High-resolution bone scintigraphy of the adult wrist.

Bony anatomic landmarks of the wrist (e.g., pisiform, hook of hamate, radioulnar joint, and styloid processes of the radius and ulna) were routinely identified in 28 adult patients examined for wrist pain. With the wrists prone and immobilized, bone scintigrams were obtained for 500,000 counts with an asymmetric (133 to 161 keV) Tc-99m energy window and either a converging (best choice) or straight-bore, high-resolution collimator. High-resolution scintigraphy precisely localized degenerative joint disease (nine patients), scaphoid fractures (five), pisiform fracture (one), lunate avascular ne

Ligamentous anatomy of the medial carpal region.

The purpose of this study is to elucidate the ligamentous anatomy of the medial carpus in order to characterize the pathologic basis of pisiform subluxation secondary to traumatic ligament rupture. Medial carpal ligamentous anatomy has been studied in the past by anatomic dissection and analyzed in terms of comparative morphology. A review of this literature reveals that the anatomy of the region requires further definition. We have performed cadaveric dissections that reveal that the ulnar or medial carpal ligament complex is composed of three structures: (1) the triangular fibrocartilage, (2

Atypical osteochondroma of the hamate that presented clinically as carpal tunnel syndrome: report of an extremely rare case and literature review.

BACKGROUND: Osteochondroma is a benign tumor that occurs mainly at the metaphysis of long bones and seldom arises from carpal bones. We describe an extremely rare case of osteochondroma of the hamate without a typical cartilaginous cap and with a spiky bony protrusion in an elderly patient. CASE PRESENTATION: A 78-year-old right-handed female housekeeper had a multilobed osteochondroma of the hamate, which caused carpal tunnel syndrome and irritation of the flexor tendons. Radiological examinations showed a morphological abnormality of the hamate comprising a spiky bony protrusion into the ca

Acute isolated traumatic anterior pisiform dislocation in an adult: A case report.

Isolated pisiform dislocation without associated carpal bone injuries is a rare clinical entity, with few reported cases. This report describes a 23-year-old, right-handed male who presented in March 2024 with acute anterior pisiform dislocation following indirect trauma while lifting furniture. The patient experienced pain, mild swelling, and tenderness in the distal ulnar region of the right hand, with initial radiographs confirming the dislocation. Spontaneous repositioning occurred during transfer to a trauma hospital after cast immobilization, eliminating the need for further imaging or i

Long-standing Ulnar Wrist Pain Caused by an Anatomic Variant of the Ulnar Nerve: A Case Report.

CASE: We present a 28-year-old woman with long-standing left ulnar wrist pain. Physical examination revealed severe tenderness over the pisiform; imaging findings were normal. Surgical exploration revealed compression of an aberrant branch, likely Kaplan's accessory branch, around the pisiform. Complete symptom relief was achieved after nerve decompression. CONCLUSION: Clinical symptoms associated with Kaplan's accessory branch are rare, indicating that many branches may be asymptomatic or overlooked despite the presence of symptoms. Kaplan's accessory branch should be considered as a potenti

Pathologic conditions of the hypothenar eminence: evaluation with multidetector CT and MR imaging.

Pain, weakness, and sensory loss occur frequently in the hypothenar eminence. However, clinical examination is difficult and nonspecific, and the prescribed imaging technique may be inadequate, or images may be misinterpreted. Different imaging modalities have various degrees of usefulness for the diagnosis of painful pathologic conditions of the hypothenar eminence. Radiography, multidetector computed tomography (CT), multidetector CT arthrography, and magnetic resonance (MR) imaging of the wrist are useful for surveying the anatomy of the hypothenar eminence, the Guyon canal, and the ulnar n

Structural changes of the carpal tunnel, median nerve and flexor tendons in MRI before and after endoscopic carpal tunnel release.

The purpose of this study is to investigate the structural changes of the carpal tunnel, median nerve, and flexor tendons in magnetic resonance imaging (MRI) before and after endoscopic carpal tunnel release (ECTR). We studied 36 hands undergoing ECTR. In MRI, the cross-sectional area of the carpal tunnel and the median nerve at the hamate and the pisiform levels were measured. The distance from the volar side of carpal bone to the median nerve or tendons and the volar displacement were measured. In post-operative MRI, the transverse carpal ligament could not be well delineated and the carpal

The role of the PTHrP/Ihh feedback loop in the unusual growth plate location in mammalian metatarsals and pisiforms.

BACKGROUND: Longitudinal skeletal growth takes place in the cartilaginous growth plates. While growth plates are found at either end of conventional long bones, they occur at a variety of locations in the mammalian skeleton. For example, the metacarpals and metatarsals (MT) in the hands and feet form only a single growth plate at one end, and the pisiform in the wrist is the only carpal bone to contain a growth plate. We take advantage of this natural anatomical variation to test which components of the PTHrP/Ihh feedback loop, a fundamental regulator of chondrocyte differentiation, are specif

Pisotriquetral arthrosis after triquetral malunion: a case report.

We report a patient with chronic ulnar palmar wrist pain caused by malunion after isolated fracture of the triquetrum. Intra-articular malunion of the triquetrum body resulted in posttraumatic pisotriquetral arthrosis. The diagnosis was made by magnetic resonance imaging, computed tomography, bone scintigraphy, and wrist arthroscopy 5 months after the injury. The patient's symptoms were relieved by excision of the pisiform.

Pisotriquetral instability causing an unusual flexor tendon rupture.

We present a case of a closed rupture of the flexor digitorum profundus tendon of the small finger. It is our hypothesis that because of pisotriquetral instability as a result of a lesion of the pisotriquetral ligaments the flexor digitorum profundus tendon was entrapped repeatedly between the pisiform and triquetrum. This resulted in an attritional lesion of the tendon. The pisotriquetral joint showed no bone abnormalities.
