Patient-specific computational fluid dynamic simulation of cerebrospinal fluid flow in the intracranial space.

BACKGROUND: Abnormal cerebrospinal fluid (CSF) flow is associated with a variety of poorly understood neurological disorders such as Alzheimer's Disease and hydrocephalus. The lack of comprehensive understanding of the fluid and solid mechanics of CSF flow remains a critical barrier in the development of diagnostic assessment and potential treatment options for these diseases. We have developed a whole brain, patient-specific computational fluid dynamics (CFD) simulation of CSF flow in the cranial cavity as a step towards comprehensive understanding of CSF dynamics and how they relate to neuro

Benefits of Patient-Specific Reconstruction Plates in Mandibular Reconstruction Surgical Simulation and Resident Education.

Poorly contoured mandibular reconstruction plates are associated with postoperative complications. Recently, a technique emerged whereby preoperative patient-specific reconstructive plates (PSRP) are developed in the hopes of eliminating errors in the plate-bending process. This study's objective is to determine if reconstructions performed with PSRP are more accurate than manually contoured plates. Ten Otolaryngology residents each performed two ex vivo mandibular reconstructions, first using a PSRP followed by a manually contoured plate. Reconstruction time, CT scans, and accuracy measuremen

Patient-specific computer simulation to predict long-term outcomes after transcatheter aortic valve replacement.

BACKGROUND: Patient-specific computer simulation may predict the development of paravalvular regurgitation (PVR) after transcatheter aortic valve replacement (TAVR). We hypothesised that patient-specific computer simulation might identify patients at risk for long-term adverse outcomes after TAVR. METHODS: A multi-centre retrospective study was performed on patients with symptomatic severe aortic stenosis who had undergone TAVR with a self-expanding transcatheter heart valve (THV). Pre-procedural cardiac computed tomography imaging was used to create finite element models of the aortic root.

Patient-specific computer simulation for transcatheter cardiac interventions: what a clinician needs to know.

Patient-specific computer simulation consists of the assessment of the interaction of the device with the host based on the integration of the detailed geometric and biomechanical properties of the device and host. Hence, it allows the prediction of valve performance (efficacy) and complications (safety) and may consequently help the physician to select the valve/device that best fits the individual patient, thereby improving outcome. There is currently little awareness and information in clinical medicine on patient-specific computer simulation. In this paper, we describe the technical backgr

Patient-Specific Computer Simulation of Transcatheter Aortic Valve Replacement in Bicuspid Aortic Valve Morphology.

BACKGROUND: A patient-specific computer simulation of transcatheter aortic valve replacement (TAVR) in tricuspid aortic valve has been developed, which can predict paravalvular regurgitation and conduction disturbance. We wished to validate a patient-specific computer simulation of TAVR in bicuspid aortic valve and to determine whether patient-specific transcatheter heart valve (THV) sizing and positioning might improve clinical outcomes. METHODS: A retrospective study was performed on TAVR in bicuspid aortic valve patients that had both pre- and postprocedural computed tomography imaging. Pr

Sensitivity of patient-specific numerical simulation of cerebal aneurysm hemodynamics to inflow boundary conditions.

OBJECT: Due to the difficulty of obtaining patient-specific velocity measurements during imaging, many assumptions have to be made while imposing inflow boundary conditions in numerical simulations conducted using patient-specific, imaging-based cerebral aneurysm models. These assumptions can introduce errors, resulting in lack of agreement between the computed flow fields and the true blood flow in the patient. The purpose of this study is to evaluate the effect of the assumptions made while imposing inflow boundary conditions on aneurysmal hemodynamics. METHODS: A patient-based anterior com

Assessing the validity of simulation-based sensitivity analysis for patient-specific quality assurance devices.

INTRODUCTION: Patient-Specific Quality Assurance (PSQA) is a crucial component of modern radiotherapy, ensuring treatment accuracy and patient safety; however assessing PSQA system sensitivity to errors remains a challenge, with no standardized methodology currently available. This study compares two sensitivity analysis methodologies for EPID-based PSQA: the Direct Error (DE) method, where errors are introduced in the delivery phase before measurement, and the Inverse Error (IE) method, where errors are incorporated into the Treatment Planning System (TPS). The aim is to determine whether the

Enhancing gastric ultrasound training with standardized patient-based drinking simulation: a self-controlled study.

BACKGROUND: Gastric cancer remains a major health burden in China, with low early screening rates and limited access to endoscopic services. Gastric ultrasound offers a noninvasive, low-cost alternative but is underutilized due to the lack of effective training models. This study aimed to evaluate a novel standardized patient (SP)-based drinking simulation for teaching gastric ultrasound to residents. METHODS: In a self-controlled design, 32 ultrasound residents underwent a structured training program comprising a 90-minute theoretical lecture and a 2-hour hands-on workshop. Trainees acted as

Intraoperative Tension Pneumothorax in a Trauma Patient: An Adult Simulation Case for Anesthesia Residents.

Anesthesiologists may encounter multiple obstacles in communication when attempting to collect information for emergency surgeries. Occult tension pneumothorax that was asymptomatic in the emergency department (ED) could become apparent upon positive pressure ventilation and pose a critical threat to the patient intraoperatively. Here, we describe a simulation exercise that was developed as a curriculum module for the Indiana University (IU) Anesthesiology residency program. It is primarily designed for first-year clinical anesthesia residents (CA-1/PGY-2). It is a 50 min encounter with two sc

Correction of hysteretic respiratory motion in SPECT myocardial perfusion imaging: Simulation and patient studies.

PURPOSE: Amplitude-based respiratory gating is known to capture the extent of respiratory motion (RM) accurately but results in residual motion in the presence of respiratory hysteresis. In our previous study, we proposed and developed a novel approach to account for respiratory hysteresis by applying the Bouc-Wen (BW) model of hysteresis to external surrogate signals of anterior/posterior motion of the abdomen and chest with respiration. In this work, using simulated and clinical SPECT myocardial perfusion imaging (MPI) studies, we investigate the effects of respiratory hysteresis and evaluat

A Framework for Patient-Specific Spinal Intervention Simulation: Application to Lumbar Spinal Durotomy Repair.

We present a functional and patient-specific lumbar phantom for the training of spinal durotomy and dura closure under microscopic view, consisting of a lumbar model, pressurized dural surrogate, together immersed in a tissue-mimicking layer simulating fat, muscle and skin. The lumbar model was derived from a patient computed tomography scan, preserving the natural shape and curvature of the lumbar column. The inclusion of the simulated soft-tissue layer was critical for preserving the surgical ergonomics and presented a realistic view under the surgical microscope. As the success of dura repa

Involving Patients Throughout Sensitive Simulation.

BACKGROUND: While people with lived experience (PLE) are increasingly included in health professions education, their role in simulation often remains limited to narrative sharing. This initiative explored how involving PLE throughout the design and delivery of a pharmacy simulation on PrEP (a preventive HIV treatment) and sexual and gender diversity shaped students' learning about inclusive, person-centred communication in consultations involving stigma and marginalisation. APPROACH: A PrEP-related simulation involving a non-binary patient was integrated into a third-year pharmacy course. Se

Acoustic simulation of a patient's obstructed airway.

This research focuses on the numerical simulation of stridor; a high pitched, abnormal noise, resulting from turbulent airflow and vibrating tissue through a partially obstructed airway. Characteristics of stridor noise are used by medical doctors as indication for location and size of the obstruction. The relation between type of stridor and the various diseases associated with airway obstruction is unclear; therefore, simply listening to stridor is an unreliable diagnostic tool. The overall aim of the study is to better understand the relationship between characteristics of stridor noise and

Incorporating simulators in a standardized patient exam.

OBJECTIVE: Using simulated patients during a clinical skills exam that involves many students has the advantage of standardizing the delivery of historical data. One major disadvantage is the inability to standardize the physical exam findings. We designed a simulated patient exam that incorporates simulated abnormal physical exam findings. DESCRIPTION: The simulated patient exam case was divided into three separate stations: (1) the simulated patient's history, (2) the simulated physical exam, and (3) the presentation station. Dyspnea was chosen as the chief complaint because of the broad di
