Alveolar epithelial cells orchestrate DC function in murine viral pneumonia.

Influenza viruses (IVs) cause pneumonia in humans with progression to lung failure. Pulmonary DCs are key players in the antiviral immune response, which is crucial to restore alveolar barrier function. The mechanisms of expansion and activation of pulmonary DC populations in lung infection remain widely elusive. Using mouse BM chimeric and cell-specific depletion approaches, we demonstrated that alveolar epithelial cell (AEC) GM-CSF mediates recovery from IV-induced injury by affecting lung DC function. Epithelial GM-CSF induced the recruitment of CD11b+ and monocyte-derived DCs. GM-CSF was a

Serum lipidome remodeling in viral pneumonia: from pathophysiology to therapeutics.

Viral pneumonia remains a preeminent global health threat, frequently culminating in acute respiratory distress syndrome (ARDS) and systemic organ failure. While traditional paradigms have centered on protein-based inflammatory cascades, this review synthesizes a vast body of emerging evidence to redefine viral pneumonia as a profound systemic metabolic crisis, specifically characterized by the radical remodeling of the host lipidome. Based on a meta-synthesis of clinical and mechanistic data, we delineate a consistent "metabolic crash" during severe infection, where a precipitous decline in s

GRP78 in viral pneumonia: dual regulatory mechanisms and translational prospects.

Glucose-regulated protein 78 (GRP78), a core molecular chaperone governing the endoplasmic reticulum (ER) stress response, exerts dual regulatory effects during the pathogenesis of viral pneumonia. Beyond serving as a critical cofactor that promotes viral invasion and replication, GRP78 functions as an indispensable protective chaperone that preserves pulmonary cellular homeostasis and attenuates lung injury. This review characterizes two distinct subcellular localizations of GRP78 in viral pneumonia, namely ER-resident GRP78 and cell surface GRP78 (csGRP78), and systematically summarizes its

Characteristics of Children With Clinically Suspected Pneumonia and Viral Infection.

This prospective study sought to assess whether the clinical and radiographic characteristics of children with suspected pneumonia differed by virus. We observed that among 2115 children undergoing chest radiography for suspected pneumonia, 718 (33.9%) had viral testing performed, among which 126 (17.5%) had respiratory syncytial virus, 55 (7.6%) had influenza, and 18 (2.5%) had COVID-19 infection. Clinical and radiographic findings varied by virus. Tachypnea, retractions, rales, and viral radiographic features were more common among children with respiratory syncytial virus. Children with inf

Post Varicella lung fibrosis: a lingering scar of viral pneumonia.

Varicella pneumonia occurs in 5%-15% of adults with primary varicella infection. While acute pulmonary complications of varicella pneumonia are well described, there are limited data on its long-term sequelae. We report a rare case of post varicella lung fibrosis that presented to us 2½ years after an uneventful recovery from varicella pneumonia. What makes this case unique is the demonstration of characteristic inclusion bodies in the histology specimen and the positive PCR test for varicella DNA, which substantiated the diagnosis and elucidated the possibility of long-term viral persistence.

Viral community-acquired pneumonia: what's new since COVID-19 emerged?

INTRODUCTION: All over the world, viral pneumonia has a significant impact on morbidity and mortality, especially among vulnerable populations. The most common respiratory viruses causing pneumonia include influenza virus, respiratory syncytial virus, adenoviruses and rhinovirus. The COVID-19 pandemic has changed the landscape of viral pneumonia and has reshaped our understanding of the role of viruses in this disease. We are now more aware of the importance of early diagnosis, the impact of co-infections, the effects of viral variants, and the long-term consequences of post-viral pneumonia.

Antiviral treatment for viral pneumonia: current drugs and natural compounds.

In recent years, viral pneumonia has become a significant challenge to global public health, particularly during the COVID-19 pandemic. Viral pneumonia can be caused by various viruses, including influenza virus, RSV, and adenovirus. These viruses trigger inflammatory responses by invading the respiratory epithelial cells, leading to lung damage. Existing antiviral drugs such as ribavirin, adobiravir, and oseltamivir exert their therapeutic effects by inhibiting different stages of the viral life cycle but face issues such as increasing drug resistance. Natural components like astragalus sapon

Dysphagia Is an Underrecognized Risk Factor for Viral Pneumonia Severity.

The aim of this study was to examine the role of pre-existing dysphagia as a risk factor for COVID-19 severity among adults ≥50 years of age presenting to the emergency department (ED). This was a retrospective cohort study that used electronic health record data from two Midwestern EDs in the same health care system. The sample included patients ≥50 years of age who tested positive for SARS-COV-2 during an ED visit between March 15, 2020 and November 19, 2020. Patients were dichotomized based on documented history of dysphagia. The primary outcome was the highest World Health Organization COV

Respiratory Viral Infection-Induced Microbiome Alterations and Secondary Bacterial Pneumonia.

Influenza and other respiratory viral infections are the most common type of acute respiratory infection. Viral infections predispose patients to secondary bacterial infections, which often have a more severe clinical course. The mechanisms underlying post-viral bacterial infections are complex, and include multifactorial processes mediated by interactions between viruses, bacteria, and the host immune system. Studies over the past 15 years have demonstrated that unique microbial communities reside on the mucosal surfaces of the gastrointestinal tract and the respiratory tract, which have both

Review of Non-Bacterial Infections in Respiratory Medicine: Viral Pneumonia.

Although bacteria are the main pathogens involved in community-acquired pneumonia, a significant number of community-acquired pneumonia are caused by viruses, either directly or as part of a co-infection. The clinical picture of these different pneumonias can be very similar, but viral infection is more common in the pediatric and geriatric populations, leukocytes are not generally elevated, fever is variable, and upper respiratory tract symptoms often occur; procalcitonin levels are not generally affected. For years, the diagnosis of viral pneumonia was based on cell culture and antigen detec

Middle East respiratory syndrome (MERS): a new zoonotic viral pneumonia.

Coronaviruses have traditionally been associated with mild upper respiratory tract infections throughout the world. In the fall of 2002, a new coronavirus emerged in in Asia causing severe viral pneumonia, i.e., severe acute respiratory syndrome (SARS). Nearly a decade following the SARS epidemic, a new coronavirus causing severe viral pneumonia has emerged, i.e., middle east respiratory syndrome (MERS). Since the initial case of MERS-CoV occurred in June of 2012 in Saudi Arabia there have been 688 confirmed cases and 282 deaths in 20 countries. Although both SARS and MERS are caused by corona

Comprehensive virome analysis of the viral spectrum in paediatric patients diagnosed with Mycoplasma pneumoniae pneumonia.

BACKGROUND: Among hospitalized children suffering from community-acquired pneumonia, Mycoplasma pneumoniae (MP) is one of the most common pathogens. MP often exists as a co-infection with bacteria or viruses, which can exacerbate the clinical symptoms. We investigated the pathogen spectrum in MP-positive and MP-negative samples from hospitalized children with respiratory tract infections in Beijing, China. METHOD: This study included 1038 samples of nasopharyngeal aspirates obtained between April, 2017 and March, 2018 from hospitalized children under 6 years of age with respiratory tract infe

[Non-pneumococcal bacterial pneumonias (II). Respiratory infections by Mycoplasma and Chlamydia. Viral pneumonias].

Non-pneumococcal and viral pneumonias are included in the group termed atypical pneumonias. They are caused by a wide range of bacteria and viruses; in most cases an aetiological diagnosis is not reached. Their clinical presentation is usually mild, although fatal cases have been described, principally in immunocompromised patients. Occasionally, extrapulmonary symptoms are associated, which makes diagnosis difficult. Microbiological tests are not indicated for diagnosis, except in the event of therapeutic failure or if the patient needs to be hospitalised. Early, empirical antibiotic treatmen

Using Precision Medicine for the Diagnosis and Treatment of Viral Pneumonia.

The COVID-19 pandemic has drawn considerable attention to viral pneumonia from clinicians, public health authorities, and the general public. With dozens of viruses able to cause pneumonia in humans, differentiating viral from bacterial pneumonia can be very challenging in clinical practice using traditional diagnostic methods. Precision medicine is a medical model in which decisions, practices, interventions, and therapies are adapted to the individual patient on the basis of their predicted response or risk of disease. Precision medicine approaches hold promise as a way to improve outcomes f

Adipocyte inflammation and pathogenesis of viral pneumonias: an overlooked contribution.

Epidemiological evidence establishes obesity as an independent risk factor for increased susceptibility and severity to viral respiratory pneumonias associated with H1N1 influenza and SARS-CoV-2 pandemics. Given the global obesity prevalence, a better understanding of the mechanisms behind obese susceptibility to infection is imperative. Altered immune cell metabolism and function are often perceived as a key causative factor of dysregulated inflammation. However, the contribution of adipocytes, the dominantly altered cell type in obesity with broad inflammatory properties, to infectious disea

Characteristics of viral pneumonia in the COVID-19 era: an update.

Influenza virus, rhinovirus, and adenovirus frequently cause viral pneumonia, an important cause of morbidity and mortality especially in the extreme ages of life. During the last two decades, three outbreaks of coronavirus-associated pneumonia, namely Severe Acute Respiratory Syndrome, Middle-East Respiratory Syndrome, and the ongoing Coronavirus Infectious Disease-2019 (COVID-19) were reported. The rate of diagnosis of viral pneumonia is increasingly approaching 60% among children identified as having community-acquired pneumonia (CAP). Clinical presentation ranges from mild to severe pneumo

[The Effects and Management of Viral Pneumonia on Lung Cancer Patients].

The worldwide epidemic of three coronaviruses and one influenza virus in 21st century have seriously threatened human health. Infection with these viruses can cause respiratory symptoms. The patients with lung cancer are more susceptible to viral infection and have a worse prognosis due to the advanced age and the systemic immunosuppressive state caused by malignancy itself and the anticancer treatments. In addition, without sufficient clinical awareness, a missed diagnosis of viral pneumonia may occur due to the fever and respiratory symptoms caused by lung cancer and its secondary diseases.

Role of Traditional Chinese Medicine in the Management of Viral Pneumonia.

Viral pneumonia is one kind of acute respiratory tract infection caused by the virus. There have been many outbreaks of viral pneumonia with high contagiousness and mortality both in China and abroad, such as the great influenza in 1918, the severe acute respiratory syndrome (SARS) coronavirus in 2003, the Influenza A (H1N1) virus in 2009, and the Middle East Respiratory Syndrome coronavirus (MERS-CoV) in 2012 and the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) in 2019. These outbreaks and/or pandemic have significant impact on human life, social behaviors, and economic develo

The impact of viral infections in children with community-acquired pneumonia.

For many years viral causes of community-acquired pneumonia were often been given limited attention. The number of published studies on influenza alone increased fivefold from 2001 to 2010. Specifically several studies have underlined that the involvement of viruses in community-acquired pneumonia has been underestimated, and this underestimation has been attributed to a lack of appropriate diagnostic methods. Now, with the advent of modern molecular assays, it is well recognized that viruses account for the largest proportion of community-acquired pneumonia in preschool children in both devel
