Evaluation of sleep-disordered breathing. Is polysomnography necessary?

To determine whether polysomnography is necessary to assess the presence and severity of sleep-disordered breathing, bedside observations by physicians were compared with the results of polysomnography in 37 patients with clinically suspected obstructive sleep apnea. Physician observations correlated with objective findings from polysomnography in detecting the presence of obstructive apnea (p less than 0.01), and had a high specificity and positive predictive value. The 20 patients correctly identified by clinical observation had a longer duration of apneic episodes (p = 0.02), increased seve

Why could sleep medicine never do without polysomnography?

Given the clinical, methodological, conceptual and modelling challenges within the field of sleep medicine, polysomnography (PSG) has emerged as a central diagnostic tool over time. It has been highly beneficial to clinical practice over the years, thanks to the scientific data that it provides. More recently, sleep medicine has sought answers in precision medicine, broadening its quest for biomarkers that take into account environmental factors, big data, and nosological refinement. However, despite these innovative developments that are relatively independent of PSG, sleep medicine remains i

Ambulatory blood pressure monitoring in children undergoing polysomnography.

Ambulatory blood pressure monitoring (ABPM) is recommended for the diagnosis of hypertension in children at high risk, such as children with obesity or obstructive sleep apnea (OSA). Nocturnal hypertension is highly predictive of cardiovascular outcomes. ABPM allows for early detection of nocturnal hypertension in children. Although OSA is the most common sleep disorder associated with hypertension, studies have also shown an increase in cardiovascular risk in adult patients with other sleep disorders; therefore, there is an imperative need to provide early diagnosis in children at high risk.

Accuracy of Automatic Polysomnography Scoring Using Frontal Electrodes.

STUDY OBJECTIVES: The economic cost of performing sleep monitoring at home is a major deterrent to adding sleep data during home studies for investigation of sleep apnea and to investigating non-respiratory sleep complaints. Michele Sleep Scoring System (MSS) is a validated automatic system that utilizes central electroencephalography (EEG) derivations and requires minimal editing. We wished to determine if MSS' accuracy is maintained if frontal derivations are used instead. If confirmed, home sleep monitoring would not require home setup or lengthy manual scoring by technologists. METHODS: O

Polysomnography Dataset for Sleep Analysis in Ischemic Stroke Patients.

Sleep architecture and integrity significantly influence neural recovery and cognitive restoration. These are particularly relevant in ischemic stroke survivors where sleep-disordered breathing (SDB) is a common comorbidity. To address the lack of stroke-specific sleep data, we present the Polysomnography Dataset for Sleep Analysis in Ischemic Stroke Patients (iSLEEPS), the first Asian and one of the largest stroke-specific sleep databases. Data collection was carried out between September-2018 and December-2021 at NIMHANS, India. iSLEEPS comprises 100 overnight PSG recordings with comprehensi

Laxatives are Associated with Poorer Polysomnography-derived Sleep Quality.

OBJECTIVE: To characterize 1) the relationship between laxative use and objective sleep metrics, and 2) the relationship between laxative use and self-reported insomnia symptoms in a convenience sample of middle-aged/elderly patients who completed in-laboratory polysomnography. METHODS: We cross-sectionally analyzed first-night diagnostic in-laboratory polysomnography data for 2946 patients over the age of 40 (mean age 60.5 years; 48.3% male). Laxative use and medical comorbidities were obtained through self-reported questionnaires. Patient insomnia symptoms were based on self-report. Associa

[Neonatal polysomnography in newborn infants with severe birth asphyxia].

INTRODUCTION: Neonatal polysomnography studies (electroencephalogram, electrooculography, body movements, cardiorespiratory frequencies) were performed in 101 newborn full-term infants diagnosed with severe birth asphyxia. PATIENTS AND METHODS: To analyse results, the sample was divided into two groups, depending on whether hypoxic-ischaemic encephalopathy (HIE) had occurred or not. The results of the polysomnography studies were correlated with those from the full-term neurological examination and the sequelae from the neurological development during the first two years of life. RESULTS: Si

Periodic limb movement counting in polysomnography: effects of amplitude.

BACKGROUND AND PURPOSE: This study investigates the relationship between periodic limb movement (PLM) counts obtained with standard scoring criteria and PLM counts scored without amplitude criterion (AC). PATIENTS AND METHODS: Twenty-four sleep laboratory patients with a PLM index (PLMI) >5 per hour of sleep in a previous polysomnography (PSG) underwent a full night of digital PSG. PLM were twice scored manually: first, according to standard criteria, and second, without AC. RESULTS: The overall PLMI for time in bed was 34.4+/-30.7 with AC and 50.2+/-36.4 without AC. The PLMI in non-rapid ey

Reference values for nocturnal home polysomnography in primary schoolchildren.

Abbreviated home polysomnography may be an alternative to laboratory polysomnography in children but is not yet generally accepted, partly due to a lack of reference values. Also, there are no normative data on respiratory events obtained using nasal prongs. We determined the prevalence and frequency of central, obstructive, and mixed apneas and hypopneas in a population-based sample of 50 children (mean age 10.1 years) using abbreviated home polysomnography and nasal prongs. We also determined the frequency of movements/arousals and body position changes. All children had central apneas. Obst

Diagnostic techniques for obstructive sleep apnoea: is polysomnography necessary?

Obstructive sleep apnoea (OSA) is a common condition of childhood with significant associated morbidity. The comprehensive evaluation of children who present with suggestive symptoms involves the overnight recording and assessment of both sleep and respiration by polysomnography in a sleep laboratory. These studies require resources and facilities that are not widely available and thus simpler, more available and less expensive alternatives have been sought. This review discusses the available alternatives to polysomnography for the evaluation of the child with suspected obstructive sleep apno

[The nocturnal eating syndrome (2 case reports and polysomnography)].

The nocturnal eating syndrome (NES) is a nocturnal sleep disorder caused by repeated awakening and the inability to fall a sleep again unless the patient ingests some food or drinks something. In children the NES is frequent, in adults rather rare and may be associated with various pathological conditions. The latter include in particular somnabulism, periodic movements of the lower extremities, narcolepsy, chronic triazolam intoxication, probably anorexia nervosa, and other eating disorders. The authors submit two case-histories of middle aged women (30 and 37 years old) without disturbances

Prediction of cardiovascular outcomes using electrocardiography from overnight polysomnography.

STUDY OBJECTIVES: Polysomnography (PSG) is the gold standard for diagnosing sleep disorders, which are recognized as cardiovascular risk factors. Single-lead electrocardiograms (ECGs) are recorded during PSG but rarely analyzed, presenting an opportunity to leverage overnight ECGs for cardiovascular risk prediction. MATERIALS AND METHODS: We applied a deep residual neural network with attention to single-lead ECGs from PSG, combined with sleep stage data, to predict 10-year risk of atrial fibrillation (AF), stroke, myocardial infarction (MI), heart failure (HF), and death. The network, traine

BACKGROUND: Sleepiness significantly contributes to traffic accidents, with obstructive sleep apnea syndrome (OSA) being the leading medical cause. Effective treatment of OSA has been shown to reduce accident-related deaths and injuries. Turkish legislation has been updated to address driver's license issues for individuals with OSA, but implementation challenges and debates persist. METHODS: Data from 159 patients who applied to obtain or renew their driver's licenses and underwent polysomnography between January 2020 and January 2024 were evaluated retrospectively. RESULTS: The body mass i

Diagnosis of Paediatric Obstructive Sleep-Disordered Breathing beyond Polysomnography.

Obstructive sleep-disordered breathing (SDB) has significant impacts on health, and therefore, a timely and accurate diagnosis is crucial for effective management and intervention. This narrative review provides an overview of the current approaches utilised in the diagnosis of SDB in children. Diagnostic methods for SDB in children involve a combination of clinical assessment, medical history evaluation, questionnaires, and objective measurements. Polysomnography (PSG) is the diagnostic gold standard. It records activity of brain and tibial and submental muscles, heart rhythm, eye movements,

Propriospinal myoclonus: diagnostic value of polymyography and video polysomnography.

UNLABELLED: Propriospinal myoclonus is a hyperkinetic movement disorder characterized by painless jerks of the axial muscles, mainly in the trunk and hips. A 53-year-old woman was referred to the Sleep Unit with trunk flexion movements in the supine position during the wake-sleep transition and during sleep, with premonitory sensation. We performed 2 video polysomnographic recordings. In the first video polysomnogram, the recording showed jerks of the trunk and abdomen that appeared when the posterior dominant alpha rhythm disappeared; during these jerks the patient stayed at stage 1 or stage

Delayed Sleep-Wake Phase Disorder: Can Polysomnography Be Useful?

BACKGROUND: Delayed sleep-wake phase disorder (DSWPD) is a chronic condition with a multifactorial etiology that primarily affects adolescents, significantly influencing their quality of life. In clinical practice, the contribution of intrinsic and behavioral factors is difficult to determine. The aim of our study was to compare data from clinical interviews, sleep diaries, actigraphy, and nocturnal polysomnography (PSG) in a cohort of adolescents with DSWPD and to assess psychiatric/neurodevelopmental comorbidity. METHODS: Thirty-one patients (22 male; mean age 15.4 ± 2.2 years, range 12 to

Polysomnography and Treatment-Related Outcomes of Childhood Sleep Apnea.

BACKGROUND: Polysomnography is central to the diagnosis and management of childhood obstructive sleep apnea (OSA). However, it is not known whether the treatment-related outcomes of OSA are causally associated with its resolution or changes in severity as determined by polysomnography. METHODS: Polysomnographic, cognitive, behavioral, quality-of life, and health outcomes at baseline and at 7 months were obtained from the Childhood Adenotonsillectomy Trial, a randomized trial comparing the outcomes of early adenotonsillectomy to watchful waiting in children with OSA. We used causal mediation a

OBJECTIVES: Neuroendocrine cell hyperplasia of infancy (NEHI) is a children's interstitial and diffuse lung disease of unknown etiology that presents in infancy with characteristic findings of tachypnea, retractions, crackles, and hypoxemia. At the present, the mainstay of treatment is oxygen supplementation to normalize oxygen saturations and decrease work of breathing. There are characteristic pulmonary function, radiographic, and histologic findings, but polysomnography (PSG) data has not been reported. We sought to report PSG data and implications for management and treatment of NEHI patie
