[Physiotherapeutic and training therapeutic options for instable patellofemoral joints].

In order to allow an adequate and timely restoration of physical performance with an instable patellofemoral joint within the framework of complex therapy strategies, individual post-traumatic cause-and-effect chains and their influence on the hip-leg axis of both the active and passive musculoskeletal systems must be considered as well as joint-specific aspects. For this reason selected relevant neurophysiological aspects of one and multiple joint movement stereotypes will be firstly presented. Subsequently some selected physiotherapeutic measures will be presented and their indication-specif

The effect of overstuffing the patellofemoral joint on the extensor retinaculum of the knee.

Overstuffing the patellofemoral compartment during TKR leads to complications such as maltracking and wear, predisposing to early failure. However, there is no data describing how the patellar construct thickness affects the retinacula. This study instrumented cadaveric knees that had a Genesis II (Smith & Nephew, Memphis, TN, USA) TKR in situ. Sutures were passed along the medial patellofemoral ligament (MPFL) and the deep transverse fibre band of the lateral retinaculum, from the ilio-tibial band (ITB) to the patella. These sutures were attached to displacement transducers. Length changes in

Double-contrast CT arthrography of the cartilage of the patellofemoral joint.

For assessment of the value of computed tomography (CT) as a method for evaluating the cartilage of the patellofemoral joint, 100 knees were examined in 91 patients. With the patient in a supine position with the knee flexed at a 30 degree angle, a sufficient quantity of air and positive contrast material was required to obtain an accurate CT picture of the cartilage. Normal cartilage images of 53 knees and abnormal images of 47 knees were obtained by use of CT. The latter group was classified into eight types. When the CT findings of 70 knees were compared with arthroscopy and/or arthrotomy,

Intraoperative patellofemoral load varies between total knee replacement and patellofemoral joint resurfacing, and across TKR implant systems.

INTRODUCTION: Patellofemoral joint loading is influenced by implant design and surgical execution during knee replacement. Altered patellofemoral mechanics have been implicated in postoperative symptoms, yet the magnitude and variability of intraoperative load changes across different replacement procedures and implant systems remain incompletely characterised. The aim of this registry study was to describe and quantify intraoperative patellofemoral joint load changes following total knee replacement (TKR) and patellofemoral joint resurfacing (PFJR), focusing on cumulative and compartment-spec

Understanding patellofemoral pain with maltracking in the presence of joint laxity: complete 3D in vivo patellofemoral and tibiofemoral kinematics.

Patellofemoral pain is widely accepted as one of the most common pathologies involving the knee, yet the etiology of this pain is still an open debate. Generalized joint laxity has been associated with patellofemoral pain, but is not often discussed as a potential source of patellar maltracking. Thus, the objective of this study was to compare the complete 6 degree of freedom patellofemoral and tibiofemoral kinematics from a group of patients diagnosed with patellofemoral pain syndrome and maltracking to those from an asymptomatic population. The following null hypotheses were tested: kinemati

Patellofemoral Joint Stress during Running with Added Load in Females.

Patellofemoral joint (PFJ) pain syndrome is a commonly reported form of pain in female runners and military personnel. Increased PFJ stress may be a contributing factor. Few studies have examined PFJ stress running with added load. Our purpose was to analyze PFJ stress, PFJ reaction force, quadriceps force, knee flexion angle, and other kinematic and temporospatial variables running with and without a 9 kg load. Nineteen females ran across a force platform with no added load and 9.0 kg weight vest. Kinematic data were collected using 3D motion capture and kinetic data with a force platform. Mu

EFFECT OF HEEL LIFTS ON PATELLOFEMORAL JOINT STRESS DURING RUNNING.

BACKGROUND: Patellofemoral pain is a debilitating injury for many recreational runners. Excessive patellofemoral joint stress may be the underlying source of pain and interventions often focus on ways to reduce patellofemoral joint stress. PURPOSE: Heel lifts have been used as an intervention within Achilles tendon rehabilitation programs and to address leg length discrepancies. The purpose of this study was to examine the effect of running with heel lifts on patellofemoral joint stress, patellofemoral stress impulse, quadriceps force, step length, cadence, and other related kinematic and spa

Sagittal plane trunk posture influences patellofemoral joint stress during running.

STUDY DESIGN: Cross-sectional, repeated-measures. Objectives To examine the association between sagittal plane trunk posture and patellofemoral joint (PFJ) stress, and to determine whether modifying sagittal plane trunk posture influences PFJ stress during running. BACKGROUND: Patellofemoral pain is the most common injury among runners and is thought to be the result of elevated PFJ stress. While sagittal plane trunk posture has been shown to influence tibiofemoral joint mechanics, no study has examined the influence of trunk posture on PFJ kinetics. METHODS: Twenty-four asymptomatic recreat

Does knee joint proprioception alter following medial patellofemoral ligament reconstruction?

BACKGROUND: This study firstly aimed to determine whether proprioception deficits, as measured by joint position sense (JPS), occur in people following recurrent patellar dislocations. Secondly, to determine whether JPS changes following medial patellofemoral ligament reconstruction (MPFL) reconstruction for patellar instability. METHODS: Thirty people following recurrent patellar dislocation were recruited. Pre-operative JPS was assessed using the passive angle reproduction test. Through this, an assessor moved a participant's limb to a target position. This was returned to neutral, before f

Does preoperative patellofemoral joint state affect medial unicompartmental arthroplasty survival?

One contested contraindication to medial unicompartmental knee arthroplasty (UKA) has been status of the patellofemoral joint. Surgeons have avoided UKA when the patellofemoral joint has radiographic evidence of arthritic changes. However, recent studies advocate ignoring patellofemoral joint status when considering UKA. The purpose of this study was to compare the failure rate of mobile-bearing, medial UKA in patients with and without preoperative radiographic evidence of patellofemoral joint degeneration. Preoperative radiographs from a random selection of 503 patients (638 knees) treated wi

The Warwick patellofemoral arthroplasty trial: a randomised clinical trial of total knee arthroplasty versus patellofemoral arthroplasty in patients with severe arthritis of the patellofemoral joint.

BACKGROUND: Severe arthritis of the knee is a disabling condition, with over 50,000 knee replacements performed each year in the UK. Isolated patellofemoral joint arthritis occurs in over 10% of these patients with the treatment options being patellofemoral arthroplasty or total knee arthroplasty. Whilst many surgeons believe total knee arthroplasty is the 'gold standard' treatment for severe knee arthritis, patellofemoral arthroplasty has certain potential advantages. Primarily, because this operation allows the patient to keep the majority of their own knee joint; preserving bone-stock and t

Effect of toe-out gait modification on patellofemoral joint loading.

BACKGROUND: Toe-out gait has been proposed as a conservative treatment to reduce medial tibiofemoral joint loading. However, patellofemoral joint loading during toe-out gait is not yet understood. RESEARCH QUESTION: Does the toe-out gait modification affect patellofemoral joint loading? METHODS: Sixteen healthy adults were enrolled in this study. The natural gait and toe-out gait were measured using a three-dimensional motion analysis and a force plate. The knee flexion angle and external knee flexion moment during the stance phase were calculated. Thus, dynamic knee joint stiffness, a proxy

Patellofemoral joint kinematics and contact pressures in total knee arthroplasty.

Patellofemoral joint kinematics, contact areas, and contact pressures were measured concomitantly before and after total knee arthroplasty in 10 fresh frozen human cadaver knees using an Instron machine, a custom patellofemoral joint testing jig, axial bone markers, a continuous video digitizing system, and Fuji pressure sensitive film. The implant used in this study was the Kirschner Performance Knee System with an all polyethylene, domed patellar component. For all tests, the patella was aligned in its anatomically neutral position. Patellofemoral joint contact areas decreased as much as 19-

Mid-Term Outcomes of Isolated Trochlear Arthroplasty for Patellofemoral Arthritis: A Joint-Preserving Alternative to Complete Patellofemoral Replacement.

Background Trochlea-only arthroplasty (TOA) is a conservative surgical approach for isolated patellofemoral arthritis, aimed at preserving the native patella and avoiding complications associated with its resurfacing. This study evaluates the mid-term survivorship, functional outcomes, and revision profile of patients undergoing TOA and compares outcomes with historical data from conventional patellofemoral arthroplasty (PFA). Methods We retrospectively reviewed 50 patients who underwent isolated TOA between 2008 and 2019 by a single surgeon. A medial parapatellar approach was used, with intra

Clinical outcome after mosaicplasty of knee articular cartilage defects of patellofemoral joint versus tibiofemoral joint.

BACKGROUND: The purpose was to investigate survival of cartilage repair in the knee joint by osteochondral autograft transfer stratified by location of the lesion; patellofemoral joint (N = 26) versus the medial or lateral femoral condyles (N = 58). METHODS: For survival analyses, "failure" was defined as the event of a patient reporting a poor Lysholm score (below 65 points) or undergoing a knee replacement procedure. RESULTS: The survival distribution was not significantly different between the patellofemoral joint and the tibiofemoral joint groups. CONCLUSIONS: The current study suggest

Axial computed tomography of the patellofemoral joint with and without quadriceps contraction.

Computed tomography was used to analyze the patellofemoral relationship during the first 60 degrees of knee flexion in patients with chronic patellofemoral pain syndrome (49 knees) and a healthy control group (15 knees). The patellofemoral joints were imaged axially through the center of the patella articular cartilage with the knee flexed 0 degrees, 0 degrees with maximal quadriceps muscle contraction, 30 degrees, and 60 degrees. In 0 degrees of knee flexion, the sulcus angle was greater in the symptomatic group than in normal controls. The patella displaced further laterally, and the lateral
