cerebral is the most common form (44%-49%). Management includes early diagnosis, control of the underlying diseases, and aggressive medical and surgical intervention. In patients undergoing maxillectomies, the use of immediate surgical obturators has become the gold standard. Th

Large clefts in the lip and palate are common congenital anomalies. If the cleft palate is large enough, conventional feeding techniques may not provide proper nutrition for the infant. Feeding obturators will aid in the ability of the infant to attain suction and help the infant to feed adequately. It is necessary for the infant to have sustained weight gain prior to surgery to correct the cleft lip and/or palate. Fabrication of an infant feeding obturator is a simple technique using materials found in every dental office. An impression is made using modeling plastic impression compound. This

Clefts of the lip and palate are congenital anomalies that affect the orofacial region. Completely edentulous patients with a cleft palate present a unique challenge during complete denture fabrication. Complete dentures with an obturator and palatal extension need to be accurately fabricated to fulfill the functions of esthetics, speech, deglutition, and mastication. The case report presents the successful prosthodontic rehabilitation of a cleft palate female patient using a complete denture with a palatal obturator and pharyngeal extension. The palatal obturator extension provided good reten

OBJECTIVE: The virtual cone beam computed tomography-derived 3-dimensional model was compared with the scanned conventional model used in the fabrication of a palatal obturator for a patient with a large palatal defect.
DESIGN: A digitally derived 3-dimensional maxillary model incorporating the palatal defect was generated from the patient's existing cone beam computerized tomography data and compared with the scanned cast from the conventional impression for linear dimensions, area, and volume. The digitally derived cast was 3-dimensionally printed and the obturator fabricated using traditio

Intranasal cocaine abuse may cause significant local ischaemic necrosis and destruction of the nasal and midfacial bones and soft tissue, leading to development of a cocaine-induced midline destructive lesion. Review of the English-language literature reveals only a few case reports describing hard and/or soft palatal perforation related to cocaine inhalation. To date, among the reconstructive techniques of the palate, different surgical options have been reported such as local, regional and free flaps. Common prosthetic obturators have also been used. Presented here are six cases of cocaine a

Defects confined to the hard palate can be managed with relative ease because it is a static shelf creating oro- nasal separation. Since the soft palate is a dynamic separator between the oral and nasal cavities, defects involving the soft palate require careful consideration. Instrumental visualization methods can aid the fabrication of obturators in patients with such defects and prevent problems of under or over obturation. This case report presents the prosthodontic management of a patient with a Veau class II cleft palate using a palatopharyngeal obturator in order to re-establish normal

Conventional obturator prostheses might cause dissatisfaction in patients with hard or soft palate defects due to inadequate retention and function during speech, mastication, and swallowing. Thus, surgical reconstruction and implant-supported obturators are considered as alternative treatments for these patients. This case-report study describes the prosthetic reconstruction of an 88-year-old patient suffering from a hard palate defect after surgical resection of verrucous carcinoma in the left side of the hard palate. Fifteen months after radiotherapy, two implants were placed in the right s

Background: Cleft lip and palate require complex treatment, often involving early surgery. Postoperative complications, such as palatal fistulas, can impair speech and feeding. While surgical correction is standard, large fistulas may pose challenges due to age, cost, and recurrence risks. Obturator prostheses provide a nonsurgical alternative, but digital impression techniques for their fabrication are underutilized. This case report explores intraoral digital impressions for creating obturator/speech aid appliances in a patient with cleft palate deformities. Methods: A 17-year-old female wit
