Perivascular epithelioid cell tumors (PEComas) are rare mesenchymal neoplasms composed of perivascular epithelioid cells that are immunoreactive for both melanocytic and myogenic markers. Recently, a fibroma-like PEComa associated with tuberous sclerosis complex (TSC) has been identified. We describe the first intra-articular case of a fibroma-like PEComa in a 44-year-old man who presented with a hypointense intra-articular knee mass, which was mistaken for tenosynovial giant cell tumor, diffuse type. This case report demonstrates that fibroma-like PEComa should be included in the extended dif

Perivascular epithelioid cell tumors (PEComas) refers to a family of mesenchymal neoplasms composed of angiomyolipomas, clear cell "sugar" tumors of the lung, and lymphangioleiomyomatoses. These tumors have a distinctive and common component of perivascular epithelioid cells that show an association with blood vessel walls and immunohistochemically display myomelanocytic differentiation. The unique neoplasms have been shown to have an expanded range through a variety of case reports, including visceral, intra-abdominal, soft tissue, and bone tumors. The retroperitoneum, abdominopelvic region,

Epithelioid angioleiomyoma (EALM) is rare in the skin and subcutaneous tissues. To the best of our knowledge, only two previous cases of this tumor have been reported. We document here the case of an 83-year-old woman who underwent complete removal of a squamous cell carcinoma of the retromolar trigone and lymph node dissection of the neck. An incidental EALM was observed in the adipose tissue. The tumor formed a unilocular, poorly demarcated neoplasm measuring 0.3 cm, and showed cavernous angiomatous spaces with villiform growth of large epithelioid cells arranged in clusters. Besides the epi

Angiomyolipoma is the most common mesenchymal tumour of the kidney, even if for a long time it has been viewed as a hamartoma rather than a neoplasm. It belongs to a family of neoplasms, named PEComa, characterised by the constant presence of perivascular epithelioid cells that co-express smooth muscle and melanogenesis markers. Angiomyolipoma can occur in patients with tuberous sclerosis, a hereditary syndrome due to the alteration of TSC1 or TSC2 genes, or sporadically. Angiomyolipoma and its variants are indolent tumours; however, some epithelioid angiomyolipomas/pure epithelioid PEComas ar

PEComa, or perivascular epithelioid cell tumor, is a distinctive mesenchymal neoplasm arising from perivascular epithelioid cells (PECs) that classically demonstrates a myomelanocytic immunophenotype. PEComas predominantly affect adults, with female predilection, and can occur at various anatomical sites, including the retroperitoneum, uterus, kidney, liver, and soft tissues. Most PEComas are considered benign and have an overall good prognosis. However, malignant PEComas are generally uncommon and tend to behave aggressively. In this clinical vignette, we present a 65-year-old woman with a ma

Pancreatic perivascular epithelioid cell tumours (PEComas) are exceptionally rare mesenchymal neoplasms. Endoscopic ultrasound-guided fine-needle biopsy shows suggestive morphologic features, while correlation with immunochemistry results supports the diagnosis. Accurate identification is crucial, as it directly influences clinical decision-making and may enable organ-preserving surgical management.

This review examines the histopathologic, immunohistochemical, ultrastructural, and molecular biologic features of epithelioid angiomyolipoma (EAML), with an emphasis on the differential diagnosis of intra-abdominal EAML. Epithelioid angiomyolipoma is an uncommon mesenchymal tumor with malignant potential, frequently associated with tuberous sclerosis complex. Histologically, EAML is characterized by sheets or nests of large polygonal epithelioid cells with abundant eosinophilic or occasionally clear cytoplasm, often with prominent nucleoli, and EAML may include multinucleated and markedly ple

Uterine perivascular epithelioid cell tumors (PEComas) are exceptional mesenchymal neoplasms of the gynecologic tract that often mimic common uterine lesions and lack pathognomonic clinical or radiologic features. We report a malignant uterine PEComa in a 44-year-old woman presenting with abnormal uterine bleeding and pelvic pain. Preoperative imaging suggested a fibroid, but definitive diagnosis followed hysterectomy: in fact, histopathology showed epithelioid cells arranged around a rich vasculature with high-grade cytologic atypia, brisk mitotic activity, and necrosis, all features consiste

In this manuscript, we summarize the main features of angiomyolipoma highlighting the recognition of epithelioid angiomyolipoma and the discovery of immunohistochemical expression of HMB45 in a group of tumors that now are referred to as as PEComas. In this scenario, Dr. Rosai believed in our intuition, demonstrating his intellectual honesty and motivated us with his experience ("when a tumor seems malignant it is malignant") and enthusiasm for the new entities ("in Verona, you use HMB45 instead of H&E"). He really pushed the improvement of the knowledge in this field.

Clear cell "sugar" tumor is a rare benign tumor considered as a member of the family of perivascular epithelioid cell tumors. To the best of our knowledge, this is the first case of clear cell "sugar" tumor presented as a mass protruding from the urethra in a 15-year-old girl. Pathologic examination revealed a tumor composed of epithelioid cells with "water" clear cytoplasm that stained positively for melanocytic and smooth muscle-specific markers. Treatment of this tumor included a surgical excision and complete removal of the urethral mass, with examination of surgical margins. Three months

Perivascular epithelioid cell tumor of the uterus is a rare mesenchymal neoplasm with variable malignant potential and nonspecific clinical and radiologic features, often mistaken preoperatively for leiomyoma. This report describes the case of a postmenopausal woman with seven months of persistent postmenopausal bleeding, pelvic pain, and symptomatic anemia. Pelvic ultrasound and magnetic resonance imaging demonstrated a markedly enlarged, irregular uterus with multiple masses interpreted as fibroids. The patient underwent total abdominal hysterectomy with bilateral salpingo-oophorectomy for r

Mesenchymal tumours with melanocytic expression can pose a diagnostic challenge because they frequently demonstrate both morphological and immunohistochemical overlap with other cutaneous melanocytic neoplasms. Therefore, they present potential pathological pitfalls that may lead to a misdiagnosis of malignant melanoma. Mesenchymal neoplasms that closely mimic melanoma include malignant melanotic nerve sheath tumour (melanotic schwannoma), epithelioid schwannoma, malignant peripheral nerve sheath, cutaneous syncytial myoepithelioma, clear cell sarcoma of soft tissue, and perivascular epithelio

BACKGROUND: Perivascular Epithelioid cell tumors (PEComa) are rare mesenchymal tumors. They generally occur in the gynecologic or digestive tract. The diagnosis of Central Nervous System PEComa is exceptional and challenging.
CASE DESCRIPTION: We report the case of a 46-year-old woman, with no particular medical history, who presented a secondary amenorrhea and a slight hyperprolactinemia. She was diagnosed on MRI with a pituitary tumor showing spontaneous hypersignal in T1-weighted images. After failure of medical treatment with cabergoline, surgical resection was required due to progressive

Nodular granular muscle degeneration (NGMD) of the appendix is a rare histologic curiosity characterized by distinctive nests of polygonal epithelioid cells with abundant pale-pink eosinophilic granular cytoplasm, mostly distributed in the inner layer of the muscularis propria or submucosa of the appendix. Although the nature of the cells of interest in NGMD of the appendix has not been completely elucidated, it is believed that they denote degenerative smooth muscle cells of the appendiceal muscularis propria, a histologic finding described as granular cells/granular cell lesions of the appen

PEComa has become a widely accepted entity, and increased recognition has led to descriptions of this tumor in a wide variety of anatomic sites, including the adrenal gland. PEComa (perivascular epithelioid cell tumor) is a mesenchymal tumor composed of perivascular cells, and the most frequent sites of PEComas are the uterus and retroperitoneum. The incidence is <1 per 1,000,000 people. We report a case of adrenal metastatic PEComa in a 63-year-old man discovered by a spontaneous hematoma of the rectus abdominis. In our case, PEComa of the adrenal gland was a significant diagnostic dilemma as

BACKGROUND: Perivascular epithelioid-cell tumour (PEComa) is a rare mesenchymal tumour with low malignant potential. PEComa can be found in many organs throughout the body. In the urinary system, it can be found in the prostate, bladder, and kidney. Given the rarity of bladder PEComas, a multidisciplinary approach is often required for diagnosis, encompassing radiological imaging, histopathological examination, and immunohistochemical staining.
METHODS: A 3 cm-diameter mass outside the bladder but related to the anterolateral wall of the bladder was excised by laparoscopic method. A minimally

PEComa is a mesenchymal neoplasm thought to derive from the perivascular epithelioid cell, a hypothetical cell type not yet identified. PEComa is a broad term that was recently expanded to include visceral, retroperitoneal, and somatic soft tissue, in addition to cutaneous tumors, which share morphological and immunohistochemical features in common with angiomyolipoma, lymphangiomyomatosis, and clear cell "sugar" tumor. The latter have distinct, site-related, clinical, morphological, and biological features. PEComas share features of concurrent melanocytic and myocytic differentiation. Most ca
