[Transjugular intrahepatic portosystemic shunt in the treatment of Budd-Chiari syndrome with extensive occlusion of the hepatic veins].

OBJECTIVE: To explore the outcome of a transjugular intrahepatic portosystemic shunt (TIPS) in the treatment of 11 patients with Budd-Chiari syndrome with extensive occlusion of the hepatic veins. METHODS: Eleven patients with Budd-Chiari syndrome with extensive occlusion of the hepatic veins were elected for TIPS. Three patients had a acute; eight, a subacute or a chronic course of the disease. All patients were associated with variceal bleeding and massive ascites. The diagnosis of Budd-Chiari syndrome was established by duplex-sonography, CT, MRI, upper digestive barium meal, angiography o

Portal hypertension.

The treatment of portal hypertensive gastrointestinal hemorrhage has seen many new and innovative advances in the past 15 years, including pharmocotherapy, sclerotherapy, transjugular intrahepatic portacaval shunt, partial portacaval shunt, and hepatic transplantation. Such an array of therapeutic options provides great flexibility for the physicians managing this complex disorder. The less invasive procedures tend to be associated with higher rates of rebleeding from esophageal varices. However, these procedures serve as excellent bridges to hepatic transplantation in poor-risk patients. Surg

[Repositioning the status of surgical treatment for portal hypertension in China].

The emergence, development, and clinical application of pharmacotherapy, endoscopic therapy, and transjugular intrahepatic portosystemic shunt have significantly improved clinical outcomes for patients with portal hypertension, leading to a marked reduction in surgical intervention needs. Consequently, the role of surgery in the management of portal hypertension has been questioned or even negated. China's cirrhosis-related portal hypertension differs markedly from Western countries in etiology, disease progression patterns, available therapeutic options, and patients' financial capacity. Ther

Evidences and perspectives of transjugular intrahepatic portosystemic shunt: A bibliometric analysis from 1990 to 2024.

BACKGROUND: Transjugular intrahepatic portosystemic shunt (TIPS) is the mainstay of cirrhotic portal hypertension therapy. The extension of TIPS indications is relatively prevalent in clinical practice these years, but its prognosis is still controversial. However, no bibliometric study in this area has been performed to date. This study aimed to provide insights and perspectives on the development, hotspots and frontiers of TIPS. METHODS: A total of 3152 related articles from 1990 to 2024 were retrieved from the Web of Science Core Collection database, and assessed using VOSviewer (version 1.

Attitudes towards transjugular intrahepatic portosystemic shunt (TIPS) in Australia: a national survey of TIPS centres.

BACKGROUND: Transjugular intrahepatic portosystemic shunt (TIPS) is a minimally invasive therapeutic option to treat the sequelae of portal hypertension. It is unclear whether current international recommendations are reflected in current clinical practice across Australia and the extent of variations in care. This study aimed to address this gap in knowledge and benchmark the current landscape of TIPS services in Australia against international guidelines. METHODS: We designed a 42-item questionnaire according to practice-based recommendations and standards of international guidelines to inv

Diagnosis and treatment of a congenital portosystemic shunt in a ferret (Mustela putorius furo).

A 3-year-old female neutered ferret presented with progressive weight loss was diagnosed with portosystemic shunting based on increased fasting bile acids, rectal ammonia tolerance testing and advanced imaging. Ammonia reference values were determined in 16 healthy ferrets. A congenital extrahepatic spleno-caval shunt was visualised with ultrasonography and CT angiography of the abdomen. Complete surgical shunt closure by suture ligation was performed, without clinical improvement after surgery. Euthanasia was elected 4 months postoperatively because the clinical condition deteriorated. This i

Advanced portal venous access techniques for transjugular intrahepatic portosystemic shunt placement.

INTRODUCTION: Transjugular intrahepatic portosystemic shunt (TIPS) placement is a well-established but technically challenging procedure for the management of sequelae of end-stage liver disease. Performed essentially blindly, traditional fluoroscopically guided TIPS placement requires multiple needle passes and prolonged radiation exposure to achieve successful portal venous access, thus increasing procedure time and the risk of periprocedural complications. Several advanced image-guided portal access techniques, including intracardiac echocardiography (ICE)-guided access, cone-beam CT (CBCT)

Hepatic venous pressure gradient measurement before TIPS for acute variceal bleeding.

Hepatic venous pressure gradient (HVPG) is an independent predictor of variceal rebleeding in patients with cirrhosis. After pharmacological and/or endoscopic therapy, the use of a transjugular intrahepatic portosystemic shunt (TIPS) may be necessary in HVPG non-responders, but not in responders. Thus, HVPG measurement may be incorporated into the treatment algorithm for acute variceal bleeding, which further identifies the candidates that should undergo early insertion of TIPS or maintain the traditional pharmacological and/or endoscopic therapy. The potential benefits are to reduce the cost

Acute cerebrovascular incident as a complication of TIPS procedure.

The formation of a transjugular intrahepatic portosystemic shunt (TIPS) is a well-established procedure in the management of complications of severe portal hypertension. It is also performed if portal hypertension is a result of acute portal vein thrombosis. We report the case of an acute cerebrovascular incident after TIPS formation in a patient with partial portal vein thrombosis. Even when no patent foramen ovale (PFO) is detectable the presence of PFO and thus the risk of cerebrovascular incident cannot be excluded. We therefore propose to inform patients with preexisting portal vein throm
