Preventing and Treating Peri-Implantitis: A Cost-Effectiveness Analysis.

BACKGROUND: A large number of treatments for peri-implantitis are available, but their cost-effectiveness remains uncertain. This study evaluates the cost-effectiveness of preventing and treating peri-implantitis. METHODS: A Markov model was constructed that followed each implant over 20 years. Supportive implant therapy (SIT) for managing peri-implant mucositis and preventing development of peri-implantitis was either provided or not. Risk of peri-implantitis was assumed to be affected by SIT and the patient's risk profile. If peri-implantitis occurred, 11 treatment strategies (non-surgical

Haptoglobin gene polymorphisms in peri-implantitis and chronic periodontitis.

AIM: The haptoglobin-hemoglobin (Hp-Hb) complex plays a significant role in regulating immune responses and acts as a bacteriostatic agent. Haptoglobin polymorphisms result in different Hb binding affinities. This study sought to assess whether Hp 2-2 could be a genetic determinant for increasing the risk of peri-implantitis and chronic periodontitis. METHODS: Of the Iranian subjects referred to the Department of Periodontics, Shahid Beheshti University, Tehran, 203 were entered into the study, including 117 patients and 86 periodontally healthy individuals. Polymerase chain reaction (PCR) wa

Interventions for replacing missing teeth: treatment of peri-implantitis.

BACKGROUND: One of the key factors for the long-term success of oral implants is the maintenance of healthy tissues around them. Bacterial plaque accumulation induces inflammatory changes in the soft tissues surrounding oral implants and it may lead to their progressive destruction (peri-implantitis) and ultimately to implant failure. Different treatment strategies for peri-implantitis have been suggested, however it is unclear which are the most effective. OBJECTIVES: To identify the most effective interventions for treating peri-implantitis around osseointegrated dental implants. SEARCH ME

Predisposing conditions for retrograde peri-implantitis, and treatment suggestions.

BACKGROUND: Recent case reports introduced the term retrograde peri-implantitis as a lesion (radiolucency) around the most apical part of an osseointegrated implant. It develops within the first months after insertion. This retrospective study aimed to find predisposing conditions for such peri-apical lesions and to evaluate treatment strategies. METHODS: All single implants (426 in the upper, 113 in the lower jaw, all Brånemark system type) placed at the department of Periodontology of the University Hospital (Catholic University Leuven) were included in this retrospective evaluation to chec

Overview of Treatment Modalities for Peri-Implantitis (Part 2).

Peri-implantitis is a biofilm-associated inflammatory disease that results in progressive loss of the supporting tissues around dental implants and remains one of the greatest challenges in implant therapy. Successful treatment extends beyond implant surface decontamination and often requires adjunctive therapies to control infection, reduce inflammation, and facilitate regeneration of the lost peri-implant tissues. This second part of this review of peri-implantitis summarizes the current evidence regarding local and systemic antibiotic therapy, laser-assisted treatment, implantoplasty, and r

Clinical evaluation of peri-implantitis relation with implant material.

Peri-implantitis, characterized by inflammation of peri-implant tissues and progressive bone loss, remains a major biological complication affecting long-term implant success. Material composition, surface characteristics and biocompatibility influence microbial colonization, host immune response and implant durability. Therefore, it is of interest to investigate the relationship between different dental implant materials and the incidence of peri-implantitis among patients treated at a tertiary dental center. Clinical parameters including probing depth, bleeding on probing, radiographic bone

TRPV1-linked neuropeptide axes in periodontitis and peri-implantitis.

Periodontitis and peri-implantitis are the two most prevalent destructive diseases encountered in clinical dentistry, both of which arise from complex interactions among microbial challenge, host immune responses, and bone metabolism. Although these conditions share similar clinical manifestations, accumulating evidence indicates that the peripheral nervous system plays fundamentally distinct roles in their pathogenesis. In this narrative review, a structured literature search was conducted across multiple databases to synthesize and weigh current evidence on neuro-osteo-immune regulation in o

Laser applications in peri-implantitis therapy: A narrative review.

INTRODUCTION/OBJECTIVES: The treatment of peri-implantitis remains one of the most challenging areas in implant dentistry, with no universally accepted gold standards for long-term disease resolution. As the prevalence of peri-implantitis continues to rise with increased implant placement, the need for predictable and effective therapies becomes increasingly critical. Laser-assisted treatment has emerged as a promising modality in surgical and non-surgical management of peri-implant diseases due to its ability to decontaminate the implant surface, reduce the bacterial load, and debride bone de

Management of Retrograde Peri-Implantitis Using Guided Bone Regeneration.

Retrograde peri-implantitis (RPI) is characterized by atypical and often progressive bone loss at the periapex of a dental implant. Although RPI reportedly has a lower prevalence than marginal peri-implantitis, if left untreated it can lead to loss of dental implants. In this clinical case series, two patients with different presentations of RPI were treated with guided bone regeneration (GBR) using bone graft and collagen membrane. Longterm follow-up showed clinical and radiographic improvements that resulted in radiographic bone fill and reduced probing depths around implants. Observations r

Experimental Model of Ligature-Induced Peri-Implantitis in Mice.

Dental implants have a high success and survival rate. However, complications such as peri-implantitis (PI) are highly challenging to treat. PI is characterized by inflammation in the tissues around dental implants with progressive loss of supporting bone. To optimize dental implants' longevity in terms of health and functionality, it is crucial to understand the peri-implantitis pathophysiology. In this regard, using mouse models in research has proven clear benefits in recreating clinical circumstances. This study aimed to describe an experimental model of ligature-induced peri-implantitis i
