Breast cancer screening in older women: practices and barriers reported by primary care physicians.

Annual mammography, in combination with clinical breast examinations, can reduce mortality from breast cancer. However, surveys of both patients and physicians suggest that mammography is underutilized. This study examined whether physicians' reported breast cancer screening practices and barriers to mammography varied with patients' age. Data from 576 primary care physicians (internal medicine, family/general practice, and obstetrics/gynecology) who participated in a mailed statewide survey were analyzed. Physicians reported screening elderly women significantly less often than younger women,

Echocardiographic screening of pregnant women by non-physicians with remote interpretation in primary care.

BACKGROUND: Impact of heart disease (HD) on pregnancy is significant. OBJECTIVE: We aimed to evaluate the feasibility of integrating screening echocardiography (echo) into the Brazilian prenatal primary care to assess HD prevalence. METHODS: Over 13 months, 20 healthcare workers acquired simplified echo protocols, utilizing hand-held machines (GE-VSCAN), in 22 primary care centres. Consecutive pregnant women unaware of HD underwent focused echo, remotely interpreted in USA and Brazil. Major HD was defined as structural valve abnormalities, more than mild valve dysfunction, ventricular systol

"Just relax": physicians' experiences with women who are difficult or impossible to examine gynecologically.

INTRODUCTION: The internal pelvic exam is a critical component of women's reproductive health care; however, it can be perceived as uncomfortable, embarrassing, and painful, which may lead some women to avoid this procedure. AIMS: The purpose of this study was to survey physicians with respect to their experiences with female patients who are difficult or impossible to examine gynecologically. METHODS: Six hundred and fifty-eight obstetrician-gynecologist and family physicians were sent a 15-item questionnaire by mail and 424 participants responded (64% response rate). The survey consisted o

The power with us: experiences of resisting oppression among Black women physicians in academic medicine.

Black women physicians often face bias, discrimination, and mistreatment within the academic environment because of their racial and gender identities. This shows up as microaggressions, exclusion, and minimal opportunities to advance their careers despite their expertise, contributions, and leadership to support institutions with creating environments that prioritize broad representation, welcoming and respectful spaces, and addressing barriers that prohibit individuals from thriving. Their first-hand experiences with intersecting oppressions told in their own voices as clinicians, educators,

Emergency physicians' patterns of treatment for presumed gonorrhea and chlamydia in women: one center's practice.

No indicator reliably predicts if a woman has gonorrhea or chlamydia (sexually transmitted diseases [STDs]) during an Emergency Department (ED) visit. Before culture results return, emergency physicians (EPs) must choose whom to treat. We evaluated EP treatment of STDs within our institution. EPs voluntarily completed anonymous surveys while evaluating women requiring both a pelvic examination and STD cultures, except for sexual assault victims. The questionnaires asked for patients' demographics, history, physical examination, and in-ED laboratory tests, and whether any particular section of

National patterns in the treatment of urinary tract infections in women by ambulatory care physicians.

BACKGROUND: Trimethoprim-sulfamethoxazole has consistently been the recommended drug for uncomplicated urinary tract infections in women. Which antibiotics physicians use has implications for patient outcomes, antimicrobial resistance, and costs. METHODS: This study was based on a sample survey of practicing physicians participating in the National Ambulatory Medical Care Survey from 1989 through 1998. Eligible visits were limited to those by women aged 18 to 75 years diagnosed with uncomplicated acute cystitis or urinary tract infection (N = 1478). We evaluated trends in the proportions of v

What leads to good health in midlife women physicians? Some clues from a longitudinal study.

Data from a longitudinal study of women physicians is used to study psychosocial and current implications of good health in midlife. The women comprise the classes 1964 to 1967 of the University of California School of Medicine at San Francisco. The subjects were interviewed and tested using the California Psychological Inventory while they were still in medical school (Time 1, average age 24 years) and at midlife (Time 2, average age 46 years). The women in good health at midlife, in contrast to their peers, were characterized by certain personality traits, including a higher degree of intell

Physician Level Assessment of Hirsute Women and of Their Eligibility for Laser Treatment With Deep Learning.

OBJECTIVES: Hirsutism is a widespread condition affecting 5%-15% of females. Laser treatment of hirsutism has the best long-term effect. Patients with nonpigmented or nonterminal hairs are not eligible for laser treatment, and the current patient journey needed to establish eligibility for laser hair removal is problematic in many health-care systems. METHODS: In this study, we compared the ability to assess eligibility for laser hair removal of health-care professionals and convolutional neural network (CNN)-based models. RESULTS: The CNN ensemble model, synthesized from the outputs of five

The effect of physician gender on women's perceived pain and embarrassment during pelvic examination.

In this article we try to determine if the examiner's gender affects women's perceived pain and embarrassment during emergency department pelvic examination, using a prospective comparative study in a university teaching hospital. Test subjects were taken from a convenience sample of female emergency department (ED) patients undergoing pelvic examination as part of their evaluation. 100 mm visual analog scales were used to gauge each subject's perceived pain and embarrassment. Subject age and complaint, and the examiner's gender and level of training were collected. Two-tailed Mann-Whitney or

Ongoing physician guidance to achieve periodic mammography of women at an urban health center.

The use of a low-cost behavior prompt at an urban, hospital-based health center that encourages physicians to record and act upon cancer screening information is described. Brief audit forms were incorporated into medical records of all women 40+ years of age seen between June 1994 and March 1996 (approximately 4,700 office visits). Audit forms alerted physicians about women's breast and cervical cancer screening histories and encouraged them to note any screening done or recommended during an office visit. Patient attributes did not readily distinguish who was and was not screened with mammog

The Only Woman in the Room: Oral Histories of Senior Women Physicians in a Midwestern City.

Introduction: The female students of the 1960s and 1970s have been at the forefront of issues for women in medicine throughout their careers. They have personally experienced the diverse challenges and opportunities that have continued to arise, for women in medicine over the past 50 years. Capturing their stories can provide a unique contribution to the history of women in medicine, especially in documenting the crucial transitional decades during which women entered the profession in increasing numbers. Their experiences can also inform programs to improve the careers of current and future w
