The 1960s cervical screening incident at National Women's Hospital, Auckland, New Zealand: insights for screening research, policy making, and practice.

BACKGROUND AND OBJECTIVES: This article examines a cervical screening incident from the 1960s and draws lessons for screening policy. STUDY DESIGN AND SETTING: Concern about harmful overtreatment of symptomless lesions prompted university gynecologist Herbert Green to study, between 1965 and 1970, a 'special series' of 33 women with carcinoma in situ (CIS) who were managed with only limited punch or wedge biopsy. These women were carefully followed up but not treated unless they showed evidence of progression to invasive cancer. This paper examines source documents and subsequent publications

The role of mathematical modelling in aiding public health policy decision-making: A case study of the BC opioid overdose emergency.

The province of British Columbia is currently experiencing the highest rate of apparent opioid-related deaths within Canada. This dramatic increase in overdose deaths has been primarily driven by the increase of fentanyl and fentanyl-analogues within the unregulated, highly unpredictable and toxic street drug supply. A public health emergency was declared in B.C. in April 2016. After the emergency was declared, overdose-related death rates continued to rise, reaching unprecedented levels. In the context of enhanced collaboration between government organizations and researchers, a series of mat

DISEASE RISK ANALYSIS--A TOOL FOR POLICY MAKING WHEN EVIDENCE IS LACKING: IMPORT OF RABIES-SUSCEPTIBLE ZOO MAMMALS AS A MODEL.

Disease control management relies on the development of policy supported by an evidence base. The evidence base for disease in zoo animals is often absent or incomplete. Resources for disease research in these species are limited, and so in order to develop effective policies, novel approaches to extrapolating knowledge and dealing with uncertainty need to be developed. This article demonstrates how qualitative risk analysis techniques can be used to aid decision-making in circumstances in which there is a lack of specific evidence using the import of rabies-susceptible zoo mammals into the Un

Development of a Clinical Forecasting Model to Predict Comorbid Depression Among Diabetes Patients and an Application in Depression Screening Policy Making.

INTRODUCTION: Depression is a common but often undiagnosed comorbid condition of people with diabetes. Mass screening can detect undiagnosed depression but may require significant resources and time. The objectives of this study were 1) to develop a clinical forecasting model that predicts comorbid depression among patients with diabetes and 2) to evaluate a model-based screening policy that saves resources and time by screening only patients considered as depressed by the clinical forecasting model. METHODS: We trained and validated 4 machine learning models by using data from 2 safety-net c

High HIV risk behavior among men who have sex with men in Kigali, Rwanda: making the case for supportive prevention policy.

Rwanda has responded strongly to HIV/AIDS, but prevention among men who have sex with men (MSM) has not yet been addressed due to a strong cultural resistance to homosexuality, and a lack of data showing the public health value of attending to the sexual health needs of this group. We conducted an exploratory study on HIV risk among MSM in Kigali using snowball sampling involving peer leaders. The 99 respondents were demographically, socially, and sexually diverse. Respondents reported relatively high numbers of male and female partners, and considerable HIV risk behaviors including commercial

"I think all of us should have […] much better training in ethics." Ethical challenges in policy making during the COVID-19 pandemic: Results from an interview study with Swiss policy makers and scientists.

BACKGROUND: The COVID-19 pandemic posed many unprecedented challenges to health care systems and public health efforts worldwide. Policy making and science were deeply intertwined, in particular with regard to the justification of health policy measures. In this context, ethical considerations were often at the core of decision-making trade-offs. However, not much is known about the actual ethical challenges encountered by policy makers and scientists involved in policy advice. With this study, we therefore aim to explore the ethical challenges during COVID-19-related political decision-making

You don't need criminal law to make cops: Reading carceral care work into reproductive healthcare policies in Vancouver.

This paper presents a critical review of the health and social policy regulating clinicians in Vancouver Canada. For this review the authors investigated how policy embeds carceral thinking in healthcare delivery dispensed in the lead up to, during, and up to one year after pregnancy, including reproductive technologies that facilitate conception, planned and emergency abortion, labour and birth, and child welfare services. Clinical care in all these areas becomes carceral when clinicians are implicated in work that coerces, criminalizes, or punishes patients. State investments in control and

Making progress on One Health policies and programmes: Perspectives on community engagement and gender equity in four West African countries.

INTRODUCTION: Gender equity and community engagement are increasingly recognised as important for the implementation of equitable One Health (OH) actions, yet no published studies examine how these are operationalised in policies and programmes. We analyse policy content and stakeholder perspectives on community engagement and gender equity in OH policies and programmes in Guinea, Liberia, Nigeria and Sierra Leone. METHODS: Mixed methods encompassed: content analysis of OH documents (7 core and 9 supporting); key informant interviews (KIIs) with policy stakeholders (n = 47) and focus group di

Impact of the One-Child Policy on Reproductive Decision-Making Among People of Chinese Descent in the United States: An Exploratory Study.

INTRODUCTION: China's one-child policy was implemented in 1980 primarily through mandatory intrauterine devices, tubal surgery, or abortion for "unauthorized pregnancies." While it was replaced in 2015, it affected millions of persons and its effects on reproductive decision making are not well known. METHODS: We designed, validated, and performed a cross-sectional survey of reproductive age Chinese-born or first-generation women of Chinese descent to describe the policy's impact on reproductive decision-making. Descriptive statistics and multivariate logistic regression were used to identify

The need to update Ethiopia's national health and medicine policies: essential tools for informed decision-making in healthcare and the pharmaceutical sectors.

BACKGROUND: Health policy involves decisions at national, state, and local levels to achieve healthcare goals, with medicine policy being a critical component that requires integration and potential reform. The World Health Organization (WHO) recommends periodic monitoring and evaluation of the National Medicine Policy (NMP) under the National Health Policy (NHP), ideally every 2 to 3 years or comprehensively every 4 to 5 years. OBJECTIVES: This perspective aims to urge the Ministry of Health (MOH) of Ethiopia to update its national health and medicine policies to address emerging health issu

Food Fortification With Folic Acid for Prevention of Spina Bifida and Anencephaly: The Need for a Paradigm Shift in Evidence Evaluation for Policy-Making.

Context-specific evidence evaluation is advocated in modern epidemiology to support public health policy decisions, avoiding excessive reliance on experimental study designs. Here we present the rationale for a paradigm shift in evaluation of the evidence derived from independent studies, as well as systematic reviews and meta-analyses of observational studies, applying Hill's criteria (including coherence, plausibility, temporality, consistency, magnitude of effect, and dose-response) to evaluate food fortification as an effective public health intervention against folic acid-preventable (FAP

Mapping evidence for health policy and systems decision-making: a spectrum approach bridging tacit and scientific knowledge across local and global contexts.

BACKGROUND: Research on evidence-informed decision-making has commonly focused on scientific evidence. However, this does not reflect the diversity of evidence used in real-world policy settings or give decision-makers a clear way to characterize and compare different evidence sources. This article describes the development of a spectrum approach to map the types of evidence used in health policy decisions and support more systematic and transparent judgements about their nature and applicability to different decisions. METHODS: The approach was developed in four stages. First, we conducted a

Leveraging lived experience: a qualitative study on the impact of formerly incarcerated consultants in public policy making to improve safety net services.

BACKGROUND: Los Angeles County-wide criminal justice reform and policy decision-making focused on reentry and diversion from incarceration usually include only the voices of law enforcement and other public-sector employees, with little input from impacted community members. The Reentry Health Advisory Collaborative (RHAC), founded in 2020 with grant funding, was established to engage formerly incarcerated individuals and their communities in county safety-net health systems. We aimed to assess the impact of RHAC and the importance of formerly incarcerated community input in safety-net program

A tutorial on discrete-event simulation for health policy design and decision making: optimizing pediatric ultrasound screening for hip dysplasia as an illustration.

BACKGROUND: It is increasingly recognized that healthcare is a complex system with limited resources and many interacting sources of both positive and negative feedback. Discrete-event simulation (DES) is a tool that readily accommodates questions of capacity planning, throughput management and interacting resources. As a result the use of DES in informing healthcare decision making is increasing. However, understanding when and how to build a DES model and use it for policy making is not yet a common knowledge. METHODS: The steps in building a DES model will be demonstrated using a real-worl

Strengthening advocacy efforts with empirical evidence: A case example of the conduct, uptake and utilisation of research in drug policy decision-making in Vietnam.

During the last decade, international aid agencies and advocates have been working with Southeast Asian governments to move away from punitive responses towards people who use drugs to more public health, humane approaches. The lack of local scientific evidence about the effectiveness of different treatment approaches has made this advocacy work more challenging. This paper reflects on a generation of treatment research evidence and how it can assist advocacy efforts. The case example is the cost-effectiveness research, comparing centre-based compulsory treatment with community-based voluntary

Policy Making to Build Relationships: A Grounded Theory Analysis of Interviews and Documents Relating to H1N1, Ebola, and the U.S. Public Health Preparedness Network.

In the last five years, the American public health emergency preparedness and response system has been tested by two significant threats, H1N1 and Ebola. While neither proved as dangerous as initially feared, these viruses highlighted on-going issues with collaborations in the field of public health and health care. Strengths were identified within the network, but also challenges that must be resolved before the U.S. faces a major pandemic. Employing interview data from public health emergency response practitioners and documentary evidence from the H1N1 and Ebola responses, this qualitative

BACKGROUND: Hospital policies play a role in healthcare providers' decision-making about reporting birthing people who use drugs to child welfare. This study sought to understand how these specific hospital policies are made. METHODS: We conducted semi-structured interviews with healthcare professionals involved in developing or revising hospital policies related to child welfare reporting for birthing people who use drugs.
