It is textbook knowledge that human infective forms of Trypanosoma brucei, the causative agent of sleeping sickness, enter the brain across the blood-brain barrier after an initial phase of weeks (rhodesiense) or months (gambiense) in blood. Based on our results using an animal model, both statements seem questionable. As we and others have shown, the first infection relevant crossing of the blood brain border occurs via the choroid plexus, i.e. via the blood-CSF barrier. In addition, counting trypanosomes in blood-free CSF obtained by an atlanto-occipital access revealed a cyclical infection

Twenty-eight patients who suffered traumatic obsolete incomplete paralysis were treated with intradural lysis and peripheral nerve implantation by a microsurgery technique. The endorachis was opened and the fibrous bands adhering to the spinal cord from the arachnoid, pia mater spinalis, ligamenta denticulatum, and the initial part of the nerve root were completely relieved. The abnormal spinal cord was then opened by three to six incisions, which were each 0.1 mm to 0.2 mm deep and longer than the abnormal portion. A cyst found in the back or side of the spinal cord was opened and the liquid

The meninges form a highly specialized barrier and immune interface that protects the central nervous system (CNS), regulates cerebrospinal fluid (CSF) dynamics, and coordinates communication between the CNS and the periphery. Each layer—dura mater, arachnoid mater and pia mater—possesses distinct structural, vascular and immunological features that collectively shape CNS homeostasis. A broad range of anatomical and molecular studies has revealed that meningeal compartments are far more heterogeneous and functionally complex than traditionally recognized, particularly with respect to their bar

Optic pathway gliomas are the most common central nervous system neoplasms in patients with neurofibromatosis type 1. Perineural arachnoidal gliomatosis is a rare and distinctive growth pattern of optic nerve glioma, in which the tumor infiltrates through the pia mater and pre-dominantly involves the subarachnoid space around the optic nerve. Here, we report an 8-year-old girl with perineural arachnoidal gliomatosis associated with neurofibromatosis type 1.

Traumatic brain injury (TBI) is a significant problem in global health that affects a wide variety of patients. Mild forms of TBI, commonly referred to as concussion, are a result of rapid accelerations of the head from either direct or indirect impacts. Kinetic energy from the impact is transferred into deformation of the brain, leading to cellular disruption. This transfer of energy is in part mediated by the pia-arachnoid complex (PAC), a layer of anatomical structures that forms the physical connection between the brain and the skull. The importance of properly quantifying the mechanics of

A case of chronic cerebrospinal meningitis is described which caused considerable diagnostic difficulties. Postmortem examination demonstrated parasitic character of the disease. Although all cysts were sterile, the authors suggest in the discussion that in the case the cause was infestation with the larval from of the canine tapeworm Taenia multiceps.

INTRODUCTION: Toward further cerebro-spinal flow quantification in clinical practice, this study aims at assessing the variations in the cerebro spinal fluid flow pattern associated with change in the morphology of the subarachnoid space of the cervical canal of healthy humans by developing a computational fluid dynamics model.
METHODS: 3D T2-space MRI sequence images of the cervical spine were used to segment 11 cervical subarachnoid space. Model validation (time-step, mesh size, size and number of boundary layers, influences of parted inflow and inflow continuous velocity) was performed a 4

A 39-year-old man received empiric treatment for pulmonary tuberculosis (TB). After developing sensory seizures he was restarted on anti-TB drugs when a brain MRI showed a 4.3 cm left parietal enhancing lesion with extensive edema. After TB treatment, imaging showed a reduction in size and edema. Later, he developed headache and seizures, and MRI showed recurrent edema and an enlarging lesion. Neurosurgery decided to biopsy the lesion to obtain a diagnosis. At craniotomy, a frozen section was reported as granulomatous inflammation. However, he returned postOP with a new right hemiparesis and M

The results of surgical treatment of 63 patients with basal meningiomas with supra-subtentorial spread were analyzed. The study has shown the effectiveness of one-stage tumor removal via supra-transtentorial approach with skull base bones resection. It is recommended to estimate an extent of operation on the basis of intraoperative assessment of biological behavior of the tumor, including interrelationship of tumor capsule and brainstem pia mater. Mortality rate was decreased from 23.8% to 11.1% following introduction of these principles.

BACKGROUND: The pia arachnoid complex (PAC) is a cerebrospinal fluid-filled tissue conglomerate that surrounds the brain and spinal cord. Pia mater adheres directly to the surface of the brain while the arachnoid mater adheres to the deep surface of the dura mater. Collagen fibers, known as subarachnoid trabeculae (SAT) fibers, and microvascular structure lie intermediately to the pia and arachnoid meninges. Due to its structural role, alterations to the biomechanical properties of the PAC may change surface stress loading in traumatic brain injury (TBI) caused by sub-concussive hits. The aim

In the literature screening, aquaporins were found in the cerebral structures including the pia mater, choroid plexus, ependyma, piriform cortex, hippocampus, dorsal thalamus, supraoptic and suprachiasmatic nuclei, white matter and subcortical organ. Among these, the most common are AQP1, AQP4, and AQP9. The roles of aquaporins have been demonstrated in several diseases such as cerebral edema, various central nervous system tumors, Alzheimer's Disease and epilepsy. In this review, the relationship between brain/brain-injury and aquaporin, has been reviewed.

Central nervous system (CNS) metastasis from systemic cancers can involve the brain parenchyma, leptomeninges (pia, subarachnoid space and arachnoid mater), and dura. Leptomeningeal metastases (LM), also known by different terms including neoplastic meningitis and carcinomatous meningitis, occur in both solid tumors and hematologic malignancies. This review will focus exclusively on LM arising from solid tumors with a goal of providing the reader an understanding of the epidemiology, pathophysiology, clinical presentation, prognostication, current management and future directions.

Leptomeninges, consisting of the pia mater and arachnoid, form a connective tissue investment and barrier enclosure of the brain. The exact nature of leptomeningeal cells has long been debated. In this study, we identify five molecularly distinct fibroblast-like transcriptomes in cerebral leptomeninges; link them to anatomically distinct cell types of the pia, inner arachnoid, outer arachnoid barrier, and dural border layer; and contrast them to a sixth fibroblast-like transcriptome present in the choroid plexus and median eminence. Newly identified transcriptional markers enabled molecular ch

Focal brain injury or perforant-path transections respectively led to an increase in the number of glial-fibrillary acidic protein (GFAP)-immunopositive astrocytes around the focal wound or in the terminal fields of the perforant path in the dentate molecular layer. This GFAP accumulation occurred 48-72 h after focal brain injury or perforant-path transection (wallerian degeneration). Focal brain injury also led to an accumulation of c-fos protein (Fos) in glial cells, ependyma and cells in the pia mater of the brain within 6 h of injury and this effect dissipated within 72 h. However, perfora

The clinical course of a patient with mycosis fungoides, which could be confirmed histologically in the premycotic stage, 2 years after the onset of the disease is reported. The disease led to death after a further 3 years, during which the infiltrative and tumorous stage had appeared. At autopsy, apart from the skin changes typical for mycosis fungoides, involvement of the pia mater, plexus chorioideus, cerebrum and cerebellum was established. This constituted the only manifestation of the disease in the internal organ systems. There were no detectable neurological defects.

The pia-arachnoid complex (PAC), composed of pia mater, arachnoid mater, and arachnoid trabeculae, plays a critical role in transmitting mechanical forces between the brain and skull during head trauma. Despite its importance, the material properties of the PAC microstructure are unknown, limiting the accurate representation of the brain-skull interface in simulations of traumatic brain injuries. To address this unmet need, this study combined in situ, pressure-controlled subarachnoid inflation with optical coherence tomography imaging and inverse finite element (FE) analysis to calculate the

The surface of the pineal organ of the rat is covered by a leptomeningeal tissue, the continuation of the corresponding meningeal layers of the diencephalon. The pineal leptomeninx consists of stratified arachnoid and of pia mater cells which follow the vessels into the pineal nervous tissue. The pineal arachnoid contains electron-lucent and electron dense cells differing from each other in their cytoplasmic components. Corpora arenacea of various size and density occur among these arachnoid cells and can grow into the pineal organ alongside pia mater tissue. Acervuli often form groups in circ

The pathogenesis of cranial and spinal dysraphia has been controversial. Studies of spinal dysraphia have shown that the relationships of the pia and dura to the cutaneous layers were best understood as the result of a primary abnormality of mesenchymal structures, with the nervous system lesions occurring as a result of exposure of the bare spinal cord on the body surface. This study was undertaken to determine if the relationship of the cutaneous layers in anencephaly were similar to those found in spinal dysraphia. We reviewed serial histologic sections of the cranial structures of 10 anenc

BACKGROUND: Acetylcholinesterase inhibition at the spinal level has been shown to produce a potent antinociceptive effect. However, the site of cholinesterase inhibition is unknown. To determine whether the spinal meninges participate in acetylcholine metabolism, the spinal meninges of monkeys and pigs were assayed for cholinesterase activity.
METHODS: Spinal cord, dura mater, and arachnoid mater specimens from anesthetized pigs and monkeys were mechanically homogenized and cholinesterase activity was determined quantitatively using a commercially available colorimetric assay. The ability of
