Periprosthetic femoral fractures above total knee replacements.

Periprosthetic femoral fractures above total knee replacements can be managed by a variety of methods, including casting, open reduction and internal fixation, external fixation, or revision arthroplasty. Because no single method has emerged as the optimal choice for all such fractures, it is important to understand which options are appropriate for each fracture pattern. Early classification systems focused on displacement as a major indication for either surgical or nonsurgical management. However, recent techniques and current implants have made surgical management preferable for most perip

Periprosthetic femoral fractures following total knee arthroplasty.

Supracondylar and distal femoral fractures involving total knee arthroplasties present a relatively new and challenging problem. The alignment of the total knee components prior to and subsequent to the fracture, as well as the patients' range of motion, symptoms, and level of function following their treatment, were investigated in 14 cases. Ten of the patients had nonoperative treatment. Four had had an operative procedure. Osteoporosis, notching of the femur, and revision arthroplasty were identified as predisposing factors with femoral fractures. The results of treatment depended primarily

The epidemiology and outcomes of periprosthetic femur fractures : the Scottish National Audit of Periprosthetic Femur Fractures (SNAP Femur) study.

AIMS: Periprosthetic femur fracture (PFF) data capture in national registries is recognised to be limited. The aim of this study was to use hospital level data to establish patient characteristics, describe PFF management and mortality (including standardized mortality rate (SMR)), and compare outcomes to native hip fractures (NHF) using Scottish Hip Fracture Audit (SHFA) data. METHODS: A nationwide retrospective cohort study was conducted of all patients aged ≥ 50 years sustaining a PFF managed in 16 centres between 1 January and 31 December 2019. Data collected included patient demographic

Distal femoral fractures: periprosthetic fractures have four times more complications than non-periprosthetic fractures and cerclage should be avoided: retrospective analysis of 206 patients.

BACKGROUND: Distal femoral fractures account for less than 1% of all fractures. The therapy of choice is usually surgical stabilization. Despite advances in implant development over the past few years, complication rate remains comparatively high. The aim of this study is to analyze our results with plate fixation of distal femoral fractures with a focus on complication and fracture healing rates. METHODS: In this retrospective cohort study, patients (> 18 years) with distal femoral fractures treated at an urban level I trauma center between 2015 and 2022 were analyzed. RESULTS: In total, 20

A Periprosthetic Femoral Fracture with Characteristics of Atypical Femoral Fracture.

Although the definition of atypical femoral fracture (AFF) excludes periprosthetic femoral fracture (PFF), the number of reports about PFF with characteristics of AFF is increasing. We present the case of such a fracture in this report. An 87-year-old woman who underwent bipolar hip arthroplasty for a femoral neck fracture 38 months prior reported left thigh pain with no history of trauma. Radiographs showed a simple transverse fracture at the level of the stem distal end with features of AFF: periosteal thickening of the lateral cortex, a medial spike, and a noncomminuted fracture. She presen

Periprosthetic Femur Fracture Around the Hip.

Periprosthetic femur fractures around the hip are one of the most common aseptic complications following total hip arthroplasty. Understanding the risk factors of periprosthetic femur fracture can aid surgeons in the prevention of these injuries. The Vancouver classification provides a reproducible description of the factors that should be considered in the treatment of patients with periprosthetic femur fractures: fracture site, implant stability, and bone stock. It is important for the surgeon to review the epidemiology and risk factors for periprosthetic femur fractures around the hip along

Periprosthetic fractures of the knee: a comprehensive review.

Demographic changes have resulted in an increase in the number of older patients diagnosed with degenerative joint disease. Developments in the field of joint arthroplasty allow a broader population to improve their lifestyles. An increased demand for knee arthroplasty has led to a rise in operations performed worldwide. Although there has been a constant propagation of technology and an increase in medical staffing at a professional level, many patients still encounter complications. Though rare, these factors may lead to life-threatening scenarios and a devastating effect on the success of t

Diagnosing Infection in the Setting of Periprosthetic Fractures.

BACKGROUND: The diagnosis of periprosthetic joint infection is particularly challenging in patients with periprosthetic fractures. The purpose of this study was to investigate the utility of commonly used diagnostic tests for periprosthetic joint infection in patients with a periprosthetic fracture. METHODS: Of 121 patients treated with a periprosthetic fracture (97 hips, 24 knees, mean age: 72.9), 14 (11.6%) met Musculoskeletal Infection Society criteria for infection. Diagnostic variables were evaluated using logistic regression models for the prediction of infection and receiver operating

Periprosthetic fracture fixation in osteoporotic bone.

Fixation techniques of periprosthetic fractures are far from ideal although the number of this entity is rising. The presence of an intramedullary implant generates its own fracture characteristics since stiffness is altered along the bone shaft and certain implant combinations affect load resistance of the bone. Influencing factors are cement fixation of the implant, intramedullary locking and extramedullary or intramedullary localization of the implant and the cortical thickness of the surrounding bone. Cerclage wires are ideally suited to fix radially displaced fragments around an intramedu

Treatment of periprosthetic femoral fractures of the knee.

PURPOSE: We report a continuous series of periprosthetic femoral fractures after knee arthroplasty treated with a locking plate. We hypothesize that minimally invasive surgery and immediate weight-bearing improve functional recovery. METHODS: From June 2002 to December 2008, 15 patients with 16 fractures were treated. Median age was 81 years. The autonomy level according to the mobility score of Parker and Palmer showed a median of 5 (0-9). Osteosynthesis was performed mainly through a minimally invasive approach using a locking compression plate. The rehabilitation protocol consisted of imme

[Periprosthetic femoral fractures: the orthopaedic disease of affluence?].

A total hip prosthesis (THP) is an effective treatment for hip osteoarthritis. Complications like periprosthetic fractures (PPF) are increasing due to an aging population. PPF accounts for 15% of THP revisions in the Netherlands, with rising incidence driven by risk factors such as prior revisions, older age, osteoporosis, falls, and cementless stems. Cemented stems offer a protective factor, particularly in octogenerians. PPF treatment is complex and costly, often requiring surgical intervention through prosthesis revision and/or open reduction and internal fixation (ORIF), both associated wi

Atypical Vancouver B1 Periprosthetic Fractures: The Unsolved Problem.

Atypical femoral fractures (AFF) are stress or insufficiency fractures induced by low energy trauma or no trauma, frequently correlated with prolonged bisphosphonate therapy. The diagnosis follows major and minor criteria, originally described by the Task Force of the American Society for Bone and Mineral Research in 2010 and updated in 2014. However, the definition of AFFs in the report excluded periprosthetic fractures. When atypical fractures occur close to a prosthetic implant the situation become critical, the surgical treatment is often demolitive and supported by medical treatment. More

KNEE PERIPROSTHETIC FRACTURES IN THE ELDERLY: CURRENT CONCEPT.

Periprosthetic fractures around total knee arthroplasty in elderly represent an emerging cause of implant revision and their incidence seems destined to further increase in the upcoming years, considering the ever-increasing number of implanted prostheses. These are complex injuries with very high complication rates. It has been estimated that the incidence of femoral periprosthetic fractures after T.K.A. ranged between 0,3 to 2,5%, but increases up to 38% when considering revision T.K.A. Patient-related risk factors for T.K.A. periprosthetic fracture (T.K.A.P.F.) include osteoporosis, age, fe

Risk Factors for Wound Complications After Periprosthetic Fractures.

The number of hip and knee arthroplasties performed annually continues to rise. Revision rates are projected to increase by 137% to 601%, with periprosthetic fractures to be among the leading cause of revision. Wound complications following surgical treatment of periprosthetic fractures are a major source of patient morbidity and health care costs. This study evaluated risk factors for wound healing complications in patients undergoing surgical management of periprosthetic fractures around the hip and knee. This was a retrospective analysis of 67 consecutive lower-extremity periprosthetic hip

OBJECTIVE: Stabilization of the humeral shaft and the restoration of shoulder function dependent on the osseous integrity of the prosthetic stem component. INDICATIONS: In cases of a stable prosthesis stem, an isolated plate osteosynthesis is possible. Prosthesis stem replacement is indicated in cases of a loose stem. With sufficient bone stock of the proximal humeral segment, a change to a shorter humeral shaft component with subsequent plate fixation of the fracture is possible. If the bone stock is poor, conversion to a long revision stem is necessary. CONTRAINDICATIONS: Inoperability of
