Magnetic resonance imaging pelvimetric measurements as predictors for emergent cesarean delivery in obstructed labor.

OBJECTIVE: This study aimed to investigate the usefulness of various magnetic resonance imaging (MRI) pelvimetric parameters for predicting emergent cesarean delivery due to obstructed labor. STUDY DESIGN: This was a prospective observational study. MRI pelvimetry was performed in cases of a clinically suspected maternal narrow pelvis, maternal short stature, fetal overgrowth, and abnormal placental position. MRI pelvimetry was performed at 34.7 ± 4.2 gestational weeks using a 1.5 T MRI system. The pelvic inlet angle, pelvic inclination, obstetric conjugate, sacral outlet diameter (SOD), and

Radiographic study of the axial and appendicular skeleton of Callithrix jacchus and Callithrix penicillata.

BACKGROUND: Marmosets are small arboreal primates. Knowledge of normal radiographic parameters and frequent disorders is limited. The objectives of this study were to evaluate possible variations between quantitative analyses and injuries to the appendicular and axial skeleton of Callithrix jacchus and Callithrix penicillata and their relationship with the environment. METHODS: Radiographic images of 29 marmosets were used. Skeletal lesions were described, long bones were measured, and pelvimetry, vertebrae, and sternebras were counted. RESULTS: Measurements of long bones and pelvimetry were

[Obstetrical study of the pelvis: a historical overview].

The female pelvis is fundamental to life itself, but it took ages to grasp this truism. During Antiquity, physicians had no need for the concepts of pelvic dystocia and cephalopelvic disproportion. When Aranzo (16th century), a practising physician, stated that a disproportion between the size of the fetus and that of the bony pelvis was the main culprit of difficult labor, his suggestion was overlooked, so much so that it was Hendrik van Deventer (1701) who came to be coined "father of the pelvic theory". The need for an accurate knowledge of the dimensions of the maternal pelvis and of the f

Three-dimensional visualization of the total mesorectal excision plane for dissection in rectal cancer surgery and its ability to predict surgical difficulty.

Total mesorectal excision (TME) for rectal cancer is often technically challenging. We aimed to develop a method for three-dimensional (3D) visualization of the TME dissection plane and to evaluate its ability to predict surgical difficulty. Sixty-six patients with lower rectal cancer who underwent robot-assisted surgery were retrospectively analyzed. A 3D TME dissection plane image for each case was reconstructed using Ziostation2. Subsequently, a novel index that reflects accessibility to the deep pelvis during TME, namely, the TME difficulty index, was defined and measured. Representative b

Maternal and neonatal short-term outcome after vaginal breech delivery >36 weeks of gestation with and without MRI-based pelvimetric measurements: a Hannover retrospective cohort study.

OBJECTIVES: Planning the mode of delivery of a full-term breech singleton remains a challenging task. The aim of this work is to compare the neonatal and maternal short-term outcomes after planned vaginal delivery and caesarean section and to evaluate the influence of an MRI pelvimetry on the short-term outcomes in order to provide appropriate advice to pregnant women with breech presentation. METHODS: This is a retrospective monocentric analysis of all deliveries with singleton pregnancies from breech presentation >36 + 0 weeks of gestation between 08/2021 and 09/2023. Short-term maternal an

[MRI-based prognostic factors predicting surgical complexity in rectal cancer].

BACKGROUND: Colorectal cancer is one of the most prevalent oncological diseases, with rectal cancer accounting for a substantial proportion of cases. Surgical intervention remains the cornerstone of treatment, with its success significantly influenced by individual pelvic anatomy. Nevertheless, there is a notable scarcity of studies specifically focused on evaluating surgical complexity based on pelvic MRI pelvimetry. OBJECTIVE: To identify the prognostic factors of pelvic MRI pelviometry that affect the complexity of surgical treatment of rectal tumors. MATERIAL AND METHODS: A retrospective

Evaluation of the natural birth prediction system for assessing cephalopelvic disproportion: a multicentre prospective study.

OBJECTIVES: To assess the potential value of the Natural birth prediction system (NBPS) on predicting cephalopelvic disproportion (CPD) and compare the value with a pelvimetry model and the foetal-pelvic index. METHODS: A multicentre prospective study was conducted between November 2023 and September 2024 in three hospitals in China. Singleton nulliparas suspected with CPD and willing to attempt vaginal delivery were included. Patients were categorised according to NBPS results (absence CPD, borderline CPD, and presence CPD). The software results were compared with actual delivery modes. Rece

Ambulatory urodynamics: extramural testing of the lower and upper urinary tract by Holter monitoring of cystometrogram, uroflowmetry, and renal pelvic pressures.

This article elucidates the clinical applicability and state of the art of ambulatory urodynamics. Ambulatory urodynamics have evolved into practical investigations like EAC, HFM, and EAC combined with renal pelvimetry. EAC has been shown to be the method of preference if detrusor overactivity is involved. Conventional filling cystometry has proved to be an unreliable way to exclude detrusor instability. De novo instability after suspension surgery often indicates that an existing detrusor overactivity was not identified preoperatively. EAC including flowmetry has shown considerable variance i

Magnetic resonance imaging (MRI) in obstetrics. I. Maternal anatomy.

We assessed the ability of magnetic resonance, a recently introduced imaging technique, to demonstrate the maternal anatomy in obstetrics. The signal intensity of different maternal tissues using T1 and T2 weighted sequences was examined. The bony pelvis is depicted with sufficient clarity to provide an alternative to conventional X-ray pelvimetry. The placenta and cervix have a distinctive appearance facilitating the diagnosis of placenta praevia. The unique demonstration of cervical morphology will offer the potential for investigation into the ill-understood conditions of cervical dystocia

Guidelines for computed tomography and magnetic resonance imaging use during pregnancy and lactation.

There has been a substantial increase in the use of computed tomography (CT) and magnetic resonance imaging (MRI) in pregnancy and lactation. Among some physicians and patients, however, there are misperceptions regarding risks, safety, and appropriate use of these modalities in pregnancy. We have developed a set of evidence-based guidelines for the use of CT, MRI, and contrast media during pregnancy for selected indications including suspected acute appendicitis, pulmonary embolism, renal colic, trauma, and cephalopelvic disproportion. Ultrasonography is the initial modality of choice for sus

Efficacy of the fetal-pelvic index as a predictor of fetal-pelvic disproportion in women with abnormal labor patterns that require labor augmentations.

The fetal-pelvic index, which compares fetal head and abdomen circumferences with respective maternal inlet and midpelvic circumferences, was introduced in 1986 as a means of identifying the presence or absence of fetal-pelvic disproportion. In this study the efficacy of the fetal-pelvic index was evaluated in 46 patients with abnormal labor patterns that required labor augmentations and was compared with that of two other means (Colcher-Sussman x-ray pelvimetry and ultrasonographically derived estimated fetal weight greater than or equal to 4000 gm). Of the 24 women who required operative int

Textbook typologies: Challenging the myth of the perfect obstetric pelvis.

Medical education's treatment of obstetric-related anatomy exemplifies historical sex bias in medical curricula. Foundational obstetric and midwifery textbooks teach that clinical pelvimetry and the Caldwell-Moloy classification system are used to assess the pelvic capacity of a pregnant patient. We describe the history of these techniques-ostensibly developed to manage arrested labors-and offer the following criticisms. The sample on which these techniques were developed betrays the bias of the authors and does not represent the sample needed to address their interest in obstetric outcomes. C

Prediction of transabdominal total mesorectal excision difficulty according to the angle of pelvic floor muscle.

BACKGROUND: Total mesorectal excision (TME) is challenging to perform in a deep, narrow pelvis. While previous studies used pelvimetry to assess bony pelvic structures, there is no consensus on exact definition of deep, narrow pelvis. We hypothesized that the shape of pelvic floor muscle may impact the performance of transabdominal pelvic dissection. We aimed to evaluate which parameters of the shape of pelvic floor muscle impact the difficulty of TME and present a predictive reference value for TME difficulty. METHODS: From January 2015 to December 2015, 85 consecutive patients who had under

[Vaginal delivery in a patient with an artificial urinary sphincter and augmentation enterocystoplasty].

Few cases of management of pregnancy in patients with an artificial sphincter (AS) have been reported in the literature, but this situation is very likely to become increasingly frequent with the more frequent use of AS in children. The authors report the case of a pregnancy in a 36-year-old woman with lumbosacral spina bifida, treated by artificial urinary sphincter and augmentation enterocystoplasty. The pregnancy and vaginal delivery did not raise any particular problems. Vaginal delivery is possible provided several precautions are observed: pelvimetry, MRI at the 8th month of pregnancy an
