Perinatal Care for Individuals With a History of Sexual Trauma.

Many women and individuals assigned female at birth experience sexual assault or abuse in their lives leading to sexual trauma. In this article, we review the effects of sexual trauma and resulting chronic stress on the body and during the perinatal period. Maternal, fetal, and neonatal health implications are discussed. Routine screening for sexual assault and violence can assist with early identification and intervention. A variety of modalities and methods for managing the effects of sexual trauma have been identified, including pharmacologic treatment, psychotherapy, complementary and alte

Anticipation, Accompaniment, and a Good Death in Perinatal Care.

The ethics of perinatal care, and the experiences of families who receive such care, remains a nascent area of inquiry. It can be hard to see how existing "good death" constructs apply to the experiences of fetal patients and their families. In this paper, we explore two themes raised by a case at our fetal health center: anticipation and accompaniment. In this case, a mother presented to our fetal health center; her unborn son, our fetal patient, was diagnosed with life-threatening hypoplastic left heart syndrome and endocardial fibroelastosis. The parents were told that their son's life expe

Trends in perinatal regionalization and the role of managed care.

OBJECTIVE: To describe trends in regionalization of perinatal care and identify factors that predict the extent of regionalization. METHODS: Data were drawn for four states for every year between 1989 and 1998. Panel data models estimated the effect of managed care enrollment on site of delivery for low, very low, and extremely low birth weight neonates. RESULTS: Strong evidence for regionalization over time was observed for North Carolina and Illinois, with little change in site of delivery in Washington. A shift from level III to level II hospitals was observed for low and very low birth w

Delivery of perinatal care in India: priorities and policies.

Delivery of appropriate perinatal care at primary, secondary and tertiary levels is becoming an urgent and important issue in countries of the Third World where maternal, perinatal and neonatal mortality rates remain at an unacceptably high level. A recent WHO report on perinatal morbidity and mortality in the South-east Asian region (Table I) shows the current state of the problem in this region (1). In India, the perinatal mortality rate (PNMR) varies from 12.5 to 147.2/1000 (2), with most centres reporting rates of 70-90/1000 (Table II) (2-5). Ghosh et al. (6) have reported intranatal asphy

Expanding Perinatal Care Beyond Obstetrics: The National Veterans Affairs Maternity Care Coordinator Program.

Pregnant and postpartum individuals in the United States experience high rates of adverse pregnancy outcomes, despite multiple obstetric initiatives to address this public health crisis. Perinatal care that meaningfully addresses long-term factors that impact pregnancy outcomes, such as chronic mental and physical conditions, adverse social and environmental factors, structural racism, birth-related morbidity and trauma, and transitions to parenting, must go beyond obstetric care sites to include health care providers who deliver care across the lifespan. Veteran's Health Administration (VHA),

Assessment of Perinatal Care Satisfaction Amongst Mothers Attending Postnatal Care in Ibadan, Nigeria.

INTRODUCTION: Patient satisfaction has been identified as a major index in the assessment of quality of healthcare globally. Mothers judge the quality of perinatal care received based on their satisfaction with the services provided, thus influencing their utilization of the available health facilities. There is currently a dearth of literature on users' satisfaction of services rendered at the primary level of care, which is the first port of call to the health system aimed at serving majority of the populace. OBJECTIVES: The study was set out to investigate maternal satisfaction with perina

Perinatal depression: Therapeutic shared decision-making in primary care.

The increasing use of antidepressants, escalating rates of unintended pregnancy, and disproportionately high rate of maternal suicide in the United States, as compared with other high-income countries, all highlight the urgent need for more clarity on available treatment options for perinatal depression. Misrepresentative data obtained from animal experiments and observational human studies, many of which do not account for underlying mental illness as a confounder, largely overestimate the teratogenicity of antidepressants. These outdated data-coupled with a lack of clear guidance from leadin

Prenatal Care and Perinatal Regionalization for Congenital Heart Defects.

IMPORTANCE: Regionalization is a system by which neonates with congenital heart defects (CHDs) are directed to risk-appropriate levels of care. However, the role of prenatal care in delivery location remains unclear. OBJECTIVE: To estimate associations between prenatal care adequacy and the likelihood of delivery at a pediatric cardiac center. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study examined retrospective data from the Illinois Department of Public Health's Adverse Pregnancy Outcomes Reporting System. Participants included neonates born with CHDs in Illinois from 2013 t

Current Strategies for Perinatal Mental Health Care in Japan.

Perinatal mental health care is required to maintain the emotional well-being of pregnant women, as well as their children, partners, and families. The mental and physical support for the pregnant and/or postpartum women with serious mental health problems should be provided with multidisciplinary collaboration in the perinatal area. The adverse outcomes related to impaired perinatal mental health are suicide and child abuse, which are the top reasons why mental health care in pregnant and/or postpartum women is important. Mental health care during the perinatal period should be provided proac

Antenatal Care Strategies to Improve Perinatal and Newborn Outcomes.

BACKGROUND: Antenatal care strategies (ANC) play a pivotal role in ensuring a healthy gestational period for expectant mothers and promote optimal outcomes for their babies. Implementing these interventions can contribute to a supportive environment for pregnant women, resulting in positive perinatal and neonatal outcomes. SUMMARY: We summarize evidence for a total of twenty-seven interventions pertaining to ANC from Every Newborn Series published in The Lancet 2014 by identifying the most recent systematic reviews, extracting data from each review, and conducting a subgroup analysis for low-

Perinatal Care of Childhood Sexual Abuse Survivors: Scoping Review.

BACKGROUND: An estimated one in nine women seeking perinatal care is a survivor of childhood sexual abuse (CSA), yet CSA may be unknown to nurses and other health care providers. Childhood sexual abuse can have adverse physical and psychological effects for survivors, and the intimacy of perinatal care can trigger distress like intrusive thoughts. PURPOSE: To explore available literature about CSA survivors and perinatal care. Specific aims were to 1) identify nursing actions that ease undesirable feelings during perinatal care for CSA survivors and 2) identify gaps in the literature on perin

Perinatal Palliative Care: ACOG COMMITTEE OPINION SUMMARY, Number 786.

Perinatal palliative care refers to a coordinated care strategy that comprises options for obstetric and newborn care that include a focus on maximizing quality of life and comfort for newborns with a variety of conditions considered to be life-limiting in early infancy. With a dual focus on ameliorating suffering and honoring patient values, perinatal palliative care can be provided concurrently with life-prolonging treatment. The focus of this document, however, involves the provision of exclusively palliative care without intent to prolong life in the context of a life-limiting condition, o

Clinical practice guidelines for perinatal bereavement care - An overview.

BACKGROUND: High quality perinatal bereavement care is critical for women and families following stillbirth or newborn death. It is a challenging area of practice and a difficult area for guideline development due to a sparse and disparate evidence base. AIM: We present an overview of the newly updated Perinatal Society of Australia and New Zealand/Stillbirth Centre of Research Excellence guideline for perinatal bereavement care. The guideline aims to provide clear guidance for maternity health care providers and their services to support the provision of care that meets the needs of bereaved

Perinatal outcome in hospital and birth center obstetric care.

OBJECTIVE: Our purpose was to compare birth complications and fetal outcome in hospitals and birth centers. METHOD: We retrospectively compared all 801 deliveries between 1992 and 1994 from two free-standing birth centers against 3271 hospital deliveries in Berlin. The hospital collective was selected according to the same risk criteria of the birth centers. RESULTS: The birth center group had significantly fewer medical interventions, with a similar cesarean section rate (3.0% vs. 4.6%, P = 0.057) and occurrence of severe perineal lesions. The episiotomy rate was significantly higher (P < 0

Expanding care for perinatal women with depression (EXPONATE): study protocol for a randomized controlled trial of an intervention package for perinatal depression in primary care.

BACKGROUND: Depression is common among women during perinatal period and is associated with long-term adverse consequences for the mother and infant. In Nigeria, as in many other low- and-middle-income countries (LMIC), perinatal depression usually goes unrecognized and untreated. The aim of EXPONATE is to test the effectiveness and cost-effectiveness of an intervention package for perinatal depression delivered by community midwives in primary maternal care in which physician support and enhanced patient compliance are implemented using mobile phones.
