Enterovirus and Parechovirus Neurologic Infections in Children: Clinical Presentations and Neuropathogenesis.

Enteroviruses (EVs) and parechoviruses (PeVs) are common pathogens of childhood. Enteroviral infections cause a range of clinical syndromes from mild illness to neurologic manifestations of meningitis, encephalitis, and acute flaccid myelitis. Disease manifestations are driven by a combination of viral replication and host immune response. Despite ubiquitousness and clinical importance, there are no approved targeted therapies for these viruses and most are without an available vaccine. Studies of EV neuropathogenesis began with poliovirus and are ongoing for other nonpolio EVs and PeVs. Many

Viral pathogens of acute gastroenteritis in Egyptian children: role of the parechovirus.

BACKGROUND AND AIM: Human parechovirus (HPeV) has emerged as a pathogen associated with acute gastroenteritis (AGE). AIM: To detect the presence of HPeV in the stool samples from Egyptian children with AGE seeking care and the possibility of its co-infection with other enteric viruses. METHODOLOGY: One hundred stool samples were collected from children attending Mansoura University Children's Hospital with AGE. HPeV and astrovirus were detected by reverse transcriptase-polymerase chain reaction (RT-PCR). At the same time, detection of rotavirus antigen and norovirus was achieved by enzyme-li

Human Parechovirus as a Cause of Isolated Pediatric Acute Liver Failure.

Among infants, almost half of acute liver failure cases are classified as indeterminate, whereas only a small number of cases show a documented viral infection. We present the first reported case of isolated acute hepatic failure in an infant in the setting of a human parechovirus (HPeV) infection. HPeV also may have been contributory to the posttransplant complication of 2 intussusceptions. This is a 10-month-old girl who presented with only symptoms of fussiness and was noted to have progressive decline in synthetic liver function as well as worsening coagulopathy requiring a liver transplan

A Rare Case of Adult-Onset Human Parechovirus Type A3 Encephalitis.

Adult-onset human parechovirus (HPeV) encephalitis is extremely rare. We report the first case of HPeV encephalitis in Japan in an 82-year-old man with diabetes, who presented with an impaired consciousness and status epilepticus (SE). A cerebrospinal fluid (CSF) analysis revealed a normal cell count with elevated protein levels. Magnetic resonance imaging showed hyperintense signals in the thalamus and parietal lobe, and HPeV type 3 was detected in the CSF. SE resolved following antiepileptic treatment, and the patient's cognitive function recovered. Adult HPeV encephalitis may present with S

Parechovirus infection causing sepsis-like illness in newborns: a NICU approach.

Human parechovirus (HpeV) is an important emerging infection in young infants, able to cause sepsis-like disease and meningoencephalitis, especially in newborns. Among the 19 identified genotypes, HPeV1, 3 and 6 are the most common types involved in human infections; HPeV3 is the type mainly responsible for neonatal infections and for infections involving the central nervous system. Signs and symptoms overlap with those of a bacterial infection and patients are usually treated with broad spectrum antibiotics. In the majority of cases lumbar puncture shows absence of pleocytosis, even in the pr

Human Parechovirus 3 in Infants: Expanding Our Knowledge of Adverse Outcomes.

BACKGROUND: Human parechovirus particularly genotype 3 (HPeV3) is an emerging infection affecting predominantly young infants. The potential for neurologic sequelae in a vulnerable subset is increasingly apparent. A review of 2 epidemics of human parechovirus (HpeV) infection in 2013 and in 2015 in Queensland, Australia, was undertaken, with an emphasis on identifying adverse neurodevelopmental outcome. METHODS: All hospitalized cases with laboratory-confirmed HPeV infection between October 2013 June 2016 were identified. Clinical, demographic, laboratory and imaging data were collected and c

Human parechovirus type 6 and Guillain-Barré syndrome: a case report.

A previously healthy 6-year-old boy was admitted to hospital with hypotonia and hyposthenia of lower limbs. Electromyography and slow motor nerve conduction velocity test identified a lower limb acute motor axonal neuropathy. Brain and spinal cord magnetic resonance imaging demonstrated multifocal cortical gray matter lesions in both cerebral hemispheres consistent with gray matter acute disseminated encephalitis otherwise with viral/Mycoplasma pneumoniae encephalitis, and signs of involvement of anterior nerve roots of the cauda equina consistent with Guillain-Barré syndrome. The patient resu

Strategies to improve detection and management of human parechovirus infection in young infants.

Human parechovirus infections are the second most common cause of viral meningitis in children. These infections are most frequently seen in infants younger than 90 days. Clinical manifestations include encephalitis, meningitis, myocarditis, and sepsis, which can lead to serious neurodevelopmental sequelae in young infants. Molecular techniques, including PCR assays, are the preferred diagnostic methods and have contributed to an increase in reported cases, along with an increasing awareness of the causal role of human parechovirus in infant diseases. However, focused clinical and diagnostic i

Severe human parechovirus type 3 myocarditis and encephalitis in an adolescent with hypogammaglobulinemia.

Human parechovirus (HPeV) belongs to the Picornaviridae family of RNA viruses. HPeV infections can be asymptomatic, lead to mild respiratory and/or gastrointestinal symptoms, or less frequently cause severe diseases such as sepsis, meningitis, encephalitis, and myocarditis. Severe neurological HPeV infections occur most commonly in infants and neonates. There are currently 16 recognized types of HPeV. HPeV type 3 (HPeV3) has been the predominant type associated with severe central nervous system disease in neonates and newborns since its discovery in 1999. Although HPeV-related infections have

Neonatal CNS Human Parechovirus Infections in Western Pennsylvania in the 2024 Season.

Human Parechovirus A (PeV-A) is a virus with near-universal infection by age five; however, neonatal infections can lead to meningoencephalitis, sepsis, and death. Prior to the COVID-19 pandemic, PeV-A showed biennial seasonality with late summer peaks, but multiple viruses have had shifted circulation post-pandemic. PeV-A is not universally included in neonatal sepsis testing; thus, the frequency and clinical spectrum of PeV-A neonatal meningoencephalitis are not fully described. We sought to evaluate the epidemiology, seasonality, and clinical presentation of neonatal PeV-A in the 2024 seaso

Parechovirus meningitis in infants: report of the first cluster of cases in Saudi Arabia.

Parechoviruses (PeVs) are RNA viruses that belong to the Picornaviridae family. PeV-A is known to cause human disease, particularly in neonates and young infants. The symptoms of infection can range from mild febrile and respiratory illnesses to severe sepsis-like conditions and central nervous system (CNS) infections. In 2022, clusters of PeV infections were reported in various parts of the world, prompting public health agencies to issue a health advisory to alert physicians about circulating PeVs causing CNS infections in children. Reports of PeV infections in Gulf Council countries are sca

First report of human parechovirus type 3 infection in a pregnant woman.

Human parechovirus type 3 (HPeV3) can cause serious conditions in neonates, such as sepsis and encephalitis, but data for adults are lacking. The case of a pregnant woman with HPeV3 infection is reported herein. A 28-year-old woman at 36 weeks of pregnancy was admitted because of myalgia and muscle weakness. Her grip strength was 6.0kg for her right hand and 2.5kg for her left hand. The patient's symptoms, probably due to fasciitis and not myositis, improved gradually with conservative treatment, however labor pains with genital bleeding developed unexpectedly 3 days after admission.
