Virtual Care and Telehealth for Improving Healthcare Access in Rural Western Canada and the Western United States: A Scoping Review and Narrative Synthesis.

Background/Objectives: Western Canadian and U.S. communities outside urban centres remain underserved by primary, specialist, emergency, mental health, and chronic-disease services. These access problems reflect distance, weather, workforce shortages, specialist maldistribution, primary care attachment gaps, broadband limitations, and the governance realities of Indigenous and Tribal communities. This scoping review with narrative synthesis examined how telehealth and virtual-care models affect rural access in western Canada and the western/frontier United States. Methods: Searches were comple

Geospatial patterns of access to self-care resources for obesity among endometrial cancer survivors in a high poverty urban community.

ObjectiveTo examine endometrial cancer survivors' access to recommended obesity-related self-care resources.MethodsParticipants included women treated 2010-2015 for endometrial cancer at an academic medical center who lived in the surrounding 16 ZIP code area on Chicago's South Side. Demographic and health data were abstracted from medical records. A socioeconomic status (SES) score (SES-1 = low, SES-5 = high) was generated for each patient using census block group-level data. Self-care resources for exercise, healthy weight, and diet were obtained from a community resource census. Geospatial

Digital Pathology Archives as Long-Term Clinical Memory: A Self-Sovereign Identity-Based Governance Model.

Background/Objectives: Digital pathology is transforming diagnostic workflows through whole-slide imaging (WSI), digital archiving, artificial intelligence (AI)-assisted analysis, and remote consultation. The existing literature only partly addresses patient access to pathology records and cross-institutional sharing within a holistic governance framework. This study proposes a Self-Sovereign Identity (SSI)-based conceptual governance model that treats digital pathology archives as components of long-term clinical memory. Here, clinical memory refers to a pathology-specific information layer t

Capture and analysis of radiation dose reports for radiology.

Radiographic imaging systems can produce records of exposure and dose parameters for each patient. A variety of file formats are in use including plain text, bit map images showing pictures of written text and radiation dose structured reports as text or extended markup language files. Whilst some of this information is available with image data on the hospital picture archive and communication system, access is restricted to individual patient records, thereby making it difficult to locate multiple records for the same scan protocol. This study considers the exposure records and dose reports

Barriers to growth hormone access in pediatric patients at an academic medical center.

PURPOSE: Evaluating barriers that may delay or limit human growth hormone (hGH) access is important to ensure timely and equitable treatment. We examined time to hGH access and whether time to approval was associated with patient factors and impacts on patient growth. METHODS: This was a single-center, retrospective review of the electronic medical records of pediatric patients starting hGH treatment for an indication approved by the FDA at the Vanderbilt Pediatric Endocrinology Clinic between January 1, 2018, and December 31, 2020. A Cox proportional hazards model was used to assess factors

Impact of health information exchange access on medication management recommendations in a community-based pharmacy setting.

BACKGROUND: Community-based pharmacists have historically lacked access to electronic medical records and clinical markers. Research was needed to assess the impact of obtaining clinical markers from a health information exchange (HIE) on pharmacist recommendations during a medication management encounter. OBJECTIVE: The objective of this project was to quantify and characterize clinical recommendations by pharmacists, resident pharmacists, or student pharmacists within an independent pharmacy setting that had access to patient information via an HIE. PRACTICE DESCRIPTION: Moose Pharmacy is

Venovenostomy for outflow venous obstruction in patients with upper extremity autogenous hemodialysis arteriovenous access.

Outflow venous obstruction is a common problem in patients with autogenous upper extremity hemodialysis access. Surgical revision to divert blood flow into the patent vein is sometimes possible to restore unobstructed flow. The result of this type of procedure is not yet well known. The purpose of this report is to describe our experience with outflow venovenostomy (VV) to salvage dysfunctional autogenous upper extremity arterial venous access (AVF) threatened by venous outflow obstruction in hemodialysis-dependent patients. All patients who underwent VV procedures between September 1999 and O

OpenNotes in dentistry as a strategy for patient engagement: opportunities, barriers, and a framework for implementation.

Patient engagement is increasingly recognized as a cornerstone of high-quality, patient-centered care. Although the OpenNotes initiative, which gives patients direct access to their clinical notes through electronic health records, has demonstrated significant benefits in medicine, its application in dentistry remains largely unexplored despite the elective and decision-intensive nature of dental care. This targeted narrative review was informed by a PubMed search conducted on March 26, 2026, using the terms ("OpenNotes" OR "shared clinical notes" OR "patient access to records") AND ("dentistr

Monitoring Digital Health Equity Across the Digital Front Door: U.S. Trends in Broadband Access, Portal Offers, and Patient IT Use in 2017-2020.

Background/Objectives: Patient portals and patient-facing information technologies are now routine channels for healthcare delivery, but healthcare organizations need practical ways to monitor whether these channels are equitably accessible and used. This study examined U.S. national trends in digital health equity from 2017 to 2020 using three Healthy People/HINTS indicators representing an access-offer-use pathway: broadband access, being offered online access to medical records, and using information technology to track or communicate about care. Methods: We conducted a repeated cross-secti

Hospital and community surveys reveal the severe public health problem and socio-economic impact of human echinococcosis in Ningxia Hui Autonomous Region, China.

A comprehensive study of human echinococcosis (caused by Echinococcus granulosus or E. multilocularis), including assessment of hospital records, community surveys and patient follow-up, was conducted in Ningxia Hui Autonomous Region (NHAR), China. In contrast to hospital records that showed 96% of echinococcosis cases were caused by cystic echinococcosis (CE), 56% of cases detected in active community surveys were caused by alveolar echinococcosis (AE). The AE and CE cases co-existed frequently in the same village, even occurring in the same patient. A serious public health problem caused by

Cohort Builder: A Software Pipeline for Generating Patient Cohorts with Predetermined Baseline Characteristics from Medical Records and Raw Ophthalmic Imaging Data.

In clinical research, the analysis of patient cohorts is a widely employed method for investigating relevant healthcare questions. The ability to automatically extract large-scale patient cohorts from hospital systems is vital in order to unlock the potential of real-world clinical data, and answer pivotal medical questions through retrospective research studies. However, existing medical data is often dispersed across various systems and databases, preventing a systematic approach to access and interoperability. Even when the data are readily accessible, clinical researchers need to sift thro

Analysis of the security and privacy requirements of cloud-based electronic health records systems.

BACKGROUND: The Cloud Computing paradigm offers eHealth systems the opportunity to enhance the features and functionality that they offer. However, moving patients' medical information to the Cloud implies several risks in terms of the security and privacy of sensitive health records. In this paper, the risks of hosting Electronic Health Records (EHRs) on the servers of third-party Cloud service providers are reviewed. To protect the confidentiality of patient information and facilitate the process, some suggestions for health care providers are made. Moreover, security issues that Cloud servi
