Ultrasound-guided piriformis muscle injection. A new approach.

Piriformis syndrome is an uncommon cause of buttock and leg pain. Some treatment options include the injection of piriformis muscle with local anesthetic and steroids. Various techniques for piriformis muscle injection have been described. Ultrasound allows direct visualization and real time injection of the piriformis muscle. We describe 5 consecutive patients, diagnosed of piriformis syndrome with no improvement after pharmacological treatment. Piriformis muscle injection with local anesthetics and steroids was performed using an ultrasound technique based on a standard technique. All 5 pati

Evaluation of Accuracy of Shear Wave Elastography and Superb Microvascular Imaging Methods in Diagnosis of Piriformis Syndrome: A Preliminary Study.

Piriformis syndrome is a neuromuscular disease resulting from sciatic nerve compression caused by an abnormal condition in the piriformis muscle. Superb microvascular imaging (SMI) is a new ultrasound imaging technique that visualizes low-velocity and small-diameter blood vessel flow. In our prospectively designed study, we aimed to evaluate the accuracy of diagnosis of piriformis syndrome with innovative methods such as shear wave elastography (SWE) and SMI. Thirty-two patients diagnosed with unilateral piriformis syndrome were accepted to the statistical stage. The side without symptoms was

Piriformis syndrome: anatomic considerations, a new injection technique, and a review of the literature.

BACKGROUND: Piriformis syndrome can be caused by anatomic abnormalities. The treatments of piriformis syndrome include the injection of steroid into the piriformis muscle and near the area of the sciatic nerve. These techniques use either fluoroscopy and muscle electromyography to identify the piriformis muscle or a nerve stimulator to stimulate the sciatic nerve. METHODS: The authors performed a cadaver study and noted anatomic variations of the piriformis muscle and sciatic nerve. To standardize their technique of injection, they also noted the distance from the lower border of the sacroili

Piriformis syndrome: a case with non-discogenic sciatalgia.

Piriformis syndrome is a clinical picture of non-discogenic sciatica caused by compression of the sciatic nerve by the piriformis muscle. It has variable etiologies and the patho-physiology is not fully understood. The major etiology was known to be the spasm, edema and inflammation of the piriformis muscle and sciatic nerve compression of the muscle later on. Patients can be diagnosed immediately with a comprehensive clinical examination and early diagnosis makes the treatment much easier. Diagnosis of the piriformis syndrome, a very rare cause of low back pain, first requires that this syndr

Botulinum toxin injection strategy of intractable and relapsed piriformis syndrome: A case report.

RATIONALE: Piriformis syndrome (PS) is neuromuscular disorder caused by sciatic nerve compression by piriformis muscle and related to sciatic-type pain. When the conservative care fails, local injection or surgery can be also performed into piriformis. In recent years, botulinum toxin (BoNT) has also been considered as a new therapeutic option of piriformis syndrome. PATIENT CONCERNS: A man in his late 40s came to pain clinic for left low back pain. The symptom was aggravated with sitting position. DIAGNOSIS: Piriformis syndrome. INTERVENTIONS: The patient underwent BoNT injection with 100

The piriformis syndrome: a review.

Piriformis syndrome is a controversial cause of hip and low back pain whose etiology stems from anatomic irregularities of the piriformis muscle and surrounding structures of the posterior hip region. Although this syndrome has been described by many clinicians, the association of piriformis muscle dysfunction and pain in the hip region remains conjectural. The diagnosis is one of exclusion of other more common pain generators in the hip, pelvis, or low back. The authors of this article will review the anatomy of the region as it relates to piriformis muscle dysfunction and delineate the variou

Swelling of the leg, deep venous thrombosis and the piriformis syndrome.

BACKGROUND: The piriformis syndrome, which was first described 60 years ago, is a well recognized cause of sciatica, leg pain and low back pain, due to the entrapment of the sciatic nerve in the piriformis and other rotator muscles. Very few complications relating to this syndrome have been described. AIMS: To discuss how the piriformis syndrome may cause venous engorgement in the lower limb, and how the piriformis syndrome should be included as a possible cause of acute deep venous thrombosis in a not initially swollen leg. Both complications can occur independently. METHODS: Two cases of s

A rare bimuscular conglomeration gluteopiriformis case report.

The present paper reports a rare anomalous muscle in the gluteal region, which appears to be a conglomeration of gluteus maximus and piriformis muscles. The muscle was proximally attached to the gluteus maximus and distally merged with the piriformis muscle. The innervation of the anomalous muscle was derived from the inferior gluteal nerve. Recognition of such a muscle variant may facilitate early clinical diagnosis and treatment of patients with symptoms of piriformis syndrome or sciatica of unexplained etiology. Contraction of such an anomalous muscle could lead to altered biomechanics of t

Perisciatic injection of steroid for the treatment of sciatica due to piriformis syndrome.

BACKGROUND AND OBJECTIVES: Piriformis syndrome causing sciatica is sometimes refractory to conventional treatments including physical therapy, piriformis injections, and even caudal epidural steroid injections. Surgical release of the piriformis muscle has been described for difficult cases of piriformis syndrome, but is occasionally accompanied by morbidity. Another approach to treating piriformis syndrome is presented. METHODS: A perisciatic injection of steroid using simple landmarks and utilizing a nerve stimulator to locate and inject near the sciatic nerve and into the piriformis muscle

Piriformis syndrome surgery causing severe sciatic nerve injury.

Piriformis syndrome is a controversial entrapment neuropathy in which the sciatic nerve is thought to be compressed by the piriformis muscle. Two patients developed severe left sciatic neuropathy after piriformis muscle release. One had a total sciatic nerve lesion, whereas the second had a predominantly high common peroneal nerve lesion. Follow-up studies showed reinnervation of the hamstrings only. We conclude that piriformis muscle surgery may be hazardous and result in devastating sciatic nerve injury.

Targeted Ultrasound-Guided Perineural Hydrodissection of the Sciatic Nerve for the Treatment of Piriformis Syndrome.

Piriformis syndrome is a common cause of lumbar, gluteal, and thigh pain, frequently associated with sciatic nerve symptoms. Potential etiologies include muscle injury or chronic muscle stretching associated with gait disturbances. There is a common pathological end pathway involving hypertrophy, spasm, contracture, inflammation, and scarring of the piriformis muscle, leading to impingement of the sciatic nerve. Ultrasound-guided piriformis injections are frequently used in the treatment of these pain syndromes, with most of the published literature describing injection of the muscle. We descr

Physiotherapy for Piriformis Syndrome Using Sciatic Nerve Mobilization and Piriformis Release.

Piriformis syndrome is also synonymous with sciatica or buttock pain. This is a condition where the muscle irritates the sciatic nerve. This nerve passes above, below, or in between the piriformis muscle piercing it. The muscle tightens or shortens, thus compressing the nerve and disturbing the impulses passing from it. The sciatic nerve is a combination of nerve roots from L4 to S3. Piriformis works as a lateral rotator and is a synergistic muscle of the flexor and abductor group. Females most commonly present with piriformis syndrome than males. Many causative factors are responsible for the

Piriformis Syndrome: A Narrative Review of the Anatomy, Diagnosis, and Treatment.

Piriformis syndrome is a form of sciatica caused by compression of the sciatic nerve by the piriformis muscle. It is a relatively uncommon, but not insignificant, cause of sciatica. The diagnosis of piriformis syndrome is complicated by the large differential diagnosis of low back and buttock pain with many diagnoses having overlapping symptoms. This narrative review highlights the relevant anatomy, history, physical exam maneuvers, electrodiagnostic findings, and imaging findings that are used to diagnose piriformis syndrome. Also discussed are posterior gluteal myofascial pain syndromes that

Exploring the anatomical basis of piriformis syndrome: A systematic review.

INTRODUCTION: Piriformis syndrome remains underdiagnosed, particularly due to the anatomical complexity of the gluteal region. The aim of this study is to identify the anatomical variations most at risk for piriformis syndrome. METHODS: A systematic search was conducted in English in PubMed and Scopus between November 2024 and December 2025. The search covered various types of studies reporting extractable data on the anatomical aspects of piriformis syndrome. The selection process for this study was conducted by a single reviewer in accordance with PRISMA guidelines. RESULTS: A total of 16

[Reflex sympathetic dystrophy secondary to piriformis syndrome: a case report].

Piriformis syndrome is a rare cause of hip and foot pain which may be due to sciatic nerve irritation because of anatomic abnormalities of sciatic nerve and piriformis muscle or herniated disc, facet syndrome, trochanteric bursit, sacroiliac joint dysfunction, endometriosis and other conditions where sciatic nerve is irritated. There has been no reflex sympathetic dystrophy (RSD) case presented due to piriformis syndrome before. A sixty-two-year-old female patient had right foot and hip pain (VNS: 8), redness and swelling in the foot since 15 days. Her history revealed long walks and travellin

Piriformis Syndrome (Sciatic Nerve Entrapment) Associated With Type C Sciatic Nerve Variation: A Report of Two Cases and Literature Review.

Although anatomical variation of the sciatic nerve and piriformis muscle at the greater sciatic notch is considered an important cause of piriformis syndrome, there are few reports on the surgical treatment of piriformis syndrome owing to specific anatomical variations of the sciatic nerve and piriformis muscle. In this report, we describe 2 cases of piriformis syndrome caused by a rare type C sciatic nerve variation that were surgically treated using the transgluteal approach. The first patient reported unremitting left hip and leg pain that occurred following blunt trauma to the hip. The sec

When Sciatica Is Not Disk Disease.

In brief Piriformis muscle syndrome is an often unrecognized cause of sciatic pain. The sciatic nerve, which normally passes beneath the piriformis muscle deep in the midbuttock, can be compressed when the muscle goes into spasm or is injured. The resultant pain is often confused with discogenic disease, but a thorough history and physical exam can pinpoint the true cause. Conservative treatment of piriformis syndrome-with a focus on stretching-is often successful.

The piriformis syndrome.

Piriformis syndrome is an often misdiagnosed cause of sciatica, leg, or buttock pain, and disability. The sciatic nerve may be compressed within the buttock by the piriformis muscle, with pain increased by muscular contraction, palpation, or prolonged sitting. A thorough medical history and physical examination are essential to proper diagnosis. Diagnostic testing may be used to differentiate piriformis syndrome from other causes of sciatica, lower extremity weakness, and pain. This article reviews the pathophysiology and management of piriformis syndrome.

[Relationships between piriformis muscle and sciatic nerve: radioanatomical study with 104 buttocks].

OBJECTIVE: The relationships between the piriformis muscle and the sciatic nerve are close and may be changing. In some cases, these relationships are the cause of a piriformis syndrome, an under-diagnosed etiology of non discal sciatica. The aim of the study was to explore, by the MRI, the sciatic nerve and the piriformis muscle. MATERIELS AND METHODS: One hundred and four buttocks were explored by MRI for 52 randomly selected patients. RESULTS: In 59.6% of cases, no variation of the piriformis muscle and sciatic nerve were found. In 26% of cases, the sciatic nerve was divided into the pelvi
