Penis Transplantation: First US Experience.

OBJECTIVE: We describe the first successful penis transplant in the United States in a patient with a history of subtotal penectomy for penile cancer. BACKGROUND: Penis transplantation represents a new paradigm in restoring anatomic appearance, urine conduit, and sexual function after genitourinary tissue loss. To date, only 2 penis transplants have been performed worldwide. METHODS: After institutional review board approval, extensive medical, surgical, and radiological evaluations of the patient were performed. His candidacy was reviewed by a multidisciplinary team of surgeons, physicians,

Penis allotransplantation model in the rat.

In this study we present the first experimental vascularized penile allogenic transplantation model in the rat. In group 1 (n = 6), the vascular anatomy of the male rat perineal region was determined. In 12 Lewis rats of group 2 (n = 24), the penile composite graft was transplanted as isotransplant by microvascular anastomosis of the pedicle of the penis to the saphenous vessels and in another 12 Lewis rats of group 2, penile composite graft was transplanted without vascular anastomosis. In group 3 (n = 12), penis composite graft was transplanted from 6 Lewis-Brown-Norway donors to 6 Lewis rec

Vascularized composite allotransplantation of the penis: current status and future perspectives.

Given the multifunctional role of the penis in daily life, penile loss can be a physically and emotionally devastating injury. Options to restore penile loss have traditionally relied on autogenous free flap, local flaps, and skin grafts. These techniques provide satisfactory outcomes but carry high rates of urologic and prosthesis-related complications. Vascularized composite allotransplantation may offer a novel solution for these patients through reconstruction with true penile tissue. Still, penile transplants pose ethical, logistical, and psychosocial challenges. These obstacles are made

Factors influencing male sexual impotence after renal transplantation.

Sexual impotence has been reported sixfold after sequential renal transplantation. The effects of race, age, diabetes mellitus, hypertension, uremia, arteriosclerosis (by ABI = ankle BP/brachial BP), penile blood flow (by PBI = penile BP/brachial BP), length of dialysis and transplantation, and patency of hypogastric arteries (by angiogram and operative description) on impotence were examined in a retrospective study of 61 male transplant patients followed from six to 108 months and a prospective study of 15 patients evaluated before and after transplantation with a six-month follow-up. Age (g

Vascular-induced erectile impotence in renal transplant recipients.

The incidence and pathogenesis of sexual dysfunction were studied in 16 male renal allograft recipients. In 9 patients with bilateral renal transplants in whom both internal iliac arteries were occluded nocturnal erections were absent and a penile arterial pulse could not be palpated. Of 7 patients with 1 renal allograft 4 demonstrated penile nocturnal tumescence and the penile pulse could be palpated in 5. Since interruption of pelvic arterial blood supply appears to contribute to impotence in renal transplant recipients consideration should be given to the use of the external iliac artery fo

Penile Microdissection: A Live Donor Feasibility Study in Feminizing Gender-Affirming Surgery.

Femininizing gender affirmation surgery includes the creation of external female genitalia such as a new clitoris, labia, and vagina with removal of the glans and urethral remnants and full corpora cavernosa. We evaluated the possibility of using preserved cavernosal bodies with glans and urethral remnants for potential live-donor penile transplantation. Between March 2021 and February 2023, penile microvascular dissection followed by gender-affirming vaginoplasty was performed in 41 patients aged 18 to 57 years (mean 30.5 years). The mean follow-up was 15 months (ranging from 6 to 26 months).

Modified Surgical Model for Genitourinary Vascularized Composite Allotransplantations: An Anatomical Study.

SUMMARY: Total penile reconstruction represents a surgical challenge. Autologous reconstructions offer limited aesthetic results and variable urinary function recovery. To date, five genitourinary vascularized composite allotransplantations have been performed worldwide. However, in all cases, vascular complications have been reported. The objective of this study was to develop a modified surgical model for penile allotransplantation to limit the number of microsurgical anastomoses and ensure vascular reliability. The authors studied penile allotransplantation by performing dissections on 12 s

Sex and organ-specific risk and temporal trends of human papillomavirus-associated anogenital cancer among solid organ transplant recipients in the United States.

Solid organ transplant recipients (SOTRs) have an elevated risk of persistent infection of human papillomavirus (HPV) and associated anogenital (cervical, vulva, and vaginal among women, penile among men and anal among both men and women) cancers. Risk stratification and temporal trends of cancer incidence will help early detection, evaluation, management, and treatment of post-transplant cancers over time. We analyzed data of 239,613 heart, lung, liver, and kidney transplant recipients registered from 1987 to 2020 in the U.S. Scientific Registry of Transplant Recipients (SRTR) with cancer dia

Penile allotransplantation for penis amputation following ritual circumcision: a case report with 24 months of follow-up.

INTRODUCTION: Ritual circumcision complicated by gangrene is a leading cause of penile loss in young men in South Africa. This deeply rooted cultural tradition is unlikely to be abolished. Conventional reconstructive techniques using free vascularised tissue flaps with penile implants are undesirable in this often socioeconomically challenged group because donor site morbidity can hinder manual labour and vigorous sexual activity might lead to penile implant extrusion. The psychosociological effects of penile loss in a young man are devastating and replacing it with the same organ is likely to

Risk Factors for Penile Intraepithelial Neoplasia: A Population-based Register Study in Sweden, 2000-2012.

Studies on risk factors for penile intraepithelial neo-plasia have been small in size, have not distinguished penile intraepithelial neoplasia from invasive cancer, and have relied on self-reported information. This study investigated risk factors for penile intraepithelial neoplasia in a cohort of 580 penile intraepithelial neoplasia cases and 3,436 controls using information from 7 Swedish registers. Cases with penile intraepithelial neoplasia had increased odds ratios (ORs) for inflammatory skin diseases (14.7, 95% CI 6.5-33.4) including lichen planus (12.0, 95% CI 3.0-48.0), indicating lic

Conventional Penile Reconstruction Versus Penile Allotransplantation: A Comprehensive Review.

Penile loss from trauma, oncologic resection, congenital anomalies, or gender-affirming needs represents a devastating condition with profound functional and psychosocial implications. Conventional reconstructive techniques, including radial forearm, fibula, scapular, and anterolateral thigh flaps, have evolved over decades and provide patients with an acceptable neophallus capable of voiding and sexual activity. However, these methods remain limited by donor-site morbidity, prosthesis-related complications, high revision rates, and the inability to restore natural erectile tissue or fully rep

Fournier's gangrene after unrelated cord blood stem cell transplantation.

A 16-year-old boy with refractory acute myelogenous leukemia developed Fournier's gangrene as an early complication after two-antigen HLA-mismatched unrelated cord blood stem cell transplantation. On day 25 after the transplantation, he noted abrupt onset of penile swelling with miction pain. The penile inflammation rapidly extended posteriorly to involve the scrotum and perianal tissues, inferiorly to involve the thighs, and superiorly up the lower abdominal region within the next 36 h, and he died from sepsis on day 27. Fournier's gangrene presenting as a genitoperineal necrotizing fasciitis

Dirt Under the Finger-Nails-More Malignant Than Malignancy.

A young boy developed steroid refractory GVHD post haploidentical transplant for relapsed B-ALL. He was on systemic immunosuppression with two immunosuppressants, and had history of CMV reactivation and tuberculosis. Eight months post-transplant, he was hospitalized with multi-drug-resistant gram-negative sepsis, and during the same episode, he developed penile lesions which progressed to dry gangrene of glans-penis. Fusarium grew in blood and penile lesions. On retrospective thinking, it was discerned that his onychomycoses were the probable cause of his penile lesions with hematogenous disse

Penile squamous cell carcinoma with posterior urethra involvement: A case report.

Penile squamous cell carcinoma (SCC) is a rare malignancy that is more prevalent among immunocompromised individuals. Given that the tumor primarily originates along the penile shaft, it typically affects the anterior urethra. In contrast, primary urethral cancers (PUC) more commonly involve the posterior urethra. The imaging features of penile SCC and PUC often overlap, making the pattern of urethral infiltration a key factor in distinguishing between these 2 conditions. Magnetic resonance imaging (MRI) plays a crucial role in local staging and in identifying the patterns of urethral involvem

Narrative review of the history of microsurgery in urological practice.

The clinical need for magnified visualization during surgery spurred the evolution of microscope and microsuture technology. Innovative surgeons across various surgical specialties recognized the importance of utilizing and advancing these technologies. Operative microscopy allows human dexterity to perform beyond direct visual limitations. Microsurgery started in otolaryngology and ophthalmology, became popular in reconstruction and transplantation, and was then adopted in urology. Microsurgery in urology involves renal and penile revascularization, penile transplantation and free flap phallo

Surgical correction of vasculogenic impotence in a patient after bilateral renal transplantation.

This article documents a vascular cause of erectile dysfunction in a patient with ligation of both internal iliac arteries after bilateral renal transplantation. The diagnosis was made by the noninvasive measurement of the penile arterial systolic pressure and confirmed by pelvic arteriography. Correction of the patient's impotence was accomplished by restoration of penile blood flow by a saphenous vein bypass graft between the external iliac and internal iliac arteries.

Impotence after kidney transplantation.

Sexual performance was evaluated in 28 men, thirty to sixty years of age, with functioning renal allografts: 13 were potent (controls), 10 had moderate sexual dysfunction, 5 had marked sexual dysfunction. Penile blood pressures, serum hormone levels, plasma zinc levels, and penile venous angiography were evaluated in search of causes of impotence. Thirteen of 15 impotent transplant patients (87 per cent) had marked abnormalities in at least one of the four areas studied. Systematic search for etiologic factors may permit specific therapy for impotence, which occurred in 54 per cent of the 28 k
