Anal sphincter injury in women with pelvic floor disorders.

OBJECTIVE: 1) To estimate the rate of anal incontinence and anal sphincter injury in a group of women with pelvic floor disorders; 2) to evaluate the relationship between anal incontinence and anal sphincter injury as demonstrated by endoanal ultrasonography; 3) to explore any associations between operative vaginal delivery and anal sphincter injury in this population. METHODS: A cohort of 100 women with stage II or greater pelvic organ prolapse and/or urinary incontinence completed the Rockwood-Thompson Fecal Incontinence Severity Index Questionnaire (FISI). Pelvic organ prolapse was recorde

Management of pelvic floor disorder in a diethylstilbestrol daughter.

Between 1947 and 1971, the synthetic estrogen diethylstilbestrol (DES) was prescribed to pregnant women to prevent spontaneous abortions. Many studies have demonstrated adverse consequences of prenatal DES exposure. This report describes a DES daughter treated for uterine cancer who presented with a foreshortened vagina of 4 cm and stage III pelvic organ prolapse. She underwent successful vaginal native tissue reconstructive surgery with uterosacral colpopexy and anterior and posterior repair to lengthen her vagina and treat her prolapse. She then developed stress incontinence and underwent pe

The Epidemiology of Pelvic Floor Disorders and Childbirth: An Update.

Using a lifespan model, this article presents new scientific findings regarding risk factors for pelvic floor disorders (PFDs), focusing on the role of childbirth in the development of single or multiple coexisting PFDs. Phase I of the model includes predisposing factors, such as genetic predisposition and race. Phase II includes inciting factors, such as obstetric events. Prolapse, urinary incontinence (UI), and fecal incontinence (FI) are more common among vaginally parous women, although the impact of vaginal delivery on risk of FI is less dramatic than prolapse and UI. Phase III includes i

[The role of ultrasound in the exploration of pelvic floor disorders].

Dynamic ultrasound, especially perineal and introital, allows the appreciation of the prolapses (cystoptosis, bladder neck and urethral mobility, enterocele, rectocele). It remains, however, clearly more limited in the precise study of posterior colpoceles, and especially in anorectal disorders, than colpocystodefecography or dynamic MRI. Endoanal ultrasound is the first line morphological examination of the anal sphincter. Perineal and introital ultrasound examinations are useful to appreciate certain complications with suburethral tape and pelvic mesh. For an appreciaton of the morphology of

Progress in the genetics and epigenetics of pelvic floor disorder.

Pelvic floor disorder (PFD) is a common gynecological disorder, and with the ageing of the population, PFD has a serious impact on the physical and mental health of patients and their quality of life. The most prominent of these are pelvic organ prolapse (POP) and urinary incontinence (UI), about which the etiology is still unclear, and it is urgent to explore their pathogenesis. Advances in genetics and epigenetics have provided new insights into the pathophysiology of PFD. Candidate genes and genome-wide association studies have identified susceptibility genes for POP and UI. These susceptib

The Role of MRI in the Diagnosis of Pelvic Floor Disorders.

PURPOSE OF REVIEW: This article provides an overview of MRI as a dynamic imaging modality as well as recent updates on the role of MRI in the evaluation of female pelvic floor disorders, including pelvic organ prolapse (POP), rectal prolapse, and defecatory dysfunction. RECENT FINDINGS: Pelvic floor disorders in women are common, and demand for treatment is high. Recently, multidisciplinary guidelines for dynamic MRI have been published to reflect this need. Despite criticisms that MRI is an expensive modality, cost-analysis studies are demonstrating utility for MRI in surgical decision-makin

Raising Awareness of Pelvic Floor Disorders Among Cultural Health Brokers to Improve Refugee and Immigrant Women's Pelvic Floor Health.

INTRODUCTION AND HYPOTHESIS: Pelvic Floor Disorders (PFDs) are common and negatively affect women's quality of life. Despite availability of treatments, women often do not seek medical care. Patient knowledge is a known barrier to accessing care. It has been shown that knowledge of PFDs is lower in Canadian immigrant communities. We hypothesized that educating Cultural Health Brokers (CHBs) on PFD will significantly increase the CHBs' knowledge and empower them to effectively communicate this knowledge to their communities, improving awareness and access to care among immigrant women. METHODS

Compliance and Adherence to Pelvic Floor Exercise Therapy in People with Pelvic Floor Disorders: A Systematic Review and Meta-Analysis.

BACKGROUND: The impact of muscle-training treatment on quality of life and functional outcomes in people with pelvic floor dysfunction may be related to adherence rates. METHODS: Nine electronic databases were searched for studies published up to 15 October 2024. A qualitative synthesis was used to describe the relationship between adherence or compliance with treatment, quality of life, and symptomatic severity. A meta-analysis of data from selected studies was performed that assessed quality of life and symptomatic severity in the short term. RESULTS: Seven studies with 2190 participants w

Social Determinants and Time to Surgery for Pelvic Floor Disorders.

IMPORTANCE: Patients with social determinants of health (SDoH)-related barriers have reduced access to care for pelvic floor disorders (PFDs), but whether SDoH influence time to surgical decision making is unknown. OBJECTIVES: We aimed to assess the association of various SDoH with time to surgical decision making in patients who had surgery for a PFD. STUDY DESIGN: We conducted a retrospective cross-sectional study of patients who underwent surgery for overactive bladder, stress urinary incontinence, or pelvic organ prolapse between January 2013 and December 2023 at a large metropolitan hea

Postpartum Anorectal and Pelvic Floor Disorders: Evaluation, Treatment, and Prevention.

PURPOSE OF REVIEW: Postpartum anorectal and pelvic floor disorders (PFD) are common, though under-recognized. There is limited knowledge regarding their diagnosis, treatment, and prevention. Here we provide a critical review of this topic and highlight knowledge gaps and treatment options for these problems. RECENT FINDINGS: Recent advances include dynamic 3D and 4D ultrasound of the pelvic floor to reveal pathology, anal sphincter defects, and pelvic organ prolapse. Treatments for fecal incontinence include anal inserts, vaginal inserts, translumbosacral neuromodulation therapy (not yet stud

Primary care providers practice patterns regarding female pelvic floor disorders.

INTRODUCTION: Pelvic floor disorders (PFDs) pose substantial physical and psychological burdens for a growing number of women. Given the ubiquity of these conditions and known patient reluctance to seek care, primary care providers (PCPs) have a unique opportunity to increase treatment and provide appropriate referrals for these patients. METHODS: An online survey was administered to PCPs to assess provider practices, knowledge, comfort managing and ease of referral for PFDs. Logistic regression was used to assess the association between demographic/practice characteristics of PCPs and two pr

Benign Anorectal Disorders and Pelvic Floor Disease After Bariatric Surgery.

The correlations between morbid obesity, bariatric surgery and gastrointestinal disorders are well known and reported. Symptoms like abdominal pain, constipation, bloating, heartburn and gastroesophageal reflux disease are known to be significantly more prevalent in overweight patients and body mass index is associated positively with abdominal pain and diarrhea. In spite of these conditions, less is known about the lower gastrointestinal tract. Of these, anorectal disorders and pelvic floor disease are both believed to be more frequent in obese patients compared to the general population. Wei

Association of Delivery Mode With Pelvic Floor Disorders After Childbirth.

IMPORTANCE: Pelvic floor disorders (eg, urinary incontinence), which affect approximately 25% of women in the United States, are associated with childbirth. However, little is known about the course and progression of pelvic floor disorders over time. OBJECTIVE: To describe the incidence of pelvic floor disorders after childbirth and identify maternal and obstetrical characteristics associated with patterns of incidence 1 to 2 decades after delivery. DESIGN, SETTING, AND PARTICIPANTS: Women were recruited from a community hospital for this cohort study 5 to 10 years after their first deliver

Prevalence of symptomatic pelvic floor disorders among gynecologic oncology patients.

OBJECTIVE: To describe the prevalence of urinary incontinence and pelvic organ prolapse (POP) in patients with gynecologic cancer before cancer treatment. METHODS: A screening questionnaire on pelvic floor dysfunction was administered as part of the baseline health questionnaire to 549 consecutive new patients presenting to a gynecologic oncology practice. Patients were asked whether they felt a bulge from their vagina or experienced loss of urine associated with activity or urge to urinate. The prevalence of urinary incontinence, POP, or both was determined for each malignancy and benign con

Medical and surgical management of pelvic floor disorders affecting defecation.

Pelvic floor disorders that affect stool evacuation include structural (for example, rectocele) and functional disorders (for example, dyssynergic defecation (DD)). Meticulous history, digital rectal examination (DRE), and physiological tests such as anorectal manometry, colonic transit study, balloon expulsion, and imaging studies such as anal ultrasound, defecography, and static and dynamic magnetic resonance imaging (MRI) can facilitate an objective diagnosis and optimal treatment. Management consists of education and counseling regarding bowel function, diet, laxatives, most importantly be

Organising a clinical service for patients with pelvic floor disorders.

The evolution of the multidisciplinary approach to the management of chronic conditions is a reflection of how medicine has evolved from a singular to a plural effort recognising the complex causations and consequences of such disorders. This thinking should not be confined to tertiary centres alone and should be adapted where local expertise is available. Such an approach is especially important in pelvic floor disorders, where the correlation between structure and function is not always straightforward. There is a need to avoid over-investigation by accurate clinical assessment allied to tai
