[Parapharyngeal space lesions: with a reference to biopsy].

We received seven cases of parapharyngeal space lesions and discussed on necessity of biopsy. Tumors originating in the parapharyngeal space were represented by parotid pleomorphic adenomas which showed bulging of soft palate and tonsillar fossa, and benign neurogenic tumors which showed bulging of the lateral wall of the meso-pharynx. In these cases, preoperative biopsy was not necessary unless the lesion was suspected, of malignancy by CT, MRI and so on. In the case of bulging of soft palate and tonsillar fossa, if CT or MRI finding shows that the tumor is localized in the median portion of

Transcervical approach to benign parapharyngeal space tumors.

OBJECTIVES: We describe our experience with the postoperative sequelae, complications, and recurrences associated with resection of a series of parapharyngeal space (PPS) tumors via a transcervical approach without submandibular gland excision, parotidectomy, or mandibulotomy. METHODS: We performed a retrospective review of 51 cases, 40 of which were pleomorphic adenomas and 11 of which were lipomas or schwannomas. RESULTS: Of 30 fine-needle aspirations performed, 24 indicated pleomorphic adenoma before operation. Twenty-eight of the 30 fine-needle aspirations yielded diagnoses that were con

Intraoral sonographic evaluation of parapharyngeal space tumors.

We performed transcutaneous (external) sonography and then intraoral sonography to evaluate 3 parapharyngeal space tumors (1 vagal paraganglioma, 1 pleomorphic adenoma, and 1 carcinoma arising in a pleomorphic adenoma). All 3 patients had medial displacement of a tonsil. In all cases, we could measure the size and see the borders of the tumors and their relation to other structures on intraoral sonography. Intraoral sonography is useful for differentiating between unilaterally enlarged and medially displaced tonsils.

[Measurement of parapharyngeal space using CT images].

Parapharyngeal space can be defined as a potential space surrounded by deglutitional and masticator muscles and their covering, superficial and middle layer of deep cervical fascia. It is easily recognized in images provided by modern scanning devices, such as CT and MRI. Parapharyngeal space has traditionally been divided by styloid process and fascia of tensor veli palatini muscle (nasopharyngeal level) or fascia of stylopharyngeus muscle (oropharyngeal level) into two compartments, prestyloid and postsyloid spaces. The latter is often called as carotid space. Prestyloid portion exclusively

A rare parapharyngeal space branchial cleft cyst.

The second branchial cleft cyst may arise anywhere from the tonsillar fossa to the supraclavicular area. It usually lies along the anterior border of the sternocleidomastoid muscle or deep into the muscle. Occurrence of the second branchial cleft cyst in the parapharyngeal space is extremely uncommon. We report one such case of second branchial cleft cyst arising from the parapharyngeal space and discuss its differential diagnosis and treatment modalities in detail.

Pleomorphic adenomas recurrences within the parapharyngeal space.

This study was undertaken to evaluate and concentrate on the recurrent pleomorphic adenomas (PAs) of the parapharyngeal space. The records of 9 patients treated for recurrent PAs occupying parapharyngeal space were retrospectively reviewed. The study population comprised 6 women and 3 men with a mean age of 51 (range, 35-79 years). Before the first revision surgery, imaging studies and clinical examinations showed 1 solitary mass in 4 patients, 2 solitary masses in 3 patients, and 3 or more masses in 2 patients. After first revision surgery, 4 patients had rerecurrences with multinodular disea

Plunging ranula with prestyloid parapharyngeal space, masticator space, and parotid gland extension.

UNLABELLED: Plunging ranula with prestyloid parapharyngeal space, masticator space, and parotid gland extension. INTRODUCTION: Ranulas develop from mucous extravasation secondary to sublingual gland duct obstruction or trauma. Plunging ranula usually dive into the submandibular space. METHODS: This is the first reported case of a plunging ranula with direct extension to the prestyloid parapharyngeal space, masticator space, and parotid gland with avoidance of the submandibular space. RESULTS: The patient presented with a tender parotid mass, of which the differential is broad, including par

Pterygoid muscle abscess secondary to parapharyngeal space lymphadenitis.

We describe the case of a young boy with severe clinical symptoms which mimicked several findings consistent with a peritonsillar abscess (PTA). After the patient exhibited no improvement with medical management, imaging revealed an intramuscular medial pterygoid abscess secondary to parapharyngeal space lymphadenitis. This is the first reported case of a patient with a medial pterygoid abscess secondary to lymphadenitis. We also describe a minimally invasive surgical approach for incision and drainage of the abscess that has not previously been described in the literature.

Transoral robotic surgery in retrostyloid parapharyngeal space schwannomas.

Parapharyngeal space (PPS) tumors are very rare, representing about 0.5% of head and neck neoplasms. An external surgical approach is mainly used. Several recent papers show how transoral robotic surgery (TORS) excision could be a prospective tool to remove mainly benign lesions in PPS; no cases of neurogenic tumors from the retrostyloid space treated with TORS have been reported. We present two cases which underwent TORS for schwannomas from the retrostyloid compartment of the parapharyngeal space. Clinical diagnosis of schwannoma was performed by magnetic resonance imaging (MRI). In the firs

Parapharyngeal space tumors: anatomical and image analysis findings.

OBJECTIVE: The parapharyngeal space (PS) is defined as the deepest space in the neck and it consists of the pre- and post-styloid regions. PS tumors originating in these regions are thought to dislocate the carotid artery (CA) in either the posterior or anterior direction. To determine the precise anatomy of the PS and its relationship with the CA in diagnostic images, we conducted cadaveric and imagining analysis. MATERIALS AND METHODS: We examined the posterior and lateral aspects of the PS in three cadavers. We also examined 17 patients who suffered from PS tumors, in which the carotid art

Extravascular papillary endothelial hyperplasia arising from parapharyngeal space.

A 66-year-old man visited our clinic suffering from swelling of right pharyngeal space. He was not aware of other symptoms. The contrast CT showed patchy enhancement with calcification. The enhanced MRI revealed a non-homogeneous signal pattern and patchy central enhancement. During surgery, egg-shaped hard elastic tumor was observed in parapharyngeal space without any sign indicating an invasion into surrounding nerves or feeding vessels. Then the tumor was removed without significant hemorrhage or damage to the cranial nerve. Histological examination of the tumor revealed a nodular lesion wi

[Syncope due to parapharyngeal space lesions syncope-syndrome].

The clinic association between tumors in the neck and syncopal attacks has been well documented but uncommon. The pathogenesis of this type of syncope has been attributed to an specific sinus node disfunction and several diseases that affect pacemaker activity, cardiac output and blood supply to the brain are able to produce it. The most common reflex cardiovascular sindromes linked to the IX nerve are carotid sinus syndrome and glossopharyngeal neuralgia-asystole syndrome. Some patients with a pathological involvement of the parapharyngeal space also suffers from syncope but the clinical pict

Parapharyngeal space tumour presenting as recurrent uvular oedema.

Neoplasms of the parapharyngeal space are uncommon and usually present as an intra-oral or neck mass. They often elude early diagnosis due to their deep-seated nature. Here we report a case presenting with recurrent oedema of the uvula. The pathophysiology of this previously unreported mode of presentation is discussed.

First branchial cleft anomaly extending to parapharyngeal space.

First branchial cleft anomalies are quite rare, and the majority of them are found in and around the ear canal, mostly superficial to the facial nerve. Very rarely, the anomalous tract of the first branchial cleft can go deeper to the facial nerve, necessitating a meticulous and extensive surgery. A 21-year-old student presented with slowly increasing cystic swelling in the infra-auricular region. Findings of the magnetic resonance imaging were consistent with the first branchial cleft cyst, which also exhibited a deeper extent of the lesion into the parapharyngeal space. The entire tract was

Transoral robotic parapharyngeal approach to the submandibular space.

The aim of the study was to demonstrate a novel transoral parapharyngeal approach to the submandibular space. The method used was the clinical case of a patient presenting with an anteriorly based tumor in the left submandibular space. The video shows the transoral robotic technique for access to the submandibular space through the parapharyngeal space for removal of the tumor. This approach allows for high definition visibility and complete access to structures in the submandibular space including the submandibular gland. For robotic head and neck surgeons performing lateral oropharyngectomie

Endoscope-assisted transoral approach to parapharyngeal space tumors.

BACKGROUND: Parapharyngeal space (PPS) tumors are rare tumors for which the surgical management differs widely. The purpose of this study was to discuss the advantages and disadvantages of the endoscope-assisted transoral approach over the traditional approaches. METHODS: This prospective study included 4 patients who were diagnosed with PPS tumors and treated with the endoscope-assisted transoral approach. RESULTS: The endoscope-assisted transoral approach described here safely allowed for the resection of benign neoplasms with high surgical view to the parapharyngeal neurovascular structur

Malignant ossifying fibromyxoid tumor of the parapharyngeal space.

Ossifying fibromyxoid tumor (OFMT) is a rare, recently described entity. As such, there is a paucity of information in the literature regarding this neoplasm. According to most reports, the tumor usually develops subcutaneously in the soft tissues of the extremities. Malignant forms of the tumor are far more rare than their benign counterparts. We present a new case of a malignant OFMT of the parapharyngeal space in a 33-year-old Pakistani man. The tumor was excised, and the patient did well with no complications. This case represents a rare occurrence of OFMT of the parapharyngeal space.
