Comorbidity of borderline personality disorder with other personality disorders in psychiatric outpatients: how does it look at 2-year follow-up?

BACKGROUND: The rates of remission of comorbid personality disorders for patients with a primary borderline personality disorder (BPD) have previously only been addressed in samples of inpatients or former inpatients with BPD receiving treatment-as-usual. AIMS: The aim of the current study was to assess the rates of remission of comorbid axis II personality disorders in a sample of 32 outpatients with a primary BPD receiving open-ended non-manualized psychotherapy. METHODS: Structured psychiatric interviews for DSM-IV were conducted by independent assessors at intake, discharge and 2-year fo

Preadolescent children of substance-dependent fathers with antisocial personality disorder: psychiatric disorders and problem behaviors.

We compared psychiatric disorders and problem behavior scores in pre-adolescent children of fathers with alcohol or other drug dependence and ASP (SD+/ASP+), children whose fathers had substance dependence without ASP (SD+/ASP-), and children whose fathers were without either disorder (SD-/ASP-). SD+/ASP+ children showed elevated rates of major depression, conduct disorder, attention deficit hyperactivity disorder, oppositional defiant disorder, and separation anxiety disorder when compared to SD+/ASP- and SD-/ASP- children. SD+/ASP+ children had higher internalizing and externalizing problem

Are DSM-IV-TR borderline personality disorder, ICD-10 emotionally unstable personality disorder, and CCMD-III impulsive personality disorder analogous diagnostic categories across psychiatric nomenclatures?

This study examined the validity of the borderline construct which encompasses diagnostic criteria from the DSM-IV-TR Borderline Personality Disorder (BPD), ICD-10 Emotionally Unstable Personality Disorder (EUPD), and CCMD-III Impulsive Personality Disorder (IPD) in a sample of 1,419 Chinese psychiatric patients. Participants completed the Chinese Personality Disorder Inventory and the Chinese Personality Assessment Inventory-2 assessing various disordered personality features. Adequate internal consistency was found for the borderline construct (α = .83). Exploratory factor analysis revealed

Gender differences in the clinical characteristics and psychiatric comorbidity in patients with antisocial personality disorder.

Gender is an important variable in the study of mental health because of the actual and perceived differences between men and women. Relatively little is known how males and females differ in their manifestations of antisocial personality disorder (ASPD). Demographic and clinical features of 323 participants with ASPD were assessed and recorded. Women had fewer episodes of antisocial behavior involving or not involving police, higher scores on the Childhood Trauma Questionnaire (CTQ) and on Emotional Abuse and Sexual Abuse subscales of the CTQ compared to men. CTQ scores positively correlated

Personality disorders and adult attachment dimensions in a mixed psychiatric sample: a multivariate study.

The associations between personality disorders and adult attachment dimensions were assessed in a sample of 487 consecutively admitted psychiatric subjects. Canonical correlation analysis showed that two sets of moderately correlated canonical variates explained the correlations between personality disorders and adult attachment patterns. The first and second attachment variates closely resembled the avoidance and anxiety attachment dimensions, respectively. The first personality disorder variate was mainly characterized by avoidant, depressive, paranoid, and schizotypal personality disorders,

Cocaine use and dependence in young adults: associated psychiatric disorders and personality traits.

Current and lifetime prevalence of substance use and psychiatric disorders was determined by administering the NIMH-DIS, revised to cover DSM-III-R diagnoses, to a sample of 1007 young adults. Personality and affectivity were measured also. Increased rate of any Substance Use Disorder was related to use of cocaine more than 5 times over the lifetime regardless of whether or not criteria for Cocaine Dependence were met. Increased rate of any Affective Disorder was related to dependence in those who used cocaine more than 5 times. Cocaine use was associated with increased neuroticism, psychotici

Psychiatric Emergency Visits of Autistic Adults With or Without Documented Borderline Personality Disorder.

Autism and borderline personality disorder (BPD) are each associated with increased psychiatric emergency department (PsyED) use, yet the service impact of coexisting autism-BPD remains unclear. We conducted a retrospective chart review of 1027 PsyED visits by 345 autistic adults (2018-2020) at an urban Canadian psychiatric hospital to compare service patterns between those with and without a BPD diagnosis. Sociodemographics, presenting reasons, restraint use, and disposition were analyzed using mixed-effects logistic regression. BPD was documented in 33.5% of visits and in 11.0% of unique aut

Distribution of borderline personality disorder related impulsivity types in psychiatric clinical populations.

BACKGROUND: Impulsivity is a core feature of Borderline Personality Disorder (BPD), yet its prevalence and variations across demographics and diagnoses in clinical populations are underexplored. This study aims to investigate the frequency and distribution of impulsive behaviors related to BPD among a large clinical sample, considering gender, age, and diagnostic categories. METHODS: A total of 2862 participants were consecutively sampled from psychiatric and psycho-counseling clinics. BPD traits and symptoms were assessed using the Personality Diagnostic Questionnaire 4th Edition Plus (PDQ-4

Addressing Spiritual and Religious Experiences in Borderline Personality Disorder With Good Psychiatric Management.

Spiritual and religious experiences in the context of borderline personality disorder are underexplored by both researchers and clinicians, are central in the lived experience of some patients, and are likely to interact in complex ways with core symptoms and challenges. Effective navigation of this domain by clinicians and patients may require increasing, decreasing, or stabilizing engagement with spiritual and religious beliefs, practices, or communities, depending on the person. No empirically derived guidelines exist for how clinicians can address this area to help patients maximize benefi

Interpersonal Trust in Adolescents With Borderline Personality Disorder: Comparisons With Healthy and Psychiatric Controls.

The aim of the current study was to utilize a well-known trust versus lottery paradigm to evaluate interpersonal trust in adolescents with borderline personality disorder (BPD). The study included 126 healthy controls, 59 inpatient adolescents with a diagnosis of BPD, and 137 inpatient adolescents without BPD. Alongside diagnostic measures, a questionnaire-based measure for assessing trust beliefs was administered to probe group differences in trust beliefs and associations between game behavior and trust beliefs. No main effect for group or condition was found. A significant interaction of tr

Impulsivity and psychiatric comorbidity as risk factors for suicide attempts in borderline personality disorder.

BACKGROUND: Addressing the risk of suicidality in borderline personality disorder (BPD) is a crucial issue. The notion that impulsive individuals are more likely to plan for suicide attempts is important for many reasons in both theoretical and clinical decision-making. AIM: The aim of this study was to investigate potential risks of suicidality in BPD and to correlate it to impulsivity. SETTING: The study was conducted at the Institute of Psychiatry, Ain Shams University and Al Mashfa Private Hospital. METHODS: Ninety-one participants were included in the study: 30 patients were diagnosed

Validation of MMPI-2-RF Personality Disorder Spectra scales in a psychiatric sample.

The diagnostic operationalization of personality disorders is at the crossroads. The Diagnostic and Statistical Manual of Mental Disorders (5th ed.; DSM-5), includes 2 separate systems for the diagnosis of personality disorders (PDs)-1 that is purely categorical (Section II) and 1 that is primarily dimensional (Section III). The International Classification of Diseases (11th ed.) will use a dimensional approach for the diagnosis of PDs. The Minnesota Multiphasic Personality Inventory 2-Restructured Form (MMPI-2-RF) PD Spectra scales were developed as measures of both the traditional and the al

Brain neuroreceptor density and personality traits: towards dimensional biomarkers for psychiatric disorders.

Positron emission tomography has, for 30 years, been used in numerous case-control studies searching for hypothesized differences in the density of neuroreceptor or transporter proteins in psychiatric disorders such as schizophrenia and depression. In most cases, the results have not been conclusive. One reason could be the sizeable interindividual variability in biochemical markers, which in twin studies have shown to emanate from both environmental and genetic factors, leading to low statistical power for the detection of group effects. On the other hand, the same interindividual variability

Principal diagnoses in psychiatric outpatients with borderline personality disorder: Implications for screening recommendations.

BACKGROUND: In the Rhode Island Methods to Improve Diagnostic Assessment and Services project, we examined how often borderline personality disorder (BPD) is the principal diagnosis in patients presenting for outpatient psychiatric treatment. To inform clinicians when it is most productive to screen for BPD, we examined the prevalence of BPD in psychiatric outpatients with different principal diagnoses. METHODS: In this study, 3,674 psychiatric outpatients were evaluated with a semi-structured diagnostic interview for DSM-IV BPD. RESULTS: Slightly ≥10% of the sample was diagnosed with BPD (n

Breakdown or Personality Disorder? Psychiatric Characterization and Developmental Pathways Towards Young Adulthood in Adolescents with Pathological Personality Structure.

Background/Objectives: Research is lacking about the development of personality disorders (PDs) from adolescence to early adulthood. This study aimed to characterize the profile of high-risk adolescents compared to adolescents with full-blown PDs and adolescents with other psychiatric disorders and to identify clinical markers that constitute a risk profile. Methods: We evaluated 99 adolescents (12-17 years old) through DSM-5-based semi-structured interviews, questionnaires, cognitive tasks, and scales regarding functioning and disorder severity. Head injury as risk factor for psychiatric disorders: a nationwide register-based follow-up study of 113,906 persons with head injury.

OBJECTIVE: Studies investigating the relationship between head injury and subsequent psychiatric disorders often suffer from methodological weaknesses and show conflicting results. The authors investigated the incidence of severe psychiatric disorders following hospital contact for head injury. METHOD: The authors used linkable Danish nationwide population-based registers to investigate the incidence of schizophrenia spectrum disorders, unipolar depression, bipolar disorder, and organic mental disorders in 113,906 persons who had suffered head injuries. Data were analyzed by survival analysis

The association of psychiatric comorbidity and use of the emergency department among persons with substance use disorders: an observational cohort study.

BACKGROUND: Psychiatric and substance use problems are commonly found to be contributing factors to frequent Emergency Department (ED) use, yet little research has focused on the association between substance use and psychiatric comorbidity. This study assesses the association of a psychiatric comorbidity on (ED) use among patients with substance use disorders (SUDs). METHODS: The study focuses on 6,865 patients who were diagnosed with SUDs in the ED of a large urban hospital in the southern United States from January 1994 - June 1998. Patients were grouped by type of substance use disorder.
