The impact of occupations on brain structures has attracted considerable research interests in the last decade. The aim of this research is to find the effect of flight training on brain gray matter volume of pilots. The whole-brain structural magnetic resonance imaging (sMRI) data collected from 26 pilots and 24 controls was analyzed using Voxel-based morphological analysis method (VBM) combined with T1 data to quantitatively detect the local gray matter of brain tissue and calculate the gray matter volume. The result shows that the pilot group has larger gray matter volume in the lingual gyr

INTRODUCTION: Inflight medical incapacitations are rare events that can result in the loss of lives and aircraft. The potential for an in-flight medical event deserves the attention of certification authorities. Cardiac emergencies are among the most common serious events. The primary focus of this study was to examine whether the proportions of pilots with reported cardiac conditions could be used to identify those who had inflight medical incapacitations.
METHODS: The Civil Aerospace Medical Institute Inflight Incapacitation Registry was searched to identify airline pilots with a cardiac hi

INTRODUCTION: This study explores the U.S. experience with waivers for insulin treatment for third-class medical certificates. From 1997 through 2014, the Federal Aviation Administration (FAA) approved an estimated 1500 waivers for insulin-treated diabetes with a total of 450 active waivers as of December 31, 2014. These pilots were involved in 25 accidents, but none were attributed to medical issues.
METHODS: Data for the insulin waiver group and control group were obtained from the FAA's aeromedical certification system and matching accident data from the NTSB database. A logistic regressio

Israeli Air Force (IAF) pilots continue flying combat missions after the symptoms of natural near-vision deterioration, termed presbyopia, begin to be noticeable. Because modern pilots rely on the displays of the aircraft control and performance instruments, near visual acuity (VA) is essential in the cockpit. We aimed to apply a method previously shown to improve visual performance of presbyopes, and test whether presbyopic IAF pilots can overcome the limitation imposed by presbyopia. Participants were selected by the IAF aeromedical unit as having at least initial presbyopia and trained usin

BACKGROUND: Barotrauma is a frequent problem in aviation medicine. Eustachian tube dysfunction plays a critical role in the pathogenesis of barotrauma. Function of the Eustachian tube can be indirectly assessed by multifrequency tympanometry, which provides valuable information about the resistance and permeability of the middle ear in a wide frequency range. The aim of this study was to research whether multifrequency tympanometry could be used for assessing middle ear impairments in pilots.
METHODS: There were 140 pilots and pilot candidates between the ages of 20-55 with normal otoscopic e

BACKGROUND: Central serous chorioretinopathy (CSC), which has the characteristics of a serous retinal detachment (SRD) in the macular area, affects mostly men in the 30- to 50-yr age range. Some patients have persistent SRD, which may cause retinal thinning and photoreceptor impairment. CSC symptoms are gradual vision loss and/or metamorphopsia. Some commercial airline pilots are concerned about CSC symptoms, which can disqualify them from flying for months and can also reoccur. Thus, careful monitoring and treatment of CSC are critical for pilots, especially those with chronic or recurrent CS

OBJECTIVE: To evaluate the audiometric profile of civilian pilots according to the noise exposure level.
METHODS: This observational cross-sectional study evaluated 3,130 male civilian pilots aged between 17 and 59 years. These pilots were subjected to audiometric examinations for obtaining or revalidating the functional capacity certificate in 2011. The degree of hearing loss was classified as normal, suspected noise-induced hearing loss, and no suspected hearing loss with other associated complications. Pure-tone air-conduction audiometry was performed using supra-aural headphones and acous

Neck pain and neck injury are common complaints among military aviators flying high performance (HPF) aircraft. Since all candidates for military flying training with the Royal Norwegian Air Force have to meet HPF medical standards, medical selection procedures include a radiological examination of the spine. Nine films of the vertebral column are made with emphasis put on cervical structures. Among 232 applicants, 527 deviations from normal conditions were described--anomalies, degenerative changes and deviations of posture--which average 2.27 diagnoses per spine X-rayed. Of the candidates, 8

INTRODUCTION: Middle ear and sinus barotraumas (MEBt and SBt) are common conditions in aviation medicine, but systematic prevalence data on military pilots in real-life military flight are lacking.
METHODS: An anonymous electronic questionnaire was distributed to 170 consecutive military pilots of the Finnish Defense Forces as part of their annual flight physical examinations. The primary outcomes were the self-reported frequency, clinical characteristics, and occupational health effects of MEBt and SBt in flight. The secondary outcomes were factors associated with repeated MEBt and SBt in fl

BACKGROUND: Migraine is a common disorder with significant aeromedical implications. The variability and unpredictable nature of occurrences hampers accurate assessment of future risk. This uncertainty results in a necessarily conservative approach to aeromedical recommendations, which unfortunately may lead to over-restrictive dispositions. Limited long-term follow up information is available on migraine outcomes in pilots, particularly assessing for impact of potential modifiable aggravating factors. Methods: This retrospective study reviewed 159 U.S. Air Force pilots with migraine who had be

BACKGROUND: Fighter pilots undergo extensive medical screening but may still miss rare diseases like latent autoimmune diabetes in adults (LADA). LADA patients have circulating autoantibodies directed against pancreatic beta cell antigens and present with frank diabetes late in life which may elude conventional military flight screening.CASE REPORT: Two fifth-generation fighter pilots, a 38-yr-old man (patient 1) and a 27-yr-old man (patient 2), with no significant past medical histories developed symptoms of fatigue, weight loss, episodic polyuria, and arthralgia. Patient 1's symptoms were in

INTRODUCTION: Nonhypoxic hypobaric (low atmospheric pressure) occupational exposure, such as experienced by U.S. Air Force U-2 pilots and safety personnel operating inside altitude chambers, is associated with increased subcortical white matter hyperintensity (WMH) burden. The pathophysiological mechanisms underlying this discrete WMH change remain unknown. The objectives of this study were to demonstrate that occupational exposure to nonhypoxic hypobaria is associated with altered white matter integrity as quantified by fractional anisotropy (FA) measured using diffusion tensor imaging and re

Object of investigation was types of personality of civil pilots. The investigation involved healthy pilots (n = 58, controls) and pilots afflicted with cerebral atherosclerosis (n = 67) including pilots with atherosclerotic plaques (n = 23). The personality questionnaire developed by L.N. Sobchik was used. The investigation showed that signs of personality accentuation in pilots with cerebral atherosclerosis were largely manifested by distortion of individual psychological attributes. This finding may be helpful to members of the medical flight certification board in making prognosis of pilot

BACKGROUND: Pilots who smoke are occasionally obliged to abstain from nicotine intake during flight and may during this period exhibit certain symptoms leading to performance decrement.
METHODS: We studied 20 healthy male aviators, who were regular smokers, (mean age 33.7 +/- 1.4 yr) operating military fixed- and rotary-wing aircraft (C-47 Dakota, F-16, A. Bell 205). All pilots were subjected to a 12-h abstinence from cigarette smoking, during which time they performed flight duties. After landing, we studied the intensity of the nicotine withdrawal syndrome, as well as its effect on physiolo

Plasma endocrine hormones were studied in both 24 motion sickness (orthostatic intolerance) and healthy pilots. Coriolis acceleration of 3.75, 5.00 and 6.25 pi 2 cm/s2 were given with intervals of 3-4 min AT-II, insulin, cortisol, Aldosterone and gastrin were determined by radioimmunoassay. It was found that aldosterone, AT-II, gastrin increased with increase of coriolis acceleration in all pilots. (P < 0.05), but cortisol and insulin only increased in healthy pilots (P < 0.05). It suggests excitation of the autonomic nervous system might be insufficient in orthostatic intolerant pilots and th
