Jaundice and chronic pancreatitis.

A study was conducted on a group of 48 patients presenting with jaundice during the course of chronic pancreatitis. Two different patterns of symptoms could be opposed: (1) Short-lasting cholestatic jaundice which is accompanied by pain, but free from pruritus and fever, is linked to a medium consumption of alcohol, occurs in the course of advanced chronic pancreatitis and seems to be the consequence of a stricture of the main bile duct due to pancreatic lesions. (2) Protracted, intense jaundice which is linked to a high alcohol consumption, is encountered among less-advanced cases of chronic

Recurrent acute pancreatitis: a harbinger for irreversible chronic pancreatitis.

Recurrent acute pancreatitis is beginning to be recognized as an intermediary stage in the continuous spectrum between acute and chronic pancreatitis. It is crucial to identify this disease stage and intervene with diagnostic and therapeutic modalities to prevent the painful and irreversible condition of chronic pancreatitis. We review the recent advances in diagnosing and managing this important 'call for action' condition.

Metabolic characteristics of acute necrotizing pancreatitis and chronic pancreatitis.

The etiology and pathogenesis of pancreatitis remains unclear. In the presence of pancreatic inflammation, metabolite abnormalities appear before transformation of tissue structure and changes in functions occur. Detection of abnormalities in metabolite levels facilitates a greater understanding of the pathophysiological events and aids in the early diagnosis of the disease. In this study, metabolic profiles from the pancreas of Wistar rats were examined using high-resolution proton magic angle spinning nuclear magnetic resonance (MAS NMR) spectroscopy to investigate the metabolite indicator(s

[From the chronic pancreatitis to chronic pancreatites].

Chronic alcohol intake accounts for 60-90% of the cases of chronic pancreatitis, but other etiologies have been recognized and described in the very recent years. Genetic causes include mutations of the cationic trypsinogen gene PRSS1 (100 families in France), of its inhibitor SPINK1 and of the CFTR gene involved in cystic fibrosis. Auto-immune pancreatitis is often part of an "IgG4-related systemic disease" involving the biliary tract, the salivary glands, the retroperitoneum and/or the kidneys. Diagnostic criteria are now well-defined (HISORt of the Mayo Clinic), with ductal and parenchymal

Pancreatic resection for chronic pancreatitis.

Patients with chronic pancreatitis needing operative management include those with severe pain, those with complications of pancreatitis, or those in whom it is not possible to distinguish cancer of the pancreas from chronic pancreatitis. The use of endoscopic retrograde cholangiopancreatography, CT, and angiography to define the structural abnormalities has increased the surgeon's ability to select an operation matched to the patient's needs. A longitudinal pancreaticojejunostomy should be performed in patients whose ducts are dilated. When the head of the pancreas is enlarged and thickened,

Pancreatic Endotherapy for Chronic Pancreatitis.

Extracorporeal shock wave lithotripsy is recommended as the first-line therapy for large (>5-mm) obstructive pancreatic ductal stones. Dominant pancreatic duct strictures should be initially managed with a wide-bore single plastic stent with 3 monthly exchanges for a year, even in asymptomatic patients. Recent studies have evaluated multiple plastic and self-expanding covered metal stents for refractory pancreatic ductal stricture. Pancreatic pseudocysts should be treated endoscopically with or without endoscopic ultrasound guidance.

Pancreatic autotransplantation in chronic pancreatitis.

The apancreatic state secondary to resective surgery for chronic pancreatitis is associated with a high rate of late morbidity and mortality that is due, in part, to endocrine insufficiency. Resective procedures should, therefore, be used very selectively. Over the last 2 decades we have seen a shift from extensive distal resections to limited proximal resections. This is because of the lowering of the operative mortality of pancreatic head resection and its better results in pain relief, while preserving in situ the body and tail of the gland with its metabolic functions. Islet autotransplant

[Pancreatic carcinoma in chronic pancreatitis].

Chronic pancreatitis and carcinoma of the pancreas are being diagnosed with increasing frequency throughout the world. When both occur together, the question of their causal relationship arises. Secondary chronic pancreatitis following carcinoma of the pancreas is relatively frequent and can be proven histologically in at least 10% of pancreatic cancers. How often primary chronic pancreatitis develops into carcinoma is controversial. So far, there are only a few prospective clinical studies of chronic pancreatitis which cover this problem. We have followed 146 cases of chronic pancreatitis for

[Development of chronic pancreatitis from acute pancreatitis: a pathogenetic concept].

Clinically, alcohol-induced chronic pancreatitis is commonly indistinguishable from acute pancreatitis. A relationship between these two forms of pancreatitis has therefore been suggested. This article presents a pathogenetic model which views the development and course of chronic pancreatitis as a consequence of severe acute pancreatitis.

[Endoscopic Management of Chronic Pancreatitis].

Chronic pancreatitis (CP) is a progressive fibroinflammatory disease characterized by pancreatic ductal obstruction, calcification, and fibrosis, leading to chronic pain and exocrine or endocrine insufficiency. Endoscopic management plays a central role in selected patients with painful obstructive CP, providing ductal decompression and drainage while avoiding the morbidity of surgery. Endoscopic retrograde cholangiopancreatography with pancreatic sphincterotomy, stenting, and stone extraction is the mainstay of therapy. Extracorporeal shock wave lithotripsy or pancreatoscopy-guided lithotrips

Chronic Idiopathic Pancreatitis and Pancreaticolithiasis.

Chronic pancreatitis (CP) is an inflammatory syndrome that causes irreversible damage and loss of pancreatic function. A late complication is pancreatolithiasis, which exacerbates pain due to intraductal hypertension. In this work, we demonstrate that given the availability of different non-surgical treatment methods, if we are in the presence of dilated ducts, longitudinal pancreaticojejunostomy (Puestow procedure) is the best surgical drainage option. The clinical study is presented through the case of a 56-year-old man with idiopathic CP and recurrent lithiasis. The diagnosis was confirmed

16. Pain in chronic pancreatitis.

INTRODUCTION: Chronic pancreatitis is defined as a disease of the pancreas in which recurrent inflammatory episodes result in replacement of the pancreatic parenchyma by fibrous connective tissue in individuals with genetic, environmental, and other risk factors. Pain is one of the most important symptoms of chronic pancreatitis and, in many cases, has chronic visceral nociceptive, nociplastic, and even neuropathic components, with evidence of both central and peripheral sensitization, neuroplasticity, and neurogenic inflammation. METHODS: The literature on the diagnosis and treatment of pain

Pain management in chronic pancreatitis.

Pain in chronic pancreatitis (CP) is difficult to manage. Many patients suffer from inadequate pain relief, completely incapacitating them in their daily activities. Historically, despite their well-known adverse effects, opioids have been the pillar of treatment regimens in painful CP. The management is now gradually evolving with a better understanding of the underlying pathophysiology of CP-related pain. Clinicians should follow a holistic approach to the management of CP-associated pain, which must involve lifestyle changes that are coupled with analgesic medications and other pain-relievi

Optimizing nutrition in chronic pancreatitis.

PURPOSE OF REVIEW: This review aims to discuss recent developments in the nutritional management in chronic pancreatitis. RECENT FINDINGS: Nutritional assessment should be comprehensive and include dietary history, anthropometry, and biochemical nutritional parameters. Micronutrients should be evaluated at least yearly and dual-energy X-ray absorptiometry (DEXA) at every 2-yearly intervals. Studies on pancreatic enzyme replacement therapy (PERT) have primarily evaluated coefficient of fat excretion (CFA), coefficient of nitrogen excretion (CNA), and stool weight. Two RCTs, in which patients w

Endoscopic management of chronic pancreatitis.

Chronic pancreatitis (CP) is an inflammatory process characterized by irreversible morphological changes in the pancreas. Pain is the predominant symptom observed during the course of CP. The etiopathogenesis of pain in CP is multifactorial and includes ductal hypertension due to obstruction of the pancreatic duct (PD), neuropathic causes, and extrapancreatic complications of CP like pseudocyst and distal biliary obstruction. A sizeable proportion of patients with CP are amenable to endoscopic treatment. The mainstay of endotherapy includes decompression of PD with one or more plastic stents i

Chronic Pancreatitis: Diagnosis and Treatment.

Chronic pancreatitis is an irreversible and progressive disorder of the pancreas characterized by inflammation, fibrosis, and scarring. Exocrine and endocrine functions are lost, often leading to chronic pain. The etiology is multifactorial, although alcoholism is the most significant risk factor in adults. The average age at diagnosis is 35 to 55 years. If chronic pancreatitis is suspected, contrast-enhanced computed tomography is the best imaging modality for diagnosis. Computed tomography may be inconclusive in early stages of the disease, so other modalities such as magnetic resonance imag

Endoscopic therapy for chronic pancreatitis.

PURPOSE OF REVIEW: To summarize recent data on techniques, efficacy and complications of endoscopic management of chronic pancreatitis. RECENT FINDINGS: Extracorporeal shock wave lithotripsy with or without endoscopic retrograde cholangiopancreatography is the first-line treatment for large painful obstructive pancreatic duct calculi. Use of preextracorporeal shock wave lithotripsy secretin could result in better stone clearance. The first-line treatment for dominant pancreatic duct strictures is placement of a single 10-Fr polyethylene stent with planned exchanges every 3 months until 1 year
