This article discusses Michel Foucault's main writings on "madness and psychiatry" from his early works up to the 1970s. On the one hand, we reconstruct the overall theoretical and methodological development of his positions over the course of the different periods in his oeuvre. On the other hand, we also take a closer look at Foucault's philosophical considerations regarding the subjects of his investigations. After an initial introduction of our conceptual approach, we draw on the most recent research on Foucault to show to what extent the phenomenological description of the topic at hand a

Since John Locke, regnant conceptions of personhood in Western philosophy have focused on individual capabilities for complex forms of consciousness that involve cognition such as the capability to remember past events and one's own past actions, to think about and identify oneself as oneself, and/or to reason. Conceptions of personhood such as Locke's qualify as cognition-oriented, and they often fail to acknowledge the role of embodiment for personhood. This article offers an alternative conception of personhood from within the tradition of phenomenology of the body. The article presents a p

This paper seeks to illuminate the nature of empathy by reflecting upon the phenomenology of depression. I propose that depression involves alteration of an aspect of experience that is seldom reflected upon or discussed, thus making it hard to understand. This alteration involves impairment or loss of a capacity for interpersonal relatedness that mutual empathy depends upon. The sufferer thus feels cut off from other people, and may remark on their indifference, hostility or inability to understand. Drawing upon the example of depression, I argue that empathy is not principally a matter of 's

Since ancient times, the study of cognitive deterioration has been a medical and philosophical constant. Unitl the 18th century, this study took place alongside that of other mental illnesses, for which reason it could not be separated, except in certain special cases, from other processes that were genuinely psychiatric. This century has witnessed a Mental Revolution as a consequence of the divorce between neuropsychiatry on the one hand, and philosophy and religious ideas on the other, of the spread of the ideas of Cullen, Chiarugi and Pinel, the emergence of the Enlightenment and Vitalism.

The author presents examples of a critical attitude towards medicine and physicians which may be observed, with changing tendencies, throughout the Middle Ages. First, medicine was despised as a mechanical art or suspected of paganism because of its literary sources. Later, the divine nature of disease and healing on the one hand and the uselessness of medicine on the other were stressed. Finally, the integration, at the universities, of medicine in the system of scholastic learning was criticized by more practically minded men. Similar criticism may be observed in the Islamic world. In this paper, the problem of correct ascriptions of consciousness to patients in neurological intensive care medicine is explored as a special case of the general philosophical 'other minds problem'. It is argued that although clinical ascriptions of consciousness and coma are mostly based on behavioral evidence, a behaviorist epistemology of other minds is not likely to succeed. To illustrate this, the so-called 'total locked-in syndrome', in which preserved consciousness is combined with a total loss of motor abilities due to a lower ventral brain stem lesion, is presented as a touchstone f

This paper attempts to address the general question whether information technologies, as applied in the area of medicine and health care, have or are likely to change fundamental concepts regarding disease and health. After a short excursion into the domain of medical informatics I provide a brief overview of some of the current theories of what a disease is from a more philosophical perspective, i.e. the "value free" and "value laden" view of disease. Next, I consider at some length, whether health care informatics is currently modifying fundamental concepts of disease. To this question I wil

Philosophy in medicine is often described through application: philosophical concepts are brought to bear on illness, clinical relationships, ethics, or education. Yet this language can obscure forms of inquiry in which clinical experience also reshapes philosophical questions. This paper develops a conceptual account of reciprocal clinical philosophizing: a bidirectional, open-ended, and dialogical practice in which philosophical inquiry and clinical experience can each generate, constrain, and revise questions posed by the other. The argument does not claim that philosophy of medicine has ge

In September 2020, the surgeon Paulo Macchiarini, who used stem cell technology to enable the transplants of artificial and donor trachea, was charged with aggravated assault in Sweden. In this comment, we argue that the Ethics Council of the Karolinska Institute should have considered issues from philosophy of science when they were brought to their attention, rather than dismiss them as irrelevant to research ethics. We demonstrate how conceptual issues of a philosophy-of-science-kind about clinical research and medical practice should be integral to research ethics.

The current state of medical practice is going through tremendous and rapid changes. There is an increasing prevalence of burnout among physicians, where they are questioning the value of practicing medicine. There is also a growing frustration among patients over reduced access to physicians, feeling rushed at appointments and generally feeling that they are not being heard. These conditions point to a sense among doctors and patients that the doctor-patient relationship is compromised without a viable pathway to repair this vital connection. In this paper, I want to show how applying philoso

In recent decades, a disciplinary and subdisciplinary proliferation has triggered both in the medical fields and science in general. This trend may be partially explained by two diachronic, dialectically interconnected facts: the deepening of technical, social and international division of labor in the globalized capitalist world, and the triumph of the reductionist program, mainly developed by the logical empiricism of the Vienna Circle. This paper aims to deepen the debate on the intricate links between medicine, biology, philosophy, reductionism and complex thought, by using two examples: a

John Freind (1675/76-1728) achieved distinction in several walks of life, first as a classical scholar, then as a physician and as a chemist who advocated Newtonian philosophy. His clinical practice was generally conservative and he was against the newly introduced practice of inoculating the smallpox. His principles were Tory and High Church; his loyalty to the house of Stuart involved him in the Jacobite plot of 1722, and a spell in the Tower of London. His money was part of the foundation of Dr Lee's benefaction to Christ Church, which still survives in name in scientific posts in Oxford. H

Conception of biological nature of a man included in "Timajos" did not limit itself to presentation of the outline of anatomy and of physiology, but also touched on problems connected with origin of diseases and their treatment. Explanation of mentioned issues was based on the idea of a man as an element of the universe. Plato subscribed opinion that body as all the universe, consisted of four basic primeval elements. Excess or shortage of one of them, exchange of place with other elements or taking over another species property caused disturbances inside the organism leading to diseases. Plat

When an individual is comatose while parts of her brain remain functional, the question arises as to whether any mental characteristics are still associated with this brain, that is, whether the person still exists. Settling this uncertainty requires that one becomes clear about two issues: the type of functional loss that is associated with the respective profile of brain damage and the persistence conditions of persons. Medical case studies can answer the former question, but they are not concerned with the latter. Conversely, in the philosophical literature, various accounts of personal ide

Miranda Fricker's influential concept of epistemic injustice (Oxford University Press, Oxford, 2007) has recently seen application to many areas of interest, with an increasing body of healthcare research using the concept of epistemic injustice in order to develop both general frameworks and accounts of specific medical conditions and patient groups. This paper illuminates tensions that arise between taking steps to protect against committing epistemic injustice in healthcare, and taking steps to understand the complexity of one's predicament and treat it accordingly. Work on epistemic injust

Jeremy de Drijvere or Thriverius (1504-1564) has been an important medical personality in our Habsburgian Low Countries in the 16th century. In 1537 he got the medical doctor's degree at the University of Leuven, where he was appointed few years later as professor of medicine. He was mostly interested in pathology and has translated and commented several works of Hippocrates and Galen. Moreover, Thriverius has published many original medical treatises in which he gives account of his personal experience and of certain diagnostic innovations, amongst others concerning the symptoms of scabies.
