BACKGROUND/OBJECTIVE: Persistent postural-perceptual dizziness (PPPD) was defined by expert consensus for the forthcoming International Classification of Diseases, 11th edition. Its diagnostic criteria were derived from phobic postural vertigo and chronic subjective dizziness. Two key symptoms are postural unsteadiness and visually induced dizziness. We observed that patients with PPPD tended to perform poorly on multiple conditions of the EquiTest® Sensory Organization Test (SOT) and sought to investigate this phenomenon systematically.
METHODS: We examined cross-sectional, pair-wise group d

OBJECTIVE: To assess whether a posturographic assessment of sway misperception (PASM) discriminates persistent postural-perceptual dizziness (PPPD) from peripheral balance disorders.
METHODS: We retrospectively reviewed adults (≥ 18 years) evaluated at Saitama Medical University Hospital between September 2024 and April 2025 with PPPD (Bárány Society diagnostic criteria) or peripheral balance disorders who underwent PASM; central/psychogenic disorders were excluded. On a stabilometer, postural sway area (PSA) and velocity of path length (VPL) were recorded during eyes-closed standing. Then, w

BACKGROUND: Persistent postural-perceptual dizziness (PPPD) is a functional vestibular disorder characterized by persistent dizziness, unsteadiness, and non-spinning vertigo. It is the most common cause of chronic vestibular syndrome, but its pathogenesis is currently unclear. Recent studies have indicated that sensory integration may be altered in PPPD patients.
OBJECTIVE: Using independent component analysis (ICA) combined with seed-based functional connectivity analysis, we aimed to analyze changes in brain network functional connectivity (FC) in PPPD patients during the resting state and

Persistent postural-perceptual dizziness (PPPD) is a recently defined syndrome with chronic dizziness interrupting daily life. Although the high levels of anxiety and functional changes in postural control strategy and multi-sensory information processing and integration may be underlying the pathophysiology, its neural mechanisms are poorly understood. The aim of this study was to examine the regional cerebral blood flow (rCBF) in patients with PPPD using single photon emission computed tomography (SPECT). A total of 25 patients with PPPD and 25 healthy controls participated in the study. All

One of the most common complaints in ENT clinics is dizziness. Although most cases are caused by vestibular neuritis and benign paroxysmal positional vertigo (BPPV), a significant number of patients experience non-vestibular dizziness. One non-vestibular cause is postural orthostatic tachycardia syndrome (POTS), which presents with symptoms similar to vestibular migraine (VM) and persistent postural perceptual dizziness (PPPD). Dizziness from POTS is related to dysautonomia rather than the vestibular system. In otolaryngology, understanding the autonomic nature of non-vestibular dizziness is i

INTRODUCTION: Persistent postural-perceptual dizziness (PPPD) and vestibular migraine (VM) share symptoms of visual vertigo and motion sickness that can be confusing for clinicians to distinguish. We compare the severity of these symptoms and dynamic subjective visual vertical (dSVV) in these two common vestibular conditions.
METHOD: Twenty-nine patients with PPPD, 37 with VM, and 29 controls were surveyed for subjective symptoms using the visual vertigo analogue scale (VVAS) and motion sickness susceptibility questionnaire during childhood (MSA) and the past 10 years (MSB). dSVV is a measure

This study used resting state functional magnetic resonance imaging (rsfMRI) to investigate whole brain networks in patients with persistent postural perceptual dizziness (PPPD). We compared rsfMRI data from 38 patients with PPPD and 38 healthy controls using whole brain and region of interest analyses. We examined correlations among connectivity and clinical variables and tested the ability of a machine learning algorithm to classify subjects using rsfMRI results. Patients with PPPD showed: (a) increased connectivity of subcallosal cortex with left superior lateral occipital cortex and left m

Background/Objectives: Persistent postural-perceptual dizziness (PPPD) and anxiety disorder (AD) share overlapping clinical features that make differentiation with conventional assessments challenging. This study examined whether virtual reality (VR)-based vestibular assessment and immersive phobic scenarios can reveal distinct response profiles between these conditions. Methods: In this preliminary, single-center, cross-sectional study, 32 participants (PPPD: n = 15, AD: n = 17) underwent joint otorhinolaryngological/audiological and psychiatric evaluation, including the Clinical Test of Sens

INTRODUCTION: Persistent Postural-Perceptual Dizziness (PPPD), as an umbrella term for functional dizziness, encompasses a wide range of subjective symptoms affecting visual, vestibular, and motor functions. We developed the Athens-Lübeck Questionnaire (ALQ) as a bedside tool to differentiate specific symptom subtypes, which could inform more targeted research into the pathogenesis of the syndrome and facilitate tailored physiotherapeutic interventions.
METHODS: A total of 96 patients with primary or secondary PPPD were included in a prospective cross-sectional study conducted at two tertiary

The clinical characteristics of persistent postural-perceptual dizziness (PPPD) vary according to patient age and inducing factors. We aimed to analyze the differences in the clinical characteristics of PPPD with different patient age groups and different etiologies. A total of 122 PPPD patients hospitalized in the vertigo ward of Nanjing Brain Hospital from December 2018 to July 2021 were enrolled. According to whether dizziness symptoms were secondary to organic diseases, PPPD patients were divided into the primary (p-) and secondary (s-) PPPD groups; subgroups were created according to age

BACKGROUND AND PURPOSE: Patients with persistent postural-perceptual dizziness (PPPD) have comorbid low activity levels and the highest prevalence of poor sleep quality and insomnia across dizziness syndromes. Vestibular rehabilitation encourages physical activity and improves sleep in dizzy patients, so we sought to evaluate the effects of an online movement therapy course on physical activity and sleep in patients with PPPD. This study aims to compare sleep and physical activity in PPPD patients before and after online movement physiotherapy (OMP) sessions and investigate the correlation bet

BACKGROUND: Persistent Postural-Perceptual Dizziness (PPPD) is a prevalent functional neuro-otologic disorder characterised by non-spinning vertigo, perceived instability and visual motion sensitivity. Diagnostic criteria currently prioritise physical symptoms; although psychological factors are clinically linked to symptom onset and maintenance. This study investigates psychological factors in vestibular disorders by comparing PPPD patients with healthy controls, and exploratorily, with Bilateral Vestibulopathy (BVP) patients, identifying differentiating constructs and potential treatment tar

Introduction Persistent postural-perceptual dizziness (PPPD) is a functional vestibular disorder characterized by chronic dizziness, unsteadiness, and hypersensitivity to motion. Preexisting anxiety disorders and neurotic personality traits confer vulnerability to PPPD. High anxiety during acute vertigo or dizziness incites it. A functional magnetic resonance imaging (fMRI) study of chronic subjective dizziness found unexpectedly hypoactive responses to vestibular stimulation in cortical regions that integrate threat assessment and spatial perception. Objective This fMRI study used non-movin

BACKGROUND: Persistent postural-perceptual dizziness (PPPD) is a common functional vestibular disorder. Vestibular balance rehabilitation therapy (VBRT) is recommended, but access to adjunctive therapies that support self-management remains limited. This study aimed to assess the feasibility and preliminary clinical effects of home-based virtual reality (HB-VR) as an adjunct to individualized VBRT in adults with PPPD.
METHODS: This prospective, single-centre controlled feasibility trial was conducted at a tertiary vestibular clinic between June 2022 and October 2024. Twenty adults with confir

Persistent Postural-Perceptual Dizziness (PPPD) is a common cause of chronic vestibular syndrome. Although previous studies have identified central abnormalities in PPPD, the specific neural circuits and the alterations in brain network topological properties, and their association with dizziness and postural instability in PPPD remain unclear. This study includes 30 PPPD patients and 30 healthy controls. Resting-state functional magnetic resonance imaging is used to construct whole-brain functional connectivity matrices, followed by network-based statistic and graph theoretical analysis. Netw

OBJECTIVE: Persistent postural-perceptual dizziness (PPPD) is a syndrome described as secondary, when it is the consequence of an organic disorder (s-PPPD), or primary, when no somatic triggers can be identified. We evaluated a group of patients diagnosed as s-PPPD, with Benign Positional Paroxysmal Vertigo (BPPV) as the main somatic trigger, with the aim of identifying the predictive clinical elements of evolution towards PPPD.
STUDY DESIGN: Retrospective case review.
SETTING: Tertiary referral center.
