Modification of the Abramson procedure for minimally invasive repair of pectus carinatum: introduction of a pectus carinatum compression system.

Pectus carinatum is characterized by a protruding sternum. This deformity can be surgically corrected through the minimally invasive Abramson technique. In this procedure, a presternal metal correctional bar, secured to rib-attached stabilizers, is implanted to redress the sternum to a neutral position. To anticipate the intended position of the sternum, manual compression is applied over the sternal deformity. We describe a modified version of the Abramson procedure, encompassing a table-mounted PectusAssist™ System which generates a constant mechanical compression over the protruding sternum

Repair of pectus carinatum, carinatum/excavatum complex patients with doubly double bar technique.

Surgical correction is needed for patients with pectus carinatum who do not adapt to bracing therapy. We performed the doubly double bar technique for ten patients who did not adapt to bracing therapy for patients with pectus carinatum and/or carinatum/excavatum complex type. A complete correction was achieved for all patients, and there were no complications. Our initial experience suggests that the doubly double bar technique can be performed effectively for pectus carinatum and/or carinatum/excavatum complex type patients.

Bracing of pectus carinatum: A quantitative analysis.

BACKGROUND/PURPOSE: Primary treatment of pectus carinatum (PC) is performed with an external brace that compresses the protrusion. Patients are 'prescribed' a brace tightening force. However, no visual guides exist to display this force magnitude. The purpose of this study was to determine the repeatability of patients in applying their prescribed force over time and to determine whether the protrusion stiffness influences the patient-applied forces and the protrusion correction rate. METHODS: Twenty-one male participants (12-17years) with chondrogladiolar PC were recruited at the time of bra

Effect of the compressive brace in pectus carinatum.

BACKGROUND: Patients with pectus carinatum complain of cosmetic problems because they stand out in spite of wearing clothes. Surgical treatment of pectus carinatum is resection of the deformed cartilage but a wide operative scar, postoperative pain and complications related with the operation can occur. Therefore we have performed compressive brace therapy as a non-operative treatment of pectus carinatum, and observed the effects and the efficiency of this treatment. MATERIALS AND METHODS: From January 2001 to December 2007, 119 patients with pectus carinatum were treated with a compressive b

Results of operative treatment of the pectus carinatum.

AIM OF THIS STUDY: Evaluation of the efficiency of the operative treatment for pectus carinatum depending on the type of deformity. MATERIAL AND METHODS: 65 subjects following surgical procedures due to pectus carinatum deformity between 1997 and 2003 were analyzed. Surgical correction of the deformity depended on the localization of the sternal protrusion, grade of asymmetry and severity of the deformity. Radiological severity index of the deformity (Haller's index-HI) and patients' subjective evaluation were collected to assess the grade of deformity and the results of treatment. RESULTS:

Surgical correction of pectus excavatum and carinatum.

The author presents three decades of experience in the management of anterior chest wall deformities. During this period more than 800 operations were performed on patients with pectus excavatum and carinatum. In this series, there was no death and serious complications were rare. The author believes that the principles on which surgical treatment of pectus excavatum should be based are as follows: (1) bilateral removal of the "culprit" costal cartilages, (2) adequate mobilization of the sternum and correction of the sternal positional deformity by transverse osteotomy, (3) stabilizing the cor

[Costal cartilage changes in children with pectus excavatum and pectus carinatum].

UNLABELLED: Pectus excavatum (PE) and pectus carinatum (PC) in children are the most common congenital deformities that cause complications in the thoracic organs; however, the role of chondrocytes and cartilage canals in the pathogenesis of these conditions remains unexplored. OBJECTIVE: To investigate qualitative and quantitative changes of cartilage lacunae and canals in the costal cartilages in children with PE and PC compared to those with normal chests. SUBJECT AND METHODS: Costal cartilages were investigated in 10 children with normal chests (a control group), in 12 children with PE,

Acute anterior myocardial infarction with pectus carinatum.

We presented a case of acute anterior myocardial infarction caused by left anterior descending artery occlusion in a patient with pectus carinatum. The electrocardiogram (ECG) on admission showed counterclockwise rotation and T wave inversion only in leads V1-V2. Computed tomography revealed that this patient with pectus carinatum had greater septal angle. Electrocardiographic counterclockwise rotation due to greater septal angle in pectus carinatum led to atypical ECG findings of acute myocardial infraction.

PECTUS CARINATUM: A NOVEL METHOD OF STERNAL FIXATION.

The traditional method for fixing the sternum during surgical repair of pectus carinatum is through the use of a stainless steel bar (Adkin’s strut). In this article we describe a new method of sternal fixation using nonabsorbable sutures which are placed in a transverse and crossed fashion anterior to the sternum. This method provides stable sternal fixation and spares the patient a second operation to remove the steel bar. The absence of metallic implants allows clearer view of the thoracic structures in future X-rays, CT scans and MRI, and is likely to be more acceptable to patients than th

[The outcome of modified procedure for pectus carinatum correction].

OBJECTIVE: To improve the surgical procedure to correct pectus carinatum. METHODS: From 1990 to 2003, 9 patients with pectus carinatum were treated, whose ages ranged from 3 years and 6 months to 16 years. The conventional operation was performed on 4 patients, the modified procedure on 5 patients. The modified procedure included: (1) the shortening rib periosteum was not sutured transversally; (2) the corrected position of the sternum was stabilized with the metal strut. RESULTS: In 4 patients corrected by the conventional procedure, the sternum depression as pectus excavatum occurred in 1

[Surgical correction of pectus carinatum: a case report].

A 12-year-old girl with a keel chest, underwent cosmetic repair of the severe deformity. The deformity was corrected by subperichondrial resection of the all deformed costal cartilages, transverse osteotomy through the anterior cortex of the sternum, detachment of the xiphoid process, bending of the posterior cortex of the sternum, and resection of the lower end of the sternum. The sternum was maintained in its corrected position by traction force of the rectus muscles through the reattached xiphoid and the shortened perichondrial tissues and by reapproximating the pectoral flaps in the midlin

Echocardiography Reveals Heart Abnormalities in Pediatric Pectus Carinatum.

BACKGROUND: Symptoms including chest pain and palpitations are commonly described by pediatric patients with pectus deformity. Cardiac anomalies are thought to be less common in patients with pectus carinatum (PC) than those in patients with pectus excavatum; however, no literature supports this presumption. Echocardiogram (echo) assesses heart structure and function. We hypothesized that a screening echo would 1) determine the relationship between symptoms and echo findings and 2) define the incidence of cardiac defects in patients with PC. MATERIALS AND METHODS: This is an institutional rev

Surgical management of pectus carinatum: 30 years' experience.

Pectus carinatum is an uncommon malformation that is often more symptomatic than the appearance suggests, and one that physicians often do not refer for surgical correction. Hospital records of 90 patients who underwent repair of pectus carinatum deformities between 1970 and 2000 were reviewed. During the same period another 445 patients underwent repair of excavatum deformities. Minimal deformity was observed before the age of 10 years for 81 of 90 patients; only 7 of the 90 underwent repair before age 11 years. All patients were symptomatic; 84 had exertional dyspnea and exercise limitation,
