A patient-specific numerical modeling of the spontaneous coronary artery dissection in relation to atherosclerosis.

The spontaneous coronary artery dissection (SCAD) is a clinical complication of angioplasty leading to an initiation of a tear/crack in the intima layer of the artery. The crack can propagate to the interface of the intima-media layer following by intramural hematoma. The relation between the SCAD and atherosclerosis is a controversial issue, as some studies stated no correlation between them while others showed that a crack can initiate in the intima but cannot propagate into the atrophied media layer. To investigate the relation between the intraluminal crack propagation in the atherosclerot

Modeling the Pathological Long-Range Regulatory Effects of Human Structural Variation with Patient-Specific hiPSCs.

The pathological consequences of structural variants disrupting 3D genome organization can be difficult to elucidate in vivo due to differences in gene dosage sensitivity between mice and humans. This is illustrated by branchiooculofacial syndrome (BOFS), a rare congenital disorder caused by heterozygous mutations within TFAP2A, a neural crest regulator for which humans, but not mice, are haploinsufficient. Here, we present a BOFS patient carrying a heterozygous inversion with one breakpoint located within a topologically associating domain (TAD) containing enhancers essential for TFAP2A expre

A validated methodology for patient specific computational modeling of self-expandable transcatheter aortic valve implantation.

Leakage of blood alongside the implant is a relatively frequent and life-limiting complication after transcatheter aortic valve implantation. The aim of this study is to develop and validate a workflow to simulate the implantation prior to the intervention. Based on the simulation outcome, the amount of leakage is estimated in order to evaluate the risk of a severe complication. A finite element model of the stent implantation in 10 patients was created based on a pre-operative computed tomography scan. All 10 patients also received a follow-up computed tomography scan, after the implantation.

Influence of interpersonal geometrical variation on spinal motion segment stiffness: implications for patient-specific modeling.

STUDY DESIGN: A validated finite element model of an L3-L4 motion segment is used to analyze the effects of interpersonal differences in geometry on spinal stiffness. OBJECTIVE: The objective of this study is to determine which of the interpersonal variations of the geometry of the spine have a large effect on spinal stiffness. This will improve patient-specific modeling. SUMMARY OF BACKGROUND DATA: The parameters that define the geometry of a motion segment are vertebral height, disc height, endplate width, endplate depth, spinous process length, transverse process width, nucleus size, lord

Using Computational Modeling to Capture Schizophrenia-Specific Reinforcement Learning Differences and Their Implications on Patient Classification.

BACKGROUND: Psychiatric diagnosis and treatment have historically taken a symptom-based approach, with less attention on identifying underlying symptom-producing mechanisms. Recent efforts have illuminated the extent to which different underlying circuitry can produce phenotypically similar symptomatology (e.g., psychosis in bipolar disorder vs. schizophrenia). Computational modeling makes it possible to identify and mathematically differentiate behaviorally unobservable, specific reinforcement learning differences in patients with schizophrenia versus other disorders, likely owing to a higher

Refinement of MLC modeling improves commercial QA dosimetry system for SRS and SBRT patient-specific QA.

PURPOSE: Mobius 3D (M3D) provides a volumetric dose verification of the treatment planning system's calculated dose using an independent beam model and a collapsed cone convolution superposition algorithm. However, there is a lack of investigation into M3D's accuracy and effectiveness for stereotactic radiosurgery (SRS) and stereotactic body radiotherapy (SBRT) quality assurance (QA). Here, we collaborated with the vendor to develop a revised M3D beam model for SRS/SBRT cases treated with a 6X flattening filter-free (FFF) beam and high-definition multiple leaf collimator (HDMLC) on an Edge lin

Patient-specific modeling of ventricular activation pattern using surface ECG-derived vectorcardiogram in bundle branch block.

Patient-specific computational models have promise to improve cardiac disease diagnosis and therapy planning. Here a new method is described to simulate left-bundle branch block (LBBB) and RV-paced ventricular activation patterns in three dimensions from non-invasive, routine clinical measurements. Activation patterns were estimated in three patients using vectorcardiograms (VCG) derived from standard 12-lead electrocardiograms (ECG). Parameters of a monodomain model of biventricular electrophysiology were optimized to minimize differences between the measured and computed VCG. Electroanatomic

Enhancing patient-specific computational modeling of cardiac mechanics by leveraging high spatiotemporal resolution magnetic resonance imaging.

BACKGROUND AND OBJECTIVE: Computational models of cardiac mechanics can improve diagnosis, surgical planning, and device design; yet their accuracy is limited by the quality of the underlying imaging data used in routine care. Our team recently reported a free-breathing, four-dimensional cardiac magnetic resonance imaging (MRI) protocol (AutoCMR), which provides 30 volumetric frames per cardiac cycle at 1.6mm isotropic resolution. Here, we investigate whether this high spatiotemporal resolution imaging data, along with cuff systolic and diastolic blood pressures (SBP, DBP), improves non-invasi

Fully coupled patient-specific fluid-structure interaction modeling of post-TAVI hemodynamics compared with echocardiographic measurements.

To develop a fully coupled, patient-specific fluid-structure interaction (FSI) framework for quantitative assessment of post-transcatheter aortic valve implantation (TAVI) hemodynamics and valve biomechanics, and to compare selected simulation-derived hemodynamic indices with post-procedural echocardiographic measurements. Patient-specific geometries were reconstructed from pre-operative computed tomography angiography in five subjects treated with SAPIEN 3 Ultra (S3) devices. Structural TAVI deployment was simulated using Abaqus/Explicit and subsequently coupled with FlowVision for performing

Finite element modeling with patient-specific geometry to assess clinical risks of percutaneous pulmonary valve implantation.

BACKGROUND: Percutaneous pulmonary valve implantation (PPVI) is a non-surgical treatment for right ventricular outflow tract (RVOT) dysfunction. During PPVI, a stented valve, delivered via catheter, replaces the dysfunctional pulmonary valve. Stent oversizing allows valve anchoring within the RVOT, but overexpansion can intrude on the surrounding structures. Potentially dangerous outcomes include aortic valve insufficiency (AVI) from aortic root (AR) distortion and myocardial ischemia from coronary artery (CA) compression. Currently, risks are evaluated via balloon angioplasty/sizing before st

Looking towards the future: patient-specific computational modeling to optimize outcomes for transcatheter mitral valve repair.

Severe mitral valve regurgitation (MR) is a heart valve disease that progresses to end-stage congestive heart failure and death if left untreated. Surgical repair or replacement of the mitral valve (MV) remains the gold standard for treatment of severe MR, with repair techniques aiming to restore the native geometry of the MV. However, patients with extensive co-morbidities may be ineligible for surgical intervention. With the emergence of transcatheter MV repair (TMVR) treatment paradigms for MR will evolve. The longer-term outcomes of TMVR and its effectiveness compared to surgical repair re

An approach for patient-specific multi-domain vascular mesh generation featuring spatially varying wall thickness modeling.

In this work, we present a computationally efficient image-derived volume mesh generation approach for vasculatures that implements spatially varying patient-specific wall thickness with a novel inward extrusion of the wall surface mesh. Multi-domain vascular meshes with arbitrary numbers, locations, and patterns of both iliac bifurcations and thrombi can be obtained without the need to specify features or landmark points as input. In addition, the mesh output is coordinate-frame independent and independent of the image grid resolution with high dimensional accuracy and mesh quality, devoid of

Anatomical compatibility of precontoured clavicular locking compression plates: a 3D patient-specific modeling approach with clinical correlation.

BACKGROUND: Precontoured clavicular locking compression plates (LCP) aim to minimize implant-related complications by providing a close fit to the clavicle's complex morphology. Nevertheless, the inherent anatomical variation observed across patients can lead to discrepancies between implant and bone, potentially contributing to suboptimal fit and an increased risk of complications requiring reoperation. OBJECTIVE: This study analyzed the anatomical compatibility of precontoured clavicular LCPs, detailing their fit characteristics and exploring the correlation between these findings and clini

Working toward the future of transcatheter aortic valve replacement: integrating computational modeling with patient-specific clinical care.

INTRODUCTION: Transcatheter aortic valve replacement (TAVR) is rapidly evolving to become the standard of care for aortic stenosis (AS) patients, as opposed to open-heart surgery. The emergence of new commercial TAVR devices, as well as technological advancements in AS diagnosis and procedural planning, combat post-TAVR complications but pose new challenges with patient selection and optimal timing of valve replacement. Computational modeling is a powerful tool that may help streamline solutions for heart teams facing these complexities. AREAS COVERED: This narrative review outlines the evolv

Modeling Patient-Specific Muscular Dystrophy Phenotypes and Therapeutic Responses in Reprogrammed Myotubes Engineered on Micromolded Gelatin Hydrogels.

In vitro models of patient-derived muscle allow for more efficient development of genetic medicines for the muscular dystrophies, which often present mutation-specific pathologies. One popular strategy to generate patient-specific myotubes involves reprogramming dermal fibroblasts to a muscle lineage through MyoD induction. However, creating physiologically relevant, reproducible tissues exhibiting multinucleated, aligned myotubes with organized striations is dependent on the introduction of physicochemical cues that mimic the native muscle microenvironment. Here, we engineered patient-specifi

Modeling Human Bile Acid Transport and Synthesis in Stem Cell-Derived Hepatocytes with a Patient-Specific Mutation.

The bile salt export pump (BSEP) is responsible for the export of bile acid from hepatocytes. Impaired transcellular transport of bile acids in hepatocytes with mutations in BSEP causes cholestasis. Compensatory mechanisms to regulate the intracellular bile acid concentration in human hepatocytes with BSEP deficiency remain unclear. To define pathways that prevent cytotoxic accumulation of bile acid in hepatocytes, we developed a human induced pluripotent stem cell-based model of isogenic BSEP-deficient hepatocytes in a Transwell culture system. Induced hepatocytes (i-Heps) exhibited defects i

Patient specific modeling of palpation-based prostate cancer diagnosis: effects of pelvic cavity anatomy and intrabladder pressure.

Computational modeling has become a successful tool for scientific advances including understanding the behavior of biological and biomedical systems as well as improving clinical practice. In most cases, only general models are used without taking into account patient-specific features. However, patient specificity has proven to be crucial in guiding clinical practice because of disastrous consequences that can arise should the model be inaccurate. This paper proposes a framework for the computational modeling applied to the example of the male pelvic cavity for the purpose of prostate cancer

Modeling neural crest induction, melanocyte specification, and disease-related pigmentation defects in hESCs and patient-specific iPSCs.

Melanocytes are pigment-producing cells of neural crest (NC) origin that are responsible for protecting the skin against UV irradiation. Pluripotent stem cell (PSC) technology offers a promising approach for studying human melanocyte development and disease. Here, we report that timed exposure to activators of WNT, BMP, and EDN3 signaling triggers the sequential induction of NC and melanocyte precursor fates under dual-SMAD-inhibition conditions. Using a SOX10::GFP human embryonic stem cell (hESC) reporter line, we demonstrate that the temporal onset of WNT activation is particularly critical
