Aneurysmal cyst of the petrosal bone.

An aneurysmal cyst of the petrosal bone presenting as hearing loss and recurrent bacterial meningitis is reported. None of the clinical or radiographic signs described previously were present. Because other diagnostic methods are not reliable, it is recommended that coronal thin section computed tomography be performed in every case of suspected malformation of the skull base and in the diagnosis of recurrent bacterial meningitis.

Petroclival meningiomas and the petrosal approach.

As with other meningiomas, the goal of petroclival meningioma treatment should be curative total removal by achieving a Simpson I resection of the tumor, dura, and bone. Petroclival meningiomas particularly pose a great technical challenge because of their deep location and intimate relation with vital neurovascular structures. They require adequate exposure and trajectory to achieve total removal while minimizing the manipulation and risk to critical neurovascular structures, such as the vertebrobasilar system, the brainstem, and the cranial nerves. Here, we describe the indications and techn

Supralabyrinthine approach to petrosal cholesteatoma.

Petrosal cholesteatomas are rare lesions, which may be congenital or acquired in nature. We report an exceptional case occurring in a seven-year old girl who presented with a unilateral conductive hearing loss, despite normal tympanic membrane appearance. Early diagnosis was facilitated by computed tomography (CT) scanning. Although this case satisfied the criteria for congenital cholesteatoma, it is likely that the petrous apex was secondarily involved. Complete cholesteatoma removal was accomplished using a transtemporal supralabyrinthine approach, which allowed for hearing preservation, whi

The petrosal approach with hearing preservation.

Twenty-six patients with petroclival lesions were operated on via a petrosal approach designed to preserve hearing. The surgical pathology included 14 meningiomas, three chordomas, three epidermoid cysts, four vertebrobasilar aneurysms, and two pontine cavernous malformations. The approach allowed complete resection of 14 of 20 tumors and definitive treatment of all six vascular lesions. Complications included cerebrospinal fluid leakage in three patients, high-frequency sensorineural hearing loss in three, meningitis in one, and cranial nerve palsies (which were usually transient). This appro

Petrositis caused by fluconazole-resistant candida: case report and literature review.

BACKGROUND: Petrositis is a rare and fatal complication associated with otitis media. It is most likely caused by bacterial infections, but in some cases it is caused by fungal infections. CASE STUDY: The case in this report is associated with fungal petrositis. The clinical symptoms are: ear pain from chronic otitis media, severe headache, peripheral facial palsy and diplopia. The case was finally confirmed through imaging of middle ear, bacterial culture, pathology, and blood Metagenomic next-generation sequencing (mNGS) test. The patient was treated with sensitive antifungal drugs. CONCLU

Petrositis With Bilateral Abducens Nerve Palsies complicated by Acute Otitis Media.

Petrous apicitis is a rare but fatal complication of otitis media. An infection within the middle ear can extend within the temporal bone into the air cells of the petrous apex. With only the thin dura mater separating the trigeminal ganglion and the 6th cranial nerve from the bony petrous apex, they are vulnerable to inflammatory processes, resulting in deep facial pain, lateral rectus muscle paralysis, and diplopia. In 1904, Gradenigo described a triad of symptoms related to petrous apicitis, including acute suppurative otitis media, deep facial pain resulting from trigeminal involvement, an

The hypoplastic inferior petrosal sinus: a potential source of false-negative results in petrosal sampling for Cushing's disease.

Our purpose was to describe the hypoplastic or plexiform inferior petrosal sinus as a potential cause of false-negative sampling results in patients with Cushing's disease. Five hundred and one patients with surgically proven Cushing's disease and negative or equivocal magnetic resonance imaging scans of the pituitary gland underwent petrosal sinus sampling. Four patients (0.8%) with surgically proven Cushing's disease had false-negative results of petrosal sinus sampling. Retrograde inferior petrosal sinograms in these patients were reviewed to evaluate the anatomy of the inferior petrosal si

Recurrent petrositis due to nasopharyngeal carcinoma in an adult patient: Gradenigo's syndrome.

The early diagnosis of very rare nasopharyngeal carcinoma (NPC) is so difficult, to surrounded by structures such as the orbital skull base. With the invasion of the disease, the symptoms are manifested in different ways so that it may be confusion and delay in diagnosis and treatment in this situation. Including otological symptoms (serous otitis media), Gradenigo's syndrome is a rare clinical condition. The majority of published cases involve children, and the most common etiology is otitis media. We report a case of a diabetic man who presented with repeating Gradenigo's syndrome symptoms d

[Petrositis due to acute otitis media: a case report and literature review].

OBJECTIVE: To study the clinical features and conservative treatment of petrositis. METHOD: One case of petrositis was reported and literatures were reviewed. Opacificated air cells in CT, and anomalous, discontinuous hypersignal in MRI T1 and T2 provided evidence of infection of the apex of the petrous temporal bone. RESULT: The patient recovered totally after conservative treatment with intravenous antibiotic, glucocorticoid and neurotrophic therapy, and no recurrence occurred in 1 year follow-up. CT rescan showed the clear air cells of the petrous apex 4 months later. CONCLUSION: Petrosi

Superior petrosal vein in trigeminal neuralgia.

During microvascular decompression procedure for trigeminal neuralgia, surgical attention is mainly focused at the neurovascular conflict at trigeminal root entry zone. However, many major complications of this surgery are often related to venous structures in the posterior fossa, including the venous sinuses and the superior petrosal vein(s). The author reviews surgical anatomy of the superior petrosal vein, highlights important surgical steps related to its microsurgical dissection and discusses avoidance of complications, as well as their management during microvascular decompressive surger

Combined petrosal approach to petroclival meningiomas.

OBJECTIVE: To study the use and advantages of combining the posterior petrosal approach with the anterior petrosal approach to petroclival meningiomas. METHODS: Seven cases of petroclival meningiomas operated on via the combined petrosal approach were retrospectively analyzed. The basis on which this approach was selected was assessed, as were its benefits and risks. RESULTS: Gross total resection was achieved in five of the seven patients. No mortality or decrease in Karnofsky performance score was observed at the time of the last follow-up examination. Six of the seven patients had service

Responses to hypoxia of petrosal ganglia in vitro.

NaCN is a classical stimulus used to elicit discharges from carotid body chemoreceptors. The effect is assumed to be mediated by glomus (type I) cells, which release an excitatory transmitter for the excitation of carotid nerve endings. Since the sensory perikarya of the glossopharyngeal nerve (from which the carotid nerve branches) are located in the petrosal ganglion, we tested whether application of this drug to the petrosal ganglion superfused in vitro elicits antidromic discharges in the carotid nerve. NaCN did indeed cause an intense and prolonged burst of nerve impulses in the carotid n

Inferior petrosal sinus sampling in acromegaly.

In view of the diagnostic value of bilateral simultaneous inferior petrosal sinus sampling (BSIPSS) in localising preoperatively the site of the microadenoma in pituitary dependent Cushing's disease, the clinical value of BSIPSS was evaluated in five acromegalic patients with equivocal or negative pituitary CT scans. Intersinus GH gradients were obtained for all patients (range 1.6-4.2) but in only one case was the gradient correctly localised to the side of the tumour. Gradients of several other pituitary hormones, particularly prolactin (range 1.6-4.0), also demonstrated gradients parallel t

Bilateral inferior petrosal sinus sampling.

Bilateral inferior petrosal sinus sampling, the gold standard assay in diagnosing pituitary secretion of adrenocorticotropic hormone in Cushing disease, is highly accurate and safe when performed by experienced interventionalists. We review the anatomic and technical considerations essential for safe and reliable practice.

[Selective catheterization of the inferior petrosal sinus].

The inferior petrosal sinus is a dura sinus which traverses laterally and dorsally from the dorsal aspect of the cavernous sinus. It represents the nearest catheterization point for blood sampling from the pituitary gland. Inferior petrosal sinus venous sampling is used for diagnosis and lateralization of ACTH-secreting and growth hormone-producing hypophyseal adenomas. It is also used in the differential diagnosis of ectopic production of ACTH or other similar peptides. We briefly describe the anatomy of the inferior petrosal sinus and the catheterization technique, based on our experience fr

The petrosal approach: indications, technique, and results.

Surgical access to the clivus and petrous apex remains a formidable challenge. Intradural tumours at the clivus and petroclival area are superbly exposed via the petrosal approach described here. To date, we have operated on 33 patients having benign tumours using this approach. Total removal was achieved in all patients except 3 with meningiomas. There was no mortality, morbidity included 1 patient with hemiparesis and several with cranial nerve deficits.
