Perilymphatic fistula and Meniere's disease. Clinical series and literature review.

There continues to be considerable confusion concerning the diagnosis and treatment of perilymphatic fistulas (PLFs) and Meniere's disease. This paper reports an 8-year experience with patients whose symptoms were compatible with Meniere's disease but who had some other element that raised the possibility of their having a PLF. This review is a retrospective report on 64 patients who had fluctuating hearing loss, vertigo, tinnitus, and aural fullness typical of Meniere's disease. However, they also had a positive fistula test and/or symptoms beginning immediately after head trauma. They all un

Perilymphatic fistula and pneumolabyrinth without temporal bone fracture: a rare entity.

Post-traumatic pneumolabyrinth is an uncommon clinical entity, particularly in the absence of temporal bone fracture. We report the case of a patient who presented to our emergency department with a headache, sudden left hearing loss and severe dizziness which began after a traumatic brain injury 3 days earlier. On examination, the patient presented signs of left vestibulopathy, left sensorineural hearing loss and positive fistula test, normal otoscopy and without focal neurological signs. The audiometry confirmed profound left sensorineural hearing loss. Cranial CT revealed a right occipital

[Traumatic perilymphatic fistula of round and oval windows (four cases reported)].

OBJECTIVE: To explore the diagnosis and treatment to traumatic perilymph fistula (PLF) of round and oval window. METHOD: Traumatic PLF was diagnosed by the traumatic history of head, neck and ear, the examinations of auditory and vestibular function, and the exploratory tympanotomy. The PLF of round and oval windows were repaired by fascia graft or tragus perichondrium and gelform. Four cases with traumatic PLF of round and oval window were reported in this paper, which included 2 cases hitten by hand, 1 by brick, 1 insulted by middle ear surgery. Exploratory tympanotomy was performed from on

Penetrating Trauma-Induced Perilymphatic Fistula: A Case Report and Literature Review.

This article highlights the importance of early identification and surgical treatment for extremely rare traumatic perilymphatic fistula (TPF) caused by an earpick, which can pose the risk of irreversible hearing loss. Herein, we have described two cases of TPF and reviewed the literature primarily based on surgical treatment for penetrating ear trauma-induced TPF.  We highlight the case of two females who sustained an accidental penetrating injury in the ear caused by the introduction of an earpick, leading to hearing loss and dizziness. Pure tone audiometry detected elevation of the bone-con

Congenital Membranous Stapes Footplate Producing Episodic Pressure-Induced Perilymphatic Fistula Symptoms.

Introduction: Recent third window syndrome studies have revealed that the intact bony labyrinth and differences in the stiffness of the oval and round windows are essential for proper cochlear and vestibular function. Herein we report a patient with a congenital dehiscence of the right stapes footplate. This dehiscence caused long-standing episodic pressure-induced vertigo (Hennebert sign). At the time of presentation, her increased thoracic pressure changes induced the rupture of the membranous stapes footplate. Perilymph leakage was confirmed by imaging and a biochemical test [perilymph-spec

Transcanal endoscopic ear surgery for perilymphatic fistula after electric acoustic stimulation.

Transcanal endoscopic ear surgery (TEES) will become a very useful therapeutic option. A perilymphatic fistula (PLF) is defined as sudden sensorineural hearing loss and/or vertigo caused by leakage of the perilymph through a fistula from the oval window and/or round window. We report a case of PLF after electric acoustic stimulation (EAS), a kind of cochlear implant, successfully treated by TEES. A 38-year-old man presented to our hospital with vertigo and hearing loss (HL). His vertigo was induced by Valsalva maneuvers. Eight months ago, he underwent EAS for his right ear for congenital senso

Effects of endolymphatic-perilymphatic fistula on endolymphatic hydrops in guinea pig.

The endolymphatic-perilymphatic shunt operation between the scala media and scala tympani was performed in 22 guinea pigs with endolymphatic hydrops induced by the silver nitrate injection method 2 months before the operation. Two (n = 10), 7 (n = 6), and 28 (n = 6) days after the operation, the condition of the fistula and the inner ear pathology were studied histopathologically. In half of the animals, the fistula was open, whereas in the other half, the fistula was closed. In the group examined after 2 postoperative days, the animals with open fistulas showed a slight collapse of Reissner's

Perilymphatic gusher in stapedectomy: demonstration of a fistula of internal auditory canal.

Gusher is a very rare phenomenon, generally associated with congenital stapes fixation or otosclerosis in adult age, that may present during stapedectomy. A sudden perilymph flow occurs following platinotomy, due to congenital malformation (abnormally wide cochlear aqueduct or internal auditory canal fistula), that causes an abnormal connection between subarachnoid and perilymphatic spaces. This report deals with a case of bilateral gusher, occurring during stapedectomy, caused by an osseous fistula between bottom of internal auditory canal and the osseous labyrinth later observed at computed

Accuracy of diagnostic tests for perilymphatic fistula: protocol for a systematic review.

UNLABELLED: Perilymphatic fistula (PLF) is a challenging inner ear condition, characterized by the abnormal connection between inner and middle ear and subsequent leakage of inner ear liquid (perilymph) into the middle ear. Early surgical intervention for PLF might lead to favorable audiological and vestibular outcomes. The conventional reference standard of PLF detection is the intraoperative visualization of perilymph leakage during explorative tympanotomy or endoscopic inspection; the specificity and sensitivity of this method is unknown. Systematic reviews assessing the accuracy of diagnos

Imaging Analysis for Cholesteatoma Extension to the Perilymphatic Space in Labyrinth Fistulae.

OBJECTIVES/HYPOTHESIS: Disturbed perilymph behind a labyrinth fistula can lead to hearing deterioration; thus, delicate manipulation is required during surgery for cholesteatomatous fistulae with matrix extension to the perilymphatic space (EPS). However, it remains challenging to identify the EPS preoperatively. This study aimed to evaluate the diagnostic value of computed tomography (CT) for preoperative prediction of the EPS of cholesteatomatous fistulae. STUDY DESIGN: Retrospective study. METHODS: We included serial high-resolution CT images showing a cholesteatomatous bone defect in the

The performance of Cochlin-tomoprotein detection test in the diagnosis of perilymphatic fistula.

BACKGROUND: Perilymphatic fistula (PLF), defined as an abnormal communication between the inner and middle ear, presents with a symptomatology of hearing loss and vestibular disorder that is indistinguishable from a number of other inner ear diseases. Methods of diagnosis remain controversial. We have previously shown that Cochlin-tomoprotein (CTP) is selectively detected in the perilymph. To establish a definite diagnostic test for PLF using CTP as a biochemical marker, we examined the diagnostic performance of the CTP detection test. METHODS: CTP detection test was performed by Western blot

Ventriculoperitoneal shunt as treatment for perilymphatic fistula: a report of six cases.

We report six cases of perilymphatic fistula in patients who received ventriculoperitoneal shunts as part of their final mode of therapy. The last of our 6 patients actually received a ventriculoperitoneal shunt as her initial mode of therapy. All but one had benign intracranial hypertension. All six felt better (less disequilibrium, tinnitus, and pressure and occasional hearing improvement) after LP with removal of 15-20 ml of cerebrospinal fluid.

Use of the glycerin test in the diagnosis of post-traumatic perilymphatic fistulas.

A new application of the glycerin test in the diagnosis of post-traumatic perilymphatic fistulas is described. Temporary disappearance of abnormal responses to the fistula and Quix tests and improvement in the hearing occurred. The glycerin test was useful in confirming the diagnosis of post-traumatic perilymphatic fistula in 13 patients in whom fistulas were found at middle ear exploration.

Electrocochleography with postural changes in perilymphatic fistula and Menière's disease: case reports.

Perilymphatic fistula can be difficult to diagnose differentially prior to exploratory surgery. In this study, we investigated a new test technique in four case studies of subjects with perilymphatic fistula and compared our findings to results obtained in 20 normally hearing subjects with no history of vertigo and 10 subjects with Meniere's disease. Recordings from an eardrum electrode were obtained with the subject in an upright position, after the subject had been lying in a horizontal position for 30 minutes with the test ear up and after 15 minutes with the test ear down. Stimuli consiste

Traumatic perilymphatic fistula: how long can symptoms persist? A follow-up report.

In the past 18 years 68 ears (average 3.8 per year) were explored for perilymphatic fistula (PLF). A total of nine (13%) ears had a fistula identified at operation. Patients with a previous history of otologic surgery were excluded from this review. The most common etiology for PLF was head trauma (4 of 9). Most patients had persistent symptoms lasting months (average 6.7). Eighty-three percent of all patients had sudden or fluctuating hearing loss, 77 percent had vertigo or dysequilibrium, and 61 percent had tinnitus. Vertigo was the most commonly improved symptom postoperatively, and only 25

The missing perilymph sign on MRI indicates a perilymphatic fistula: A case report.

A perilymphatic fistula (PLF) is a rare condition in which perilymph leaks from the cochlea or vestibule into the surrounding cavities, most commonly through round and oval windows, and causes cochlear and vestibular symptoms. However, vague symptoms and the lack of a clear diagnostic test have made the existence of PLF a controversial subject for decades. Here, we report a case of definite PLF confirmed by surgery in a patient who underwent mastoidectomy 20 years prior, revealing a specific sign of missing perilymph on MRI. T2-weighted MRI revealed a nodular bright signal in the cochlea and v

Traumatic perilymphatic fistula secondary to stapes luxation into the vestibule: a case report.

A penetrating ear injury with a perilymphatic fistula is not an uncommon occurrence in otolaryngologic practice, but stapes luxation is rare. We report the case of an 11-year-old boy who developed a traumatic perilymphatic fistula secondary to an atypical stapes luxation into the vestibule. After sustaining a penetrating injury to the right ear, the patient presented with otalgia, vertigo, vomiting, gait unsteadiness, and hearing loss. High-resolution computed tomography (HRCT) of the temporal bone detected pneumolabyrinth, indicating a perilymphatic fistula. The stapes had pivoted on the foot

Perilymphatic fistula in cabin attendants: an incapacitating consequence of flying with common cold.

A perilymphatic fistula (PLF) is an abnormal communication between the inner ear and the middle ear that leaks perilymph. PLF is considered rare, but is known to occur during childbirth, straining, weightlifting, head trauma, and diving with middle ear equalizing problems. It has also, anecdotally, been described in connection with flying. The symptoms are uncharacteristic vertigo and, in some cases, hearing impairment and tinnitus. This study describes four cases of PLF during a period of 6 mo in a major Scandinavian airline company employing approximately 3000 cabin attendants (CAs). None of
