Post-exposure prophylaxis in leprosy (PEOPLE): a cluster randomised trial.

BACKGROUND: Post-exposure prophylaxis (PEP) using single-dose rifampicin reduces progression from infection with Mycobacterium leprae to leprosy disease. We compared effectiveness of different administration modalities, using a higher (20 mg/kg) dose of rifampicin-single double-dose rifampicin (SDDR)-PEP. METHODS: We did a cluster randomised study in 16 villages in Madagascar and 48 villages in Comoros. Villages were randomly assigned to four study arms and inhabitants were screened once a year for leprosy, for 4 consecutive years. All permanent residents (no age restriction) were eligible to

Rabies in Morocco: Epidemiological Aspects and Post-exposure Prophylaxis Management.

Background Rabies is a zoonosis transmitted accidentally to humans and is fatal once clinical signs appear. In Morocco, rabies remains a difficult disease to eradicate, with dogs being the main source of contamination. Transmission of this illness can be avoided by promptly implementing post-exposure prophylaxis (PEP) following animal exposure. Inappropriate or delayed PEP increases the risk of acute infection. This study aims to describe and analyze the characteristics of exposure, investigate the factors related to delay in initiating PEP, and evaluate the attitudes and practices of patients

[Revised multidisciplinary guidelines 'Varicella'; broader indication for post-exposure prophylaxis].

Post-exposure prophylaxis (PEP) with varicella zoster immunoglobulins (VZIG) should be administered as soon as possible after exposure to the virus, but always within ten days; in the previous guidelines this was within 96 hours. In cases of perinatal exposure, PEP with VZIG should be administered to neonates if the mother develops clinical chickenpox between seven days before delivery and seven days after delivery; in the previous guidelines this was between five days before delivery and two days after delivery. A new chapter on the treatment of chickenpox has been added to the guidelines.

Recurrent temporary paralysis reported after human rabies post-exposure prophylaxis.

Adverse events can occur after rabies post-exposure prophylaxis (PEP), and linkage to causality is often difficult to determine. We report a case of recurrent temporary paralysis that began immediately after the initiation of rabies PEP in a man exposed to a bat. The recurrent temporary paralysis first occurred in the patient after his initial dose and then again after day 3 of his rabies PEP. The PEP was terminated prior to a serologic response. The patient continued to experience numerous discrete episodes of temporary paralysis for over two years.

Post-exposure prophylaxis (PEP) of rabies-infected Indian street dogs.

A rabies post-exposure prophylaxis study was carried out to examine the efficacy of two commercially available rabies vaccines and the efficacy of a 5- or 3-dose vaccination regime. Healthy, native breed dogs (N = 40), seronegative for rabies antibody, were challenged intramuscularly with virulent rabies virus brain suspension (10(4.4) MLD50) by direct inoculation into the masseter muscle. The dogs were divided into four equal groups and injected intramuscularly with either Nobivac Rabies (Intervet), Rabisin (Merial) or placebo on multiple occasions (3 or 5-times) over the next 28 days. All do

Identification of risk factors for rabies exposure and access to post-exposure prophylaxis in Chad.

Rabies remains a global public health problem, with Africa as one of the most affected continents. Endemic transmission in the unvaccinated domestic dog population of developing countries leads to many exposures with subsequent death in humans due to lack of access to existing effective prevention tools. The presented study identifies factors of exposure and rabies risk in Chad on the household and health facility levels and highlights the challenges of access to Post Exposure Prophylaxis (PEP). Data on bite exposure and prevention was collected through a representative cross-sectional survey

Rabies exposures, post-exposure prophylaxis and deaths in a region of endemic canine rabies.

BACKGROUND: Thousands of human deaths from rabies occur annually despite the availability of effective vaccines following exposure, and for disease control in the animal reservoir. Our aim was to assess risk factors associated with exposure and to determine why human deaths from endemic canine rabies still occur. METHODS AND FINDINGS: Contact tracing was used to gather data on rabies exposures, post-exposure prophylaxis (PEP) delivered and deaths in two rural districts in northwestern Tanzania from 2002 to 2006. Data on risk factors and the propensity to seek and complete courses of PEP was c

[Post-exposure prophylaxis following exposure to HIV: adaptation to the situation may be indicated].

A woman aged 36 injured herself on a needle that had been used to take an iliac-crest biopsy from an HIV-positive patient and a man aged 34 and a woman aged 35 had sexual contact with their HIV-positive partners during which the condom tore. They were given post-exposure prophylaxis (PEP) which was formulated using medication and virus resistance data from the HIV-positive individual. At 3 and 6 months the patients were all still HIV-negative. After occupational or non-occupational exposure to HIV, PEP is initiated if there is a reasonable risk of transmission of HIV. [Sexual exposure to HIV, sexual behaviour and use of pharmacological post-exposure prophylaxis (PEP)].

In this study we examined the characteristics of 60 subjects (49 M and 11 F, average age 32.3) out of 195 post-exposure pharmacological prophylaxis (PEP) to HIV, taken in our hospital from 2001 to 2008. The above-mentioned subjects are sexually exposed (or presumably exposed) to HIV. We considered both their sexual intercourse behaviour and protective measures, and sought to infer some trends in sexual behaviour in Italy. All the subjects were monitored until 180 days after exposure, as established by the national guidelines. Only one of the 60 people presented a seroconversion (he dropped out

Instruments for Assessing Population Knowledge of Pre-exposure Prophylaxis (PrEP) and/or Post-exposure Prophylaxis (PEP) for HIV: A Scoping Review.

Pre-exposure prophylaxis (PrEP) and post-exposure prophylaxis (PEP) are central strategies for HIV prevention; however, a comprehensive understanding of how knowledge about these strategies is measured across different contexts remains limited. This scoping review aimed to map the instruments used to assess knowledge of PrEP and/or PEP. The study followed the recommendations of the JBI Reviewer's Manual, with a systematic search conducted on August 27, 2024, in PubMed, Embase, Web of Science, the Virtual Health Library, and grey literature. Original studies assessing knowledge of PrEP and/or P

Doxycycline post-exposure prophylaxis as prevention of STIs - the golden bullet?

INTRODUCTION: Doxycycline post-exposure prophylaxis ('doxyPEP') is an emerging strategy to prevent bacterial sexually transmitted infections (STIs). Users take 200 mg of doxycycline up to 72 hours after condomless sex, and data from randomized controlled trials and real-world implementation have shown doxyPEP to be effective in preventing syphilis, chlamydia, and to a lesser extent gonorrhea, in gay, bisexual, and other men-who-have-sex-with-men (GBMSM) and transgender women. AREAS COVERED: We discuss the potential benefits, risks, and important considerations for doxyPEP implementation, draw

UK guideline for the use of HIV post-exposure prophylaxis 2021.

We present the updated British Association for Sexual Health and HIV (BASHH) guidelines for post-exposure prophylaxis (PEP) to HIV following sexual exposures, occupational exposures and other nonoccupational exposures in the community. This serves as an update to the 2015 BASHH guideline on PEP following sexual exposures and the 2008 Expert Advisory Group on AIDS guidelines on HIV PEP. We aim to provide evidence-based guidance on best clinical practice in the provision, monitoring and support of PEP for the prevention of HIV acquisition following sexual, occupational and other nonoccupational

Effectiveness of varicella vaccine as post-exposure prophylaxis: a meta-analysis.

BACKGROUND: To evaluate the effectiveness of varicella vaccine (VarV) as post-exposure prophylaxis (PEP) among children during varicella outbreaks. MATERIAL AND METHODS: A comprehensive literature search was performed in PubMed, Embase, Web of Science, SinoMed, Wanfang and CNKI. Relevant outcomes included the incidence of varicella. Pooled estimates were calculated using a fixed-effects or random-effects model according to the heterogeneity among studies. RESULTS: A total of 15 studies with 7,474 children that received one or two dosages of VarV as PEP and 183,827 children who received no Va

Rabies encephalitis in a child: a failure of rabies post exposure prophylaxis?

Rabies remains a serious public health problem in many developing countries. The diagnosis is easy when a non-immunised patient presents with hydrophobia and hypersalivation after a bite by a known rabid animal but more difficult when a patient presents atypical symptoms after having received rabies postexposure prophylaxis. Rabies postexposure prophylaxis failure is rare. We report a case of a 6-year-old boy who presented febrile seizure with agitation and cerebellar signs, without hydrophobia or hypersalivation, 17 days after a dog bite. Despite four doses of rabies vaccine and immunoglobuli

Factors associated with delay in post-exposure prophylaxis in bitten people.

BACKGROUND: Although the incidence of animal bite is increasing in Iran, there is no sufficient knowledge about delayed Post-Exposure Prophylaxis (PEP). Hence, the aim of this study was to evaluate delayed PEP and its associated factors in bitten people. METHODS: This cross-sectional study was conducted in Tuyserkan district of Hamadan Province (West of Iran) from February 2011 to February 2012 and included 425 cases of animal bites recorded in the Rabies Treatment Center (RTC) using the census method. The associations between delayed PEP and each of the potential risk factors were investigat

Cost effectiveness of rabies post exposure prophylaxis in the United States.

There is growing concern in the United States about the excessive use of rabies post exposure prophylaxis (PEP) treatment. In this paper we have estimated the cost effectiveness of rabies PEP treatment under various scenarios of rabies transmission. When the risk of a patient getting rabies is deemed greater than 0.7%, then giving PEP will be cost saving (societal perspective). For lower risks of transmission, cost effectiveness estimates ranged from approximately $500,000 per life saved to $billions. The uncertainty caused by the wide range of cost effectiveness can only be resolved through a

[Pre- and post-exposure rabies prophylaxis: who to vaccinate and how?].

Switzerland is rabies free since 1996 (except bats). However sporadic cases in animals (imported dogs and local bats essentially) may occur from time to time. The disease is always fatal, which implies special attention when someone gets bitten or scratched by a wild or unknown animal, not only in a country with a high endemicity, but also in Switzerland. In this article, we will explore in detail the modalites of application of pre- and post-exposure prophylaxis following the publication of new recommandations from the Public Health authorites of Switzerland.
