Peripartum Cardiomyopathy: A Review Article.

Peripartum cardiomyopathy (PPCM) is an important cause of heart failure (HF) in northern Nigeria and many other regions of the world. Although the aetiology is unknown, several aetiopathogenic mechanisms have been proposed, including myocarditis, vasculo-hormonal (16-kDa prolactin and Cathepsin D), genetic susceptibility and selenium deficiency hypotheses. The peripartum cardiomyopathy in Nigeria (PEACE) registry has revealed that three socioeconomic factors (lack of formal education, unemployment, underweight status), pre-eclampsia and selenium deficiency were independently associated with hi

Electrocardiographic findings in peripartum cardiomyopathy.

BACKGROUND: There is limited data on electrocardiographic (ECG) abnormalities and their prognostic significance in women with peripartum cardiomyopathy (PPCM). We sought to characterize ECG findings in PPCM and explore the association of ECG findings with myocardial recovery and clinical outcomes. HYPOTHESIS: We hypothesized that ECG indicators of myocardial remodeling would portend worse systolic function and outcomes. METHODS: Standard 12-lead ECGs were obtained at enrollment in the Investigations of Pregnancy-Associated Cardiomyopathy study and analyzed for 88 women. Left ventricular ejec

New Insights in Peripartum Cardiomyopathy.

Significant progress in understanding the pathophysiology of peripartum cardiomyopathy, especially hormonal and genetic mechanisms, has been made. Specific criteria should be used for diagnosis, but the disease remains a diagnosis of exclusion. Both long-term and recurrent pregnancy prognoses depend on recovery of cardiac function. Data from large registries and randomized controlled trials of evidence-based therapeutics hold promise for future improved clinical outcomes.

Peripartum cardiomyopathy: approach to management.

PURPOSE OF REVIEW: To provide recommendations for the diagnosis and management of patients with peripartum cardiomyopathy (PPCM). RECENT FINDINGS: PPCM is pregnancy-associated myocardial disease that occurs during pregnancy or postpartum and is characterized by the development of heart failure due to marked left ventricular (LV) systolic dysfunction. The disease is relatively uncommon, but its incidence is increasing, and it remains an important cause of cardiac-related maternal morbidity and mortality in previously healthy young women. With early diagnosis and treatment, the majority of the

Peripartum cardiomyopathy: a contemporary review.

Peripartum cardiomyopathy is a rare and potentially fatal disease. Though approximately half of the patients recover, the clinical course is highly variable and some patients develop refractory heart failure and persistent left ventricular systolic dysfunction. It is diagnosed when women present with heart failure secondary to left ventricular systolic dysfunction towards the end of pregnancy or in the months following delivery, where no other cause of heart failure is found. Etiology remains unclear, and treatment is similar to other cardiomyopathies and includes evidence-based standard heart

[Peripartum cardiomyopathy in young women].

Two women aged 26 and 41 were diagnosed with peripartum cardiomyopathy (PPCM). They presented with shortness of breath and oedematous ankles. The first woman presented in her 37th week of pregnancy. Her father had had dilated cardiomyopathy. A caesarean section was carried out. Her left ventricular function declined and she was therefore treated by means of an Impella heart pump and later, a left-ventricular assisting device. She eventually underwent urgent heart transplantation and recovered. The second woman presented 6 weeks after having given birth to twins. She was treated with a diuretic

Coronary embolus complicating peripartum cardiomyopathy.

Peripartum cardiomyopathy is a rare disorder in which heart failure occurs during the last month of pregnancy or within 5 months of delivery, in the absence of any other etiology or prior heart disease. We present the case of a 42-year-old woman with peripartum cardiomyopathy. She was admitted with an acute myocardial infarction. Multiple mobile ventricular thrombi were seen in the echocardiogram. Coronary angiogram showed consequential coronary embolus occluding the left anterior descending artery. A successful embolectomy was performed followed by coronary stenting. There have been only two

Peripartum cardiomyopathy: a short review.

Peripartum cardiomyopathy is an unusual form of dilated cardiomyopathy, which manifests as acute heart failure in the last trimester of pregnancy or early postpartum period. Its aetiology is currently unknown. The presenting signs and symptoms are those of congestive heart failure and more specifically those of left ventricular failure. Its importance lies in the fact that it has a high mortality rate and strikes the patient in the prime of life. Peripartum cardiomyopathy has far reaching implications for the anaesthesiologist. The reason for this is that many of the signs and symptoms of norm

Peripartum cardiomyopathy: a comprehensive review.

Peripartum cardiomyopathy (PPCM) is a rare disorder in which left ventricular dysfunction and symptoms of heart failure occur in the peripartum period in previously healthy women. Incidence of PPCM ranges from 1 in 1300 to 1 in 15,000 pregnancies. The etiology of PPCM is unknown, but viral, autoimmune, and idiopathic causes may contribute. The diagnostic criteria are onset of heart failure in the last month of pregnancy or in first 5 months postpartum, absence of determinable cause for cardiac failure, and absence of a demonstrable heart disease before the last month of pregnancy. Risk factors

Peripartum cardiomyopathy--a case report.

Peripartum cardiomyopathy in the form of congestive heart disease of unknown etiology appears relatively rarely during the last month of pregnancy or within 5 months after delivery, and it is potentially life-threatening. This case report describes a 34-year-old female, G4P1, at 36 weeks' gestation who was admitted for cesarean section under lumbar epidural anesthesia due to twin pregnancy. She had no past history of cardiovascular diseases. However, rapid onset of dyspnea at ward and acute cardiac failure developed 15 h after cesarean section. Peripartum cardiomyopathy was diagnosed. After a

Peripartum cardiomyopathy: a comprehensive review.

The objective of this article is to review the etiology, diagnosis, treatment, prognosis, and natural history of peripartum cardiomyopathy. The English language medical literature was accessed though MEDLINE from 1966 to the present; additional sources were obtained by cross-referencing. Because of the limited number of studies and patients, metaanalysis could not be performed; however, the existing data regarding the etiology, diagnosis, treatment, and prognosis of peripartum cardiomyopathy are presented. Approximately 1000 U.S. women will have peripartum cardiomyopathy this year, and for man

[Peripartum cardiomyopathy with fatal outcome].

The authors reported the case of a 26 year-old primipara who had unexplained congestive cardiac failure on the 9th week after delivery. There was no previous disease in her history however her pregnancy endangered because of imminent abortion was preceded by 1 artificial and 2 spontaneous abortions. In the acute phase she had to be resuscitated because of ventricular fibrillation and then she had a pulmonary embolism followed by pneumonia and therefore she was in septic state for 4 weeks. After a 4 month clinical remission she died unexpectedly at home. At autopsy the histological appearance o

Current perspectives in peripartum cardiomyopathy.

Peripartum cardiomyopathy (PPCM) is a rare, but potentially life-threatening condition that typically presents during the final month of pregnancy, or within the first few months postpartum. PPCM is characterized by a decrease in left ventricular function that leads to heart failure in pregnant women with no previous history of cardiovascular disease, and for which the etiology of heart failure cannot be identified. Despite advances in understanding its molecular and clinical underpinnings, PPCM remains a diagnosis of exclusion. This review provides an overview of PPCM, including discussions o

Peripartum cardiomyopathy: disease or syndrome?

Peripartum cardiomyopathy (PPCM) is a rare form of pregnancy-associated heart failure and is considered to be a diagnosis of exclusion. There are many hypotheses on the aetiology of PPCM; however, the exact pathophysiological mechanism remains unknown. It shows many resemblances to other conditions, such as familial dilated cardiomyopathy or myocarditis, and therefore it can be hard to make a definite diagnosis. We describe four cases of peripartum-onset heart failure in women who were suspected of having PPCM. We discuss the differential diagnosis, pathophysiological mechanisms and various di

Peripartum Cardiomyopathy Presenting as Bradycardia.

Peripartum cardiomyopathy (PPCM) is a disease that typically affects young otherwise healthy women. As PPCM is associated with significant mortality, timely diagnosis is necessary to ensure appropriate care. To our knowledge, this represents the first reported case of PPCM presenting as symptomatic bradycardia. We describe the patient's clinical presentation and relevant findings and review the potential etiology and ramifications of bradycardia in patients with PPCM.

Practical management of peripartum cardiomyopathy.

Peripartum cardiomyopathy (PPCM) is an idiopathic cardiomyopathy that causes systolic heart failure (HF) in previously healthy young women. Despite latest remarkable achievement, unifying pathophysiologic mechanism is not well established. Considering close temporal relationship to pregnancy, the recent prolactin theory is promising. Abnormal short form of 16-kDa prolactin may be produced in the oxidative stress milieu, show anti-angiogenic effect and damage cardiovascular structure in late pregnancy. Future study is needed to determine whether abnormal prolactin system is useful as a biomarke

Electrocardiographic predictors of peripartum cardiomyopathy.

OBJECTIVE: To identify potential electrocardiographic predictors of peripartum cardiomyopathy (PPCM). METHODS: This was a case-control study carried out in three hospitals in Kano, Nigeria. Logistic regression models and a risk score were developed to determine electrocardiographic predictors of PPCM. RESULTS: A total of 54 PPCM and 77 controls were consecutively recruited after satisfying the inclusion criteria. After controlling for confounding variables, a rise in heart rate of one beat/minute increased the risk of PPCM by 6.4% (p = 0.001), while the presence of ST-T-wave changes increase

[Peripartum cardiomyopathy: a case report].

BACKGROUND: Peripartum cardiomyopathy also known as cardiomyopathy associated with pregnancy, is rarely a cause of heart failure, it affects pregnant or puerperal women in the first 5 months. Although the first case reported was in 1849, it was recognized until 1930. In 2010 the European Society of Cardiology Working Group on peripartum cardiomyopathy, defined this pathology as an idiopathic cardiomyopathy that affects pregnant women between the third trimester and five months after delivery. Characterized by a left ventricular failure with an ejection fraction of ≤45% and an end-diastolic dim
