The Copernican revolution of caring for older adults living alone with dementia: lessons from Japan.

The rise of older adults living alone with dementia demands a profound transformation in healthcare systems and societies worldwide. In Japan, where one in three households are occupied by only one person, this population faces elevated risks for adverse health outcomes, delayed diagnoses, and difficulties accessing critical services. In response, Japan launched the Study for Promoting Rights Protection in Older People Living Alone with Dementia, grounded in a human rights-based approach rather than traditional medical or social welfare models. This Forum article reflects on ten lessons from t

Constructing the death elephant: a synthetic paradigm shift for the definition, criteria, and tests for death.

In debates about criteria for human death, several camps have emerged, the main two focusing on either loss of the "organism as a whole" (the mainstream view) or loss of consciousness or "personhood." Controversies also rage over the proper definition of "irreversible" in criteria for death. The situation is reminiscent of the proverbial blind men palpating an elephant; each describes the creature according to the part he can touch. Similarly, each camp grasps some aspect of the complex reality of death. The personhood camp, in contrast to the mainstream "organism" camp, recognizes that a huma

Breadth psychology.

Freudian psychoanalysis holds a conception of personhood in which patients are divided by a conflict between their outward presentation and their hidden, inner turmoil. The Freudian analyst too is split, occupying a position within the patient's internal dialogue while also listening in from outside of it. Addressing the analyst as both self and other, the patient's confidentiality is necessarily preserved but also inherently violated. As digital technologies reshape psychic structure, this paradox in confidentiality, constitutive of the analytic situation, may no longer arise. Personhood in t

The management of daily topics in conversations with individuals with Alzheimer's disease in Chinese residential care.

BACKGROUND: Individuals with Alzheimer's Disease (AD) often experience difficulties managing topics in daily interactions, which can hinder their participation in interactions and affect their social engagement. There is a gap in exploring the interactional dynamics and the maintenance of personhood when interacting with individuals with AD in asymmetric institutional settings. OBJECTIVES: This study aims to examine the interactional competence of individuals with AD and how their vulnerabilities are managed in daily-topic interactions within institutional healthcare settings. METHODS: This

The ethical inadequacy of uninformed surrogate consent: advancing respect for persons in clinical research.

In clinical research, decision-making capacity is often equated with unspecified conceptions of autonomy, and autonomy is often equated with personhood. On this view, the loss of decision-making capacity is seen as a loss of autonomy, and the loss of autonomy subsumes a loss of personhood. An ethical concern arises at the intersection of those philosophical considerations with the legal considerations in informed consent. Because persons with inadequate decision-making capacity cannot provide legally effective consent, enrollment in research can occur only if a surrogate gives permission on th

The Directiveness of Advance Directives. Performing Euthanasia on Individuals with Severe Dementia.

This paper inquires whether performing euthanasia in accordance with an advance directive is acceptable in the case of individuals with severe dementia. Specifically, the Convention on the rights of persons with disabilities is examined to determine whether it provides the basis to perform euthanasia in these cases. The meaning of certain values in particular is analysed, namely, equality, (legal) personhood, autonomy, freedom, dignity and physical and mental integrity. The intricate aspect of the identity of individuals with severe dementia merits special attention.

The dignity of the corpse: an explanation.

In medico-legal literature, the notion of the "dignity of the corpse" is frequently used as a narrative device, almost romantic in nature, aimed at highlighting the values attributed to it and thereby justifying its defense. The most common explanations tend to focus on its strictly practical effects (in relation to the living), either by pointing to a certain inviolability or by emphasizing some consequences at a symbolic level. Intentionally or not, some of these theories undertake the bold task of explaining the consequences of a phenomenon (the death/dead binomial) while disregarding the s

In Vitro Fertilization is Essential Healthcare for Patients With Rheumatic Disease.

Infertility presents a profound physical, emotional, and financial burden, particularly for rheumatology patients who often face substantial barriers to family building. The 2024 Alabama Supreme Court ruling in LePage v. Center for Reproductive Medicine equating embryo destruction with wrongful death introduced new legal uncertainties, sparking concerns about access to in vitro fertilization (IVF) and reproductive health services across the United States. Although Alabama's subsequent legislative action granted temporary protections for fertility providers, the broader implications of embryo p

Longitudinal Health-Related Values Discussions for Patients Undergoing Hematopoietic Stem Cell Transplant: Do Values Change Over Time?

PURPOSE: Hematopoietic stem cell transplant (HSCT) for patients with hematologic malignancies (HM) is associated with significant morbidity. Patients who undergo HSCT benefit from receiving palliative care that addresses their health-related values (HRV). We previously reported results of a pilot study incorporating repeated HRV discussions for patients undergoing HSCT and now aim to explore how patients' values may change throughout the transplant course. METHODS: Consecutive patients with HM receiving HSCT were approached for enrollment. Patients' HRV (e.g. personhood, sources of strength,

In search of the 'self': Holistic rehabilitation in restoring cognition and recovering the 'self' following traumatic brain injury: A case report.

BACKGROUND: Following mild-moderate traumatic brain injury (TBI), an individual experiences a range of emotional changes. It is often difficult for the patient to reconcile with their post-injury persona, and the memory of pre-injury personhood is particularly painful. Insight into one's cognitive deficits subsequent to injury can lead to an existential crisis and a sense of loss, including loss of self. OBJECTIVE: Restoration of cognitive functions and reconciliation with loss of pre-traumatic personhood employing a holistic method of neuropsychological rehabilitation in a patient suffering

Brain Organoid Research in a Post-Dobbs World.

The creation and study of brain organoids may hold significant promise for understanding brain functions, disorders, and diseases. This research may also raise novel considerations and ethical concerns, but it has significant public and professional support when thoughtfully undertaken. Current legislative and judicial restrictions on abortion and pronouncements about fetal personhood could, however, have a surprisingly broad and unintended reach, even conceivably restricting the development and use of brain organoids and other biomedical and bioengineered research tools. Brain organoid resear

[Dignity, ageing and artificial intelligence].

Dignity is a concept frequently invoked in bioethics, both in the literature and in practice. However, those who use the term rarely describe or define it, despite the word's multiple meanings. This article explores its various meanings, highlighting the relational context in which it is situated, whether it is attributed or intrinsic. The discussion focuses on the impact of AI-powered assistive robots, designed to combat isolation and loneliness, as well as on relationality and dignity in the broadest sense.

Life stories as a tool for preserving autonomy and dignity in people with dementia.

The life story represents a useful tool that enables people living with dementia to better preserve their identity and sense of continuity, that is, the connection between who they were and who they still are. At the same time, they provide care providers with a deeper understanding of the needs and values of people with dementia and enable the provision of care that is truly individualized, sensitive, and respectful. In practice, information from life stories is used as a valuable source of knowledge for care planning. However, this approach raises several ethical questions. Who has the right

Stillbirth and the law: options for law reform and issues for the coronial jurisdiction.

In spite of its relative frequency, stillbirth is a phenomenon that has been relatively little discussed and requires further research for comprehensive clinical understanding. It impacts upon the legal system by way of the need to determine whether a life has come into being such that legal consequences such as criminal law, probate law and the coronial jurisdiction can attach to it. This requires contemporary clinically informed interpretation and application of the ancient "born alive rule" in respect of the ramifications of matters such as pulseless electrical activity in the heart, brain

Non-consequentialist and egalitarian objections to the dead donor rule.

Unlike some critics of the dead donor rule, I do not defend consequentialism or deny the personhood of any potential organ donor. Instead, I argue that the principles of ethics and action theory that support the dead donor rule also prohibit widely accepted procedures, including kidney and partial liver transplants from living donors, fetal surgeries that permanently damage the uterus, sequential or domino liver transplants and lethal palliation. If physicians sometimes may cause death as a result of relieving a patient's pain, then they sometimes may cause death as a result of fulfilling the

[The human dignity of elderly people admitted to geriatric wards].

A testament to longevity, old age is a time of great vulnerability. An ageing and ailing body often weakens the immune system and leaves a person susceptible to illness. Ageing therefore presents a challenge for the medical profession, which addresses it through a specialised branch of medicine: geriatrics. In this field of medicine for older people, which focuses on health and quality of life, the medical profession has both positive and negative legal obligations aimed at protecting dignity.

The impact of anencephaly on parents: A mixed-methods study.

This study used a convergent parallel mixed-method design to explore the impact of an anencephalic pregnancy on parents. Twenty women and four men between 18-59 years old participated. Interview transcripts were analyzed using interpretive phenomenology and synthesized with Perinatal Grief Intensity Scale scores using a Pearson's correlation. Overall, 75% of parents scored intense grief. Qualitative patterns included overwhelming trauma, patient-centeredness as critical, stigmatizing perinatal loss, embracing personhood, and reframing reality. Control over care was associated with decreased gr

Assessing Health-Related Values: Imperative for Person-Centered Care.

Person-centered care, a gold standard in serious illness care and palliative care, emphasizes the importance of understanding a patient's personhood and values. Nurses are uniquely suited to provide high-quality, person-centered care and to support patients as they communicate their worries, goals, hopes for the future, and sources of support. By assessing and understanding an individual's health-related values, nurses are better able to complete their nursing responsibilities, advocate for their patients, and confidently encourage shared decision-making. Nurses have strong communication skill

Systematic scoping review on moral distress among physicians.

BACKGROUND: Concepts of moral distress (MD) among physicians have evolved and extend beyond the notion of psychological distress caused by being in a situation in which one is constrained from acting on what one knows to be right. With many accounts involving complex personal, professional, legal, ethical and moral issues, we propose a review of current understanding of MD among physicians. METHODS: A systematic evidence-based approach guided systematic scoping review is proposed to map the current concepts of MD among physicians published in PubMed, Embase, PsycINFO, Web of Science, SCOPUS,
