KL-6 as a Tumor Marker of Primary Peritoneal Cancer in Patients with Connective Tissue Diseases.

We herein report two patients with connective tissue disease who developed primary peritoneal cancer (PPC). Serum Krebs von den Lungen-6 (KL-6) levels increased when PPC was diagnosed, and these levels were correlated with the treatment and worsening of PPC in both cases. In one patient with systemic sclerosis, serum KL-6 levels increased despite stable interstitial lung disease (ILD), leading to a diagnosis of PPC. In the other patient with dermatomyositis and no ILD, PPC was diagnosed with elevated KL-6 levels four months post-treatment, without ILD development. Clinicians should be reminded

Patients with immunological diseases or on peritoneal dialysis are prone to false positive flow cytometry crossmatch.

Despite implementation of virtual crossmatches, flow cytometry crossmatches (FCXM) are still used by many transplant centers to determine immunological risk before kidney transplantation. To determine if common profiles of patients prone to false positive FCXM exist, we examined the demographics and native diseases of kidney patients tested with autologous FCXM (n = 480). Improvements to FCXM and cell isolation methods significantly reduced the positive rate from 15.1% to 5.3%. Patients with native diseases considered 'immunological' (vasculitis, lupus, IgA nephropathy) had more positive autol

Peritoneal dialysis in polycystic kidney disease patients. Report from the French peritoneal dialysis registry (RDPLF).

BACKGROUND: It is commonly believed that polycystic kidney disease (PKD) patients on peritoneal dialysis (PD) are over-exposed to technique failure and peritonitis compared with other patients. This study was carried out to assess whether PKD is associated with technique failure and to evaluate the outcome of PKD patients on PD. METHODS: This was a retrospective cohort study based on the data of the French Language Peritoneal Dialysis Registry. We analysed 4162 incident non-diabetic patients who started PD between January 2002 and December 2007. The end of the observation period was 31 Decemb

Clinical course of Klebsiella variicola peritonitis in end-stage kidney disease patients receiving peritoneal dialysis.

Klebsiella variicola is part of the K. pneumoniae complex and has been recently identified to have pathogenic properties. Emerging reports show that it can cause bloodstream, respiratory and urinary tract infections. It has been shown to be an important causative agent in immunocompromised individuals. We report here three cases of K. variicola peritonitis encountered in our peritoneal dialysis patients.

Concurrent development of encapsulating peritoneal sclerosis and calciphylaxis in a patient with peritoneal dialysis for end-stage renal disease.

Both encapsulating peritoneal sclerosis (EPS) and calciphylaxis are rare but severe complications involving patients with end-stage renal disease. In this report, we discuss a unique case of a 73-year-old female patient who had undergone 8 years of peritoneal dialysis for IgA nephropathy and concurrently developed these two synchronous complications within 3 months of each other. Diagnosis and management of both conditions were discussed in detail as well as the possible association between the two. With surgical treatment for EPS and measures to minimise bone mineral disorder abnormalities, b

Neisseria cinerea-Mediated Peritonitis in an End-Stage Renal Disease Patient on Continuous Ambulatory Peritoneal Dialysis.

Peritonitis can be a lethal outcome of peritoneal dialysis (PD), often leading to significant morbidity and mortality. It is caused mostly by gram-positive organisms. Neisseria cinerea is a gram-negative nasal and oropharyngeal commensal, rarely reported as an etiology of peritonitis in PD patients. Our patient was a 37-year-old female on continuous ambulatory peritoneal dialysis for the last seven years, who developed peritonitis found to be from Neisseria cinerea. She didn't respond to broad-spectrum antibiotics well and had to be switched to intermittent hemodialysis. We highlight this impo

Urgent-start peritoneal dialysis versus conventional-start peritoneal dialysis for people with chronic kidney disease.

BACKGROUND: Urgent-start peritoneal dialysis (PD), defined as initiation of PD within two weeks of catheter insertion, has been emerging as an alternative mode of dialysis initiation for patients with chronic kidney disease (CKD) requiring urgent dialysis without established permanent dialysis access. Recently, several small studies have reported comparable patient outcomes between urgent-start and conventional-start PD. OBJECTIVES: To examine the benefits and harms of urgent-start PD compared with conventional-start PD in adults and children with CKD requiring long-term kidney replacement th

Isolated peritoneal hydatid disease: a rare case report.

Hydatid disease is a zoonotic parasitic infection caused by Echinococcus granulosus and is not uncommonly encountered in South Africa, especially in certain endemic areas. It generally affects the liver and lungs, with primary isolated peritoneal involvement being extremely rare. We report the case of a 14-year-old female with an isolated peritoneal hydatid cyst, initially thought to be a simple ovarian cyst, with no apparent hepatic or pulmonary involvement. This case highlights the potential consideration of hydatid disease in the differential of peritoneal cystic masses, even in the absence

[Benign multicystic peritoneal mesothelioma in a patient with Crohn disease].

Benign multicystic peritoneal mesothelioma is an uncommon lesion arising from the peritoneal mesothelium. It is asymptomatic or presents with unspecific symptoms. Imaging techniques may reveal it, however the final diagnosis can only be made by histopathology. Surgery is the only effective treatment considering its high recurrence rate. We report a 19 years old male with Crohn’s disease. Due to persistent abdominal pain, an abdominal magnetic resonance imaging was performed, showing a complex cystic mass in the lower abdomen. The patient underwent surgery and the lesion was completely resected

[Our experience in the surgical treatment of peritoneal hydatid disease].

The Authors report on a series of 9 patients with peritoneal hydatidosis submitted to surgery during the period between 1974 and 2003. In 5 cases the cyst was solitary, while in 3 patients the cysts were multiple, in 1 spread and in 2 there was a concomitant liver hydatidosis. In 2 patients the disease was asymptomatic, while 4 cases were revealed by pain and 3 by an abdominal mass. The diagnosis was made at the operation in 3 patients, while it was performed by ultrasonography in 1 case, by CT scan in 4 and by both methods in 1. All patients were operated on: 7 underwent total cystectomy and

Malignant peritoneal mesothelioma mimicking mesenteric inflammatory disease.

A case of malignant peritoneal mesothelioma mimicking mesenteric inflammatory disease (MID) is presented. The patient had mesenteric and omental lesions characterized at biopsy by extensive fibrosis of fat tissue with mild to moderate inflammation. One year later, post-mortem examination revealed a well-differentiated epithelial mesothelioma. Immunohistochemical stains for keratin and vimentin were diffusely positive, whereas EMA showed a membranous staining of scattered cells. CEA, Ber-EP4, B72.3 and Leu-M1 were negative. In addition, actin monoclonals decorated groups of cells pertaining to

A nomogram for predicting the risk of peritoneal dialysis-associated peritonitis in patients with end-stage renal disease undergoing peritoneal dialysis: model development and validation study.

OBJECTIVE: This study aimed to develop and validate a nomogram to predict the risk of peritoneal dialysis-associated peritonitis (PDAP) in patients undergoing peritopreneal dialysis. METHODS: A retrospective analysis was conducted on clinical data from 376 patients at Nanhai District People's Hospital in Foshan City, Guangdong Province, between December 2017 and December 2024. The dataset was randomly divided into a training set (n = 244) and a validation set (n = 132). Risk factors for PDAP were identified using Least Absolute Shrinkage and Selection Operator (LASSO) regression and logistic

Insulin resistance in cardiovascular disease, uremia, and peritoneal dialysis.

Diabetic nephropathy is highly correlated with the occurrence of other complications of type 1 diabetes (T1D) and type 2 diabetes (T2D) mellitus; for example, hypertension with cardiovascular disease (CVD) being the most frequent cause of death in patients with end-stage renal disease and undergoing renal dialysis. Hyperglycemia and insulin resistance (IR) are responsible for the micro- and macrovascular complications of diabetes through different mechanisms. In particular, IR plays a key role in the etiology of atherosclerosis in both diabetic and non-diabetic patients. IR - exacerbated by or

A Comparison of Hemodialysis and Peritoneal Dialysis in Patients with Cardiovascular Disease.

The high prevalence of cardiovascular disease is caused by the traditional cardiovascular risk factors common among end-stage renal disease patients, and nontraditional risk factors attributed to underlying kidney disease, including chronic inflammation, anemia, bone mineral disease, and the dialysis procedure itself. Individualization of the treatment of cardiovascular disease in end-stage renal disease that could impact the underlying mechanisms of the cardiovascular diseases is important to improve outcomes. This article reviews and compares hemodialysis and peritoneal dialysis in associati
