Popliteal vein entrapment presenting as deep venous thrombosis and chronic venous insufficiency.

PURPOSE: This report describes popliteal vein entrapment in three patients and demonstrates that it may present with manifestations of typical venous disease. METHODS: This report was compiled from a review of inpatient and outpatient records. RESULTS: In the first case, a 28-year-old woman was seen with left leg popliteal and calf deep vein thrombosis without obvious cause. She described long-standing calf discomfort, and passive dorsiflexion of the left foot caused disappearance of arterial pulsations at the pedal level. She was given the anticoagulants heparin and sodium warfarin (Coumadi

Acute pulmonary thromboembolism from deep vein thrombosis induced by trauma to the popliteal vein with a tennis racket.

Acute pulmonary thromboembolism (PTE) is mainly caused by deep vein thrombosis (DVT) and sometimes leads to a fatal outcome. Few cases of sport-related lower extremity DVT, involving direct external trauma, have been reported. A 58-year-old man, without any risk factors for thromboembolism suffered acute PTE from DVT after playing tennis. A detailed history revealed that he had hit his popliteal vein with each swing of his tennis racket and the site of the trauma, at popliteal fossa, was exactly the same as the site of the DVT formation. Therefore, the cause of DVT was suspected to be the repe

[MEP-21] Management of Cardiac Bullet Embolism Originating From the Popliteal Vein.

Intravascular missile embolism is comparatively uncommon; however, vascular injury is a frequent consequence of firearm-related accidents. To date, there have been 38 documented cases of intravascular missile embolization to the heart. This case report described the successful surgical management of a patient with a bullet embolism in the right ventricle after a gunshot injury to the left popliteal vein. A 54-year-old male presented with a gunshot wound to the left popliteal fossa. The popliteal vein, which was partially damaged, was primarily repaired. Radiological imaging, including radiogra

Pulmonary embolism secondary to popliteal vein aneurysm: case report and literature review.

Aim: Aneurysms are rarely detected in the popliteal vein as only a few cases have been reported in the literature so far. However, such aneurysms can be fatal due to thromboembolic complications or rupture. Case presentation: A 47-year-old male who had multiple bilateral pulmonary embolisms secondary to saccular right popliteal vein aneurysm discovered by lower limb duplex ultrasound and successfully treated with tangential aneurysmectomy with venorrhaphy. Conclusion: Popliteal vein aneurysm should be ruled out as a cause of pulmonary embolism, and medical treatment should be started rapidly,

Ruptured popliteal cyst stimulating deep vein thrombosis with false positive radiofibrinogen uptake.

A case is described in which deep vein thrombosis of the calf was initially diagnosed on the basis of the clinical signs and the radiofibrinogen uptake in a patient suffering from rheumatoid arthritis. The correct diagnosis of ruptured popliteal cyst was later established by arthrography and phlebography. Attention is drawn to the unreliability of the radiofibrinogen uptake test in the presence of ruptured popliteal cyst and alternative diagnostic methods are discussed.

Popliteal cysts are not a risk factor for lower extremity deep vein thrombosis.

BACKGROUND: Popliteal cysts (PC) result from distension of the gastrocnemio-semimembranosous bursa. Published reports indicate coincident PC and deep vein thrombosis (DVT). Whether the presence of PC increase the risk of deep vein thrombosis (DVT) remains unclear. METHODS: Lower extremity venous Duplex ultrasound (DUS) reports were evaluated across the Mayo Clinic Enterprise (Rochester, Minnesota, Jacksonville, Florida, Scottsdale, Arizona, and the Mayo Clinic Health System) in patients ≥ 18 years of age. Natural language processing (NLP) algorithms were created and validated to identify acut

Closed plication is a safe and effective method for treating popliteal vein aneurysm.

BACKGROUND: Popliteal vein aneurysms are a rare vascular anomaly first reported in the 1980s. Degeneration of elastic fibers and smooth muscle cell reduction, possibly secondary to inflammation, are implicated as integral steps in the development of these aneurysms. Given the rarity of this clinical entity, significant controversy exists regarding ideal treatment strategies, including the role of observation, medical management with anticoagulation, and surgical intervention. Retrospective reviews have demonstrated a failure rate of >40% with anticoagulation alone, with patients often presenti

Popliteal Vein Blood Sampling and the Postmortem Redistribution of Diazepam, Methadone, and Morphine.

Postmortem redistribution (PMR) refers to the site- and time-related blood drug concentration variations after death. We compared central blood (cardiac and subclavian) with peripheral blood (femoral and popliteal) concentrations of diazepam, methadone, and morphine. To our knowledge, popliteal blood has never been compared with other sites. Intracardiac blood (ICB), subclavian blood (SB), femoral blood (FB), and popliteal blood (PB) were sampled in 30 cases. To assess PMR, mean concentrations and ratios were compared. Influence of postmortem interval on mean ratios was also assessed. Results

Successful vein bypass in patients with an ischemic limb and a palpable popliteal pulse.

Patients presenting with an ischemic foot who have a palpable popliteal pulse have not always been considered to be suitable candidates for arterial reconstruction. Since 1976, 32 distal bypasses were performed in 30 such patients, 77% of whom were diabetic. Proximal anastomoses were from the distal superficial femoral or popliteal artery. Autologous vein was used in all cases. There were six graft failures, all of which occurred within eight months. Three failures, which all occurred within the first month, required below-the-knee amputation. Using life-table analysis, the one- and three-year

Thrombosed Popliteal Venous Aneurysm Treated With a Great Saphenous Vein Roll Interposition Technique.

Popliteal venous aneurysms (PVAs) are rare vascular anomalies that can result in serious thromboembolic complications. We present a case of a 77-year-old woman with a thrombosed right PVAs complicated by pulmonary embolism. Following initial anticoagulation, surgical resection of the aneurysm was performed, and individualized venous reconstruction was achieved using an autologous great saphenous vein (GSV) roll graft. This technique was selected over conventional panel or spiral grafts due to its simplicity, adaptability to size discrepancy, and reduced suture lines. To our knowledge, the use

Early Results of Popliteal Vein External Banding for Treatment of Chronic Venous Insufficiency.

BACKGROUND: Chronic venous insufficiency (CVI) encompasses a myriad of clinical manifestations including lower extremity swelling and pain, ulcerations and chronic skin changes such as stasis dermatitis, and lipodermatosclerosis. CVI effects greater than 25 million Americans and has a significant socioeconomic and psychosocial impact. Treatment of CVI varies depending on the etiology. For those patients with deep venous reflux, restoration of the deep venous valvular system is critical. Popliteal vein external banding is a novel technique to treat deep venous reflux. Our study aims to retrospe

Comparisons between prosthetic vascular graft and saphenous vein graft in femoro-popliteal bypass.

PURPOSE: Infrainguinalfemoropopliteal bypass (IFPB) is recommended to peripheral arterial disease (PAD) with a long occlusion of the superficial femoral artery (SFA). The aims of our study were to determine the patency of graft materials, and identify the risk factors of graft failure. METHODS: From January 1995 to April 2011, we had performed 380 IFPBs in 351 patients, including 302 femoro-above the knee (AK) bypasses and 78 femoro-below the knee (BK) bypasses. We compare age, sex, severity of ischemia between polytetra-uoroethylene (PTFE) graft and saphenous vein (SV) graft, and evaluate pa

Use of Endoscopic Vein Harvest Yields Improved Outcomes in Popliteal Artery Aneurysm Repair.

BACKGROUND: There is growing literature showing that endoscopic vein harvest (EVH) is safe, with excellent patency rates and decreased wound complications when treating infrainguinal occlusive disease. Our institution has performed EVH since 2003 with a dedicated team of providers specializing in endoscopic vein harvest. The purpose of this study was to evaluate major outcomes of EVH as an adjunct to standard, open operative repair of popliteal artery aneurysms. METHODS: We performed a 12-year retrospective single-institution chart review from January 2005 to December 2017, identifying all pa

Endoscopic versus open great saphenous vein harvesting for femoral to popliteal artery bypass.

OBJECTIVE: Conflicting data exist on outcomes of open vein harvest (OVH) and endoscopic vein harvest (EVH) for infrainguinal bypass. The purpose of this study was to compare outcomes between OVH and EVH in femoral to popliteal artery bypasses. METHODS: A retrospective review was performed of all patients undergoing common femoral to popliteal artery bypass with great saphenous vein between January 1997 and June 2014. Bypasses using arm or composite vein were excluded, as were those performed for popliteal artery aneurysms or trauma. Harvest was typically performed by dedicated surgical assist

Variations in popliteal fossa venous anatomy: implications for diagnosis of deep-vein thrombosis.

BACKGROUND: To retrospectively review the bilateral venous system within the popliteal fossa to evaluate the types of variations and their frequency seen in venous anatomy. MATERIALS AND METHODS: During routine dissection of formalin-fixed cadavers, a retrospective review of 32 bilateral (64 limbs) lower limbs obtained from adult donors was performed. Deep veins present in the popliteal fossa were evaluated according to predetermined criteria for the presence of duplication of vessels and interindividual variations in venous anatomy. RESULTS: More than one deep venous vessel was seen in the

Outcomes of popliteal artery aneurysms treated by ligation and in-situ saphenous vein bypass.

BACKGROUND: Modality of elective repair (open or endovascular) of popliteal artery aneurysms (PAAs) is still debated. About open repair no strict evidence exists about the best surgical technique. The aim of this study was to report a 20-year experience with ligation and in-situ saphenous vein bypass for the elective treatment of PAAs. METHODS: A retrospective review of consecutive patients who underwent elective open surgical PAA repair in our center between January 2001 and April 2020 was performed. Ninety-two limbs in 84 patients underwent a PAA ligation and in-situ saphenous bypass. Early

Successful repair of a popliteal aneurysm with saphenous vein graft in a patient with Marfan syndrome.

Marfan syndrome is an autosomal dominant disorder caused by mutations in the fibrillin 1 gene (FBN1). This leads to defective elasticity of connective tissue in the arterial wall. Aortic aneurysms and dissections are the most common vascular anomalies; the incidence of peripheral artery aneurysms is not well understood. Treatment options for infrainguinal disease are limited as endovascular interventions are generally contraindicated. The best conduit for arterial reconstruction is also unknown because there is concern that saphenous vein may become aneurysmal. Currently, there are few case re
