Performance- and image-enhancing drug use in the community: use prevalence, user demographics and the potential role of wastewater-based epidemiology.

Performance- and image-enhancing drug (PIED) misuse is a significant public health issue. Currently, seizure data, surveys, anti-doping testing, and needle service provider data are used to estimate PIED use in populations. These methods are time consuming, single point-in-time measurements that often consist of small sample sizes and do not truly capture PIED prevalence. Wastewater-based epidemiology (WBE) has been used globally to assess and monitor licit and illicit drug consumption within the general community. This method can objectively cover large populations as well as specific subpopu

[Cardiovascular alterations associated with doping].

Doping -the abuse of anabolic-androgenic steroids in particular- is widespread in amateur and recreational sports and does not solely represent a problem of professional sports. Excessive overdose of anabolic steroids is well documented in bodybuilding or powerlifting leading to significant side effects. Cardiovascular damages are most relevant next to adverse endocrine effects.Clinical cases as well as forensic investigations of fatalities or steroid consumption in connection with trafficking of doping agents provide only anecdotal evidence of correlations between side effects and substance a

Ginger (Zingiber officinale) as an Analgesic and Ergogenic Aid in Sport: A Systemic Review.

Ginger is a popular spice used to treat a variety of maladies, including pain. Nonsteroidal anti-inflammatory drugs (NSAIDs) are frequently used by athletes to manage and prevent pain; unfortunately, NSAIDs contribute to substantial adverse effects, including gastrointestinal (GI) dysfunction, exercise-induced bronchoconstriction, hyponatremia, impairment of connective tissue remodeling, endurance competition withdrawal, and cardiovascular disease. Ginger, however, may act as a promoter of GI integrity and as a bronchodilator. Given these potentially positive effects of ginger, a systematic re

Harm Reduction in Male Patients Actively Using Anabolic Androgenic Steroids (AAS) and Performance-Enhancing Drugs (PEDs): a Review.

Anabolic androgenic steroid (AAS) and performance-enhancing drug (PED) use is a prevalent medical issue, especially among men, with an estimated 2.9-4 million Americans using AAS in their lifetime. Prior studies of AAS use reveal an association with polycythemia, dyslipidemia, infertility, hypertension, left ventricular hypertrophy, and multiple behavioral disorders. AAS withdrawal syndrome, a state of depression, anhedonia, and sexual dysfunction after discontinuing AAS use, is a common barrier to successful cessation. Clinical resources for these patients and training of physicians on manage

Potential Health Risks Associated with Sports Supplementation Use in Athletes.

As modern competitive sports continue to evolve, athletes are under increasing pressure to optimize both performance and recovery. Sports supplements have therefore become widely used among athletes because of their purported benefits in enhancing athletic performance, delaying fatigue, and accelerating physiological recovery. However, misuse, excessive intake, and other inappropriate patterns of supplement use remain common in some athletes and may impair performance, compromise health, and increase the risk of anti-doping rule violations. Although existing research has largely focused on the

Sodium bicarbonate supplementation and the female athlete: A brief commentary with small scale systematic review and meta-analysis.

ABSTRACTSodium bicarbonate (SB) is considered an effective ergogenic supplement for improving high-intensity exercise capacity and performance, although recent data suggests that women may be less amenable to its ergogenic effects than men. Currently, an apparent paucity of data on women means no consensus exists on whether women benefit from SB supplementation. The aim of the current study was to quantify the proportion of the published literature on SB supplementation that includes women, and to synthesise the evidence regarding its effects on blood bicarbonate and exercise performance in wo

New Insights into Enhancing Maximal Exercise Performance Through the Use of a Bitter Tastant.

It is generally acknowledged that for an orally administered ergogenic aid to enhance exercise performance it must first be absorbed by the gastrointestinal tract before exerting its effects. Recently, however, it has been reported that some ergogenic aids can affect exercise performance without prior absorption by the gastrointestinal tract. This is best illustrated by studies that have shown that rinsing the mouth with a carbohydrate (CHO) solution, without swallowing it, significantly improves exercise performance. The ergogenic effects of CHO mouth rinsing in these studies have been attrib

Static Stretching Acutely Reduces the Ergogenic Effects of Caffeine on Sprint Performance but Not Maximal Ball Velocity in Football Players.

This study aimed to investigate the independent and combined effects of caffeine and static stretching (SS) on maximal ball velocity and 30-m sprint performance. Sixteen male amateur football players performed 30-m sprint and an instep kicking ball velocity test under six conditions. The six conditions were control, SS only, placebo only, placebo combined with SS (PLA + SS), caffeine only, and caffeine combined with SS (CAF + SS). A repeated measures ANOVA revealed no significant main effect of condition on maximal ball velocity (F(5, 75) = 1.11, p = 0.36, η2 p = 0.069). In contrast, a signifi

Caffeine Enhances Some Aspects of Physical Performance in Well-Trained Hammer and Discus Throwers.

BACKGROUND: Caffeine is a widely recognized ergogenic aid for enhancing exercise performance. However, its effect on throwing performance has been less studied, yielding contradictory results. OBJECTIVES: The main aim of the study was to analyze the potential ergogenic effects of a moderate dose of caffeine (3 mg·kg-1 body mass) on vertical jump performance and throwing distance during a simulated competition in trained discus and hammer throwers. METHODS: In a randomized, counterbalanced, and repeated measures design, 14 well-trained throwers (9 hammer throwers and 5 discus throwers; age 24

Against Moral Panic and Citation Fiction: A Critique of "Panem, Corticoids and Circenses" and a Proposal for Editorial Gatekeeping on Reference Integrity.

The proposed Enhanced Games have become a convenient stage for bioethical sermonising about risk, authenticity, and the "spirit of sport". This is epitomized by a recent article arguing that institutionalizing pharmacological enhancement under the "pretence of medical supervision and personal autonomy" would redefine human excellence in "purely biochemical terms", erode ethical norms, normalize doping, and burden public health systems. The present paper offers a two-level response. First, I criticise the argument on its merits. Its central claims rely on rhetorical inflation, false dichotomies

The IPEDs assemblage: Tracing the entanglements of biomedicine, technology, enhancement and anti-doping policies in sport and society.

The US Anti-Doping Agency (USADA) and Ultimate Fighting Championship (UFC) recently ended their anti-doping partnership amidst controversy. We treat this decision, and the motivations underpinning it, as a means of exploring the complexities of anti-doping norms and the blurred lines between image and performance enhancing drug (IPED) use in sport and wider society. Drawing ideas from assemblage thinking, we analyse the evolving power dynamics surrounding IPED use, anti-doping policy, and the role of popular athletes in shaping societal perceptions of the use of, and potential harms associated

Adverse effects of doping with anabolic androgenic steroids in competitive athletics, recreational sports and bodybuilding.

Despite the fact that sports organizations and legislators have introduced various mechanisms to discourage athletes from using performance and appearance enhancing substances a high percentage of athletes admits to their unabated application. In competitive athletics, bodybuilding and in recreational sports anabolic androgenic steroids (AAS) continue to be the substances most abused. This review summarizes the side effects of AAS abuse on organs and system functions in both sexes. High doses of AAS cause a significant increase of erythrocytes and hemoglobin concentration, which may lead to th

The effects of the COVID-19 pandemic on the use of the performance-enhancing drugs.

The coronavirus disease (COVID-19) pandemic has been impacting the whole society in every aspect of the daily life, comprising the sport field. Several restrictive strategies have been implemented by governments in an effort to stem the spread of the disease and salvage public health. Such efforts have severely constrained access to non-essential services, leading to the closure of non-essential points of gathering and business and the enforcement of rigorous social distancing and prolonged lockdowns, in addition to masking and stay-at-home mandates. However necessary, there is no denying that

Anabolic androgenic steroids are human enhancement drugs, not necessarily substance abuse.

This essay examines human enhancement drugs - particularly anabolic-androgenic steroids - through the lens of public health theory from the Global South. It critiques prohibitionist policies, which criminalize users while disregarding social determinants, as well as extreme relativism, which reduces consumption to individual autonomy alone. In contrast, harm reduction emerges as an evidence-based, user-centered approach that rejects criminalization, upholds human rights, and provides contextualized health responses. By reframing anabolic-androgenic steroids as human enhancement drugs, the anal

A new era of doping? Use of peptide and peptide-analog drugs in recreational and professional sport and bodybuilding: a critical review.

The pursuit of pharmacological enhancement in sport has evolved from the widespread use of anabolic-androgenic steroids (AAS) to novel agents such as peptides and peptide analogues. Marketed as more selective and ostensibly safer alternatives, peptides-including growth hormone secretagogues (e.g., Ipamorelin), growth hormone-releasing hormone analogues (e.g., CJC-1295, Sermorelin), and synthetic fragments (e.g., Frag 176-191, KPV)-are promoted for muscle growth, fat metabolism, recovery, and anti-inflammatory effects. Their pharmacological profiles, including enhanced stability and receptor se

Generating evidence on the use of Image and performance enhancing drugs in the UK: results from a scoping review and expert consultation by the Anabolic Steroid UK network.

BACKGROUND: The use of anabolic androgenic steroids (AAS) and associated image and performance enhancing drugs (IPEDs) is now a global phenomenon. There is a need to develop evidence to support the development of interventions to prevent the commencement of use, to minimise the potential harms or to support those in their cessation of use. While the United Kingdom (UK) is no exception to this issue, its public health and legislative response to the phenomenon differs to other countries and requires the examination of research specific to the UK. Therefore, a scoping review has been conducted t

Vertebral artery dissection in weightlifter with performance enhancing drug use.

This case report describes a transient ischaemic attack secondary to vertebral artery dissection (VAD) in a young male body builder. This occurred following weight training with weights across the back and shoulders. The patient was also known to take multiple performance enhancing agents including anabolic steroids, slimming agents, stimulants and human growth hormone. Cases of VAD have been described with cervical manipulation in the past and an association between the use of anabolic steroids and embolic strokes has been described. To the authors knowledge, this is the first case describing

Dosing strategies for β-alanine supplementation in strength and power performance: a systematic review.

BACKGROUND: β-alanine is a well-established ergogenic aid that enhances muscle carnosine levels and buffering capacity during high-intensity efforts. However, its role in improving strength and power performance remains inconsistent across the literature. This systematic review investigates whether dosing strategy, rather than duration alone, is the critical determinant of efficacy in resistance-trained populations. METHODS: A systematic search was conducted across PubMed, SPORTDiscus, Scopus, Web of Science, and Google Scholar up to 28 May 2025, following PRISMA guidelines. Studies were incl
