Nutritional behaviour of patients on continuous ambulatory peritoneal dialysis.

Seven patients on home continuous ambulatory peritoneal dialysis (CAPD) were investigated for their spontaneous nutritional behaviour. Energy intake with dialysate glucose gain amounted to 130kJ/kg BW/day. The distribution found was: 56% carbohydrates, 35% fat and 10.9% protein. The protein intake opposed by peritoneal loss of aminoacids (1.76g/day) and of proteins (9.4g/day) amounted to 0.78g/kg BW/day. Subtraction of the peritoneal loss yields only 0.58g/kg/day; however, estimation of total nitrogen balance was slightly positive and nutrition related serum proteins remained comparable to oth

Tuberculous peritonitis in a child undergoing continuous ambulatory peritoneal dialysis.

We present a 13-year-old girl with Arnold-Chiari syndrome and uremia secondary to neurogenic bladder. She had been treated with continuous ambulatory peritoneal dialysis (CAPD) for 13 months prior to the development of peritonitis. The patient demonstrated no improvement with a 3-day therapy of intraperitoneal vancomycin and netilmicin. Meanwhile, smear of centrifuged dialysate revealed acid fast bacilli on two occasions. We, then, started anti-TB therapy with oral isoniazid (INAH), rifampin and ethambutal. The symptoms subsided within three days. In the first week, the patient lost her perito

Listeria monocytogenes peritonitis in a patient on continuous ambulatory peritoneal dialysis.

We report a case of peritonitis in a patient on continuous ambulatory peritoneal dialysis caused by Listeria monocytogenes. The patient was taking oral corticosteroids for underlying systemic lupus symptoms, and had classic signs and symptoms of peritonitis. There was initial clinical deterioration while in vitro data indicating that the organism was sensitive to vancomycin with a minimum inhibitory concentration of 0.5 micrograms/ml. The patient rapidly responded to intravenous ampicillin. We believe this to be the second reported case of L. monocytogenes peritonitis in a CAPD patient.

Continuous Ambulatory Peritoneal Dialysis Peritonitis Guidelines - Consensus Statement of Peritoneal Dialysis Society of India - 2020.

Continuous ambulatory peritoneal dialysis (CAPD) related peritonitis is a major cause of technique failure, morbidity, and mortality in patients on CAPD. Its prevention and management is key to success of CAPD program. Due to variability in practice, microbiological trends and sensitivity towards antibiotics, there is a need for customized guidelines for management of CAPD related peritonitis (CAPDRP) in India. With this need, Peritoneal Dialysis Society of India (PDSI) organized a structured meeting to discuss various aspects of management of CAPDRP and formulated a consensus agreement which

Hypomagnesemia in continuous ambulatory peritoneal dialysis patients dialyzed with a low-magnesium peritoneal dialysis solution.

OBJECTIVE: Previous studies have shown a decrease in serum magnesium (Mg) concentration when continuous ambulatory peritoneal dialysis (CAPD) patients previously maintained on a 1.0-1.2 mEq/L Mg peritoneal dialysis solution (PDS) were dialyzed with a 0.5 mEq/L Mg PDS. However, the prevalence of hypomagnesemia in CAPD patients dialyzed with low-Mg PDS is unknown. DESIGN: A retrospective study to determine the prevalence of hypomagnesemia and the factors associated with its occurrence in CAPD patients dialyzed using a 0.5 mEq/L Mg PDS. SETTING: A CAPD unit in a large Veterans Affairs Hospital.

Changes in peritoneal membrane after continuous ambulatory peritoneal dialysis--a histopathological study.

Peritoneal membrane changes in continuous ambulatory peritoneal dialysis (CAPD) patients have been widely described but poorly classified. Our aim was to identify the morphological changes occurring after CAPD treatment. To this end, 17 biopsies of parietal peritoneum (1 cm in diameter) were withdrawn at least 5 cm from the catheter entry hole and stained with Van Gieson, hematoxylin-eosin, trichrome, and some immunohistochemical stains: keratin, vimentin, CD34, CD20, CD4, CD8, desmin, and collagen IV. The morphology of mesothelium, vessels, and basement membrane (BM) of mesothelium and vessel

The epidermo-peritoneal potential in patients treated with continuous ambulatory peritoneal dialysis.

At physiologic pH, S. epidermidis moves along an electrical potential gradient. We measured the epidermo-peritoneal electrical potential (EPP) in 23 end-stage renal failure patients treated with CAPD. There was a negative correlation between the mean EPP and the patient's age (r = 0.47, p = 0.016), but no correlation between the mean EPP and the duration of treatment (r = 0.003, p = 0.5). The EPP was greater in those patients with a history of recurrent bacterial peritonitis due to S. epidermidis [median EPP 23 mv (95% confidence limits 16-51)] compared to those with only one or no episodes of

Differences in predicting glucose absorption from peritoneal dialysate compared to measured absorption in peritoneal dialysis patients treated by continuous ambulatory peritoneal dialysis and ambulatory peritoneal dialysis cyclers.

BACKGROUND AND AIMS: Glucose-containing peritoneal dialysates are used to generate an osmotic gradient for the convective removal of water and sodium. Predictive equations were developed to estimate glucose absorption without having to formally measure changes in dialysate glucose. In view of the changes in peritoneal dialysis prescriptions over time, we compared predicted and measured glucose absorption. SUBJECTS/METHODS: We measured peritoneal glucose losses when peritoneal dialysis patients attended their first assessment of peritoneal membrane function, and compared this to glucose exposu

The Nutritional Management of Patients with Continuous Ambulatory Peritoneal Dialysis.

End-stage kidney disease (ESKD) is a severe final phase of chronic kidney disease (CKD). Currently, it is related to high morbidity and mortality rates, making it an important health issue and a catastrophic disease. There is an increase in the death rate, especially when the underlying metabolic disorders are not treated with renal replacement therapy. Continuous ambulatory peritoneal dialysis (CAPD), or continuous dialysis in the peritoneal cavity, is one of the treatment options available in Indonesia as CKD becomes more prevalent each year, in addition to hemodialysis and kidney transplant

Quality of life in automated and continuous ambulatory peritoneal dialysis.

OBJECTIVE: Despite a lack of strong evidence, automated peritoneal dialysis (APD) is often prescribed on account of an expected better quality of life (QoL) than that expected with continuous ambulatory peritoneal dialysis (CAPD). Our aim was to analyze differences in QoL in patients starting dialysis on APD or on CAPD with a follow-up of 3 years. METHODS: Adult patients in the prospective NECOSAD cohort who started dialysis on APD or CAPD were included 3 months after the start of dialysis. Laparoscopic cholecystectomy in a patient on continuous ambulatory peritoneal dialysis.

The patient was a 72-year-old man who was receiving continuous ambulatory peritoneal dialysis (CAPD) with a diagnosis of chronic renal failure. Although his response to dialysis therapy was favorable, right hypochondralgia and fever occurred, and gallstones were detected by abdominal ultrasonography and computed tomography. Drip-infusion cholangiography (DIC) revealed neither dilation nor calculus in the common bile duct. The patient was diagnosed as having acute cholecystitis and cholecystolithiasis and, in consideration of his general condition, laparoscopic cholecystectomy was carried out.

Determinants of catheter loss following continuous ambulatory peritoneal dialysis peritonitis.

BACKGROUND: Few patients are able to resume peritoneal dialysis (PD) therapy after an episode of peritonitis that requires catheter removal. PD catheter loss is therefore regarded as an important index of patient morbidity. The aim of the present study was to evaluate factors influencing catheter loss in patients suffering from continuous ambulatory PD (CAPD) peritonitis. PATIENTS AND METHODS: We retrospectively reviewed 579 episodes of CAPD peritonitis from 1999 to 2006 in a tertiary-care referral hospital. Demographic, biochemical, and microbiological characteristics were recorded. Episodes

Hemodialysis versus continuous ambulatory peritoneal dialysis: effects on the heart.

In this study we compared the influence of 2 different modalities of treatment, CAPD and hemodialysis, on the prevalence and severity of left ventricular hypertrophy and cardiac arrhythmias of chronic renal failure patients. We compared 27 patients on the CAPD program with 27 patients on the chronic hemodialysis matched for sex, age, and duration of dialysis treatment. The prevalence of hypertension was significantly lower in CAPD than in hemodialysis patient (41% vs. 81%, p = 0.0023). Blood pressure levels were also lower in CAPD than in hemodialysis patients (systolic pressure 124.9 +/- 4.7

Endocrine abnormalities in patients treated by continuous ambulatory peritoneal dialysis.

Endocrine-metabolic disturbances of renal failure have many underlying mechanisms, including abnormal secretion, transport, and target cell binding, impaired synthesis and elimination by the diseased kidney, and responses to stimuli resulting from altered homeostasis. Neither hemodialysis nor continuous ambulatory peritoneal dialysis (CAPD) removes large amounts of hormones. By correcting metabolic, fluid and electrolyte disturbances, dialysis may improve some endocrine abnormalities. Possibly because of more permeable membranes, or continuous treatment including ultrafiltration, CAPD has a so

[Encapsulating peritonitis during continuous ambulatory peritoneal dialysis. A physiopathologic hypothesis].

A case of sclerosing encapsulating peritonitis was observed in a patient who had been treated by continuous ambulatory peritoneal dialysis for 4 years. During that period the patient had always used a lactate buffered dialysate (Dianeal solution, supplied by Travenol) and no disinfectant. Nine episodes of peritonitis occurred during the first two years of dialysis. At the end of the first year the patient had decreased ultrafiltration associated with high glucose absorption. A peritoneal biopsy performed at that time showed mesothelial alterations and desquamation. Sclerosing encapsulating per

Vitamin nutrition in patients on continuous ambulatory peritoneal dialysis (CAPD).

In 10 patients who had been on CAPD for 8.75 months, blood levels of the vitamins A, E, B-complex and C were measured and a precise diet history using food weighing for 3 days was obtained. Plasma vitamin A was elevated in all; since retinol binding protein (RPB) was elevated even more, the ratio of retinol to RBP was low. Vitamin E levels were also high. The vitamins B1, B2 and B6 were measured using erythrocyte enzyme activities. Vitamin B1 was low or borderline in 5, vitamin B6 was decreased in 3 and erythrocyte pyridoxal phosphate in 8 patients. Folic acid was low or borderline in 6 patien
