Factors in the rehabilitation of patients with peripheral vascular disease.

At some point in the progression of peripheral vascular disease, a hospitalized patient may require the services of a rehabilitation unit. All patients, however, can benefit from basic rehabilitative care that should begin at the time of initial diagnosis of their disease and be considered throughout their treatment course. The nurse in the clinic , in the intensive care unit, and on the medical and surgical units should be aware of potential factors that may prevent the person with peripheral vascular disease from realizing his full potential. Physical, psychologic, social, economic, vocation

An advanced ultrasound application used to assess peripheral vascular diseases: superb microvascular imaging.

Over the past several years, a novel ultrasound imaging modality termed superb microvascular imaging (SMI) has enabled visualization of microvessels. SMI ultrasound studies of peripheral artery diseases have significantly extended our knowledge of tissue microcirculation and the arterial microenvironments of atherosclerotic lesions. We here present an overview of current knowledge on the utility of SMI assessment of vascular diseases and highlight certain peripheral microcirculation disorders for which SMI is particularly valuable. The evidence indicates that SMI can detect intraplaque neovasc

Percutaneous vascular stent: experimental studies and preliminary clinical results in peripheral arterial diseases.

A new pattern of metallic intravascular stent has been experimentally and clinically tested. When the stent is percutaneously placed through a six french introducer, it is held against the vessel wall by its natural elastic properties. In 28 animals, 47 stents of different sizes (from 3 to 5 mm of luminal diameter and from 1.5 to 5 cm of length) have been implanted in the femoro-popliteal (16), the coronary (20), the carotid (3) and the renal arteries (5) and in 3 femoral veins. Angiographic and histological results have shown the stent's low thrombogenicity when it is well fit to the vessel's

Modern approaches and innovations in the diagnosis and treatment of peripheral vascular diseases.

Amongst the three major vascular beds (coronary, cerebrovascular, and peripheral), peripheral vascular disease (PVD) has traditionally received the least attention, despite its growing global burden. The aging population has led to the increased prevalence of PVD, thereby increasing visibility to its various diagnostic and treatment modalities. In the past decade, research and development of innovations in the management of PVD has exploded. Modern advances in imaging, molecular technology, medical devices, and surgical techniques have reduced the morbidity and mortality of PVD. However, many

Screening for concomitant diseases in peripheral vascular patients. Results of a systematic approach.

BACKGROUND: Patients considered for arterial surgery, have been shown to have a high incidence of coexistent cardiac, vascular and other diseases, affecting operative risk and survival. We developed a systematic workup strategy for detecting these coexistent diseases in our vascular surgical patients, mainly based on non-invasive diagnostic techniques. METHODS: We evaluated 200 consecutive patients, admitted to the department of vascular surgery in an academic teaching hospital, in order to establish the total incidence of relevant concomitant disorders, the extent to which this screening yie

Peripheral vascular disease - a new vascular disease associated with normal tension glaucoma: a case report.

INTRODUCTION: Normal-tension glaucoma is known as a multifactorial optic neuropathy. A number of lines of evidence suggested that vascular factors played a significant role in the development of normal-tension glaucoma. The mechanisms underlying the abnormal ocular blood flow in normal-tension glaucoma are still not clear. Peripheral vascular disease seems to be associated with glaucoma populations independent of other cardiovascular risk factors. We found this presentation, for the first time, to our knowledge, as another probable vascular abnormality related to our patient with normal-tensio

Peripheral vasodilators and the management of peripheral vascular disease and Raynaud's syndrome in general practice.

There is no convincing evidence that peripheral vasodilators produce any significant improvement in exercise tolerance in patients with peripheral vascular disease, and these drugs may do more harm than good. In the treatment of severe Raynaud's syndrome, however, thymoxamine, prazosin or nifedipine is recommended. A descriptive study was carried out, firstly, to determine why these drugs are prescribed in general practice, and secondly, to describe the drug choices in the treatment of both Raynaud's syndrome and peripheral vascular disease in a representative sample of 22 practices in Norther

Role of Indocyanine Green Fluorescence to Assess Skin Vascularity in Peripheral Vascular Disease.

OBJECTIVES: The objective is to assess the level of amputation based on the skin vascularity using indocyanine fluorescence. BACKGROUND: Patients with chronic limb ischemia have a risk of amputation within 1 year and reamputation due to impaired wound healing postamputation. Therefore, the assessment of tissue perfusion and viability before the planned surgery can minimize the chances of reamputation. To qualify the level of tissue perfusion, near-infrared (NIR) fluorescence imaging is promising. METHODS: The study includes adult patients suffering from peripheral vascular disease and in nee

Evaluation of peripheral vascular system disorders in vascular symptom--free Behçet's disease.

Vascular involvement in Behçet's disease has been reported in 7-40% of patients in the literature and nearly 8% of these will have serious vascular complications during follow-up. So far, little information is available on the extent of vascular involvement in patients with Behçet's disease who are free of vascular symptoms. The study comprised 34 individuals with Behçet's disease who were free of vascular symptoms and 47 healthy volunteers. Both groups underwent non-invasive peripheral vascular examination consisting of Doppler, photoplethysmography, and segmental pressure measurements and de

[Current strategies for vascular rehabilitation in peripheral artery disease].

Recent international guidelines strongly recommend physical exercise as a cornerstone in the management of patients with symptomatic peripheral artery disease. Optimizing the vascular rehabilitation pathway is therefore critical to maximizing functional outcomes, relieving symptoms, and improving overall patient care. This article provides an overview of the key features and specific considerations involved in implementing a vascular rehabilitation program, along with a detailed review of the latest evidence-based recommendations.

[Vascular rehabilitation for patients with peripheral arterial occlusive disease].

Compared to patients with coronary heart disease, patients with PAD have a poorer attitude towards general practitioners with regard to the frequency of prescribing rehabilitative measures and controlling risk factors. The patients themselves do not perceive the increased cardiovascular risk and thus the need for lifestyle changes and compliance to the same extent as patients with coronary heart disease. The "Rehabilitation clinic with vascular expertise" certificate defines the criteria for fulfilling the specific structural and process requirements for vascular medicine.Due to the paradigm sh

Carnosine as a potential therapeutic for the management of peripheral vascular disease.

AIMS: To evaluate the potential role of carnosine in the management of peripheral vascular disease. DATA SYNTHESIS: Peripheral vascular disease is growing in its burden and impact; however it is currently under researched, and there are a lack of strong, non-invasive therapeutic options for the clinicians. Carnosine is a dipeptide stored particularly in muscle and brain tissue, which exhibits a wide range of physiological activities, which may be beneficial as an adjunct treatment for peripheral vascular disease. Carnosine's strong anti-inflammatory, antioxidant and antiglycating actions may

Rosai-Dorfman Disease Presenting as Peripheral Vascular Insufficiency.

Rosai-Dorfman (-Destombes) disease (RDDD) is a rare idiopathic disorder of histiocyte proliferation, usually involving lymph node stations. The most common clinical finding is a bilateral cervical lymphadenopathy, fever, and weight loss. Arterial or venous structures are notably not involved. We hereby present a case of a 78-year-old Caucasian man, presenting with symptoms of progressive arterial insufficiency and right lower-limb edema, along with a nonpulsatile mass at the middle third of the thigh. Initial diagnostic hypothesis was a superficial femoral artery aneurysm thrombosis with a sec

Anesthesia for Patients with Peripheral Vascular Disease and Cardiac Dysfunction.

Patients with vascular disease and cardiac dysfunction present particular challenges to the anesthesiologist. They are hemodynamically brittle, at high risk of morbidity and mortality during surgery, and often carry additional comorbidities that increase their complexity and risk. Those with peripheral vascular disease should be assumed to have coronary artery disease and tend to have other systemic vascular problems. Poor cardiac function further worsens perfusion in an already compromised peripheral vascular system. Care of these patients requires judicious monitoring, an anesthetic that opt

Diagnosis of peripheral arterial disease: utility of the vascular laboratory.

The risks of undiagnosed peripheral arterial disease (PAD) include progressive reduction in quality of life, potential limb loss, and unrecognized comorbid atherosclerotic (concomitant coronary and cerebrovascular) disease. Although a thorough historical review of symptoms and a comprehensive physical examination may lead the clinician to an appropriate index of suspicion, accurate diagnostic testing is crucial. Accredited vascular diagnostic laboratories provide a risk-free description of the presence, severity, and location of PAD. This article describes the basic noninvasive diagnostic test
