INTRODUCTION: Anterior tension wiring using Kirschner wires (K-wires) is still considered the standard treatment for patella fractures, despite its high complication rate. We report a greenstick fracture of the patella in an ambulant boy with diplegic cerebral palsy (CP). The boy was known to have knee crouch which was documented in our gait laboratory. Greenstick fractures usually occur in the long bones of children and are caused by a bending force. This is the first report of a patellar greenstick fracture and provides a unique insight into the propagation of patellar fractures in CP crouch.

Irreducible lateral patellar dislocation is rare. It has previously been described with rotation around a vertical axis. We describe a case of irreducible lateral patellar dislocation in a 66-year-old lady with an impaction fracture of medial facet of patella locked on the lateral femoral condyle. Closed reduction was unsuccessful and the patient was treated by total knee replacement. Such a case has not been described previously in the literature.

CASE: Hoffa fractures, uncommon injuries of the femoral condyle, sometimes involve both condyles, forming bicondylar fractures, typically from high-velocity trauma. We describe a 17-year-old male with an open conjoined bicondylar Hoffa fracture and a patellar fracture with incarceration following a road traffic accident. Disorders of the patella are the most common cause of anterior knee pain. The etiologies of anterior knee pain are reviewed. A case report of an acute displaced patella fracture in a bipartite union is presented. Bipartite patellar development, incidence, radiographic findings, and clinical symptoms follow. Patella fracture is a recognized complication of ACL reconstruction with an autogenous patella tendon graft. Typically, fracture occurs as a result of a fall. The incidence of fracture is approximately 0.5%. Accelerated rehabilitation protocols can place stress on the patella, especially in the initial stages of recovery. Therapists are reminded to observe constraints placed on patients by biological tissues, recovering neuromuscular status, and previous level of conditioning. Rehabilitation protocols should be revised according to these factors.

Pateller fractures in association with total knee replacement are uncommon, but potentially a cause of an unsatisfactory clinical outcome. Patient, implant, and technical factors are important predisposing causes of these patellar fractures. Prevention is the best treatment. Important outcome criteria include the integrity of the extensor mechanism, patellar implant fixation, and anatomic location. Patients requiring operative intervention often have an unsatisfactory outcome.
