Conservative treatments are easy to handle, have no side effects and no negative effects concerning later surgical treatments and are not expensive. Therefore conservative treatments should be the first line therapies for pelvic floor disorders. The following treatment options are used: lifestyle interventions, bladder training, use of pessaries, pelvic floor muscle training, pharmacologic treatment, incontinence aids/products and complementary medicine. Important for the success of these treatments are motivation and patience of the patient as well a good team consisting of doctors, specialis

The pelvic floor is a dome-shaped striated muscular sheet that encloses the bladder, uterus, and rectum, and, together with the anal sphincters, has an important role in regulating storage and evacuation of urine and stool. This article reviews the anatomy, nerve supply, pharmacology, and functions of the anal sphincters and the pelvic floor. The internal and external anal sphincters are primarily responsible for maintaining faecal continence at rest and when continence is threatened, respectively. Defecation is a somato-visceral reflex regulated by dual nerve supply (i.e. somatic and autonomi

Posterior pelvic floor compartment disorders generally refer to functional anorectal disturbances that by definition are symptom-based rather than anatomical defect-based and have a significant impact on quality of life. Symptoms attributed to the posterior compartment are often non-specific and associated with structural, neuromuscular and functional defects giving rise to symptoms of prolapse, pelvic pressure, faecal incontinence, stool trapping and constipation. They may range from mild to incapacitating and occur in varying combinations. While symptoms of constipation and incontinence may

BACKGROUND: Pelvic floor surface electromyography (sEMG) is widely used to evaluate and treat pelvic floor dysfunctions (PFDs). Based on sEMG, the Glazer protocol was developed over 20 years ago with a limited sample size, making it challenging to accurately diagnose PFDs across diverse populations and conditions. This study aims to establish a multidimensional database for monitoring pelvic floor sEMG, derive more reasonable reference ranges for sEMG parameters, and achieve accurate diagnosis of PFDs through artificial intelligence (AI).
METHODS: In this population-based, multicenter, cross-

OBJECTIVE: To investigate the diagnostic value of pelvic floor ultrasound parameters in combination for pelvic floor dysfunction (PFD), and to explore the risk factors.
METHODS: Forty PFD patients treated from April2019to December 2020(case group) and another 40 healthy women (control group) were enrolled. Their clinical data were collected, and both groups received three-dimensional (3D) ultrasound of the pelvic floor. The diagnostic value of pelvic floor ultrasound parameters for PFD was assessed by receiver operating characteristic (ROC) curves. The risk factors of PFD were evaluated by mu

AIMS: Several randomized controlled trials have demonstrated that pelvic floor muscle training is effective to treat stress urinary incontinence. The aim of the present study was to compare muscle strength increase and maximal strength in responders and non-responders to pelvic floor muscle training.
MATERIALS AND METHODS: Fifty-two women with urodynamically proven stress incontinence who had participated in a six months randomized controlled trial on pelvic floor muscle training, mean age 45.4 years (range 24-64), participated in the study. The women were classified as responders and non-res

BACKGROUND Pelvic floor dysfunction (PFD) significantly reduces quality of life. During horseback riding, the movement of the horse's back affects the tension of the pelvic floor muscles. This study aimed to compare reported symptoms of pelvic floor dysfunction in women with past, current, and no history of horse show jumping. MATERIAL AND METHODS We examined 160 women (age 23.69±3.96 years). The cohort group consisted of 74 women training currently and training in the past, and a control group of 86 women not practicing horseback riding. The study used the original questionnaire and the Austr

AIM: To measure the neuromuscular activation of the pelvic floor and abdominal muscles concurrently with vaginal closure forces induced during a hypopressive exercise (HE) and to identify the contribution of the HEs sequences (posture and maneuver) in the muscle's activation.
METHODS: A cross-sectional study design was employed. Sixty-six women who had participated in a physical therapy program focused on HEs were recruited. Pelvic floor muscle (PFM) activation was measured using surface electromyography (sEMG) in supine and in the orthostatic position, and vaginal closure force was measured

INTRODUCTION: An important cause or maintaining factor for pain in Urological Chronic Pelvic Pain Syndrome (UCPPS) may be pelvic floor muscle (PFM) dysfunction, which may also be implicated in sexual dysfunction and influenced by psychosocial factors. Pelvic floor ultrasound is a noninvasive, reliable, and relatively simple method to assess PFM morphology and function and can be assessed by the anorectal angle (ARA) and levator plate angle (LPA).
AIMS: The aim of the present study was to examine PFM morphology in men with UCPPS as compared with controls and to examine the correlation with pai

The pelvic floor is a complex structure that supports the pelvic organs and provides resting tone and voluntary control of the urethral and anal sphincters. Dysfunction of or injury to the pelvic floor can lead to gastrointestinal, urinary, and sexual dysfunction. The prevalence of pelvic floor disorders is much lower in men than in women, and because of this, the majority of the published literature pertaining to MRI of the pelvic floor is oriented toward evaluation of the female pelvic floor. The male pelvic floor has sex-specific differences in anatomy and pathophysiologic disorders. Despit

Understanding the anatomic relationship of the pelvic floor muscles with the pelvic girdle, spine, and hips aids the rehabilitation provider in diagnosis, management, and appropriate referrals. The bony anatomy of the pelvic girdle consists of 3 bones and 3 joints. The pelvic floor muscles are comprised mainly of the levator ani muscles with somatic innervation from the lumbosacral plexus. The bony and muscular pelvis is highly interconnected to the hip and gluteal musculature, which together provide support to the internal organs and core muscles. Pelvic floor physiology is centered on bladde

This article discusses the background and appraisal of endoluminal ultrasound of the pelvic floor. It provides a detailed anatomic assessment of the muscles and surrounding organs of the pelvic floor. Different anatomic variability and pathology, such as prolapse, fecal incontinence, urinary incontinence, vaginal wall cysts, synthetic implanted material, and pelvic pain, are easily assessed with endoluminal vaginal ultrasound. With pelvic organ prolapse in particular, not only is the prolapse itself seen but the underlying cause related to the anatomic and functional abnormalities of the pelvi

The clinical treatment of patients with anorectal and pelvic floor dysfunction is often difficult. Dynamic cystocolpoproctography (DCP) has evolved from a method of evaluating the anorectum for functional disorders to its current status as a functional method of evaluating the global pelvic floor for defecatory disorders and pelvic organ prolapse. It has both high observer accuracy and a high yield of positive diagnoses. Clinicians find it a useful diagnostic tool that can alter management decisions from surgical to medical and vice versa in many cases. Functional radiography provides the maxi

Vaginal delivery is known to be associated with trauma to the pelvic floor (PF) and its muscles and ligaments. In some cases, this can lead to issues with urinary incontinence and pelvic organ descent postpartum, either immediately or in the woman's later years. Some factors during a vaginal delivery can be modified in attempts to preserve as much functional integrity to the PF as possible. A trend has evolved in recent years regarding maternal requests for primary cesarean section (CS), with one goal being the reduction in PF trauma. However, evidence to date refutes this as a foolproof pract
