A Rare Case of Morel-Lavallee Syndrome Complicating an Anterior Dislocation of Hip Joint.

INTRODUCTION: Non-or partial weight bearing is frequently the standard treatment after peri-articular lower extremity fractures. Displaced talar neck fractures are severe injuries compromising vascularity of the corpus and consequently are at risk for non-union and avascular necrosis, the main reason to restrict weight bearing for up to three months according to most literature. CASE PRESENTATION: We report a case of a 31-year old male with a high impact car accident. His pelvic ring and Hawkins II talar fracture were treated by open reduction and internal fixation. Rehabilitation was based o

Early versus Standard Weight Bearing Following Operative Treatment of Tibial Plateau Fractures: Do We Really Have to Wait So Long?

Postoperative management of tibial plateau fractures classically involves a prolonged period between 10 and 12 weeks of nonweight bearing or partial weight bearing. In recent years, there has been some support for earlier weight-bearing protocols although this remains controversial. The goal of this study was to investigate the difference in outcomes between early weight-bearing (EWB) and traditional weight-bearing (TWB) protocols. This investigation is a retrospective review of 92 patients treated with open reduction and internal fixation of tibial plateau fractures at a single institution, f

[Sprain and dislocation of the hip joint].

Traumatic dislocations of the hip, while very infrequent injuries, must be treated with the utmost urgency. The later the adjustment is made, the sooner aseptic necrosis of the femoral head and periarticular ossification have to be reckoned with; these may lead to dysfunction. The results of 40 cases are reported, one-half of which was treated with two-week extension and a long period of non-weight bearing, the other half without extension, early mobilisation and partial weight bearing up the 11th week. The results in each group are equally good. Thus, we endorse Lorenz Böhler's proposal to fo

Pediatric extremity bone sarcoma reconstruction with the vascularized fibula flap: Observational study assessing long-term functional outcomes, complications, and survival.

BACKGROUND: Limb salvage is important in pediatric patients with bone sarcomas. The vascularized fibula flap is a versatile option, combined or not with a bone allograft. The authors evaluated the functional long-term outcomes, complications, and survival of using this technique in pediatric patients. METHODS: A retrospective review of 27 pediatric patients reconstructed between 2011 and 2018 with the fibula flap after bone sarcoma resection was conducted. Long-term functional outcomes, complications, and survival were assessed. Variables analyzed were age, sex, Capanna technique, follow-up,

[Controlled weight bearing after osteosynthesis].

Patient compliance with postoperative partial weight bearing can be a difficult management problem. The problem may be intentional or unintentional. There is no objective way to assess the amount of weight placed on the lower extremity by the patient. It is our clinical suspicion that patients place more weight than is desirable on the effected limb. There are few reports in the literature on this topic. One study has confirmed our suspicion of poor patient compliance with postoperative weight bearing. Our goal is to develop a system to accurately assess weight bearing and to improve this aspe

Early weight-bearing following distal femur fracture fixation - a systematic review and meta-analysis.

INTRODUCTION: The global standard of care for hip fracture surgery is early weight-bearing, but this has not extended to other lower extremity fractures in the elderly. Patients undergoing fixation of distal femur fractures are often prescribed weight-bearing restrictions, which may lead to deconditioning and other complications. The purpose of this study was to compare the outcomes and complication rates between patients permitted early versus restricted weight-bearing following distal femur fracture fixation. METHODS: Medline, Embase, Cochrane and Web of Science databases were searched for

Partial Arm Weight Support Does Not Hamper the Weight Perception of Haptically-Rendered Virtual Objects: A Psychophysics Study.

Arm weight support (AWS) from upper-limb exoskeletons aids training by reducing fatigue and increasing the movement workspace, especially in people with acquired brain injury (ABI). Simultaneously, task-specific training could be enabled via haptic rendering, simulating the interaction forces with tangible virtual objects. However, how AWS affects somatosensory perception, such as the perception of the weight of haptically rendered virtual objects, is unclear. We therefore conducted a psychophysics experiment with 40 participants investigating the effect of partial AWS on virtual object weight

Primary Retrograde Tibiotalocalcaneal Nailing in an Elderly Osteoporotic Patient With Ankle Fracture: A Case Report.

INTRODUCTION: Ankle fractures are a common orthopedic injury traditionally treated with open reduction and internal fixation or conservative management. One complication introduced with open reduction and internal fixation is the requirement for non or partial weight bearing in the post-operative period. This requirement poses a unique challenge for patients who may not comply with these weight-bearing restrictions. This case is the first known article that specifically illustrates the validity and effectiveness of retrograde tibiotalocalcaneal (TTC) nailing for patients who are unlikely to fo

Predictors of immobility following surgery for periprosthetic hip fractures.

BACKGROUND: Periprosthetic femoral fractures (PPF) following total hip arthroplasty (THA) are a growing clinical challenge, particularly in the elderly. Postoperative mobility is a key determinant of outcome, yet little is known about how surgical treatment and weight-bearing protocols influence functional recovery in this population. METHODS: We conducted a retrospective cohort study of 186 patients aged ≥ 65 years treated surgically for Vancouver type B or C PPF between January 2017 and August 2023 at a level 1 trauma center. Patients underwent either open reduction and internal fixation (OR

Slow Recovery of Weight Bearing After Stabilization of Long-Bone Fractures Using Elastic Stable Intramedullary Nails in Children.

Stabilization of diaphyseal long-bone fractures using elastic stable intramedullary nails (ESIN) in children promises early mobilization and rapid resumption of full weight bearing. We evaluated the duration of postoperative functional rehabilitation after ESIN, measured by the time from stabilization until first partial weight bearing, full weight bearing, and resumption of school sports. Fifty children with unstable, displaced fractures of the femur or lower leg treated with ESIN between 2002 and 2012 were included in this retrospective analysis. We classified fractures according to the pedi

Long-term outcome and progression of osteoarthritis in uncomplicated cases of cranial cruciate ligament rupture treated by tibial plateau leveling osteotomy in dogs.

The object of this study is to evaluate the long-term outcome of hind limb weight-bearing function and progression of stifle osteoarthritis (OA) after tibial plateau leveling osteotomy (TPLO). Groups were classified by the degree of cranial cruciate ligament (CrCL) damage and presence or absence of medial meniscus damage as macroscopically evaluated during surgery. Weight-bearing function was assessed via the peak vertical force (PVF), and OA progression was assessed via the radiographic OA score (OAS) preoperatively and 1, 3, 6, 12, 18, 24, and 36 months postoperatively. In all stifles, PVF w

A novel tool for continuous fracture aftercare - Clinical feasibility and first results of a new telemetric gait analysis insole.

Weight bearing after lower extremity fractures still remains a highly controversial issue. Even in ankle fractures, the most common lower extremity injury no standard aftercare protocol has been established. Average non weight bearing times range from 0 to 7 weeks, with standardised, radiological healing controls at fixed time intervals. Recent literature calls for patient-adapted aftercare protocols based on individual fracture and load scenarios. We show the clinical feasibility and first results of a new, insole embedded gait analysis tool for continuous monitoring of gait, load and activit

[The pararectus approach: surgical procedure for acetabular fractures].

OBJECTIVE: The pararectus approach was rediscovered several years ago for pelvic surgery and described as an alternative approach especially for the treatment of acetabular fractures of the anterior column involving the quadrilateral plate. INDICATIONS: For optimal visualization of acetabular fractures involving the quadrilateral plate, fractures of the anterior wall and anterior column, anterior column/posterior hemitransverse fractures, and fractures with central impression of dome fragments, the pararectus approach has proven to be a useful access. CONTRAINDICATIONS: The pararectus approa

[Stabilization in treatment of per- and subtrochanteric fractures of the proximal femur].

The incidence of fractures in the trochanteric region is high in the elderly, and early stabilization is required that allows immediate weight bearing. Older patients with low bone quality, limited compliance and strength, and the inability for partial weight-bearing, benefit from intramedullary stabilization of these fractures. The choice of implant and the operative technique depends on the degree of instability in the individual fracture type. Intertrochanteric fractures are frequently combined with subtrochanteric fracture types. Their treatment is still a challenge because of the high deg

Radiological study of the secondary reduction effect of early functional exercise on displaced intra-articular calcaneal fractures after internal compression fixation.

PURPOSE: Early post-operative exercise and weight-bearing activities are found to improve the functional recovery of patients with displaced intra-articular calcaneal fractures (DIACFs). We hypothesized that early functional exercise after surgery might have a secondary reduction effect on the subtalar joint, in particular the smaller fracture fragments that were not fixed firmly. A prospective study was conducted to verify this hypothesis. METHODS: From December 2012 to September 2013, patients with unilateral DIACFs were enrolled and received a treatment consisting of percutaneous leverage

A Rare Case of Talus Neck and Open Medial Malleolus Fractures with Ankle Subluxation Treated using a Single Anteromedial Approach.

INTRODUCTION: Talus fractures are uncommon and complex injuries associated with significant trauma and complications. The incidence of associated malleolar injury with talus fracture is rare. CASE REPORT: We share this unusual case of a Hawkins type-3 talus neck fracture along with a serious Grade 3B medial malleolus fracture and ankle subluxation, which was treated with cleaning the wound, realigning the ankle, and surgery to fix the bones. Post-operatively, the wound was healthy and free of infection. Despite being told to avoid weight-bearing for three months, the patient lost follow-up af
