Consequences of Patient-Ventilator Discordance.

The impact of patient-ventilator asynchrony is still unclear. Initial studies in patients with asynchrony showed that it is associated with negative physiologic and clinical outcomes. However, these studies were narrowly focused in many different ways, including the duration of observation and the types of asynchronies included in the study. Research has since not only identified a newly described asynchrony, reverse triggering, but also identified potentially important modifiers of clinical outcomes such as asynchronies occurring in clusters. Our understanding of the magnitude of the patient'

A randomized clinical trial of neurally adjusted ventilatory assist versus conventional weaning mode in patients with COPD and prolonged mechanical ventilation.

BACKGROUND: Patient-ventilator asynchrony is a common problem in mechanically ventilated patients; the problem is especially obvious in COPD. Neutrally adjusted ventilatory assist (NAVA) can improve patient-ventilator asynchrony; however, the effect in COPD patients with prolonged mechanical ventilation is still unknown. The goals of this study are to evaluate the effect of NAVA and conventional weaning mode in patients with COPD during prolonged mechanical ventilation. METHODS: The study enrolled a total of 33 COPD patients with ventilator dependency for more than 21 days in the weaning cent

Patient-Ventilator Interaction During Noninvasive Ventilation.

Patient-ventilator discordance commonly occurs in patients receiving noninvasive ventilation (NIV). Unintentional leaks contribute to trigger and cycle asynchrony. Historically, ventilators designed for NIV with an intentional leak circuit compensated for unintentional leaks better than critical care ventilators. However, many critical care ventilators now have NIV settings for leak compensation. Per bench studies and some clinical studies, NIV settings for leak compensation variably improve synchrony related to leak. Some interfaces, such as the helmet, can contribute to asynchrony. Ventilato

How I Teach Mechanical Ventilator Waveform Analysis: A Focus on Normal Ventilator Waveforms.

Identification and management of patient-ventilator asynchronies is a fundamental skill for the management of mechanical ventilation and often requires interpretation of mechanical ventilation waveform scalars. However, teaching mechanical ventilation waveform analysis can be challenging for educators due to the complexity of the material. In this entry of How I Teach, we provide an approach to teaching the concept of "normal" mechanical ventilator waveforms as an introduction to the broader topic of patient-ventilator asynchronies. We stress that intimate knowledge of normal mechanical ventil

How to Interpret Ventilator Waveforms Using the Taxonomy for Modes of Mechanical Ventilation.

Mechanical ventilation is a complex yet essential aspect of critical care. This article provides a brief review of historical developments related to descriptions of ventilators, the derivation of the foundational equation of motion for the respiratory system and its derivatives, and a short overview of the taxonomy for modes of ventilation. These topics are then applied in a step-by-step procedure for interpreting patient-ventilator interactions through waveform analysis.

Characteristics of the Five Different Types of Intermittent Mandatory Ventilation.

Intermittent mandatory ventilation (IMV) has evolved substantially from its original form, developing into 5 distinct types intended to enhance patient safety, comfort, and liberation from mechanical ventilation. Each form of IMV has different potential advantages and disadvantages that should be considered when selecting the mode of ventilation. IMV(1): the set mandatory breath rate is always delivered. IMV(2): mandatory breaths may be suppressed by spontaneous breaths if their frequency is high enough. IMV(3): mandatory breaths may be suppressed by spontaneous breaths if they achieve some pr

Defining and Measuring Patient-Ventilator Interactions: 10 Fundamental Maxims.

Patient-ventilator interaction (PVI) has been extensively addressed in the literature, but there is no agreement on terms and definitions used. The result is that articles and books present concepts and terms that are inconsistent, confusing, and many times based on obsolete legacy ideas. To solve this problem, we propose a set of basic rules and standard nomenclature from which to establish a formal taxonomy and basis for its measurement. We also highlight some areas for research on PVI definitions, measurement, and clinical needs. Taxonomies have 2 components: a set of definitions relevant t

Predictors of asynchronies during assisted ventilation and its impact on clinical outcomes: The EPISYNC cohort study.

PURPOSE: To investigate if respiratory mechanics and other baseline characteristics are predictors of patient-ventilator asynchrony and to evaluate the relationship between asynchrony during assisted ventilation and clinical outcomes. METHODS: We performed a prospective cohort study in patients under mechanical ventilation (MV). Baseline measurements included severity of illness and respiratory mechanics. The primary outcome was the Asynchrony Index (AI), defined as the number of asynchronous events divided by the number of ventilator cycles and wasted efforts. We recorded ventilator waveform

Neurally adjusted ventilatory assist (NAVA) improves patient-ventilator interaction during non-invasive ventilation delivered by face mask.

PURPOSE: To determine if, compared to pressure support (PS), neurally adjusted ventilatory assist (NAVA) reduces patient-ventilator asynchrony in intensive care patients undergoing noninvasive ventilation with an oronasal face mask. METHODS: In this prospective interventional study we compared patient-ventilator synchrony between PS (with ventilator settings determined by the clinician) and NAVA (with the level set so as to obtain the same maximal airway pressure as in PS). Two 20-min recordings of airway pressure, flow and electrical activity of the diaphragm during PS and NAVA were acquired

[Patient-noninvasive mechanical ventilation interaction].

Noninvasive mechanical ventilation is one more step in the treatment of patients with acute respiratory failure. In addition to gas exchange disorders, its primary indication to initiate it is the presence of signs of respiratory muscles fatigue. To assure successful mechanical ventilation, the ventilator and patient must be synchronized, that is, the effort the patient makes to start inspiration is recognized by the ventilator and it quickly delivers gas flow, that the flow provided by the ventilator adapts to the flow need of the patient during delivery of gas phase and that the ventilator r

Ventilator graphics and respiratory mechanics in the patient with obstructive lung disease.

Obstruction of the large and small airways occurs in several diseases, including asthma, chronic obstructive pulmonary disease, cystic fibrosis, bronchiectasis, and bronchiolitis. This article discusses the role of ventilator waveforms in the context of factors that contribute to the development of respiratory failure and acute respiratory distress in patients with obstructive lung disease. Displays of pressure, flow, and volume, flow-volume loops, and pressure-volume loops are available on most modern ventilators. In mechanically ventilated patients with airway obstruction, ventilator graphic

Patient-Ventilator Synchronization During Non-invasive Ventilation: A Pilot Study of an Automated Analysis System.

Background: Patient-ventilator synchronization during non-invasive ventilation (NIV) can be assessed by visual inspection of flow and pressure waveforms but it remains time consuming and there is a large inter-rater variability, even among expert physicians. SyncSmart™ software developed by Breas Medical (Mölnycke, Sweden) provides an automatic detection and scoring of patient-ventilator asynchrony to help physicians in their daily clinical practice. This study was designed to assess performance of the automatic scoring by the SyncSmart software using expert clinicians as a reference in patien

Patient-ventilator interaction.

Patient-ventilator interaction has been the focus of increasing attention from both manufacturers and researchers during the last 25 years. There is now compelling evidence that passive (controlled) mechanical ventilation leads to respiratory muscle dysfunction and atrophy, prolonging the need for ventilatory support and predisposing to a number of adverse patient outcomes. Although there is consensus that the respiratory muscles should retain some activity during acute respiratory failure, patient-ventilator asynchrony is now recognized as a cause of ineffective ventilation, impaired gas exch

Discordance, Pendelluft, and Electrical Impedance Tomography.

Pendelluft, the internal redistribution of gas within the lung, was first described as a phenomenon related to high breathing frequencies and regional differences in time constants. The availability of electrical impedance tomography has transformed our understanding of this form of intrapulmonary discordance, revealing that effort-related pendelluft occurs commonly in mechanically ventilated patients and may contribute to patient self-inflicted lung injury. Emerging evidence suggests associations between pendelluft and adverse clinical outcomes, though meaningful gaps in our understanding rem

Patient-Ventilator Interaction: Timing Discordance in Invasive Ventilation.

Mechanical ventilation goals include restoration of gas exchange, lung protection, and patient comfort. Improving patient comfort through synchronous patient-ventilator interaction represents a common clinical challenge in the management of these patients. Patient-ventilator discordance describes a mismatch between the patient's intrinsic respiratory timing and effort with the ventilatory support provided by the ventilator. Achieving concordance requires an understanding of physiologic principles related to the control of breathing, expertise in recognition of discordance using the pressure an

Automated Detection of Ventilator Asynchronies: A Clinical and Technological Perspective.

Patient-ventilator asynchrony is highly prevalent during invasive mechanical ventilation, yet its detection at the bedside remains limited. Conventional waveform inspection is intermittent, operator-dependent, and insufficient to capture the complexity and temporal variability of patient-ventilator interaction. Automated systems based on advanced signal processing and artificial intelligence represent a paradigm shift, enabling continuous, objective, and scalable detection of asynchrony. Recent approaches highlight the value of entropy-based metrics to quantify the irregularity of ventilatory

Additional work of breathing from trigger errors in mechanically ventilated children.

BACKGROUND: Patient-ventilator asynchrony is associated with increased morbidity and mortality. A direct causative relationship between Patient-ventilator asynchrony and adverse clinical outcome have yet to be demonstrated. It is hypothesized that during trigger errors excessive pleural pressure swings are generated, contributing to increased work-of-breathing and self-inflicted lung injury. The objective of this study was to determine the additional work-of-breathing and pleural pressure swings caused by trigger errors in mechanically ventilated children. METHODS: Prospective observational s
