A similar number of adults and children had invasive pneumococcal infection. There was male predominance, and different ethnic distribution between children and adults. The majority of adults (78%), had underlying diseases, but this was less frequent in children (24%). The presenting illness differed between adults and children. Complications of invasive pneumococcal infection occurred more frequently in adults than in children. The mortality rate in adults was 21.5%; in children, only 3.8%. The rate of penicillin-resistant pneumococci at our hospital was 23%, while cefotaxime resistance was 4

Antibiotics and chemotherapeutics have been available for about 40 years, but still bacterial infections constitute some of the greatest problems in medicine. Pneumococci causing pneumonia, bacteremia, meningitis and otitis are a leading cause of illness and death. The exact incidence and lethality of pneumococcal infections is not known, however, since they are not reportable diseases in most countries and since microbiological diagnosis is difficult. In the latest years some significant progresses have been made for the diagnosis of infections caused by pneumococci, especially pneumonia. Thi

During the period 1974-1987, the frequency of infections in alcoholics discharged from hospitals in Funen showed an increase of 5.53 +/- 3.38 (p = 0.0039) per 10,000 alcoholics discharged per year. The frequency of discharged alcoholics increased by 3.72 +/- 0.60 per 10,000 discharged patients per year. Infections were diagnosed in 0.48 per cent of the discharged alcoholics and in 0.18 per cent of the discharged non-alcoholics (p less than 0.00001). Pneumococcal infections were relatively more frequent in alcoholics (p less than 0.05).

When administered prophylactically, we show that the Toll-like receptor-2 (TLR-2) agonist PEG-Pam2Cys (pegylated-S-(2,3-bis(palmitoyloxy)propyl)cysteine) not only mediates potent anti-viral activity against influenza virus but also reduces the impact of secondary infections with Streptococcus pneumoniae (the pneumococcus) by reducing (i) pulmonary viral and bacterial burdens, (ii) the levels of proinflammatory cytokines that normally accompany influenza and S. pneumoniae secondary infections and (iii) the vascular permeability of the pulmonary tract that can allow bacterial invasion of the blo

Elderly people are more susceptible to pneumococcal infections. Data in Germany from 2005-2010 shows that especially seniors are prone to develop serious complications such as sepsis. Women are obviously less affected than men. Most of the infections occurred during the winter months. The majority of isolates, i.e., about 80%, possess capsular polysaccharide antigens which are represented in the 23-valent vaccine. Consequently, it could be assumed that the severe complications ensuing long hospital stays and associated with a high mortality could have been avoided, if the elderly people would

We retrospectively analyzed 33 cases of children with systemic pneumococcal infections, 22 bacteremia and 11 meningitis, diagnosed and treated in Asahi General Hospital between 1985 and 1999. The median age at diagnosis was 15 months old and the incidence peaked in infants between 7 and 24 months of age (57.6%). Two cases showed low serum IgG2 levels. Fever was a common symptom in all cases and 13 (39.4%) presented convulsions. Meningitis [median age: 10 months] tended to occur, if not significant, in younger children than bacteremia [16 months]. All cases of meningitis were diagnosed 12 hours

Over the past three decades, penicillin-resistant pneumococci have emerged worldwide. In addition, penicillin-resistant strains have also decreased susceptibility to other beta-lactams (including cephalosporins) and these strains are often resistant to other antibiotic groups, making the treatment options much more difficult. Nevertheless, the present in vitro definitions of resistance to penicillin and cephalosporins in pneumococci could not be appropriated for all types of pneumococcal infections. Thus, current levels of resistance to penicillin and cephalosporin seem to have little, if any,

Pneumococcal sepsis is a life-threatening complication in splenectomised patients even since the introduction of a polyvalent pneumococcal vaccine. Eight episodes of pneumococcal sepsis in seven properly vaccinated splenectomised patients are described. All but two of the causative serotypes were included in the vaccine. The fact that most of the patients were immunosuppressed may explain the failure of the vaccine in these particular patients. A survey of 59 episodes of pneumococcal bacteraemia during 1982 at the Soroka Medical Center, however, shows that 32% of the strains of pneumococci iso

The 23-valent polysaccharide vaccine and the 7-valent pneumococcal conjugate vaccine are licensed vaccines that protect against pneumococcal infections worldwide. However, the incidence of pneumococcal diseases remains high in low-income countries. Whole-cell vaccines with high safety and strong immunogenicity may be a favorable choice. We previously obtained a capsule-deficient Streptococcus pneumoniae mutant named SPY1 derived from strain D39. As an attenuated live pneumococcal vaccine, intranasal immunization with SPY1 elicits broad serotype-independent protection against pneumococcal infec

An etiologic classification of acute pneumonia was presented and the relative importance of some of the causative agents was briefly reviewed. The early developments of the therapy of pneumococcal pneumonia with type-specific antisera, sulfonamide drugs, and antimicrobial drugs were reviewed, mostly from the experiences of the author at Boston City Hospital. Changes in the occurrence and relative importance of the pneumococcus as a cause of infections associated with bacteremia, empyema, and meningitis were demonstrated, based on cases observed at Boston City Hospital during 12 selected years

Streptococcus pneumoniae is one of the important causative pathogens for both upper and lower respiratory tract infections during childhood. The current study was designed to evaluate the protection against fatal pneumococcal infections during the infant period by maternal immunization with pneumococcal surface protein A (PspA). Four-week-old females BALB/c mice were immunized with PspA and cholera toxin B (CTB) intranasally twice a week for 3 weeks. After mating, the 10-day-old offspring of these mice were intraperitoneally (i.p.) infected with S. pneumoniae to evaluate survival. Anti-PspA-sp

Pneumococcal meningitis is a subgroup of invasive pneumococcal disease with a case-fatality rate of up to 30% and long-term sequelae in more than 50% of cases in adults in developed countries. We aimed to determine risk factors for this particular form of pneumococcal disease. We conducted a prospective population-based laboratory study of invasive pneumococcal disease in adults in North-Rhine-Westphalia, Germany from February 2001 to August 2006. All isolates underwent serotyping and susceptibility testing at the National Reference Centre for Streptococci in Aachen, Germany. Data were analyse

Pneumococcal sepsis and/or meningitis are major causes of morbidity and mortality in young children with sickle cell disease. Abnormal complement activity, poor splenic function and a lack of type-specific pneumococcal antibody are responsible for the severity and frequency of these infections. A program consisting of early institution of antibiotic therapy for febrile episodes, antimicrobial prophylaxis, and administration of pneumococcal vaccine may be effective in reducing the incidence of pneumococcal disease. Specific guidelines for infection prevention are presented. Other infections tha

BACKGROUND: Neoplasms greatly increase the risk of infection-the main causes of death are pneumonia and influenza infection. Streptococcus pneumoniae, the most common pathogen causing community-acquired pneumonia, is of great significance in patients suffering from hematological neoplasms, which have a 30 times higher incidence of invasive pneumococcal disease than the general population. Vaccination plays a key role in prevention. The Standing Committee on Vaccination (STIKO) recommends vaccination for adults over 60 and immunosuppressed adults. Only about 20% of these individuals are vaccina

Pneumococcal infections continue to pose a serious medical problem. A broad range of serotypes, increasing resistance to antibiotics and high pathogenic potential of pneumococci are associated with development of various clinical forms of diseases. Some chronic diseases are an important predisposing factor for development of pneumococcal infections. The most common noninvasive forms of the disease are otitis, sinusitis, conjunctivitis; pneumonia is on the borderline between the invasive and noninvasive forms. Meningitis, sepsis, endocarditis and arthritis all belong to invasive pneumococcal di

Antimicrobial susceptibilities of 156 Streptococcus pneumoniae strains isolated from 1994 through 2002 were studied. Of this total, 38.7, 26.3, 16.7, 8.9, and 9.6% were recovered from patients with bacteremia, pneumonia, otitis media, sinusitis, and meningitis, respectively. All S. pneumoniae strains were fully susceptible to amoxicillin-clavulanic acid and ampicillin, with 9.0 and 2.6% being resistant to penicillin and ceftriaxone, respectively. The ratios of resistant strains to tetracycline, co-trimoxazole, and chloramphenicol were 73.7, 69.3, and 63.5%, respectively. Approximately 90% of s
