Ultrasonographic findings and differential diagnosis of acute calcific periarthritis in hands.

OBJECTIVES: To explore the feasibility of using high-frequency ultrasound for the diagnosis and differential diagnosis of acute calcific periarthritis (ACP). METHOD: A total of 35 ACP patients in Group A and 38 gouty patients in Group B underwent ultrasound examination. All the ultrasound images were observed and analysed, and the sizes of the tophi and calcifications in the hands were measured. The differences in ultrasound features between Group A and Group B were examined. A t test was used to determine significant differences in size between ACP calcifications and gouty tophi. RESULTS: T

Oxaprozin versus diclofenac in NSAID-refractory periarthritis pain of the shoulder.

OBJECTIVE: To evaluate the efficacy and safety of oxaprozin in comparison with diclofenac in patients with periarthritis pain of the shoulder previously unsuccessfully treated with nonsteroidal anti-inflammatory drugs other than diclofenac and oxaprozin. METHODS: In this open, multicentre, randomised, controlled study, eligible patients with periarthritis of the shoulder were randomised to receive either oxaprozin 1200 mg once daily (n = 49) or diclofenac 50 mg three times daily (n = 47). The treatment period was 15 +/- 1 days. The study was planned on a hypothesis of equivalence between the

[Humero-scapular periarthritis--a diagnosis which does not need to be used].

The periarthritis humeroscapularis (PHS) diagnosis has been used for over a century. In our country, the term PHS is customary and unavoidable in everyday practice. It has been used by everyone, from general practitioners to specialists--physiatrists, radiologists, and orthopaedists. The purpose of this study is to show the inappropriateness of the PHS diagnosis and to point to other clinical conditions with a clear anatomical structure and adequate treatment method.

Calcific periarthritis: more than a shoulder problem: a series of fifteen cases.

BACKGROUND: Calcific periarthritis, referring to a circumscribed juxta-articular deposit of minute non-birefringent mineral grains, is rarely the clinical diagnosis accompanying a pathologic specimen. Familiarity with the clinical, pathologic, and radiologic manifestations of calcific periarthritis, particularly when encountered adjacent to joints other than the shoulder, facilitates diagnosis and may obviate biopsy, avoid confusion with other entities, and speed appropriate treatment. METHODS: Pathologic specimens that fulfilled the criteria for a diagnosis of calcific periarthritis were pro

Acute calcific periarthritis of the hand and elbows in women. A study and review of the literature.

OBJECTIVE: To study the clinical and radiological features of acute calcific periarthritis (ACP) of the hand and elbow in women and to review the literature. METHODS: A retrospective study of ACP of the hand and elbow was conducted in 10 women, and the literature since 1970 reviewed, using Medline data. RESULTS: Ten women fulfilled the criteria of a single focus ACP. Mean age was 36.5 years (range 25-53). X-ray diffraction in 3 patients who underwent aspiration biopsy showed a pattern of calcium hydroxyapatite. The clinical course was short and self-limited. Most cases mimicked septic condit

Periarthritis of the shoulder. I. Aetiological considerations with particular reference to personality factors.

186 patients with periarthritis of the shoulder have been studied. The sex ratio was female:male, 1-52:1. The peak age of onset was 54-59 years in both sexes. Over 40% of the patients were referred to the clinic after 6 months had elapsed from the time of onset of the disease. The right shoulder was more frequently involved than the left, particularly in the men. One shoulder only was affected in 75% of patients. There was frequently a previous history of 'rheumatism' before the episode of periarthritis. In one-third of the women 'nonspecific rheumatism' had occurred. Cervicobrachial pain and

Acute Calcific Periarthritis of the 1st Metacarpophalangeal Joint: A Rare Diagnostic Challenge.

INTRODUCTION: Acute calcific periarthritis (ACP) is a self-limiting inflammatory condition most commonly involving the rotator cuff of the shoulder. Its presentation in the hand is rare and often mimics infectious processes, leading to potential misdiagnosis and inappropriate treatment. Prompt recognition through imaging is crucial for accurate diagnosis and management. CASE REPORT: We present a case report of a 84-year-old woman who presented with acute pain, swelling, and erythema of the right thumb. Clinical examination suggested an infectious etiology; however, plain radiographs were obta

Acute Calcific Periarthritis of the Hand: A Case Report in an Active Duty Soldier.

Acute calcific periarthritis of the hand is a condition that can easily be misdiagnosed resulting in unnecessary diagnostic studies and treatment. The condition is thought to be benign with a self-resolving course. The author presents a case of an active duty 29-year-old male who presented to a military emergency department with severe atraumatic hand pain developing overnight with initial concern for flexor tenosynovitis. Following orthopedic consultation, he was determined to have acute calcific periarthritis of the small finger (metacarpophalangeal) MCP joint with resolution over approximat

Effective period of conservative treatment in patients with acute calcific periarthritis of the hand.

BACKGROUND: Acute calcific periarthritis of the hand is a relatively uncommon painful condition involving juxta-articular deposits of amorphous calcium hydroxyapatite. Although conservative treatments have been generally considered effective, there is little evidence regarding how long they could remain effective. METHODS: We retrospectively reviewed ten patients who were diagnosed with acute calcific periarthritis of the hand from January 2015 to June 2018. We recommended the use of warm baths, nonsteroidal anti-inflammatory drugs (NSAIDs), and limited activity as initial treatments. If the

Chronic calcific periarthritis of the elbow treated by cimetidine administration: Five cases.

Calcific periarthritis or calcific tendinitis occurs most frequently in the shoulder and rarely in the elbow. Cimetidine was previously reported to be effective for chronic calcific periarthritis of the shoulder. Here, we present five patients with chronic calcific periarthritis of the elbow treated by administration of cimetidine; there were six affected elbows in these five patients. Although all patients had been treated with nonsteroidal anti-inflammatory drugs for at least 3 months, their symptoms were not relieved. All patients took oral administration of cimetidine 400 mg daily. The pai

Calcific periarthritis as the only clinical manifestation of hypophosphatasia in middle-aged sisters.

Hypophosphatasia (HPP) is the inborn error of metabolism that features low serum alkaline phosphatase (ALP) activity caused by loss-of-function mutation(s) within the gene for the tissue nonspecific isoenzyme of ALP (TNSALP). In HPP, extracellular accumulation of inorganic pyrophosphate (PPi), a TNSALP substrate and inhibitor of mineralization, leads frequently to premature tooth loss and often to rickets or osteomalacia. In affected adults, the excess PPi sometimes also causes calcium pyrophosphate dihydrate (CPPD) deposition, PPi arthropathy, or pseudogout, or seemingly paradoxical depositio
